Estimating the Prevalence of Congenital Cytomegalovirus Infection due to Primary Maternal Infection in the United States: A Probabilistic Risk Assessment Model.

BACKGROUND: Transplacental transmission of cytomegalovirus (CMV) after primary maternal infection during the first trimester of pregnancy is associated with the most severe sequelae from congenital CMV (cCMV) infection. We estimated the prevalence of cCMV infection due to primary maternal CMV infection in the United States. METHODS: We built a probabilistic risk assessment model inputting 2022 national natality data, national CMV seroprevalence estimates among females 15-49 years, and published vertical transmission rates to estimate cCMV infection prevalence and distribution by maternal age

Cerebellar Malformations in Congenital Cytomegalovirus Infection.

BACKGROUND AND PURPOSE: Congenital cytomegalovirus (cCMV) is the leading infectious cause of birth defects in the United States; its spectrum of MRI findings is broad, with cerebellar abnormalities scarcely reported in the literature. We aim to describe the neuroimaging spectrum of cerebellar anomalies and malformations in patients with cCMV infection and their associations with other typical cCMV findings in prenatal and postnatal brain MRI. MATERIALS AND METHODS: We conducted a single-center retrospective observational study at a quaternary children's hospital. Included patients had fetal a

Antiviral Treatment for Congenital Cytomegalovirus Infection in Extremely Preterm Newborn: A Case Report and Literature Review.

BACKGROUND: Congenital cytomegalovirus (cCMV) infection is one of the most common congenital infections worldwide and the leading cause of non-genetic sensorineural hearing loss. Although less frequent in preterm infants, cCMV may significantly worsen outcomes in an already vulnerable population. The risks and benefits of antiviral therapy in extremely preterm neonates remain unclear, as this group is largely excluded from clinical trials. CASE PRESENTATION: We report a case of symptomatic cCMV infection in an extremely preterm infant born at 26 weeks and 2 days of gestation to a mother with

The Management of Congenital Cytomegalovirus Infection in an Era of Universal Newborn CMV Screening.

The most common infectious disease responsible for paediatric developmental disability is congenital infection with human cytomegalovirus (cCMV). Many serious sequelae are caused by cCMV, including microcephaly, intracranial calcifications, neuronal migration defects, seizure disorders, developmental delay, and sensorineural hearing loss (SNHL). Although long-term neurodevelopmental impairment is clearly more common in newborns with clinically apparent disease at birth, more knowledge is needed about management in the era of universal cCMV screening, since screening identifies infants that, in

Eight years of vestibular screening in children at increased risk: Results and perspectives.

BACKGROUND: Since vestibular hypofunction (VH) in children can affect motor and cognitive development, standardized vestibular screening is recommended for high-risk children. However, clinical evidence on screening outcomes remains limited. This study aimed to evaluate eight years of vestibular screening results and to investigate variations across different underlying conditions. METHODS: A retrospective chart review was performed, including 229 children (aged 0-18 years) who underwent vestibular screening between 2017 and 2024. Screening indications included congenital cytomegalovirus infe

Cortical auditory evoked potential with different speech stimuli in children with asymptomatic congenital cytomegalovirus infection.

PURPOSE: To characterize cortical auditory evoked potentials with speech stimuli in children with asymptomatic CMV infection compared to non-infected children with typical development of hearing, speech, and language. METHODS: The sample included 23 children with asymptomatic congenital cytomegalovirus infection and 16 control children, matched by age, sex, head circumference, and socioeconomic status. Cortical Auditory Evoked Potentials were recorded using speech stimuli /da/ and /ga/ at 70 dB SPL, with electrodes placed at the vertex (Cz), forehead (Fpz), and earlobes (A1 and A2). The prese

Maternal transmission, neonatal outcomes, and predictors of adverse effects in congenital cytomegalovirus infection.

BACKGROUND: Congenital cytomegalovirus (cCMV) infection, caused by a DNA beta-herpesvirus, is the most common congenital infection globally, with significant neurodevelopmental and audiological sequelae. This study aimed to evaluate maternal-to-fetal transmission rates, neonatal outcomes, and the predictive value of clinical and virological markers for adverse outcomes. METHODS: This prospective longitudinal study enrolled 43 neonates born to mothers with CMV infection during pregnancy (2019-2023). cCMV was diagnosed by CMV DNA detection in urine or saliva within the first two weeks of life.

Maternal cytomegalovirus infection in the first trimester of pregnancy: timing, fetal brain injury, and long-term neurodevelopmental outcomes including autism spectrum disorder.

BACKGROUND: Congenital cytomegalovirus infection is the leading nongenetic cause of sensorineural hearing loss and is associated with a broad spectrum of neurodevelopmental outcomes. A possible association between congenital cytomegalovirus and autism spectrum disorder has been hypothesized for several decades, based on case reports and small retrospective studies. OBJECTIVE: The objective was to estimate the strength of the association between congenital cytomegalovirus and neurodevelopmental abnormalities including autism spectrum disorder, their dependence on the timing of maternal infecti

Complex Challenges in ANCA-Associated Vasculitis: A Case of COVID-19, Cytomegalovirus Pneumonitis, and Hemorrhagic Cholecystitis Following Rituximab Therapy.

