Proteomics reveals crosstalk between calcium disturbance and multiple stresses facilitating lethal ventricular arrhythmias-sudden cardiac death under the co-morbidity of acute myocardial ischemia and type II diabetes mellitus.

AIMS: Diabetes mellitus (DM) often co-exists with myocardial ischemia (MI). Whether DM increases sudden cardiac death (SCD) susceptibility in early MI remains unclear, and the underlying mechanism needs to be further explored. MATERIALS AND METHODS: We induced type II DM in mice to produce a co-morbidity model of DM and MI. Proteomic analysis was conducted to identify proteomic features associated with DM-SCD and MI. Additionally, a neonatal rat ventricular myocyte (NRVM) model was established under conditions of hypoxia and high glucose (HG) to elucidate the interplay between oxidative stres

Ivabradine Use in Premature Infants With Accelerated Idioventricular Rhythm.

INTRODUCTION: Ivabradine has been recently introduced to treat focal atrial tachycardia and junctional ectopic tachycardia in children. Its use for treating ventricular tachyarrhythmias, specifically accelerated idioventricular rhythm (AIVR), remains limited. METHODS AND RESULTS: We report two cases of premature infants with AIVR successfully treated with ivabradine as monotherapy after other antiarrhythmic agents had failed. The first infant had a structurally normal heart and the second infant a truncus arteriosus. In both patients, sinus rhythm was restored with ivabradine. Side effects on

Safety and Feasibility of Implanting Dual-Chamber Permanent Pacemakers in Patients Under 3 kg.

Purpose: Permanent pacing in the young pediatric population can be challenging, especially in small neonates. We describe our institutional experience with the surgical placement of dual-chamber permanent pacemaker (PPM) systems in neonates weighing <3 kg at the time of implantation. Methods: The Cincinnati Children's Hospital database was used to identify patients who had an epicardial dual-chamber PPM and weighed <3 kg at implantation from 2000 to 2022. Descriptive analyses focused on surgical techniques, postimplant complications, and survival. Results: Twenty patients underwent dual-chambe

Sudden Unexpected Infant and Perinatal Death: Pathological Findings of the Cardiac Conduction System.

Objective: Sudden infant death syndrome (SIDS), sudden neonatal unexpected death (SNUD), and sudden intrauterine unexpected death (SIUD) are major unsolved, shocking forms of death that occur frequently and without warning. The body of literature on the anatomo-pathological substrates in the cardiac conduction system of SIDS-SIUD and their possible relationship with risk factors and triggers is fragmentary and scarce. The work aims is to analyze the cardiac conduction system findings collected at the national referral center for SIDS-SIUD. Methods: A total of 123 autopsied cases of SIDS (59.35

Pregnancy and breastfeeding in the mother with cardiomyopathy.

Cardiomyopathies are diseases of the heart muscle, characterised by structural and functional abnormalities of the myocardium that are not caused by ischemia, valvular problems or congenital heart disease. They are responsible for one-third of pregnancy-related cardiovascular deaths. A woman may enter pregnancy with a pre-existing diagnosis, or the cardiomyopathy may emerge or develop de novo during pregnancy. The most common cardiomyopathies encountered in pregnancy are dilated cardiomyopathy, hypertrophic cardiomyopathy and peripartum cardiomyopathy. All cardiomyopathies can be complicated b

Mitochondrial Donation in a Reproductive Care Pathway for mtDNA Disease.

Pathogenic variants in mitochondrial DNA (mtDNA) are a common cause of severe, often fatal, inherited metabolic disease. A reproductive care pathway was implemented to provide women carrying pathogenic mtDNA variants with reproductive options. A total of 22 women with pathogenic mtDNA variants have commenced or completed pronuclear transfer (and thus receipt of a mitochondrial donation), and there have been 8 live births. All 8 children were healthy at birth, with no or low levels of mtDNA heteroplasmy in blood. Hyperlipidemia and cardiac arrhythmia developed in a child whose mother had hyperl

Maternal Cardiac and Perinatal Outcomes Among Pregnant Individuals With Ebstein's Anomaly.

