BACKGROUND: Recurrent neonatal infection caused by the same bacterial lineage across pregnancies is rarely documented. We describe two preterm siblings born one year apart who developed Escherichia coli sepsis caused by clonally related strains.
CASE SUMMARY: Two female infants were born preterm, 1 year apart, at 26 weeks and 4 days and 30 weeks and 3 days of gestation, respectively, following prolonged premature rupture of membranes. Both infants were evaluated for early-onset sepsis at birth with blood cultures and administered empiric ampicillin and gentamicin; initial blood cultures were

Background: Neonatal sepsis is a bloodstream infection and constitutes a major cause of morbidity and mortality among newborns. The diagnosis is primarily based on a positive blood culture result. An additional diagnostic and therapeutic challenge may be congenital infection, particularly in preterm infants. This is associated with the lack of routine screening of the maternal genital tract, except for testing for group B streptococci during earlier stages of an uncomplicated pregnancy. Case presentation: This paper presents a case of a neonate with early-onset sepsis (EONS) caused by Escheric

Early-life host-microbe interactions critically shape immune development, lifelong homeostasis, and disease susceptibility. The PRIMAL trial (Priming Immunity at the Beginning of Life) demonstrated that multistrain probiotics shifted the gut microbiota of very preterm infants toward eubiosis without affecting sepsis incidence, yet the immunological consequences remained unresolved. To explore this, we colonized germ-free female mice with fecal samples from probiotic- or placebo-treated preterm infants from the PRIMAL trial. Microbiota composition was vertically transmitted and stable across ge

Chronic granulomatous disease (CGD) is a primary immunodeficiency caused by defective phagocyte oxidative burst and may present in early life with severe or recurrent infections. We report a female infant born at 38 weeks' gestation (birth weight 2700 g) who developed fever and presumed sepsis at 5 days of life, followed by multiple recurrent hospitalizations for febrile illness with suspected meningitis, diarrhea, dehydration, and failure to thrive. Cerebrospinal fluid (CSF) evaluations across episodes demonstrated pleocytosis with elevated protein and normal-to-low glucose, while Gram stain

BACKGROUND Escherichia coli is a major pathogen causing sepsis in neonates and young infants. Although the K1 capsule is a well-established virulence factor in invasive infections, the clinical relevance of non-K1 strains and alternative virulence factors remains insufficiently characterized, particularly in cases of pediatric liver failure. The increased serum survival (iss) gene confers resistance to complement-mediated killing; however, its clinical relevance in infants has not been clarified. CASE REPORT Two full-term infants developed acute liver failure complicated by septic shock within

Stress hyperglycemia is a common metabolic response in critically ill neonates but may rarely present with extreme hyperglycemia mimicking neonatal diabetes mellitus. We report a term female neonate presenting on day 18 of life with lethargy, poor feeding, dehydration, and severe hyperglycemia (>1000 mg/dL), hypernatremia, and acute kidney injury. Insulin therapy was initiated due to suspicion of neonatal diabetes mellitus. Blood cultures grew Escherichia coli, confirming neonatal sepsis. Neuroimaging revealed a massive right fronto-parieto-temporal intraparenchymal abscess with significant ma

This secondary in silico study analysed 36 Escherichia coli draft genome assemblies from Zambia to characterize the virulence gene profiles of extraintestinal pathogenic E. coli (ExPEC) and to relate them to mobilome proxies, namely plasmid replicons and prophage content. All assemblies met plausibility thresholds for E. coli (1-17 contigs; 4.85-5.77 Mb; G+C 50.47-50.96 mol%), and species identity was confirmed by average nucleotide identity to the E. coli type strain (96.6-98.9%). Using the Achtman seven-locus MLST scheme applied to the reads, 35 of 36 genomes were assigned a sequence type (S

OBJECTIVE: To evaluate the effectiveness and safety of short (≤8 days) vs long (≥9 days) duration of antibiotic therapy for uncomplicated gram-negative (GN) bloodstream infections (BSI) among infants in the neonatal intensive care unit (NICU).
STUDY DESIGN: Retrospective analysis of infants treated for GN BSI at 7 NICUs within 2 health care systems. Infants were identified by review of positive blood cultures from the microbiology laboratory and electronic health records. Patients were excluded if they had polymicrobial BSI, meningitis/osteomyelitis/endocarditis, or died before completion of

OBJECTIVE: The optimal antibiotic duration for infants with Escherichia coli bacteremia with reliable, normal cerebrospinal fluid (CSF) studies and without meningitis is unknown. We evaluated whether treatment duration is associated with infection recurrence or mortality in these infants.
METHODS: This cohort study included infants discharged from Pediatrix Medical Group neonatal intensive care units between 2003-2022 with first instance of E coli bacteremia. Exclusion criteria included absence of CSF test results, CSF pleocytosis, positive culture or Gram stain from the CSF, delayed antimicr

