Familial Alveolar Capillary Dysplasia With Misalignment of Pulmonary Veins Caused by Paternal FOXF1 Upstream Enhancer Deletion: A Case Report.

Alveolar capillary dysplasia with misalignment of pulmonary veins (ACDMPV) is a lethal disease characterized by severe respiratory distress and pulmonary hypertension during the neonatal period, which is seldom diagnosed prenatally. Ten percent of ACDMPV cases are familial and caused by single nucleotide or copy-number variants in FOXF1 or the FOXF1 enhancer in an autosomal dominant manner. Here, we report a familial case of paternal FOXF1 upstream enhancer deletion, detected as somatic mosaicism in his blood specimen. A 37- and 35-year-old couple had four children. The first, third, and fourt

Decreased Level of TMEM100 in Neonates With Lethal Lung Developmental Disorders due to Abnormalities in SHH-FOXF1 and TBX4-FGF10 Signaling Pathways.

Lethal lung developmental disorders (LLDDs), histologically classified as alveolar capillary dysplasia with misalignment of pulmonary veins (ACDMPV), congenital alveolar dysplasia (CAD), acinar dysplasia (AcDys), and primary pulmonary hypoplasia (PH), are rare diseases associated with high neonatal mortality due to refractory respiratory failure. Although ACDMPV mostly results from single nucleotide variants (SNVs) or copy-number variants (CNVs) involving FOXF1, AcDys, CAD, and PH are often associated with abnormalities within TBX4 or FGF10. These genes interact in the SHH-FOXF1 and TBX4-FGF10

Circulating Intercellular Adhesion Molecule as a Novel Marker of Pulmonary Hypertension in Newborn.

BACKGROUND: IntercellularAQ1 adhesion molecules (ICAMs) are released from the endothelium in response to shear stress. Their role in persistent pulmonary hypertension (PPHN) remains un-investigated. METHODOLOGY: Sixty neonates (30 PPHN, 30 controls) underwent serum ICAM testing and echocardiography. RESULTS: Right and left ventricular function was depressed in PPHN cases. Tricuspid annular plane systolic excursion was reduced (Cases 8±1 vs. Controls 11±3.5). ICAM was elevated in cases (332±96 vs. 208 ±36). CONCLUSION: ICAM is a sensitive PPHN marker. Elevated ICAM may result from endothelia

Decreased endothelial micro-RNA-30b-5p impairs angiogenesis in fetal lambs with persistent pulmonary hypertension.

BACKGROUND: Impaired angiogenesis contributes to increased pulmonary vascular resistance in persistent pulmonary hypertension of the newborn (PPHN). Notch signaling, critical to lung angiogenesis, is modulated by micro-RNAs (miR). RNA sequencing of pulmonary arterial endothelial cells (PAEC) from a PPHN lamb model revealed downregulated miR-30b-5p. METHODS: We investigated whether decreased levels of miR-30b-5p in PPHN affect angiogenesis by facilitating excess Dll4, a predicted target for miR-30b-5p. We obtained PAEC from a fetal lamb model of PPHN and transfected them with miR-30b-5p mimic

Bronchopulmonary dysplasia with pulmonary hypertension associates with semaphorin signaling loss and functionally decreased FOXF1 expression.

Lung injury in preterm infants leads to structural and functional respiratory deficits, with a risk for bronchopulmonary dysplasia (BPD) that in its most severe form is accompanied by pulmonary hypertension (PH). To identify potential cellular and molecular drivers of BPD in humans, we performed single-cell RNA sequencing of preterm infant lungs with evolving BPD and BPD + PH compared to term infants. Examination of endothelial cells reveals a unique, aberrant capillary cell-state in BPD + PH defined by ANKRD1 expression. Within the alveolar parenchyma in infants with BPD/BPD + PH, predictive

Genetic features of alveolar capillary dysplasia with misalignment of pulmonary veins in Japanese infants.

BACKGROUND: The genetic features of alveolar capillary dysplasia with misalignment of pulmonary veins (ACDMPV) among infants in Japan have not been extensively evaluated. METHODS: This study enrolled sixteen infants with pathologically confirmed ACDMPV. DNA was extracted from the peripheral blood cells of the infants for genetic analysis. The samples from tree infants were not viable for analysis, so only 13 were analyzed. Sanger sequencing or next-generation sequencing of the FOXF1 coding exon was performed on the samples. When causative variations were not identified, the multiplex ligation

Transcription factor EB improves hypoxic pulmonary hypertension in fetal rats by suppressing NLRP3 inflammasome activation via induction of mitophagy.

Persistent pulmonary hypertension of the newborn (PPHN) represents a life-threatening cardiopulmonary condition characterized by hypoxia-driven pulmonary vascular remodeling. While transcription factor EB (TFEB), a master regulator of cellular adaptation to hypoxia, has been implicated in vascular pathologies, its mechanistic role in PPHN remains undefined. This study elucidates the molecular interplay of TFEB in hypoxia-induced PPHN pathogenesis. Fetal rat models of hypoxia-induced PPHN, including untreated hypoxic models and hypoxic models treated with the TFEB inhibitor Eltrombopag (EO), as

Association between nitric oxide treatment and intraventricular hemorrhage among extremely preterm neonates in Switzerland, 2012-2020.