Ebstein's anomaly is a rare congenital cardiac condition with limited data regarding the impact of this condition in pregnancy. The objective of this study was to describe the cardiac and perinatal outcomes associated with Ebstein's anomaly in pregnancy. We performed a retrospective cohort of pregnancies from Epic Cosmos, an integrated dataset from over 1,500 hospitals across the United States. Charts were queried for the coexistence of pregnancy and Ebstein's anomaly diagnosis code between July 5, 2021, and March 5, 2025. They were then compared to patients with other congenital heart disease

Electrical device therapy in the paediatric population-age, indications, strategies and limitations.

BACKGROUND: Electrical device therapy is increasingly used for paediatric patients with congenital and acquired arrhythmias and heart diseases. OBJECTIVES: This review evaluates the indications, strategies and limitations of this therapy in the young. Pacing and defibrillation systems in young patients should last longer than in adults. The paediatric population ranges from preterm newborns to young adults, and therefore recommendations and pacing strategies are variable. Recent guidelines have provided new and updated recommendations. Various systems have enhanced our ability to treat arrhyt

Nursing vigilance in preventing catheter fragment migration: a dual case report of peripheral venous catheter fractures in pediatric practice.

BACKGROUND: Indwelling venous catheters, including peripheral intravenous catheters (PIVCs), are vital in pediatric care for delivering medications and fluids. However, catheter fractures, though rare (incidence 0%-2.1%), pose serious risks such as pulmonary embolism or cardiac arrhythmias if fragments migrate. While central venous catheter fractures are well-documented, PIVC fractures are underreported despite their widespread use. CASE PRESENTATION: This report details two pediatric cases of PIVC fractures. In the first, a 1-day-old female neonate experienced a fractured left axillary cathe

Unmasking of Wolff-Parkinson-White syndrome following cardioversion of ventricular tachycardia in pregnancy: a case report.

BACKGROUND: Wolf-Parkinson-White syndrome predisposes patients to tachyarrhythmias and sudden cardiac death, with pregnancy further exacerbating arrhythmia risk due to hemodynamic, hormonal, and autonomic changes. We present a rare case of a pregnant woman with Wolf-Parkinson-White syndrome, which was unmasked after successful cardioversion of unstable ventricular tachycardia. CASE PRESENTATION: A 40-year-old Black Ethiopian pregnant woman in her 35th week of gestation presented with unstable ventricular tachycardia, requiring multiple cardioversions, along with treatment with amiodarone and

Impact of dexmedetomidine during hypothermia on initiation of enteral feeding in newborns with hypoxic-ischemic encephalopathy.

BackgroundAfter early studies suggested safety and potential for benefit of dexmedetomidine use in neonatal hypoxic-ischemic encephalopathy (HIE), our neonatal intensive care unit (NICU) decided to transition from morphine to dexmedetomidine as our standard sedative during therapeutic hypothermia (TH). The primary aim was to monitor the possible side effects of transitioning from morphine to dexmedetomidine with a primary goal of reducing the days to initiation of enteral feeds to less than 3 days, with the hypothesis that the gastrointestinal motility effects of morphine may have been hinderi

Arrhythmias in Pregnancy: A Comprehensive Review.

Cardiac arrhythmias are increasingly recognized as contributors to maternal morbidity and mortality, particularly in women with preexisting cardiovascular disease. The hemodynamic, hormonal, and autonomic changes of pregnancy can unmask or exacerbate underlying arrhythmogenic conditions. This review summarizes current data on the classification, epidemiology, and management of arrhythmias during pregnancy. Antiarrhythmic therapy requires careful consideration of maternal benefit versus fetal risk, including concerns about teratogenicity and neonatal drug exposure. Management must be individual

Elevated levels of Letm1 drives mitochondrial dysfunction and cardiomyocyte stress-mediated apoptosis in cultured cardiomyocytes.

BACKGROUND: Cardiac ischemia, a predominant cause of heart failure, is marked by profound mitochondrial dysfunction, dysregulated ion homeostasis, and maladaptive cellular remodeling, all of which compromise cardiac performance. The mitochondrial inner membrane protein Leucine zipper-EF-hand containing Transmembrane Protein 1 (Letm1), implicated in Wolf-Hirschhorn Syndrome, is essential for mitochondrial function. Although genetic alterations in Letm1 are linked to cardiomyopathies, its specific contributions to cardiac pathophysiology, particularly in the context of ischemic heart disease, re
