Therapeutic role of fluconazole in immunocompetent patients with pulmonary cryptococcosis: A case report.

The present investigation examines the potential for customized treatment of pulmonary cryptococcosis using fluconazole in an immunocompetent individual. Therapeutic drug monitoring (TDM) was utilized throughout treatment to assess fluconazole levels. Research indicated that the MIC90 for cryptococcus in China is around 8 mg/L. We evaluated the area under the curve (AUC) relative to the minimal inhibitory concentration (MIC), focusing on the AUC from 0 to 24 h (AUC0-24 h/MIC). Fluconazole's dosage was adjusted according to pharmacokinetics/pharmacodynamics (PK/PD) principles to enhance therape

A fatal case of pulmonary cryptococcosis in an apparently immunocompetent patient from Cote D' Ivoire.

Cryptococcosis is an opportunistic mycosis caused by Cryptococcus, mainly affecting the immunocompromised. We report a case of pulmonary cryptococcosis within a 31-year-old immunocompetent patient admitted for painful chest swelling evolving for 4 months prior to admission. The chest X-Ray showed a rounded pulmonary opacity. The chest ultrasound revealed a pulmonary abscess diffusing under the skin. Direct examination using the Indian ink and the May-Grunwald Giemsa staining revealed encapsulated yeasts in the pus. The Culture of the pus on the Sabouraud medium with chloramphenicol confirmed t

Atypical Cryptococcal Pneumonia in an Immunocompetent Host: A Case Report and Review of Diagnostic and Therapeutic Challenges.

Cryptococcal infections, commonly associated with immunocompromised hosts, are rare but increasingly recognized in immunocompetent individuals. This case report describes a 39-year-old immunocompetent male presenting with cryptococcal pneumonia, initially misdiagnosed as bacterial pneumonia. The patient's persistent symptoms and diagnostic complexities highlight the importance of including cryptococcal infection in the differential diagnosis of chronic respiratory conditions unresponsive to conventional treatment. Treatment with fluconazole was effective, underscoring its role in managing isol

Diagnostic performance of an in-house real-time polymerase chain reaction (PCR) assay and antigen detection from respiratory samples for the diagnosis of pulmonary cryptococcosis.

INTRODUCTION: The currently available diagnostic tests lack sensitivity to diagnose pulmonary cryptococcosis. In the current study, we developed and standardized an in-house real-time PCR assay and evaluated the antigen detection in respiratory samples for the diagnosis of pulmonary cryptococcosis. MATERIALS AND METHODS: We standardized an in-house real-time PCR assay (using URA5 and STR1 primers; index test 1) and cryptococcal antigen detection (BIOSYNEX® CryptoPS, France) from the respiratory samples (index test 2). We considered a sample positive for PCR assay when both gene targets (URA5

Pulmonary cryptococcosis in immunocompetent children presenting with chest pain: Three case reports.

BACKGROUND: Cryptococcus is a systemic opportunistic pathogenic fungus that can cause infections in both immunocompromised and immunocompetent hosts, with diverse clinical manifestations, ranging from asymptomatic pulmonary lesions to disseminated central nervous system infections. The incidence of pulmonary cryptococcosis (PC) has rapidly increased in recent years, with an increasing proportion of non-human immunodeficiency virus-infected and immunocompetent patients making its diagnosis challenging. If not properly recognized, PC can lead to systemic dissemination and high mortality rates. E

Pulmonary Cryptococcosis During Osimertinib Treatment for Epidermal Growth Factor Receptor (EGFR) L858R-Mutant Lung Adenocarcinoma: A Case Report.

Pulmonary cryptococcosis is a rare but important opportunistic fungal infection that may occur during epidermal growth factor receptor-tyrosine kinase inhibitor (EGFR-TKI) therapy for lung cancer. We describe the case of a woman in her 60s receiving osimertinib for EGFR L858R-mutant lung adenocarcinoma who developed new pulmonary lesions. A CT-guided biopsy confirmed pulmonary cryptococcosis despite negative serum cryptococcal antigen results, with markedly elevated serum β-D-glucan levels, an atypical finding in cryptococcal infections. The patient was also receiving dexamethasone for brain m

Pulmonary cryptococcosis with headache as the first presentation: A case report.

Pulmonary cryptococcosis (PC) is an invasive fungal infection caused by Cryptococcus neoformans or Cryptococcus gattii. Its clinical presentation and radiological findings are often non-specific, making early diagnosis challenging. Herein, the case of a 44-year-old male who presented with dizziness and headache is reported. Initial cranial magnetic resonance imaging and chest computed tomography (plain and contrast-enhanced) suggested lung cancer with brain metastasis. A definitive diagnosis was established only after a lung mass biopsy, followed by a cytological smear and histopathological an

Transcriptomic profiling of lung alveolar macrophages reveals distinct contribution of sterol metabolism in macrophage response to Cryptococcus gattii infection.

Alveolar macrophages are well known as the first responders to pulmonary cryptococcosis; however, their dynamic molecular responses to Cryptococcus gattii infection in vivo remain limited. Here, we investigated the transcriptional profiles of lung alveolar macrophages purified from mice intranasally infected with C. gattii and compared them to those infected with C. neoformans using RNA sequencing analysis. Alveolar macrophages from C. gattii-infected mice exhibited distinct transcriptional alterations, particularly in genes associated with sterol biosynthesis, whereas those from C. neoformans
