BACKGROUND: Pulmonary cryptococcosis (PC) is an invasive fungal infection with rising incidence in immunocompetent populations. The differential clinical and imaging characteristics between immunocompromised (ICP) and non-immunocompromised (NICP) patients remain incompletely characterized, limiting precision in diagnosis and management.
METHOD: A retrospective study was conducted on 117 patients with confirmed PC from a single tertiary center. Patients were stratified into ICP and NICP groups based on refined criteria. Clinical symptoms, laboratory findings, and chest CT imaging features were

BACKGROUND: Pulmonary cryptococcosis (PC) is increasingly diagnosed in human immunodeficiency virus (HIV)-negative and apparently immunocompetent individuals. A critical complication of PC is dissemination to the central nervous system (CNS), leading to concurrent cryptococcal meningitis (CM). Identifying CNS involvement remains a clinical challenge due to insidious presentations and non-specific radiological findings. This study aimed to identify independent clinical and laboratory predictors of concurrent CM in patients presenting with PC. METHODS: We conducted a retrospective study of patie

Cryptococcus neoformans is responsible for pulmonary cryptococcosis and life-threatening cryptococcal meningitis, particularly in immunocompromised individuals. Despite its high mortality rate and the diagnostic challenges, current antifungal therapies are hampered by limitations such as side effects and the emergence of drug resistance. A critical virulence factor of C. neoformans is its ability to form titan cells, enlarged polyploid cells that promote immune evasion and dissemination. However, therapeutic interventions specifically targeting titan cell formation remain unexplored. In this s

Introduction. Pulmonary cryptococcosis is increasingly identified in routine care, including among patients with no recorded immunocompromising conditions.Hypothesis/Gap Statement. Real-world descriptions of pulmonary cryptococcosis are difficult to interpret because diagnostic intensity and case definitions vary across studies, and many cohorts reflect pathway-based rather than population-based case capture.Aim. This study aimed to describe the clinical presentation, including asymptomatic/incidental cases, and chest computed tomography (CT) patterns of pulmonary cryptococcosis by two-level i

Sarcoidosis is a multisystem granulomatous disorder associated with immune dysregulation and impaired cell-mediated immunity, which may increase susceptibility to opportunistic infections, including cryptococcosis. The clinical and radiological overlap between sarcoidosis progression, neurosarcoidosis, and cryptococcal infection can create a significant diagnostic challenge, particularly when neurological symptoms are present. We present a 50-year-old male with a 15-year history of pulmonary sarcoidosis and CD4 lymphopenia who developed pulmonary cryptococcosis with subsequent symptoms concern

RATIONALE: Behçet's disease (BD) is a systemic vasculitis that rarely involves the lungs. Pulmonary manifestations of BD can radiographically mimic infectious pneumonia, posing a substantial diagnostic challenge and increasing the risk of inappropriate treatment. This case report describes a young female patient with BD who presented with pulmonary opacities initially misdiagnosed as cryptococcal pneumonia based on imaging findings.
PATIENT CONCERNS: A 28-year-old woman with a known history of BD presented with a 1-month history of fever, cough, and productive yellow sputum. She had been on l

UNLABELLED: Methotrexate (MTX) is a class of antifolate chemotherapeutics and immunosuppressants that are effectively used in the treatment regimen of cancer and autoimmune diseases. In the current MASLD era, methotrexate-induced hepatotoxicity must be understood in the context of host metabolic dysfunction, obesity, diabetes, and alcohol use that amplify oxidative and inflammatory liver injury. But its administration is being restricted due to hepatic toxicity, which increases asymptomatic in transaminases and progresses to fibrosis. This review examines the complex mechanisms underlying MTX-

BACKGROUND: Increasing case reports and monocentric studies suggest that co-infections may be associated with poorer outcomes in patients with pulmonary cryptococcosis (PC), including both people living with HIV (PLWH) and HIV-negative individuals. However, systematic cohort-based evidence evaluating the prognostic impact of co-infections remains limited.
METHODS: Total of 454 patients with PC (239 PLWH and 215 non-HIV) were included to compare clinical characteristics, CT imaging features, and relevant prognostic factors according to their co-infection status.
RESULTS: Among non-HIV patient

INTRODUCTION: Myxoid liposarcoma is a soft tissue sarcoma with a high tendency for lung metastasis. Its aggressive nature necessitates accurate diagnosis and treatment planning to improve outcomes.
CASE REPORT: A 31-year-old man presented with a three-year history of an asymptomatic, progressively enlarging mass in his right thigh. Magnetic resonance imaging indicated a soft tissue tumor within the sartorius muscle, and an incisional biopsy suggested myxoid liposarcoma. Staging computed tomography scans revealed two nodules in the right lung, leading to an initial diagnosis of Stage IV myxoid

Cryptococcosis is an infectious disease caused by Cryptococcus neoformans and is most often found in the lungs, followed by the meninges. However, little is known about the mechanism by which cryptococcal meningitis develops without pulmonary cryptococcosis. A 78-year-old woman visited our hospital due to decreased appetite, difficulty moving, and fever. She had right-sided hemiparesis due to an old cerebral infarction, headache, no nuchal rigidity, a National Institutes of Health Stroke Scale of 4, and no pulmonary disorders on computed tomography. On the fourth day after hospitalization for

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

BACKGROUND: Patients with poor prognosis of pulmonary cryptococcosis (PC) are prone to other complications such as meningeal infection, recurrence or even death. Therefore, this study aims to analyze the influencing factors in the poor prognosis of patients with PC, so as to build a predictive nomograph model of poor prognosis of PC, and verify the predictive performance of the model.
METHODS: This retrospective study included 410 patients (78.1%) with improved prognosis of PC and 115 patients (21.9%) with poor prognosis of PC. The 525 patients with PC were randomly divided into the training

We describe pulmonary cryptococcosis in a 28-year-old previously healthy man. Exhaustive immunological investigations revealed a primary NK cell deficiency associated with a secondary impaired anti-Cryptococcus CD8 lymphocyte response and the expansion of a CD8Vβ14 + T cell clone. This case illustrates the potential role of NK cells in immunity against Cryptococcus.

BACKGROUND: Lateral flow assay (LFA) has demonstrated high sensitivity and specificity for diagnosing cryptococcosis. However, its role in predicting therapeutic efficacy for pulmonary cryptococcosis (PC) remains underexplored.
METHODS: We conducted a retrospective analysis of HIV-negative patients with PC to describe the clinical profile and identify potential predictors of radiological prognosis.
RESULTS: All the 168 participants received antifungal therapy with a triazole agent. Of these, 84.5% experienced partial or complete absorption of pulmonary lesions. The results of the gamma test,

Lung-resident memory T cells (lung TRMs) settle in the lung and respond rapidly to external antigens, and are therefore considered to have great potential for development of respiratory vaccines. Here, we demonstrate that lung-resident memory Th2 cells (lung TRM2) protect against pulmonary mycosis caused by Cryptococcus gattii. We developed novel whole-cell intranasal vaccines using a heat-inactivated C.gattii capsule-deficient strain cap59Δ, which induced ST-2+ Gata-3+ lung TRM2 specifically responding to C.gattii whole-cell antigen. Lung fungal burden and survival rate were significantly imp