Inhaled nitric oxide (iNO) treatment is routinely used to treat newborns suffering from severe respiratory distress syndrome (RDS). While the treatment is established for term born infants, its safety and efficacy remain controversial among preterm born infants. A 2017 meta-analysis focusing on this population reported a nonsignificant increase in the occurrence of severe intraventricular hemorrhage (sIVH) if infants were treated with iNO. sIVH is a dreaded complication of extreme premature birth that impairs long-term neurodevelopment. In this study, we hypothesize that iNO treatment is assoc

Nebulized nitroglycerin as an adjuvant drug in management of persistent pulmonary hypertension of newborns: a randomized controlled trial.

The objective was to evaluate the effect of nebulized nitroglycerin (NNG) in neonates with persistent pulmonary hypertension (PPHN). The study focused on assessing changes in echocardiographic and clinical parameters following its administration. This is a randomized controlled trial that included 80 full-term newborns diagnosed with PPHN within 72 h after birth. Participants were randomized into two groups: the NNG group received nebulized nitroglycerin, while the control group did not receive it as adjunctive therapy to PPHN. Clinical parameters and echocardiographic measures were evaluated

From pulmonary to ocular protection: rethinking inhaled nitric oxide in preterm infants with pulmonary hypertension.

Persistent pulmonary hypertension of the newborn (PPHN) and retinopathy of prematurity (ROP) are traditionally regarded as distinct morbidities in preterm infants, yet both share common antecedents such as oxygen instability, hyperoxia, and inflammation. Inhaled nitric oxide (iNO) is established as first-line therapy for PPHN, but its broader impact on extra-pulmonary vascular development has remained uncertain. In a nationwide U.S. study, Cho et al. reported that PPHN was independently associated with an increased risk of ROP, and that iNO administration in infants with PPHN was linked to a r

Downregulation of KLF2 impairs pulmonary endothelial function and promotes persistent pulmonary hypertension of the newborn.

Persistent pulmonary hypertension of the newborn (PPHN) is a severe and progressive disorder with limited therapeutic options. Dysfunction of pulmonary artery endothelial cells (PAECs) underpin the pathogenesis and progression of PPHN, but the precise mechanisms remain poorly defined. Endothelial Kruppel-like factor 2 (KLF2) is a key transcription factor required for vascular homeostasis. Here, we investigated the role of KLF2 in PAEC function in PPHN. We found reduced expression of KLF2 in infants with PPHN. Silencing of KLF2 in PAECs led to dysregulated secretion of vasoactive mediators, cha

Pulmonary comorbidities and response to surfactant in late preterm infants: a multicenter cohort study.

BACKGROUND: Surfactant (SRT) therapy is well-established for respiratory distress syndrome (RDS) in very preterm infants, but its effectiveness in late preterms (≥ 34 weeks' gestation) is less defined. We evaluated post-SRT oxygenation responses in late preterm infants in relation to pulmonary comorbidities. METHODS: In this multicenter prospective cohort, 350 late preterm infants received SRT according to European RDS guidelines. Pulmonary comorbidities-including persistent pulmonary hypertension (PPHN), meconium aspiration, air leaks, pulmonary hemorrhage, and pneumonia-were documented. Oxy

Alveolar capillary dysplasia in a neonate: emphasising the importance of early recognition and biopsy in refractory pulmonary hypertension.

Alveolar capillary dysplasia with misalignment of pulmonary veins (ACD/MPV) is a rare congenital condition characterised by abnormal development of the pulmonary vasculature. It manifests as severe, refractory hypoxaemia and persistent pulmonary hypertension of the newborn (PPHN), often resulting in rapid progression to respiratory failure and early death despite aggressive interventions. Diagnosis relies on distinct histological features. We present the case of a term neonate born to parents with a history of third-degree consanguinity. The mother, a primigravida, delivered a male neonate vag

Prenatal Rare 16q24.1 Deletion Between Genomics and Epigenetics: A Review.

Alveolar capillary dysplasia with misalignment of pulmonary veins (ACDMPV) is a rare, often fatal congenital disorder characterized by severe neonatal respiratory distress and associated with complex multisystem malformations. In approximately 90% of cases, the condition is linked to deletions or mutations affecting the FOXF1 gene or its upstream enhancer region on chromosome 16q24.1. This review analyzes reported prenatal cases with 16q24.1 deletion involving FOXF1, aiming to identify recurrent sonographic features and elucidate the underlying genomic and epigenetic mechanisms. We reviewed pr

Rapid genome diagnosis of alveolar capillary dysplasia leading to treatment in a child with respiratory and cardiac failure.

Alveolar capillary dysplasia (ACD) is a fatal disorder that typically presents in the neonatal period with refractory hypoxemia and pulmonary hypertension. Lung biopsy is traditionally required to establish the diagnosis. We report a 22-mo-old male who presented with anemia, severe pulmonary hypertension, and right heart failure. He had a complicated hospital course resulting in cardiac arrest and requirement for extracorporeal membrane oxygenation. Computed tomography of the chest showed a heterogenous pattern of interlobular septal thickening and pulmonary edema. The etiology of his conditio
