Temporal trends in the clinical presentation, management, and outcomes of children with congenital cytomegalovirus infection.

UNLABELLED: Congenital cytomegalovirus (cCMV) is the most common congenital infection and a leading cause of sensorineural hearing loss as well as neurodevelopmental impairment. In recent years, awareness, screening, and management strategies for cCMV have evolved. This study aimed to assess temporal changes in the clinical presentation, treatment, and outcomes of children diagnosed with cCMV infection. We conducted a retrospective cohort analysis using data from the CCMVnet registry ( www.ccmvnet.org ), an international multicenter database collecting anonymized data on children with confirme

Chorioretinal Scars in Congenital Cytomegalovirus Infection Following Prenatal Valacyclovir Therapy.

BACKGROUND: Congenital cytomegalovirus infection is the most common congenital infection. Ocular abnormalities affect approximately 14% of symptomatic children, but fewer than 1% of asymptomatic children. Prenatal valacyclovir reduces vertical transmission, but ophthalmic outcomes in exposed infants are poorly described. METHODS: We describe 2 neonates with congenital cytomegalovirus confirmed by amniotic fluid and urine polymerase chain reaction following first-trimester maternal primary infection. Both mothers received high-dose valacyclovir (8 g daily) from 13 weeks' gestation. Infants und

Congenital Cytomegalovirus Testing Following Pennsylvania Hearing-Targeted Screening Legislation.

BACKGROUND: Congenital cytomegalovirus infection (cCMV) occurs in approximately 1/200 to 1/250 US newborns each year and can cause progressive sensorineural hearing loss. Since 2022, Pennsylvania law mandates cCMV testing for infants who fail newborn hearing screening. OBJECTIVES: To assess changes in cCMV testing rates and diagnosis before and after Pennsylvania hearing-targeted legislation. METHODS: Retrospective cohort study of all live births at 3 university-affiliated birth centers. We identified all infants tested for CMV at 21 days of age or younger and analyzed cCMV testing rates in

Pediatric hearing loss in the universal newborn hearing screening era: etiologies, risk indicators, and clinical phenotypes.

Childhood hearing loss poses significant developmental and public health challenges, impacting language acquisition, cognitive development, social participation, and educational outcomes. Universal newborn hearing screening (UNHS) has notably enhanced the early identification of congenital hearing loss, yet limitations persist. Mild, unilateral, delayed-onset, progressive, and certain neural forms of hearing loss may remain undetected during a single neonatal screening session. This review consolidates current evidence regarding the etiologies, risk indicators, and clinical features of pediatr

Neuroimaging Abnormalities in Congenital Cytomegalovirus Infection: A Systematic Review and Pooled Descriptive Analysis.

Congenital cytomegalovirus (cCMV) is the most common congenital viral infection and an important cause of neurological morbidity, yet reported neuroimaging findings vary across studies and imaging modalities. We performed a systematic review with pooled descriptive analysis to characterize neuroimaging abnormalities reported in cCMV. PubMed, Scopus, and Embase were searched for English-language, peer-reviewed studies published from January 1, 2013, through December 1, 2024, that reported extractable prenatal or postnatal neuroimaging data in confirmed cCMV. Studies using magnetic resonance ima

Prevalence of Congenital Cytomegalovirus Infection in a CMV-Endemic Ugandan Population.

Congenital cytomegalovirus (cCMV) infection is a leading cause of childhood deafness and disability worldwide. However, less is known about its epidemiology in low-income regions where most women are CMV-seropositive at conception. We found that cCMV is highly prevalent in rural Uganda (3.73%) and is associated with lower gravidity and placental malaria. We additionally report a high transmission intensity in the first years of life, with 97.3% of children acquiring CMV IgG by age 3.

Progressive Hearing Loss in Childhood - A Prognostic Factor Systematic Review.