Antineutrophil cytoplasmic antibody (ANCA)-associated vasculitis (AAV) is a rare autoimmune disease with significant infectious and hemorrhagic risks during immunosuppressive therapy. We describe an 81-year-old woman with perinuclear ANCA (p-ANCA)-associated vasculitis complicated by acute respiratory distress syndrome from concurrent COVID-19 and CMV pneumonitis following rituximab induction. Her course was further complicated by hemorrhagic cholecystitis, a rare bleeding manifestation, requiring percutaneous intervention. Careful adjustment of immunosuppression was necessary due to severe in

Mathematical modelling highlights the crucial role of early childhood immunization in preventing congenital CMV in countries with high CMV seroprevalence.

BACKGROUND: Congenital cytomegalovirus infection (cCMVi) is a major global health problem and a leading cause of childhood hearing, visual, and intellectual disability. Development of an effective vaccine against CMV is a high priority, and there is optimism that one will be achieved soon. Previous mathematical modelling to predict the impact of CMV vaccination in low CMV seroprevalence, high-income populations indicate that vaccinating infants could be a highly efficient strategy to prevent cCMVi, even with only a modestly effective vaccine (i.e., <50% protective against primary infection). Congenital cytomegalovirus infection drives oligoclonal expansion of cytotoxic γδ T cells from early fetal progenitors.

Gamma delta (γδ) T cells emerge early during human gestation and are uniquely equipped to protect the fetus and infant following infection, due to their innate-like recognition of conserved molecular ligands. Following congenital cytomegalovirus infection (cCMV), Vδ1 T cells have been shown to expand, differentiate, and upregulate cytotoxic mediators, but display markedly restricted γδTCR diversity compared to CMV-infected adults. Early fetal γδ T cells are biased toward rapid effector function and comprise a distinct ontological "layer" that can be distinguished from late gestation γδ T cells

Preventing congenital cytomegalovirus infection in transplant recipients; a focus on uterine transplantation.

Uterine transplantation (UTx) offers the only treatment for those with uterine factor infertility to carry their own pregnancy. Cytomegalovirus infection is significant in UTx, given its implications for both transplant and pregnancy. There is little evidence and no management guidance on cytomegalovirus in periconception and pregnancy in transplant recipients and no consensus or guidelines on managing cytomegalovirus in UTx. This review highlights the pertinent issues surrounding cytomegalovirus infection following transplantation and congenital cytomegalovirus infection. It summarises the pu

Congenital Cytomegalovirus Infection in Pregnancy: Challenges in Early Diagnosis, Reinfection, and Secondary Prevention.

Cytomegalovirus (CMV) remains one of the most relevant congenital and early-life infections in pediatrics because of its high global seroprevalence, lifelong latency, and potential for reactivation or reinfection. Biologically, the virus poses a particular threat during pregnancy, when maternal primary infection carries a substantially higher risk of transplacental transmission than non-primary infection, with fetal and neonatal consequences that vary according to gestational timing and host vulnerability. In children, CMV infection is common in the first years of life and may contribute to a

Congenital Cytomegalovirus Infection in Fetal Demise: A Retrospective Cohort Study.

BACKGROUND: CMV is the most common congenital infection worldwide, affecting 0.2-6% of live births and causing significant morbidity and mortality. Although most infections are asymptomatic, severe cases can lead to fetal demise. OBJECTIVE: To quantify and characterize confirmed CMV infection among fetal demises in a large pediatric hospital in the US from 2013 to 2024. METHODS: A retrospective electronic health record (EHR) review of pregnancies with CMV testing, delivery summaries, and confirmed fetal CMV infection via tissue or autopsy linkage. RESULTS: During the review period, 61,973 p

A neonatal prognostic score for infants with congenital cytomegalovirus infection (cCMVnet Score).

The objective was to identify risk factors for long-term sequelae in children with congenital cytomegalovirus (cCMV) infection and to develop a prognostic scoring system (cCMVnet score) to support individualized management. This retrospective study was conducted within the prospective European Registry of Children with cCMV and included 570 children born between 2011 and 2024 in 11 countries. Eligible participants had confirmed cCMV, early cranial ultrasound and brain MRI (≤ 90 days of life), and at least six months of follow-up. The primary outcome was sequelae, defined as sensorineural heari

Bacterial metagenome in plaque, saliva, and tumor samples from individuals with and without OSCC by next-generation sequencing.

BACKGROUND: Postoperative complications and delayed physical recovery are significant problems following emergency abdominal surgery. Physiotherapy aims to aid recovery and prevent complications in the acute phase after surgery and is commonplace in most first-world hospitals. Despite ubiquitous service provision, no well-designed, adequately powered, parallel-group, randomised controlled trial has investigated the effect of physiotherapy on the incidence of respiratory complications, paralytic ileus, rate of physical recovery, ongoing need for formal sub-acute rehabilitation, hospital length

From hip pain to MAS: A case of gonococcal arthritis in a child.

Joint pain in children presents a diagnostic challenge due to its multifaceted etiology, ranging from musculoskeletal injuries to systemic infections. This case report details a 3-year-and-5-month-old girl with bilateral hearing impairment and congenital cytomegalovirus infection, who was admitted with thigh pain, high fever, and difficulty walking. Despite being hemodynamically stable and without apparent infection signs, extensive diagnostic evaluations revealed gonococcal hip arthritis. This condition, caused by Neisseria gonorrhoeae, is unusual in young children and typically associated wi
