Postpartum pulmonary cryptococcosis combined with pulmonary embolism: a case report.

Pulmonary cryptococcosis is an opportunistic infection of the lungs caused by cryptococcus, usually occurring in immunosuppressed patients. Pulmonary embolism is a type of venous thromboembolism. Pregnancy and postpartum are risk factors for pulmonary embolism. To date, no cases of pulmonary cryptococcosis co-occurring with pulmonary embolism in the postpartum period have been documented. This case report describes a 39-year-old female who was diagnosed with HIV-negative pulmonary cryptococcosis with pulmonary embolism 42 days postpartum and experienced improvement in her condition after activ

Metagenomic next-generation sequencing facilitates precision treatment and prognostic improvement in pulmonary cryptococcosis.

BACKGROUND: The early diagnosis of pulmonary cryptococcosis (PC) remains challenging due to the low sensitivity and prolonged turnaround time of conventional diagnostic methods. Despite the broad-spectrum pathogen detection capability of metagenomic next-generation sequencing (mNGS), its clinical utility in the diagnosis and therapeutic management of pulmonary cryptococcosis remains underexplored. METHODS: In this retrospective study, 31 patients diagnosed with Cryptococcus infection through mNGS at The First Affiliated Hospital of Zhengzhou University between July 2023 to March 2025 were inc

Effectiveness of voriconazole therapy for pulmonary cryptococcosis in fluconazole treatment failure: a multicenter real-world study.

INTRODUCTION: Current guidelines recommend fluconazole as first-line therapy for pulmonary cryptococcosis. However, clinical reports indicate that fluconazole treatment fails in some patients. Voriconazole is frequently used in these cases, yet its effectiveness has not been comprehensively studied or validated. METHOD: This multicenter real-world retrospective study enrolled patients with fluconazole treatment failure who were divided into two groups based on subsequent antifungal therapy: continuation of fluconazole or switch to voriconazole. Overall treatment responses were evaluated at pre

Evaluation of a lateral flow immunochromatographic assay for detecting cryptococcal antigens in bronchoalveolar lavage fluid from HIV-negative patients with pulmonary cryptococcosis.

UNLABELLED: Cryptococcal antigen lateral flow immunochromatographic assay (CrAg-LFA) is extensively utilized for the diagnosis of cryptococcosis in serum and cerebrospinal fluid. However, its application in bronchoalveolar lavage fluid (BALF) has been relatively underexplored. This study aimed to assess the diagnostic performance of the CrAg-LFA in BALF from human immunodeficiency virus (HIV)-negative patients with pulmonary cryptococcosis (PC) over an 11-year period in a tertiary hospital in China and to compare it with serum samples. Among 514 clinically suspected PC patients at a tertiary h

Pulmonary cryptococcosis coexisting with lung cancer: a Chinese multicenter retrospective study.

BACKGROUND: Pulmonary cryptococcosis (PC) can appear as nodules or masses on chest imaging, resembling lung cancer. Multiple nodules may be misdiagnosed as metastatic cancer, whereas solitary nodules may be mistaken for lung cancer and removed surgically. Reports of PC coexisting with lung cancer are rare, mostly limited to case reports. This retrospective case series study screened 46 cases of nodular PC coexisting with lung cancer from 22 tertiary hospitals across China to explore the potential imaging manifestations and clinical characteristics of this patient population. METHODS: A total

Riboflavin inhibits growth and reduces virulence of Cryptococcus neoformans in vitro by membrane disruption and excessive accumulation of reactive oxygen species and exhibits efficacy against pulmonary cryptococcosis and meningitis.

The incidences of pulmonary cryptococcosis and meningitis cause significant morbidity and mortality. Effective and affordable drugs for treatment of cryptococcal meningitis are urgently needed. Drug reuse is an effective strategy for the development of new antifungals against Cryptococcus neoformans infection. In this study, riboflavin (RF) significantly inhibited growth of C. neoformans as determined by the broth microdilution and spot dilution methods. Moreover, RF significantly inhibited biofilm formation and reduced virulence (capsule, melanin, and urease). In addition, RF caused cell memb

Effects of novel organoantimony compounds on the fungal pathogen Cryptococcus neoformans.

