Early-onset neonatal sepsis and neonatal antibiotic exposure in Hungary a nationwide population-based study.

UNLABELLED: Neonatal sepsis remains a challenge in perinatal medicine. We aimed to estimate incidence of culture positive early-onset sepsis (CP-EOS), causative pathogens and overall antibiotic exposure in newborn infants in Hungary. Retrospective national registry analysis (2020-2023). Data were mainly collected from the Hungarian National Perinatal Registry covering all level II-III neonatal intensive care units (NICUs) in Hungary. The number of livebirths in each gestational age group was provided by Hungarian Central Statistical Office. Overall CP-EOS incidence was 0.41/1,000 livebirths (0

Bovine lactoferrin maintains antibacterial effect against neonatal Escherichia coli septicaemia isolates despite the presence of iron acquisition genes.

Introduction. Escherichia coli is a dominant cause of neonatal sepsis for which no prevention measures currently exist. Lactoferrin (LF) is an iron-sequestering antibacterial protein that has been noticed to decrease neonatal late-onset sepsis.Hypothesis/Gap Statement. LF's effect on in vitro growth of neonatal E. coli clinical isolates causing septicaemia is unknown. The prevalence of iron acquisition virulence genes which contribute to pathogenicity outcomes in these strains has also not been described in detail.Aim. To evaluate bovine lactoferrin's (bLF) effects on the in vitro growth of ne

Biotin-specific molecularly imprinted polymers as a biomimetic test line in lateral flow assays.

Lateral flow assays are widely used for rapid diagnostics at the point of care. However, their dependence on biological receptors limits their stability, scalability, and cost-effectiveness. This work demonstrates the use of biotin-specific molecularly imprinted polymers (biotin-MIPs) as synthetic recognition elements in nucleic acid lateral flow (NALF) assays. The biotin-MIPs were synthesized, structurally characterized, and integrated into the nitrocellulose membrane as a test line. Their selective binding affinity was first validated using biotinylated horseradish peroxidase as a model anal

Management of the newborn at risk for vertical sepsis.

Suspected early-onset sepsis (EOS) is one of the most frequent diagnoses in neonatal units; however, infection is not confirmed in most cases, leading to the widespread overuse of empirical antibiotherapy. This document presents a diagnostic and therapeutic approach to EOS and proposes evidence-based recommendations, supported by national epidemiological data, for the management of neonates at risk of EOS according to gestational age and individual risk level, in order to promote rational use of antibiotics in the neonatal period.

CRISPRi Screening Identifies Essential E. coli Virulence Factors for Placental Barrier Breach in a Maternal-Fetal Infection Model.

Early-onset neonatal sepsis caused by Escherichia coli (E. coli) threatens neonates' lives due to the pathogen's high virulence and multidrug resistance. The mechanisms that enable its placental barrier breach are poorly understood. Using a clinically isolated ST95 ExPEC strain from a neonatal sepsis case, along with a pregnant rat model and an in vitro placental barrier model, we performed CRISPR interference screening. This screen targeted 264 virulence factor genes and identified virulence factors for motility, iron acquisition, hemolysin secretion, and adherence/invasion as critical. We de

Prevalence and risk factors for carbapenem-resistant Enterobacterales colonization in hospitalized children in a provincial hospital in Vietnam.

BACKGROUND AND AIMS: Carbapenem-resistant Enterobacterales (CRE) colonization is an important prerequisite to hospital-acquired infections (HAIs) caused by CRE and increased mortality. This study assessed the prevalence of, and risk factors for, CRE colonization among children admitted to a provincial pediatric hospital in a high-antimicrobial-resistance setting in Vietnam. METHODS: A point prevalence survey was conducted on 15 August 2022 at a provincial pediatric hospital in the Red River Delta. Rectal swabs were collected from 376 hospitalized children after informed consent. Samples were

Natural maternal immunity protects neonates from Escherichia coli sepsis.

Escherichia coli is a leading cause of neonatal sepsis, with infection occurring in approximately one in every 1,000 live births1,2. However, with E. coli colonization beginning soon after birth3-5 and defects in neonatal host defence maturation6-9, an alternative consideration is why infection does not occur even more frequently. Here we show that newborn babies with E. coli sepsis have selectively reduced vertically transferred natural antibodies that recognize E. coli, mechanistically explaining their susceptibility to infection. Complementary preclinical studies show that preconceptual int

Nanomotion-enabled ultra-rapid antibiotic susceptibility testing with magnetic bead-based pathogen enrichment for accelerated sepsis diagnostics.

Rapid and accurate antimicrobial susceptibility testing (AST) is critical for guiding therapy in life-threatening infections such as sepsis, but current diagnostics rely on blood culture, delaying results by 48–72 h. This time lag forces empirical use of broad-spectrum antibiotics, increasing risks of treatment failure and antimicrobial resistance. We developed an approach combining bacterial growth-independent nanomotion detection with magnetic bead–based enrichment directly from blood. Using machine-learning classifiers trained on nanomotion spectra, we established ten antibiotic-specific mo

A Case of Severe Neonatal Neutropenia Secondary to Late-Onset Sepsis From E. coli Omphalitis.

