Neonatal hypertension. Incidence and risk factors.

To determine the incidence and risk factors for neonatal hypertension we studied the entire population of 3179 infants admitted to our neonatal intensive care unit over a 6-year period. We report a 0.81% (26/3179) incidence of hypertension in this population. In 13 patients (50%) the hypertension was renal in origin. In four patients (15.4%), the etiology of hypertension could not be determined. Significant risk factors for neonatal hypertension included: bronchopulmonary dysplasia (5.9% in affected infants v 0.5% in unaffected infants, P less than .001); patent ductus arteriosus (3.07% in aff

[Symptomatology and treatment of neonatal hypertension].

A newborn child of a diabetic mother is described, who developed neonatal hypertension after birth. Clinical signs were not specific and the diagnosis would have been missed if blood pressure had not been measured. The cause of the neonatal hypertension appeared to be a thrombus in the left renal artery, probably originating from the ductus arteriosus Botalli. Control of the neonatal hypertension according to a stepwise treatment regime was very difficult. Based on our experience and on study of the literature it is advised to start treatment in the acute stage with nifedipine orally or with s

Treatment of neonatal renovascular hypertension with intravenous enalapril.

Hypertension in the neonate is commonly related to renal disease. More specifically, renovascular hypertension in neonates is often associated with the placement of a umbilical artery catheter. Many medications, including angiotensin-converting enzyme (ACE) inhibitors, have been used in the treatment of neonatal hypertension. Captopril, an ACE inhibitor, is an oral agent that is effective in renovascular hypertension. We describe the use of intravenous enalapril, another ACE inhibitor, in successfully treating severe renovascular hypertension refractory to standard medical therapy in two term

Neonatal hypertension and thrombosis of the ductus arteriosus.

In a case of neonatal hypertension, radiographic findings showed multiple thromboembolism from the ductus arteriosis diverticulum. Hypertension was well controlled by captopril during the neonatal period. The long-term follow-up of 8 years with normal cardiovascular and renal function indicates a good prognosis for acquired renovascular neonatal hypertension.

Neonatal hypertension and cardiomegaly associated with a congenital neuroblastoma.

A neonate who died 12 days after birth from complications related to a congenital neuroblastoma is described. Hypertension and congestive heart failure occurred soon after birth. Hospital course was marked by a consumptive coagulopathy and the development of acute renal and hepatic failure. At autopsy the heart was hypertrophied but normally formed. Although there was elevated urinary excretion of vanilmandelic acid and homovanillic acid, levels of epinephrine, norepinephrine, metanephrine, and normetanephrine were not documented. This case shows that a congenital neuroblastoma may be associat

Neurologic complications of captopril treatment of neonatal hypertension.

The occurrence of neurologic abnormalities is described in a series of nine infants with chronic hypertension, in whom antihypertensive therapy decreased BP markedly and for a prolonged period, although to levels often within the normal range. All infants had mean systolic BPs greater than 113 mm Hg and elevated renin values to a mean of 134 +/- 128 ng/mL/h. Antihypertensive therapy, such as captopril, an inhibitor of angiotensin I-converting enzyme, consistently lowered the systolic BP by 20% from baseline per dose. However, the nine infants exhibited a total of 17 episodes of striking decrea

Arterial hypertension in the newborn infant.

Blood pressure is low at birth. It increases with age by about 1 mm Hg per day within the period of 3-8 days. It rises by about 1 mm Hg per week between 5 and 6 weeks of age. Neonatal hypertension carries a risk of cardiorespiratory failure and cerebral distress. Causes of neonatal hypertension are either secondary to congenital malformations or to acquired disease states. Congenital etiologies include: renal artery stenosis, renal artery hypoplasia, abdominal aortic atresia, coarctation of the aorta, kidney cystic disease, reflux nephropathies. Acquired causes include: thrombo-embolic renal a

Imaging techniques in the evaluation of pediatric hypertension.

Moderate or severe hypertension occurs in a small percentage of hypertensive children, but it is within this group that the surgically correctable causes of hypertension are found. Since cure rates up to 90% have been reported, it is important to diagnose a secondary cause of hypertension. Excretory urography is recommended to screen for renovascular hypertension and renal parenchymal disease. Renal scintigraphy can be substituted for the urogram, but the anatomical resolution is poorer. If renovascular hypertension is suggested by abnormal results of screening examination, arteriography shoul

Epidemiology and management of severe symptomatic neonatal hypertension.

Symptomatic neonatal hypertension, defined as a mean arterial blood pressure (MAP) greater than 70 mm Hg for 3 separate determinations, was observed in 2% of all admissions (20 of 988) to the intensive and intermediate care nurseries. Evidence of renal dysfunction occurred in 85% of these infants, including increased plasma renin, abnormal renal scintiscans, and pathologic evidence of renal vascular embolism or thrombus in 13 of 20. Cardiac complications were present in 55% of affected patients, and features of hypertensive retinopathy were noted in 53% of those examined. Medical management du

Hypertension in the first month of life.

We reviewed our 10-year experience with neonatal hypertension. Fifty-three cases were identified, which represented 0.7% of all neonatal tertiary care admissions. Causes were identified in 23 (43%) neonates. These included acute tubular necrosis (n = 7), renal vascular abnormalities (n = 8), renal structural abnormalities (n = 4), interstitial nephritis (n = 2), and coarctation of the aorta (n = 2). No cause was identified in 30 (57%) infants. If the two neonates with coarctation are excluded, infants who had normal urinalyses, blood urea nitrogen (BUN), serum creatinine and plasma renin activ

Neonatal hypertension from a unilateral multicystic dysplastic kidney.

Multicystic renal dysplasia is an extremely uncommon cause of hypertension in children and the few reported cases have not been of newborns. We report on a neonate in whom severe hypertension associated with elevated peripheral plasma renin resulted from a unilateral multicystic dysplastic kidney. Hypertension and plasma renin activity normalized after unilateral nephrectomy. No evidence of perfusion or excretory renal function in the dysplastic kidney was present on a radionuclide renal scan. This child demonstrates a relationship between hypertension and unilateral multicystic renal dysplasi

Hypertensive retinopathy in the newborn infant.

Hypertension (blood pressure greater than 70 mm Hg on three days) was noted in 1.2% of all neonatal admissions to the intensive and intermediate care nurseries. During this investigation of neonatal hypertension, a retinopathy identical to that seen in hypertensive adults was noted. Indirect ophthalmoscopy was performed in 21 neonates with elevated blood pressure. Eleven of these patients demonstrated some or all of the following abnormalities: increased ratio of venous to arterial caliber, vascular tortuosity (including arteriovenous crossing changes), superficial and deep hemorrhages, and ex