OBJECTIVE: Escherichia coli is a leading cause of Gram-negative rod bacteremia in infants. Cerebrospinal fluid (CSF) evaluation is often deferred, and guidance on antibiotic duration is lacking in these circumstances. We evaluated whether treatment duration is associated with infection recurrence or mortality.
METHODS: This cohort study included infants discharged from Pediatrix Medical Group neonatal intensive care units (NICUs) from 1997 to 2022 with first instance of E coli bacteremia. Infants with CSF evaluation, delayed antimicrobial initiation (>2 days), or death/discharge during therap

Neonatal intestinal perforation is a life-threatening surgical emergency that is more commonly described in preterm or low-birth-weight infants. Isolated descending colonic perforation in a term neonate is rare and may be difficult to recognize when symptoms are non-specific. We report a male newborn delivered at 37+2 weeks, weighing 2.6 kg, with Apgar scores of 9 and 9 at 1 and 5 minutes. Breastfeeding was started at two hours and was initially tolerated. At 12 hours, he developed poor feeding and brownish vomiting, while stool passage was present from day one. Initial laboratory tests and ch

BACKGROUND: Escherichia coli is a common cause of invasive infections such as bloodstream and cerebrospinal fluid infections in neonates. Strains positive for the K1 capsule are considered the most common cause of such neonatal invasive infections. This assumption of K1 dominance, and indeed the population genomics of E coli causing invasive infections in general is largely unstudied. We aimed to provide a comprehensive characterisation of this pathogen population using a longitudinal isolate collection.
METHODS: In this analysis we report the findings of the SENTINEL study, a longitudinal ge

Escherichia coli (E. coli) strains expressing the capsule serotype K1 (E. coli K1) are a prevalent cause of neonatal sepsis and meningitis. The gut microbiota of healthy adults is a natural reservoir of E. coli K1, from which it can spread to extra-intestinal sites or be transmitted from mother to infant during birth. Accordingly, shifting gut colonization from potentially pathogenic E. coli strains to more benign strains could reduce the risk of disease. Here, we leverage selective pressures exerted by bacteriophage and mucosal antibodies to limit gut colonization by E. coli K1 and prevent it

BACKGROUND: Extraintestinal pathogenic Escherichia coli (ExPEC) is a leading cause of urinary tract infections, sepsis, and neonatal meningitis globally. However, global patterns linking O-serogroups, sequence types (STs), virulence, and antimicrobial resistance (AMR) remain poorly resolved. METHODS: We analyzed 12,220 ExPEC genomes, including 12,160 publicly available genomes retrieved from the NCBI database and 60 representative isolates collected and sequenced in this study. Serogroups were assigned by in silico wzm/wzt/wzx/wzy and fliC gene analysis, and phylogenies constructed using maxim

Neonatal meningitis-associated Escherichia coli (NMEC) is a formidable pathogen in veterinary medicine. The emergence of atypical, multidrug-resistant (MDR) variants complicates disease control. An Escherichia coli (E. coli) strain was isolated from the brain tissue of a deceased calf with acute meningitis. Comprehensive characterizations were performed, including whole-genome sequencing (WGS), multi-locus sequence typing (MLST), antimicrobial susceptibility testing (AST), murine pathogenicity assays, and RT-qPCR evaluation of neuroinflammatory cytokines. Results: The isolate (O18ab:H14) was i

OBJECTIVE: To determine the incidence, causative pathogens, risk factors, and clinical outcomes of early-onset sepsis (EOS) in very preterm (gestational age < 32 weeks) and very low birth weight (birth weight < 1500 g) neonates.
METHODS: This retrospective study analyzed data from the Thai Neonatal Network registry (2019-2024; total 22 neonatal units). EOS was defined as bacteremia or bacterial meningitis within 72 h of birth.
RESULTS: Among 792 neonates (median [interquartile range] gestational age, 28 [26-29] weeks; median [interquartile range] birth weight, 960 [779-1190] g), the incidenc

Intestinal bacterial translocation to the bloodstream is a route of infection for late-onset sepsis (LOS) in preterm infants, highlighting the potential of fecal microbiota profiling for early risk stratification. We aimed to identify and validate LOS-specific gut microbiota signatures. Fifty-eight preterm infants (gestational age < 30 weeks) with blood culture-proven LOS (excluding coagulase-negative staphylococci) were matched to controls (1:1) across three cohorts (Discovery (DC) n = 18; Validation 1 and 2; VC1 n = 12, VC2 n = 28). Fecal samples collected up to 10 days before LOS onset unde

OBJECTIVES: Neonatal sepsis in one twin poses a dilemma in managing the healthy-appearing sibling. Shared exposures and genetic factors increase risk, and early signs of sepsis can be subtle with rapid deterioration. However, the magnitude of this risk for the healthy twin is unclear, and management strategies vary considerably.
METHODS: We systematically searched PubMed and EMBASE on June 10th 2025 for multiples in which at least one infant (0-89 days) developed culture-confirmed Group B Streptococcus or Escherichia coli early-onset or late-onset sepsis or meningitis.
RESULTS: Of 151 sets o