Severe neonatal neutropenia, defined as an absolute neutrophil count (ANC) of <500/µL, is a critical hematologic finding that signals either a severe consumptive process or a primary bone marrow disorder. We present the case of an eight-day-old, full-term female infant who presented with fever and irritability, later diagnosed with severe neutropenia (ANC: 400/µL) and elevated C-reactive protein (CRP) (150 mg/L). Omphalitis was identified as the source, with pus culture growing pan-sensitive Escherichia coli, confirming late-onset neonatal sepsis (LONS). The patient was managed with targeted i

Neonatal sepsis: any role for procalcitonin as a diagnostic marker?

INTRODUCTION: In low and middle-income countries, a key driver of neonatal morbidity and mortality is neonatal sepsis. Rapid and precise diagnosis of neonatal sepsis remains a critical challenge due to limited laboratory facilities. Blood culture results, the diagnostic gold standard, are often unavailable or delayed; therefore, procalcitonin (PCT) has been considered as a potential inflammatory biomarker of bacterial infection. This study assessed the diagnostic performance of PCT in neonates with sepsis at the Federal Teaching Hospital, Owerri, Nigeria. METHODS: This cross-sectional study rec

Evaluation and Validation of a Loop-Mediated Isothermal Amplification (LAMP) Assay for Detection of Escherichia coli from Blood Samples of Young Infants with Sepsis.

Sepsis in young infants can lead to high mortality and morbidity, and its nonspecific signs require early and on-site detection of infections. The most common cause of infections in early-onset sepsis is Escherichia coli, particularly in premature and low-birth-weight infants. Here, the authors developed a rapid method based on loop-mediated isothermal amplification (LAMP) assay for detecting E. coli in newborn blood samples. The results showed detection of E. coli with a limit of detection (LOD) of 70 CFU/mL, confirming the assay’s reliability in distinguishing positive and negative cases com

Exosome-facilitated nanoplatform for enhanced antibiotic delivery to eradicate intracellular multidrug-resistant Escherichia coli in neonatal sepsis.

Neonatal sepsis is a pathogenic infection with an estimated global fatality rate of 18% and poses a greater threat in preterm infants, where early-onset cases often result from vertical transmission from the maternal genital tract. Conventional high-dose antibiotic therapy may lead to adverse effects and reduced therapeutic efficacy due to suboptimal intracellular delivery and interactions with extracellular components before effective cellular uptake. To overcome this limitation, exosome-based nanocarriers deliver antibiotics directly into cells, preventing systemic dissociation in the blood.

Neonatal early-onset sepsis: A changing paradigm of pathogens and high-risk cohorts.

The causative pathogens of neonatal early-onset sepsis (EOS) have changed over time with a shift towards gram-negative organisms. This change is likely multifactorial and due in part to a high prevalence of maternal intrapartum antibiotic exposure leading to alterations in rectovaginal colonization with antibiotic-resistant organisms and subsequent neonatal colonization and infection. As a result, neonatal EOS secondary to gram-negative pathogens, particularly Escherichia coli (E. coli), is becoming more aggressive, resistant and difficult to treat, and not exclusive to very low birth weight i

Depletion of Neonatal Neutrophilic Cells Worsens the Outcome of E. coli Sepsis in Newborn Mice.

Neonates are particularly susceptible to infections because of a reduced ability to fight bacterial pathogens while simultaneously exhibiting enhanced inflammatory damage. Neonatal neutrophilic cells (NNC) are a heterogeneous population of innate immune cells, which differ substantially from adults. They display functional deficits in pro-inflammatory responses and include a suppressive subpopulation known as myeloid-derived suppressor cells (MDSC). It remains unclear whether pro- or anti-inflammatory properties of NNC predominate immediately after birth. This study aimed to clarify the role o

Single Immunoglobulin Interleukin1-Related Receptor-TWIST1 Axis Regulates Barrier Function in Neonatal Intestine.

BACKGROUND & AIMS: The genetic basis of impaired intestinal barrier function in preterm infants is poorly understood. Variants in single immunoglobulin interleukin1-related receptor, a negative regulator of Toll-like receptor signaling, have been identified in preterm infants with necrotizing enterocolitis. We hypothesized that single immunoglobulin interleukin1-related receptor variants associated with necrotizing enterocolitis impair gut barrier function to pathobionts. The aim of this study was to determine how single immunoglobulin interleukin1-related receptor genetic variants disrupt neo

Efficacy and Safety of Meropenem in Pregnant Women with Upper Urinary Tract Infections: A Retrospective Cohort Study in Romania.

Introduction: Upper urinary tract infections (UUTIs) are among the most common serious infections during pregnancy and may be associated with maternal and fetal complications. The increasing prevalence of multidrug-resistant pathogens has led to the use of broader-spectrum antibiotics, including meropenem. However, data regarding the safety and efficacy of meropenem in pregnant women remain limited. The aim of this study was to evaluate the indications, efficacy, and safety of meropenem treatment in pregnant women with UUTIs and its impact on maternal and fetal outcomes. Methods: We conducted
