Symptomatic Congenital Cytomegalovirus Infection in a Preterm Very-Low-Birth-Weight Infant Treated With Intravenous Ganciclovir Followed by Oral Valganciclovir.

Oral valganciclovir (VGCV) therapy has been shown to be effective against congenital cytomegalovirus (CMV) infection, with improvements in both auditory and neurological outcomes. However, therapeutic strategies remain undefined for cases involving preterm infants born before 34 weeks of gestation or those with a birth weight below 1200 g. We report a case of a male neonate born at 32 weeks and four days of gestation, with a birth weight of 1168 g, diagnosed with symptomatic congenital CMV infection. Diagnosis was based on positive CMV-DNA in urine and clinical findings, including ventriculome

Evaluation of Olfactory Function in Asymptomatic Children With Congenital Cytomegalovirus Infection.

Background and aim Congenital cytomegalovirus (cCMV) is the most prevalent congenital infection worldwide, associated with numerous long-term sequelae, even in infants asymptomatic at birth. However, data are lacking for asymptomatic-at-birth children, who account for the majority of cases. Thus, the aim of the study was to evaluate the olfactory function of asymptomatic cCMV-infected children and compare it to matched healthy controls using a validated odor Identification test. Methods A case-control study was conducted. Asymptomatic cCMV-infected children and healthy controls without cCMV hi

Congenital Cytomegalovirus Infection in Malaysia: A Scoping Review, Clinical Insights and Future Directions.

Congenital cytomegalovirus (cCMV) infection is the most common congenital infection worldwide, greatly contributing to neonatal morbidity and long-term neurological and audiological complications. Despite its clinical burden, cCMV remains underdiagnosed in Malaysia due to the absence of routine screening programmes and limited awareness among healthcare professionals. This review presents an overview of the epidemiology, clinical manifestations, diagnostic approaches, and management strategies of cCMV in Malaysia, highlighting gaps in current practices and potential future approaches. The sero

Predominance of Auditory But Not Vestibular Deficits in a Mouse Model of Congenital Cytomegalovirus Infection.

OBJECTIVES: The aim of this study was to investigate the auditory and vestibular consequences of congenital cytomegalovirus (cCMV) infection in a C57BL/6 mouse model. This model, established via intraperitoneal inoculation, was designed to mimic clinical phenotypes. METHODS: A cCMV infection model was developed by intraperitoneally inoculating C57BL/6 mice with murine cytomegalovirus. Auditory and vestibular functions were evaluated using auditory brainstem response tests, as well as open field, swim, and rotarod assessments. Histologic analysis of the inner ear was also conducted. RESULTS:

Prevalence of ocular and visual abnormalities following symptomatic and asymptomatic congenital CMV infection: a systematic review and meta-analysis.

BACKGROUND: Cytomegalovirus (CMV), the most common congenitally acquired infection, can result in visual disability in affected children. We aimed to estimate the burden of eye and vision disorders amongst children with symptomatic and asymptomatic congenital cytomegalovirus infection (cCMV), to inform the development of guidance for the provision of care. METHODS: In this systematic review and meta-analysis, we searched PubMed, Embase, and CINAHL databases up to 6th Feb 2025 for studies reporting ocular disorders or visual impairment (VI) outcomes following cCMV diagnosis. We included longit

Universal newborn screening for congenital cytomegalovirus infection.

Congenital cytomegalovirus (CMV) infection is the leading infectious cause of childhood disability, in particular sensorineural hearing loss (SNHL). Timeliness of diagnosis is crucial, since the presence of CMV in any compartment (eg, blood, urine, or saliva) after age 21 days can mean postnatal acquisition of infection, particularly in breastfed infants. Given these issues, there is considerable interest in implementation of screening programmes-either universal screening (where all newborns are tested) or targeted screening. Targeted screening is typically based on the outcome of a newborn h

Neonatal T cells unleash innate powers to combat congenital cytomegalovirus infection.

Approximately 1 in 200 newborns worldwide are affected by congenital cytomegalovirus (CMV). Most of these cases are asymptomatic due to successful control of the infection by the newborn's immune system. In this issue of the JCI, Semmes et al. characterized the cellular immune response in cord blood of neonates with CMV infection. The authors found that conventional T cells with NK-like features expanded during congenital CMV infection. To exert their antiviral function, these cells relied on Fc receptors, recognizing virus-infected cells bound by IgG. Thereby, the fetal and maternal immune sy

Agreement Between Dried Blood Spot and Plasma PCR in Infants With Congenital Cytomegalovirus Infection.

