Maternal and neonatal outcomes in pregnant patients with Brugada Syndrome - a multicentre retrospective study (2014-2025).

BACKGROUND: Brugada Syndrome is an inherited channelopathy that predisposes to ventricular arrhythmias. Triggers include fever, enhanced vagal tone, electrolyte abnormalities and certain medications, including local anaesthetics and propofol. Multicentre studies evaluating anaesthetic management in conjunction with maternal and neonatal outcomes in pregnant patients with Brugada Syndrome are lacking. METHODS: In this retrospective cohort study, outcomes from 64 pregnancies in 57 patients with Brugada Syndrome across 12 maternity units in the UK and Ireland were reviewed (2014-2025). Outcomes

Research on ultra-fast fabrication method of bidirectionally anisotropic Ion beam figuring for large-aperture nanoradian X-ray mirrors.

The grazing-incidence operating mode of large-aperture X-ray mirrors imposes bidirectional anisotropic requirements on surface figure accuracy: extremely stringent in the meridional direction, while relatively relaxed in the sagittal direction. The strict meridional accuracy requirement necessitates the use of small-beam deterministic figuring processes, making machining efficiency a critical bottleneck to be overcome. To address this issue, this paper proposes a spatially decoupled figuring strategy based on a bidirectionally anisotropic elliptical diaphragm. By exploiting the disparity in ac

Impact of hydroxychloroquine on pregnancy outcomes in systemic lupus erythematosus: A 25 years retrospective cohort study from Asia.

BackgroundSystemic lupus erythematosus (SLE) is an autoimmune disorder that affects various organs and follows a pattern of remission and relapse. Hydroxychloroquine (HCQ), an antimalarial drug, has recently become widely used in pregnant women with SLE due to its multi-level immune anti-inflammatory mechanisms and potential endothelial protective and thromboprophylaxis properties. While several studies have evaluated the impact of HCQ on SLE activity and the occurrence of neonatal lupus, its effects on improving early delivery, preeclampsia, and intrauterine growth restriction (IUGR) remain c

Association between maternal anti-Ro and anti-La antibody levels and congenital heart block: a 20-year cohort study.

OBJECTIVE: To evaluate the association between maternal anti-Ro and anti-La antibody levels and the occurrence of congenital heart block (CHB) in fetuses and newborns. METHODS: This retrospective cohort study included 182 pregnant women with positive anti-Ro and/or anti-La antibodies who received prenatal care at our tertiary center between 2002 and 2022. Maternal clinical, laboratory, and obstetric variables were analyzed. RESULTS: Thirteen fetuses (7.1%) were diagnosed with CHB. Mothers of affected fetuses had significantly higher anti-Ro (median 240 vs. 42; p < 0.001) and anti-La (median

Role of Hydroxychloroquine in Prevention of Congenital Heart Block in Pregnant Women With Positive Anti-Ro/SSA and/or Anti-La/SSB.

Neonatal lupus, a condition primarily affecting infants born to mothers with anti-Ro/SSA and/or anti-La/SSB antibodies, is linked to congenital heart block (CHB), a potentially life-threatening complication. Hydroxychloroquine (HCQ), an antimalarial with immunomodulatory properties, has been shown to reduce CHB recurrence. This review examines current evidence on HCQ's role in preventing CHB in pregnancies affected by those autoantibodies, focusing on overall, primary and secondary prevention. Evidence supports HCQ in overall and secondary prevention, but data on its effectiveness for primary

Androgen-mediated DKK1 upregulation in bronchial epithelial cells suppresses Th2 response and ameliorates asthma.

BACKGROUND: Mutations in the human mitochondrial genome have been suspected to play a significant role in the etiological development of mitochondrial diabetes. Detection of the 3243A>G mutation in the mitochondrial transfer RNALeu(UUR) gene (MTTL1), especially at low heteroplasmy levels, is highly desirable since it facilitates the diagnosis and subsequent management of the disease. The proportions of mutant mitochondrial DNA (mtDNA) can vary between tissues and are usually significantly higher in muscle than in blood, but muscle biopsies from patients with diabetes are rarely available. Refractory Neonatal Apnea Revealing Congenital Central Hypoventilation Syndrome: Improved Outcome through Early Multidisciplinary Intervention.

Background: Congenital central hypoventilation syndrome (CCHS) is a rare genetic disorder that causes alveolar hypoventilation because of impaired chemoreceptor response to hypercapnia and hypoxia occurring due to a dysfunctional central autonomic drive. CCHS is a result of pathogenic variants of the PHOX2B gene, which affects neural crest cell development. Consequently, CCHS can be accompanied by dysregulation of the autonomic system, Hirschsprung's disease and other gastrointestinal disorders, cardiac arrhythmias, and neural cell-derived tumors. Disease severity correlates with mutation type

Survivors of Cyanotic Congenital Heart Disease: A Review.

