Neonatal hypertension occurs in up to 2% of neonatal intensive care survivors and in up to 3% of all neonates. Normal blood pressure (BP) measurements are required to diagnose and manage appropriately both hypotension and hypertension in the neonate and infant. The aim of this study was to provide normative BP measurements during the first year of life of healthy infants born at term, using an oscillometric method. Neonates were enrolled from August 2003 to August 2005. Exclusion criteria included: infants of mothers with hypertension or diabetes of any type, use of illicit substances, congeni

Neonatal hypertension is an uncommon but important complication of intensive care management. The aims of this study were to identify in neonates with hypertension: antenatal and postnatal risk factors; aldosterone and renin levels; and report on outcome in early infancy. The study involved a retrospective review of neonates diagnosed with systemic hypertension from January 2001 to December 2005. Demographic data, risk factors, laboratory investigation, and follow-up data at 3-6 months of age were collected. Of the 2,572 newborn infants included, 34 (1.3%) had neonatal hypertension. Gestationa

We describe a case of mid-aortic syndrome presenting as systemic hypertension in infancy and early childhood. Angiography of the descending and abdominal aorta is the diagnostic test of choice to confirm the diagnosis of mid-aortic syndrome. Severity of hypertension is one of the major factors in determining the timing of intervention. Because of variability in the anatomic extent of mid-aortic syndrome, management options need to be individualized.

Hypertension in the neonate is a rare disease. Although the definition of hypertension in this age group has not been completely standardized, recent studies have provided new normative data that may be used to facilitate identification of such infants. The most common causes of clinically and treatable hypertension in neonates is renal artery occlusion due to thromboembolic events related to umbilical catheterization. Additionally, congenital problems such as aortic coarctation, structural renal malformations and renovascular disease, as well as acquired renal disease and certain medications

BACKGROUND: Liver fibrosis (LF) is a chronic disease, associated with many collateral diseases including reproductive dysfunction. Although the normal liver has a large regenerative capacity the complications of LF could be severe and irreversible. Hormone and sex-related issues of LF development and interactions with male reproductive have not been finally studied. The aim was to study the reproductive function of male rats in experimental CCl4-induced liver fibrosis rat model, and the capability for restoration of both the liver and male reproduction system.
MATERIALS: Studies were conducte

Neonatal hypertension occurs in 2% of all infants and it is caused by renovascular abnormalities in 70% of these infants. The gold standard for diagnosing renovascular disease is conventional renal angiography. However, in neonates the procedure is not commonly used because of its invasive and technically challenging nature. MRI and MR angiography (MRA) are less invasive yet reliable means of detecting renovascular disease in adults. There is minimal literature on the use of MRI/MRA in neonatal hypertension. We report a neonate with hypertension secondary to a renovascular abnormality in which

Neonatal hypertension, defined as systolic blood pressure greater than 95th percentile for age, size, and gender, is an uncommon but significant problem. Hypertension may develop secondary to thromboembolic complications of central lines, steroid use, or be a sign of an underlying renal or cardiac problem. Pharmacologic management of hypertension in the neonate includes use of beta blockers, angiotensin-converting enzyme inhibitors, calcium channel blockers, and/or diuretics. This article discusses issues in assessment and management of neonatal hypertension and reviews the pharmacologic agent

OBJECTIVE: To describe the clinical course of a newborn (gestational age 38 wk, 6 d) administered intravenous nicardipine to control hypertension while on extracorporeal membrane oxygen (ECMO) support.
SETTING: Urban, university teaching hospital.
CASE SUMMARY: A 1-day-old male neonate presenting with a left congenital diaphragmatic hernia developed a sustained elevation in blood pressure following a change of ECMO tubing. After several unsuccessful treatment regimens, nicardipine was started at a rate of 0.5 micro g/kg/min and aggressively titrated to 1.5 micro g/kg/min. Nicardipine produce