Congenital cytomegalovirus (cCMV) infection is diagnosed by positive urine or saliva testing within 21 days after birth. Beyond this age, newborn dried blood spot (DBS) PCR can retrospectively diagnose cCMV infection but has lower sensitivity than urine or saliva PCR testing. The DBS PCR may be negative due to the absence of blood DNAemia at birth or to the technical limit of detection for DBS PCR. The objective of this study was to distinguish these two possibilities by determining agreement between DBS and plasma CMV PCR tests among cCMV-infected infants. This single center retrospective coh

Early control of cochlear viral load limits cochlear inflammation and prevents virus-induced sensorineural hearing loss.

Human cytomegalovirus (HCMV) is the most common viral infection acquired in utero and a leading cause of neurodevelopmental abnormalities, including sensorineural hearing loss (SNHL). In previous studies using a murine model of HCMV induced SNHL, hearing loss was correlated with virus-induced cochlear inflammation but not cochlear viral load. However, these previous findings were determined at the time of auditory testing, a time poiont well past critical periods of auditory development. In the current study, cochlear virus load early in auditory development could be correlated with the magnit

Clinical Characteristics, Neuroimaging Findings and Long-Term Sequelae in Children with Congenital Cytomegalovirus Infection: A Single Centre Study.

Congenital cytomegalovirus infection is the most common congenital infection worldwide and an important cause of neurodevelopmental delay and sensorineural hearing loss. Neuroimaging represents the best prognostic marker in cCMV infection. The aim of this study was to establish the role of cranial ultrasound and brain magnetic resonance imaging in the development of long-term sequelae in symptomatic and asymptomatic children with cCMV infection. Of the 47 children enrolled in the study, 25 (53.1%) were classified as symptomatic at birth. In 27/47 patients, SNHL was diagnosed with a similar pro

Self-reported executive functioning in adults with congenital cytomegalovirus infection.

BACKGROUND: Congenital Cytomegalovirus (cCMV) is the most common prenatal infection and the main infectious cause of neurodevelopmental abnormalities in developed countries. Long-term neuropsychological outcome of cCMV infection is yet not well understood, and follow-up studies on adults screened for CMV at birth are few. The aim of this study was to investigate self-reported executive functioning (EF) in adults with cCMV infection in relation to uninfected controls. METHOD: All individuals from a universal newborn CMV screening study conducted in Southern Sweden and sampled 1977-85, was invi

Epidemiology and Economic Burden of Diagnosed Congenital Cytomegalovirus Infection in the First 2 Years of Life among Commercially Insured and Medicaid-Insured Individuals in the United States.

PURPOSE: Congenital cytomegalovirus (cCMV) is the leading infectious cause of congenital birth defects. Although approximately 20% to 25% of infants born with cCMV develop long-term health complications such as sensorineural hearing loss, developmental issues, and microcephaly, studies on the disease burden of cCMV are limited. In this study, we assessed the epidemiology, economic burden, and disease burden of clinically diagnosed cCMV in the United States using insurance claims data. METHODS: This retrospective study utilized Merative MarketScan Commercial Claims and Encounters and Multi-Sta

Universal screening for congenital Cytomegalovirus infection using saliva from the neonate.

BACKGROUND: Congenital Cytomegalovirus infection (cCMV) is the most common intrauterine infection in developed countries. The diagnostic gold standard is detection of CMV in neonatal urine. Considering difficulties in urine sample collection, saliva is an alternative specimen providing supportive material for early diagnosis and timely intervention. OBJECTIVES: Aim of this study was to evaluate a universal screening program using saliva as target specimen, and urine as confirmatory material. RESULTS: Between May 2019 and November 2024, 5168 newborns were screened for CMV shedding in saliva,

Cytomegalovirus Saliva Shedding Kinetics in Children with Congenital CMV Infection.

BACKGROUND: Congenital CMV (cCMV) is a common congenital viral infection worldwide and the most common cause of childhood non-genetic sensorineural hearing loss (SNHL). The kinetics of CMV detection (shedding) from mucosal surfaces have not been extensively described in children with cCMV due to a lack of systematic newborn CMV screening and follow-up protocols. The aim of this study is to describe the natural history of saliva CMV shedding in a cCMV cohort, which was identified through universal newborn screening. METHODS: As part of the CMV and Hearing Multicenter Screening study (CHIMES),