IMPORTANCE: Cyanotic congenital heart diseases, which occur in approximately 0.2% of live births in the United States, have high mortality rates if untreated, but survival to adulthood is common with current surgical interventions. OBSERVATIONS: Approximately 80% of all cyanotic congenital heart diseases are caused by tetralogy of Fallot (TOF), D-loop transposition of the great arteries (TGA), and congenital heart disease with single-ventricle circulation. Survivors of cyanotic congenital heart diseases benefit from multidisciplinary care including pediatric and adult cardiologists, congenita

Spectrum of potentially lethal cardiac conditions presenting with fetal sinus bradycardia: a report on 34 cases.

OBJECTIVE: Fetal sinus bradycardia (FSB) is usually a benign antenatal finding but it can be a marker of an underlying serious condition that manifests in later life. The aim of this study was to report the antenatal course and postnatal outcome of FSB. METHOD: This study was a retrospective review of all cases of persistent FSB, defined as a fetal heart rate (FHR) < 3rd percentile for gestational age (GA) with 1:1 atrioventricular conduction on at least two consecutive fetal echocardiograms, diagnosed between January 2014 and December 2024 at the Brompton Centre for Fetal Cardiology, London,

pedQTNet: A Deep Learning Approach to Estimate Corrected QT Intervals from Multi-Lead Conventional ECG Waveforms in Pediatric Patients.

Long QT syndrome (LQTS) is a primary risk factor for ventricular arrhythmias and sudden cardiac death in children. Accurate corrected QT intervals (QTc) measurement is imperative but challenging for non-heart-rhythm specialists, especially in children. We developed and evaluated pedQTNet, a deep neural network model for estimating QTc and detecting LQTS in pediatric patients. We analyzed a cohort of 37,992 patients aged 0-18 years with 65,370 ECGs annotated by pediatric electrophysiologists (PEPs) between 2010 and 2020. Using PEP-annotated QTc measurements as ground truth, pedQTNet was trained

Role of CHD chromatin remodelers in heart development.

BACKGROUND: Chromodomain helicase DNA-binding (CHD) proteins are ATP-dependent chromatin remodelers that regulate chromatin accessibility, genome organization, and lineage-specific transcription during embryonic development. There are nine members of this family of proteins. Increasing genetic and functional evidence links mutations in multiple CHD genes to congenital-heart disease, arrhythmias, and cardiomyopathies. However, the stage-specific contributions of individual CHD-family members to cardiac development and functional maturation remain incompletely defined. DATA SOURCES: This narrat

β-Arrestin-biased activation of type I angiotensin II receptors improves prognosis of murine pediatric heart failure.

The pharmacological treatment of pediatric heart failure (PHF) remains a long-standing unmet medical need. We previously reported that TRV027, a peptidyl angiotensin type 1 receptor (AT1R) agonist that selectively activates β-arrestin over G protein signaling and avoids G protein-mediated vasoconstriction, increased cardiac contractility in neonatal, but not adult, mice by activating CaV1.2 L-type Ca2 + channels via casein kinase 2 without detectable adverse effects. TRV027 also significantly increased twitch Ca2+ transients in human induced pluripotent stem cell-derived cardiomyocytes. To exa

A varying presentation of refractory supra ventricular tachycardia in a newborn: A rare and unusual case report.

Conduction abnormalities in neonates are not common and some arrhythmias may resolve spontaneously, while refractory abnormalities may persist. Some rare arrhythmias may be associated with congestive cardiac failure (CCF) or shock, requiring immediate intervention. Most supraventricular tachycardias (SVT) respond to adenosine and are rarely refractory. If not managed properly, it may result in more complications, prolonged hospital stays, and mortality. Here, we are reporting the case of a 20-day-old baby who presented with poor feeding, respiratory distress, lethargy, and tachycardia. The hea

Prenatal Diagnosis of Isolated Congenitally Corrected Transposition of the Great Arteries.

Congenitally corrected transposition of the great arteries (ccTGA) is an uncommon congenital heart defect characterized by atrioventricular and ventriculo-arterial discordance resulting from abnormal leftward looping of the primitive cardiac tube. Although this double discordance produces a physiologically corrected circulation, the morphologic right ventricle functions as the systemic ventricle, and the tricuspid valve serves as the systemic atrioventricular valve, predisposing affected individuals to late complications. Prenatal diagnosis of ccTGA is often challenging, particularly in isolat