BACKGROUND: Neonatal arterial hypertension is rare with an incidence between 0.2 - 3 %. Clinical presentation varies widely and is in some cases dramatic.
PATIENT: A 4-day old full-term neonate was admitted to the intensive care unit with severe congestive heart failure and metabolic acidosis. Mechanical ventilation was initiated and dobutamine administered because of poor systolic function. Continuous monitoring of blood pressure revealed severe arterial hypertension (30 to 40 mm Hg above the 95th percentile). Ultrasonography showed an echogenic left kidney with normal perfusion. Laboratory

OBJECTIVE: To present the importance of searching for the surgical causes of pharmacologically resistant hypertension in the neonatal population.
STUDY DESIGN: A case report and discussion are provided.
RESULTS: Severe hypertension in the neonatal period is uncommon and almost always has a secondary cause. Although a majority of hypertensive neonates can be successfully managed with medical therapy, some cases are resistant to pharmacological treatment. We report three hypertensive neonates who failed to respond to intensive multidrug therapy. This led to further evaluation and identificatio

We report a case of fetal systemic hypertension. This occurred in an ex-donor twin soon after coagulation of chorionic vessels and amniodrainage performed for severe twin-twin transfusion syndrome during the 2nd trimester of pregnancy. Systemic hypertension was suspected because of a high systolic velocity through the tricuspid valve, and Bernoulli's equation was used to estimate the right intraventricular pressure. As both pulmonary arteries and ductus arteriosus were normal, the pressure in the aorta was considered to be equal to that in the right ventricle (60 mm Hg). Fetal systemic hyperte

Hypertension in the neonatal period and early infancy in underdiagnosed due to the absence of the attention and the difficulties of diagnostic procedures. Although the neonatal systemic hypertension is rare, but it can, cause serious medical problem with an increased risk of cardiorespiratory failure, cerebral distress and growth failure. Arising questions in their daily work got the authors to review the literature. They analyse procedures and criteria of measuring of neonatal blood pressure and values of neonatal hypertension. In addition to a list of the origin of hypertension they discuss

Two newborn infants developed cardiac failure due to severe hypertension which was recognised as the heart failure was treated. Renal abnormalities were found in both infants who are normotensive off treatment at 18 months follow up. The finding of hypertension rather than hypotension in the presence of cardiac failure and the apparent absence of a cardiac abnormality should prompt a search for a renal or renovascular cause.

Treatment of hypertension with enalapril in a preterm infant is described. Enalapril is a new converting enzyme blocker with a longer plasma half-life and less side effects than captopril. Oral administration of enalapril 0.1 mg/kg caused severe hypotension and renal failure in a preterm infant. We recommend an oral starting dose of 0.01 mg/kg in preterm infants and extremely close monitoring of infants receiving the first dose of enalapril.

We present an unusual case of in utero adrenal hemorrhage associated with fetal distress. This case was further complicated by transient neonatal hypertension that resolved spontaneously and a nonfunctioning ipsilateral kidney. We discuss the possible underlying pathophysiology.

Hypertension in infants is rare and requires a thorough evaluation. The incidence of hypertension in infancy has risen in recent years, reflecting both better monitoring methods and increasingly successful salvage of smaller and smaller newborns. Overall mortality and morbidity rates for uncontrolled hypertension in infants are unknown. With appropriate treatment, the prognosis for resolution of hypertension is good. In most cases, hypertension is short-lived and blood pressures return to normal even when medication is discontinued. Recent experience with improved antihypertensive agents in in

Systolic blood pressure was measured using the Doppler technique in 149 infants who attended the neonatal follow-up clinic. One hundred and six of these infants were graduates of the neonatal intensive care unit and 81% were prematurely delivered. Most of the participants were examined only once and their postnatal age ranged between 2-191 weeks at the time the blood pressure was measured. During the first weeks of life the blood pressure in infants whose birth weight was > 2000 grams appeared to be higher than that of infants of lower birth weight infants (< 2000 grams), but the difference di
