Prevalence study of antimicrobial resistant organisms in very preterm neonates.

BACKGROUND: Preterm neonates have underdeveloped organs, a fragile skin barrier and an immature immune system rendering them susceptible to infection. These infants are at an increased risk of developing a healthcare-associated infection because of antibiotic exposure, invasive monitoring and the general risk of outbreaks within a hospital setting. The aim of this study was to investigate the prevalence of antimicrobial resistant organism carriage in very preterm neonates (i.e. born ≤ 32 weeks gestation) in the neonatal intensive care unit (NICU). METHODS: Neonates born ≤ 32 weeks' gestation

Trends of neonatal sepsis and its etiology at Hawassa, Ethiopia: a five year retrospective cross-sectional study.

BACKGROUND: Neonatal sepsis is a significant cause of morbidity and mortality in low- income countries. Neonatal sepsis is classified as early-onset neonatal sepsis (EONS) and late-onset neonatal sepsis (LONS). Etiologies responsible for EONS are mostly acquired vertically from the mother during or before birth with the possibility of prevention. The burden and etiology of neonatal sepsis is not uniform across the globe with huge disparities based on the income level of the countries. This study aimed to determine neonatal sepsis trends, prevalence, and etiologies at Hawassa University Compreh

16 S rRNA-based molecular identification of coagulase-negative Staphylococcus species in neonates with sepsis and their antibiotic resistance patterns in Ahvaz, Iran.

INTRODUCTION: Coagulase-negative staphylococci (CoNS) are among the leading causes of neonatal sepsis (NS). NS can be divided into two types: early-onset sepsis (EOS), which usually occurs less than 72 h after birth, and late-onset sepsis (LOS), which can occur 8 to 28 days after birth. According to newly published statistics, the incidence rates of EOS and LOS in neonates are 0.5-3.1% and 2-32%, respectively. This study aimed to determine the prevalence of common CoNS isolates and their antibiotic resistance patterns in NS cases in Ahvaz, Iran. METHODS: This cross-sectional study (October 20

Insights into bloodstream infections in South African paediatric burn patients: implications for antimicrobial stewardship.

INTRODUCTION: Bloodstream infections (BSIs) significantly contribute to the morbidity and mortality in paediatric burn patients from low- and middle-income countries; with common pathogens like Staphylococcus aureus, Escherichia coli, and Pseudomonas aeruginosa frequently being multidrug resistant (MDR). Due to the growing prevalence of MDR BSIs, antimicrobial stewardship needs to be improved with perhaps more targeted initial antimicrobial use. The study describes the aetiology, and timing of burn-associated BSIs and MDR infections in paediatric burn patients admitted to two paediatric surger

Seven-day versus 14-day antibiotic course for culture-proven neonatal sepsis: a multicentre randomised non-inferiority trial in a low and middle-income country.

OBJECTIVE: Definitive guidance regarding the duration of antibiotics for neonatal sepsis is lacking. We hypothesised that a 7-day antibiotic course is non-inferior to a 14-day course for treating culture-proven sepsis. DESIGN: Randomised, controlled, non-inferiority trial with masked outcome assessment in eight centres in a low and middle-income country. PATIENTS: Neonates with a birth weight (BW) ≥1000 g and blood culture-proven sepsis were randomised on day 7 of sensitive antibiotic therapy provided sepsis had clinically remitted. EXCLUSIONS: Staphylococcus aureus or fungal sepsis, and in

Reduction of Vancomycin Use in a Neonatal Intensive Care Unit: A Quality Improvement Project.

INTRODUCTION: Late-onset sepsis (LOS) is a common cause of neonatal morbidity and mortality. Professional organizations recommend avoiding empiric vancomycin use in neonates without risk factors for methicillin-resistant Staphylococcus aureus infection. We aimed to reduce the mean vancomycin antibiotic utilization rate (AUR) by 30% for 12 months in our neonatal intensive care unit (NICU). METHODS: We included neonates admitted to our level-3 NICU from March 15, 2023, to February 29, 2024, with suspected LOS in the intervention period. A multidisciplinary team used the Model for Improvement. I

Serious bacterial infections in infants younger than 90 days of age with acute bronchiolitis.

UNLABELLED: Although acute bronchiolitis is primarily caused by viral infections, antibiotics are often administered to children with acute bronchiolitis. Due to concerns about serious bacterial infections (SBI), neonates and young infants are particularly prone to antibiotic overuse. This study aimed to identify the clinical characteristics and prevalence of SBI in neonates and infants aged < 90 days with acute bronchiolitis. We retrospectively reviewed the medical records of 651 neonates and infants aged < 90 days hospitalized with acute bronchiolitis between September 2015 and August 2024.

Clinical and Microbiological Profile of Culture-Positive Neonatal Sepsis in a Tertiary Care Center in North India.

Background Sepsis in newborns is a major clinical concern, especially in resource-limited settings. Although blood culture remains the primary method for confirming infection and guiding treatment, its detection rate is often suboptimal. The objective of this study was to evaluate the incidence, clinical characteristics, maternal and neonatal risk factors, and antimicrobial resistance patterns linked to culture-confirmed neonatal sepsis in a tertiary care facility in North India. Materials and methods This prospective observational study was carried out over 18 months in the neonatal intensive

Orbital cellulitis presenting as persistent fever in a neonate after inappropriate caregiving practices.

Neonatal orbital cellulitis is an uncommon but serious infection that can lead to an orbital abscess, often requiring surgical intervention. We report a rare case of a term neonate presenting on day 20 of life with persistent fever and irritability without initial orbital signs. Blood culture revealed methicillin-resistant Staphylococcus aureus (MRSA), and intravenous vancomycin was initiated. By day 3 of admission, the infant developed periorbital swelling; imaging confirmed a retro-orbital abscess. With timely antibiotic therapy, the infection resolved without the need for surgical drainage.

Ertapenem Combined With Anti-Staphylococcal Beta-Lactam Therapy for the Treatment of Persistent Staphylococcus Aureus Bacteremia in a Child With Vertebral Osteomyelitis.

Staphylococcus aureus infection is one of the most common and serious infections that arises in children and is associated with high morbidity. S. aureus is the leading cause of acute hematogenous osteomyelitis in children. In the absence of concerns regarding resistance to methicillin, an anti-staphylococcal isoxazolyl penicillin, such as oxacillin or nafcillin, is the drug of choice for treatment of S. aureus osteomyelitis. However, first-generation cephalosporins, such as cefazolin, can also be used. There are limited antimicrobial options available for osteomyelitis and persistent or inter
