Improvement of pregnancy outcome following primary CMV infection after the introduction of valacyclovir in the clinical practice in Italy, updated data from a large real-life multicenter study.

BACKGROUND: Congenital cytomegalovirus infection (cCMV) is a leading cause of neurodevelopmental impairment and congenital sensorineural hearing loss (SNHL). Since December 2020, valacyclovir (VCV) is reimbursed by the Italian National Health System to reduce maternal-fetal CMV transmission following primary CMV maternal infection. OBJECTIVE: The aim of this study is to evaluate the effectiveness of VCV in preventing fetal CMV infection, termination of pregnancy, miscarriage, congenital CMV infection at birth and presence of signs/symptoms of cCMV in newborns. STUDY DESIGN: The current study

Maternal Cytomegalovirus Serology Results by Geographic Region in a Large Multicenter US Study.

Congenital cytomegalovirus (CMV) infection, due to both nonprimary and primary maternal infection, affects nearly 1% of neonates. In conducting a randomized trial of hyperimmune globulin for prevention of congenital CMV infection, we screened over 200,000 pregnancies across the United States for evidence of prior or primary CMV infection and evaluated their geographic variation. Although the majority were seropositive, seroprevalence and primary infection risk varied significantly by geographic region, with seroprevalence the highest (88%) and primary infection (0.2%) the lowest in the Southwe

Changes in birth certificate-reported prevalence of congenital anomalies at live birth during the COVID-19 pandemic: a nationwide study of 26 million United States live births.

BACKGROUND: Although the majority of studies have indicated that maternal severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection and coronavirus disease 2019 (COVID-19) vaccination are not associated with neonatal birth defects, it is relevant to examine whether the disease burden changed during the COVID-19 pandemic, as other influencing factors may have also changed. We aimed to compare the birth certificate-reported prevalence of congenital anomalies at live birth before and during the pandemic. METHODS: Using public birth data, all live births from 2016 to 2022 in the Unit

Temporal trends in the clinical presentation, management, and outcomes of children with congenital cytomegalovirus infection.

UNLABELLED: Congenital cytomegalovirus (cCMV) is the most common congenital infection and a leading cause of sensorineural hearing loss as well as neurodevelopmental impairment. In recent years, awareness, screening, and management strategies for cCMV have evolved. This study aimed to assess temporal changes in the clinical presentation, treatment, and outcomes of children diagnosed with cCMV infection. We conducted a retrospective cohort analysis using data from the CCMVnet registry ( www.ccmvnet.org ), an international multicenter database collecting anonymized data on children with confirme

Chorioretinal Scars in Congenital Cytomegalovirus Infection Following Prenatal Valacyclovir Therapy.

BACKGROUND: Congenital cytomegalovirus infection is the most common congenital infection. Ocular abnormalities affect approximately 14% of symptomatic children, but fewer than 1% of asymptomatic children. Prenatal valacyclovir reduces vertical transmission, but ophthalmic outcomes in exposed infants are poorly described. METHODS: We describe 2 neonates with congenital cytomegalovirus confirmed by amniotic fluid and urine polymerase chain reaction following first-trimester maternal primary infection. Both mothers received high-dose valacyclovir (8 g daily) from 13 weeks' gestation. Infants und

Congenital Cytomegalovirus Testing Following Pennsylvania Hearing-Targeted Screening Legislation.

BACKGROUND: Congenital cytomegalovirus infection (cCMV) occurs in approximately 1/200 to 1/250 US newborns each year and can cause progressive sensorineural hearing loss. Since 2022, Pennsylvania law mandates cCMV testing for infants who fail newborn hearing screening. OBJECTIVES: To assess changes in cCMV testing rates and diagnosis before and after Pennsylvania hearing-targeted legislation. METHODS: Retrospective cohort study of all live births at 3 university-affiliated birth centers. We identified all infants tested for CMV at 21 days of age or younger and analyzed cCMV testing rates in

Zika exposure during pregnancy and the risk of hospitalisation and death due to community-acquired pneumonia in early childhood: a nationwide registry-based cohort study in Brazil.

BACKGROUND: Childhood pneumonia remains a leading cause of morbidity and mortality worldwide. Prenatal Zika virus (ZIKV) infection has been associated with severe outcomes such as Congenital Zika Syndrome (CZS), linked to excess hospitalisations. However, the long-term respiratory consequences of prenatal ZIKV exposure across the full clinical spectrum, including children without CZS, remain poorly understood. We aimed to examine the association between prenatal ZIKV exposure and community-acquired pneumonia-related hospitalisation or death during childhood. METHODS: We conducted a population

Prenatal Diagnosis and Obstetrical Challenges of a Massive Fetal Ascites Affecting One Fetus in a Dichorionic Diamniotic Twin Pregnancy: A Case Report.

