Differentiating disseminated cryptococcosis from pulmonary cryptococcosis: Roles of clinical characteristics and cryptococcal antigen.

AbstractDistinguishing disseminated cryptococcosis (DC) from localized pulmonary cryptococcosis (PC) is crucial due to divergent management and disparate prognoses. This study aimed to recognize associated factors for DC from PC in non-human immunodeficiency virus (HIV) patients. Accordingly, 475 pathologically confirmed cryptococcosis cases (391 PC and 84 DC) were retrospectively reviewed over a 20-year period at a tertiary hospital in China. Clinical, radiographic, dynamic serum cryptococcal antigen (CrAg) titer profiles, treatment and prognosis were analyzed. Patients with DC were significa

Diagnosis and treatment of four cases of pulmonary cryptococcosis: case report and literature review.

INTRODUCTION: Cryptococcosis has attracted global attention due to its high mortality rate, unique geographical distribution, and tendency to cause severe and rapid disease progression. Metagenomic next-generation sequencing (mNGS) is a novel detection technology that provides diagnostic insights for identifying pathogens, particularly rare ones. The present article describes 4 individuals diagnosed with pulmonary cryptococcosis (PC) using mNGS of bronchoalveolar lavage fluid (BALF). CASE PRESENTATION: Among these patients, 3 had cancer, 1 had sarcoidosis and diabetes, and 2 were exposed to p

Pulmonary cryptococcosis mimicking metastatic progression despite systemic disease stability in advanced colon cancer: A case report and literature review.

RATIONALE: In patients with advanced cancer, newly detected pulmonary nodules are often interpreted as metastatic progression. The distinctive feature of this case was the appearance of multiple new lung nodules during otherwise controlled systemic disease: the primary colonic lesion and hepatic metastases remained stable while tumor marker levels declined. This radiologic-clinical discordance prompted reconsideration of metastatic progression and broadened the differential diagnosis. PATIENT CONCERNS: A patient with advanced colon cancer, who was undergoing systemic chemotherapy and had othe

Pulmonary Cryptococcosis in an Immunocompetent Patient: Negative mNGS but Positive Antigen Test.

BACKGROUND: Pulmonary cryptococcosis is common in patients with immunodeficiency, application of immunosuppressive and cytotoxic drugs. Its incidence has increased in recent years, with a greater number of patients with no underlying disease or history of exposure, and a greater number of immunocompetent patients. Its clinical presentation lacks specificity and imaging features are atypical, making it easy to misdiagnose or miss the diagnosis. METHODS: Diagnosis was confirmed by serum cryptococcal antigen after negative bronchoalveolar lavage fluid mNGS in an immunocompetent patient with atyp

Serum cryptococcal antigen gradient detection for monitoring the efficacy and medical guidance of the treatment of pulmonary cryptococcosis: a case report.

BACKGROUND: The duration of drug maintenance therapy for pulmonary cryptococcosis (PC) has been controversial, and good monitoring methods for drug efficacy are lacking. CASE PRESENTATION: An immunocompetent patient diagnosed with PC was admitted in 2020. He had no history of immune deficiency but reported being extremely tired at the time and possibly exposed to pigeon dropping. The diagnosis was confirmed through serum cryptococcal antigen (CrAg) testing and pulmonary high-resolution computed tomography (HRCT) imaging, and the patient received oral fluconazole therapy immediately. During the

Rethinking Routine Lumbar Puncture and Induction Therapy in Non-HIV Pulmonary Cryptococcosis: A Call for Risk-Based Management.

Current guidelines recommend lumbar puncture (LP) in patients with pulmonary cryptococcosis (PC) to exclude central nervous system (CNS) involvement. However, recent multicenter data suggest that CNS dissemination is rare in immunocompetent, asymptomatic patients with low serum CrAg titers. And, although guidelines advocate induction therapy for severe PC, most patients in practice receive fluconazole monotherapy with favorable outcomes. This Perspective evaluates the gap between guideline-based universal intervention and real-world clinical practices, advocating for a risk-stratified manageme

Two cases of refractory Cryptococcus gatti (C gatti) infection and literature review: Case report.

RATIONALE: The incidence of Cryptococcus gatti (C gatti) infection has been increasing, posing significant challenges in clinical diagnosis and treatment due to its high virulence, refractoriness, and mortality. PATIENT CONCERNS: Both patients had no immune function defects. One of the cryptococcal drug sensitivity results suggested resistance to fluconazole, which ultimately required surgical intervention. Two patients had recurrent symptoms and worsened examination results during antifungal treatment, and the course of anti-infection was more than 6 months. DIAGNOSES: Pulmonary cryptococco

Intralobar pulmonary sequestration coexisting with pulmonary cryptococcosis: A case report and literature review.