Introduction. Cryptococcus neoformans is an opportunistic fungal pathogen that causes pulmonary cryptococcosis, or an acute or chronic infection in the lungs, and cryptococcal meningitis, an infection of the brain and spinal column, in immunocompromised individuals. Fungal infections are responsible for ~1.7 million deaths each year. In contrast to antibacterial drugs, the quantity of antifungal drugs capable of combating fungal infections remains low. With high toxicity and increased resistance to antifungals in recent years, the importance of finding new options for antifungal therapy is eve

Rapid respiratory cryptococcosis detection using targeted next-generation sequencing.

Cryptococcus neoformans, a WHO critical-priority and neglected fungal pathogen, causes cryptococcosis primarily via inhalation. Clinical manifestations range from asymptomatic infection to fatal meningitis; suboptimal diagnostics cause delayed detection and high meningitis mortality. This retrospective study evaluated the clinical utility of targeted next-generation sequencing (tNGS) for diagnosing pulmonary cryptococcosis using respiratory specimens, compared to serum cryptococcal antigen (CrAg) testing and fungal culture. Over 38 months, 39 patients with confirmed cryptococcosis were enrolle

Point-of-Need Test of Cryptococcus Utilizing Aggregation-induced Emission-based Lateral Flow Immunoassay.

Cryptococcus is a top-priority fungal pathogen designated by the World Health Organization. It induces pulmonary cryptococcosis and cryptococcal meningitis that are often misdiagnosed or underdiagnosed, owing to inherent limitations of existing detection methodologies, especially in resource-constrained point-of-need (PoN) settings. Although lateral flow immunochromatography (LFIA) is clinically recommended for its rapidity, it frequently lacks adequate sensitivity for the detection of early-stage infections, restricting its utility in PoN settings. Herein, we integrate the aggregation-induced

CsDETC: detection and counting of small target Cryptococcus spp.

INTRODUCTION: Cryptococcus spp. infection can lead to cryptococcal meningitis (called CM) and pulmonary cryptococcosis, and how to diagnose Cryptococcus spp. infection accurately and timely is an urgent need in clinical practice. However, the existing methods such as cerebrospinal fluid (CSF) ink staining smear microscopy and CSF Cryptococcus spp. culture only rely on manual counting to determine the number of Cryptococcus spp., resulting in low efficiency. Thus, how to identify Cryptococcus spp. in cerebrospinal fluid and achieve automated counting of Cryptococcus spp. is of great significanc

Invasive pulmonary cryptococcosis mimicking metastatic lung cancer: A case report and review of literature.

BACKGROUND: Worldwide, there has been a steady increase in the number of cases of pulmonary cryptococcosis diagnosed in immunocompetent patients, where symptoms can range from mild to severe. Clinical and radiological distinction of disease may be made when compared with immunodeficient cases and in those presenting with primary lung carcinoma. In the latter case confusion can lead to initial misdiagnosis and delayed treatment. We report a case of disseminated cryptococcosis in an immunocompetent patient which mimicked a primary lung carcinoma with brain metastases. CASE SUMMARY: A 51-year-ol

Fungicide-tolerant persister formation during cryptococcal pulmonary infection.

Bacterial persisters, a subpopulation of genetically susceptible cells that are normally dormant and tolerant to bactericides, have been studied extensively because of their clinical importance. In comparison, much less is known about the determinants underlying fungicide-tolerant fungal persister formation in vivo. Here, we report that during mouse lung infection, Cryptococcus neoformans forms persisters that are highly tolerant to amphotericin B (AmB), the standard of care for treating cryptococcosis. By exploring stationary-phase indicator molecules and developing single-cell tracking strat

Autochthonous Cryptococcus gattii genotype VGIIb infection in a Japanese patient with anti-granulocyte-macrophage colony-stimulating factor antibodies.

A 31-year-old Japanese man presented with cerebral and pulmonary cryptococcosis. Cryptococcus gattii (C. gattii) genotype VGIIb was detected in the patient's sputum and cerebrospinal fluid specimens. The serum levels of anti-granulocyte-macrophage colony-stimulating factor (GM-CSF) antibodies were elevated in this patient, which has been associated with pulmonary alveolar proteinosis and is considered a risk factor for C. gattii infection. After undergoing >12 months of antifungal treatments, the patient showed improvements in symptoms and findings on brain and lung imaging. Several Japanese p