INTRODUCTION: Progressive hearing loss (PHL) in childhood can significantly impact speech, language, and educational outcomes. While congenital and acquired aetiological factors for PHL are well documented, their influence on hearing deterioration and the trajectory of PHL remains unclear. Understanding prognostic factors is essential to enable targeted surveillance and timely intervention. METHODS: A PROSPERO-registered systematic review (ID: CRD42023492129) was conducted to identify and evaluate prognostic factors associated with childhood PHL. Bilateral sensorineural hearing loss in children: causes and long-term progression.

PURPOSE: Childhood hearing loss (CHL) in need of assisting listening devices require a constant surveillance for hearing loss (HL) progression and adjustment of interventions. Despite advances in aetiological investigations, there are still important gaps in understanding causal determinants and progression patterns of CHL. The aim of this study was to analyze the prevalence and yield of aetiological investigations, and to identify causes associated with HL progression in older children. METHODS: This descriptive retrospective study included data from 194 children with moderate-profound senor

Pooled extreme-phenotype genome-wide association study XP-GWAS reveals an association between 4-hydroxyphenylpyruvate dioxygenase and β-carotene variation in Capsicum annuum.

BACKGROUND: Carotenoids are essential plant pigments with key roles in stress tolerance and human nutrition. β-carotene is a major provitamin A carotenoid, and understanding the genetic basis of its natural variation in Capsicum annuum is important for nutritional improvement. However, carotenoid accumulation is a complex quantitative trait influenced by multiple metabolic and regulatory pathways. METHODS: An extreme-phenotype genome-wide association study (XP-GWAS) was conducted using pooled genomic DNA from 92 C. annuum accessions representing contrasting extremes of β-carotene content. Fru

The first two complete mitochondrial genomes for the genus Neotrichoporoides (Hymenoptera, Eulophidae) and their phylogenetic analysis.

Neotrichoporoides belongs to the family Eulophidae (Hymenoptera: Chalcidoidea). As a group of parasitic wasps, it plays an indispensable role in the biological control of agricultural and forest pests and in maintaining ecosystem balance. To date, only nine complete mitochondrial genomes of Eulophidae have been sequenced worldwide, including the two newly sequenced species in this study. To enrich our understanding of the mitochondrial genomic diversity of Eulophidae and to provide preliminary insights into its phylogenetic relationships, we sequenced and comparatively analyzed the mitochondri

Recent insights into the immunobiology of congenital cytomegalovirus infection.

Human cytomegalovirus (HCMV) is the leading cause of congenital viral infection, and therapeutic interventions remain limited. This review highlights recent findings on the immunobiology of HCMV during pregnancy, exploring the roles of maternal and fetal/neonatal T cell responses in controlling infection. The utility of in vitro models of infection is discussed, with emphasis on the use of novel trophoblast stem cells and placental organoids to study HCMV transmission at the maternal-fetal interface and the response to infection. Efficacious vaccines have yet to be developed. Recent clinical t

Retrospective screening for congenital cytomegalovirus infection in the Survey of Neonates in Pomerania shows very low disease burden.

BACKGROUND: Congenital cytomegalovirus (cCMV) infection is considered one of the most common infectious pathologies in neonates, showing a wide range of clinical manifestations, ranging from completely asymptomatic to critically ill newborns. Primary infection during the first trimester of pregnancy carries the highest risk of severe fetal involvement. However, the majority of infected fetuses show an asymptomatic or mild course of disease. The most common long-term sequela is sensorineural hearing loss, which is detected in only about 50% of affected children during newborn hearing screenings

Cytomegalovirus in pregnancy: prevention, maternal screening, and the role of antivirals.

Cytomegalovirus (CMV) is the most common congenital infection in Australia and a leading cause of preventable childhood disability. Current Australian guidelines recommend targeted antenatal screening of women at higher risk for CMV infection. Serology testing should also be considered in women with clinical symptoms suggestive of CMV. Women with suspected CMV infection in pregnancy should be promptly referred to a maternal-fetal medicine or infectious diseases specialist. High-dose valaciclovir can reduce in utero transmission to the fetus following first-trimester maternal primary infection;