Pulmonary sequestration (PS) is a rare congenital foregut abnormality characterized by non-functioning lung tissue that receives an arterial blood supply from an anomalous systemic artery, typically the aorta, and is separated from the normal tracheobronchial tree. It often presents as a mass-like, cystic, cavitary or pneumonic lesion on imaging. Patients often present with symptoms of recurrent pulmonary infection; however, cryptococcal infection is rare. The present case study reports on a 52-year-old man with PS and cryptococcal infection. The mass in the lower lobe of the right lung was re

[A case of pulmonary cryptococcosis in dust-exposed worker].

Cryptococcus is an opportunistic pathogen, which belongs to yeast-type fungi. It mainly exists in bird droppings, rotten wood and soil. Respiratory tract is the main portal of invasion. It can occur in immunocompromised and immunocompetent hosts. The clinical manifestations, symptoms, signs and imaging of pulmonary cryptococcosis caused by it have no obvious specificity, which may lead to misdiagnosis and mistreatment. This article reported a case of a patient with atelectasis complicated by cryptococcal pneumonia, diagnosed and treated at Qingdao Central Hospital of Health and Rehabilitation

Diagnosis of a patient with pulmonary and skeletal cryptococcosis: A case report and literature review.

Cryptococcosis is an invasive mycosis and can disseminate an infection in immunocompromised hosts. The main sites of infection include the central nervous system and lungs; skeletal infections are rarely reported. We describe the case of a Chinese man in his early 40s with a 1-year history of dermatomyositis and prolonged use of corticosteroids and immunosuppressants. During re-examination of a previously discovered pulmonary nodule, lung computed tomography revealed that the nodule had grown in size, and a bone scan revealed local bone abnormalities and mass formation in the left scapula. Pulmonary Cryptococcosis Mimicking Lung Metastasis in a Patient with Pancreatic Cancer: A Case Report and Review of the Literature.

Pulmonary cryptococcosis is a fungal infection that mimics metastatic lung tumors. A man with pancreatic head cancer and liver metastases harboring a germline BRCA2 mutation received FOLFOX chemotherapy. Despite a favorable response from the primary tumor and a decline in the CA19-9 levels, a rapidly enlarging solitary pulmonary nodule appeared. Because of the discordance between the response of the primary tumor and the lung nodule, thoracoscopic partial lung resection was performed for a definitive diagnosis, which confirmed pulmonary cryptococcosis. Postoperatively, fluconazole was administ

Recurrent Pulmonary Nodules, from Pulmonary Cryptococcosis to ANCAAssociated Vasculitis: A Case Report.

INTRODUCTION: On chest Computed Tomography (CT), pulmonary cryptococcosis and antineutrophil cytoplasmic antibody (ANCA)-associated vasculitis (AAV) can both present as multiple non-cavitating pulmonary nodules. As a result, when such nodules worsen during antifungal therapy in a patient with pulmonary cryptococcosis, it is difficult to determine whether this represents progression of the infection or the development of concomitant AAV, posing a significant diagnostic challenge. CASE PRESENTATION: A 67-year-old man presented with multiple non-cavitating pulmonary nodules and was diagnosed wit

[Pulmonary cryptococcosis in a non-HIV patient].

Cryptococcosis is an opportunistic and conditional disease. The lungs and central nervous system are common organs invaded by Cryptococcus. Most patients with pulmonary cryptococcosis are immunocompromised, such as acquired immune deficiency síndrome, subjects with immunosupressive drugs and hematological malignancies. We present an immunocompetent 46-year-old man with diabetes mellitus, who had cough, chest pain and weight loss. Computed tomography of thorax demonstrated a pulmonary nodule in the right upper lobe, measuring 8.5 cm. To confirm the diagnosis, the patient underwent CT-guided bio

Fluconazole-induced liver injury in patients with pulmonary cryptococcosis: a comprehensive study integrating clinical cohort analysis, network toxicology, molecular docking, and transcriptomics.

Fluconazole is key for pulmonary cryptococcosis (PC), but its hepatotoxicity (HT) risk is unclear. This study characterized fluconazole-induced liver injury in HIV-negative PC patients and explored its mechanism. We integrated clinical cohort analysis (n = 123), network toxicology, molecular docking, and transcriptomics. HT incidence was 23.6%, predominantly cholestatic (55.2%). Immunocompromised status independently increased HT risk (aRR = 2.67, P = 0.004). Baseline ALT >16.5 U/L was the best predictor (AUC = 0.69) among liver enzymes, which showed modest discriminatory value. A model combin