Fetal ascites is a rare prenatal condition; it may result from a wide range of underlying etiologies, including structural anomalies, genetic disorders, congenital infections, and metabolic diseases. Despite considerable advances in prenatal investigations, etiology remains unidentified in some cases. Massive fetal ascites may also complicate obstetric management by distorting fetal biometric measurements and influencing delivery planning. We report the case of a 34-year-old woman with a spontaneously conceived dichorionic diamniotic twin pregnancy. At 24 weeks of gestation, massive fetal asci

Rubella and congenital rubella syndrome in the GCC countries: A retrospective ecological analysis of elimination progress and scenario-based projections, 2020-2024.

INTRODUCTION: Rubella and congenital rubella syndrome (CRS) remain vaccine-preventable priorities. Although the Gulf Cooperation Council (GCC) countries maintain high routine rubella-containing vaccine (RCV) coverage and well-established immunisation programmes, the region is also characterised by large migrant populations, intense international mobility, and complex population immunity profiles that may sustain vulnerability to rubella transmission even in high-coverage settings. This study assessed progress toward rubella and CRS elimination in the GCC during 2020-2024 and examined the polic

Wesselsbron virus at the livestock-human interface: A practical guide to disease recognition and diagnosis.

Wesselsbron virus (WSLV) is a mosquito-borne Orthoflavivirus of the yellow fever virus (YFV) serocomplex, widely distributed across sub-Saharan Africa. It is neglected, and its true impact is likely underestimated. WSLV causes abortion, congenital malformations, and neonatal mortality in sheep, the primary recognized vertebrate host, but infections have also been documented in goats, cattle, camels, wildlife, and humans. The virus is maintained in complex cycles predominantly involving Aedes mosquitoes and incompletely defined domestic and wild vertebrate hosts. Direct infection via the placen

Pediatric hearing loss in the universal newborn hearing screening era: etiologies, risk indicators, and clinical phenotypes.

Childhood hearing loss poses significant developmental and public health challenges, impacting language acquisition, cognitive development, social participation, and educational outcomes. Universal newborn hearing screening (UNHS) has notably enhanced the early identification of congenital hearing loss, yet limitations persist. Mild, unilateral, delayed-onset, progressive, and certain neural forms of hearing loss may remain undetected during a single neonatal screening session. This review consolidates current evidence regarding the etiologies, risk indicators, and clinical features of pediatr

Anti-Rubella antibodies seroprevalence among pregnant women in South-Western Romania.

INTRODUCTION: Rubella infection during pregnancy poses significant risks, including congenital rubella syndrome (CRS), miscarriage, and fetal death. Vaccination programs have reduced rubella incidence globally, but regional differences in coverage and immunity persist. The objective of the study was to assess the seroprevalence of anti-rubella IgM and IgG antibodies among pregnant women in South-Western Romania, with a focus on temporal trends, demographic factors, and the impact of vaccination eligibility. METHODS: This observational, cross-sectional study included pregnant women screened fo

Zika virus and host innate immunity: mechanisms of activation and evasion.

Zika virus (ZIKV) is an Orthoflavivirus that poses a significant public health threat due to its unique capacity to cause congenital malformations, neurological disorders, and reproductive complications. Upon infection, host cells sense viral RNA and danger signals through pattern recognition receptors, including Toll-like receptors, RIG-I-like receptors, inflammasomes and the cGAS-STING pathway, leading to the production of type I interferons and the subsequent expression of interferon-stimulated genes. Additional intrinsic restriction mechanisms, such as apoptosis, autophagy, RNA interferenc

Neuroimaging Abnormalities in Congenital Cytomegalovirus Infection: A Systematic Review and Pooled Descriptive Analysis.

Congenital cytomegalovirus (cCMV) is the most common congenital viral infection and an important cause of neurological morbidity, yet reported neuroimaging findings vary across studies and imaging modalities. We performed a systematic review with pooled descriptive analysis to characterize neuroimaging abnormalities reported in cCMV. PubMed, Scopus, and Embase were searched for English-language, peer-reviewed studies published from January 1, 2013, through December 1, 2024, that reported extractable prenatal or postnatal neuroimaging data in confirmed cCMV. Studies using magnetic resonance ima

Prevalence of Congenital Cytomegalovirus Infection in a CMV-Endemic Ugandan Population.

Congenital cytomegalovirus (cCMV) infection is a leading cause of childhood deafness and disability worldwide. However, less is known about its epidemiology in low-income regions where most women are CMV-seropositive at conception. We found that cCMV is highly prevalent in rural Uganda (3.73%) and is associated with lower gravidity and placental malaria. We additionally report a high transmission intensity in the first years of life, with 97.3% of children acquiring CMV IgG by age 3.
