Meckel's Diverticulum in a Five-Day-Old Neonate: An Unusual Case.

Meckel's diverticulum is a congenital anomaly of the gastrointestinal tract. We report the case of a five-day-old male infant who presented to the pediatric emergency department with lethargy, feeding refusal, grunting, bilious vomiting, and abundant bowel movements. Upon observation, he was hypotonic, jaundiced, and poorly perfused, with cool extremities and a soft abdomen. He was admitted to the neonatal intensive care unit, underwent nasogastric tube insertion, and was placed on nothing by mouth. The imaging studies were not conclusive. Subsequently, he progressed to metabolic acidosis. Due

Burden, risk factors, and emerging microbiological trends of Gram-negative neonatal sepsis in Jordan: a retrospective cohort study.

BACKGROUND: Neonatal sepsis (NS) is a major cause of morbidity and mortality, particularly in low- and middle-income countries. Gram-negative bacteria are increasingly predominant and associated with severe disease. In Jordan, data on Gram-negative NS are limited, highlighting the need for local evidence to guide early detection and treatment. OBJECTIVE: To determine the incidence, risk factors, bacterial profile, and antimicrobial resistance patterns of Gram-negative NS in a tertiary Neonatal Intensive Care Unit (NICU) in Jordan. METHODS: A retrospective cohort study reviewed 4,804 blood cu

Surveillance of bacterial colonization and infection in high-risk neonates admitted to tertiary referral hospital in Indonesia.

BACKGROUND: Neonatal mortality remains high in Indonesia, particularly at our center, Dr. Hasan Sadikin General Hospital, where sepsis is the most common cause of death. However, data on colonization dynamics-which are essential for preventing Multi-drug Resistance Organism (MDRO) acquisition-remain limited, particularly in lower-middle income country (LMIC). This study aims to investigate the dynamics of bacterial colonization among high-risk neonates and determine the pattern and timing of MDRO acquisition. METHODS: Between May and December 2025, 46 neonates were included in this prospectiv

Multiple cross displacement amplification coupled with a lateral flow biosensor for ultra-rapid and highly sensitive detection of Escherichia coli in neonatal sepsis.

UNLABELLED: Escherichia coli (E. coli) is a predominant pathogen causing neonatal sepsis, and rapid pathogen identification is critical for timely antimicrobial therapy. However, conventional diagnostic methods are often limited by prolonged turnaround time, or insufficient sensitivity. In this study, we developed a multiple cross-displacement amplification assay coupled with a lateral flow biosensor (MCDA-LFB) targeting the uidA gene for rapid visual detection of E. coli. Using a set of 10 specific primers, the reaction conditions were optimized at 63°C for 40 min. The analytical limit of det

Case Report: Effect of setmelanotide treatment in a young patient with acquired hypothalamic obesity following Escherichia coli sepsis and meningoencephalitis with brain abscess.

Acquired hypothalamic obesity (aHO) is a rare and severe condition caused by injury to hypothalamic centers regulating appetite, energy balance, and neuroendocrine function. It is characterized by rapid weight gain, hyperphagia, and significant metabolic complications, and current treatment options remain limited. We report the case of a 22-month-old child who developed severe acquired hypothalamic obesity following neonatal Escherichia coli sepsis complicated by meningoencephalitis and brain abscess. Treatment with the melanocortin-4 receptor agonist setmelanotide was initiated at 0.25 mg dai

Genomic characterization of the type VI secretion system in extraintestinal pathogenic Escherichia coli.

Extraintestinal pathogenic Escherichia coli (ExPEC) is a distinctive group of Escherichia coli strains capable of causing infections outside the gastrointestinal tract of humans and certain animals. ExPEC includes the pathotypes avian pathogenic E. coli (APEC), neonatal meningitis E. coli (NMEC), sepsis-associated E. coli (SEPEC), and uropathogenic E. coli (UPEC). These pathotypes share multiple virulence factors, including the Type VI Secretion System (T6SS), a nanomachinery that has been described as important for E. coli pathogenesis. In this study, we applied genome-wide association analys

Plasmid-driven evolution of a multi-drug resistant ETEC/ExPEC hybrid E. coli associated with neonatal septicemia in lambs, a challenge for veterinary diagnostics.

AIMS: Precise characterization of Escherichia coli isolates plays a crucial role in the treatment and prevention of diseases in animal production. The classical diagnostic approach to define pathotypes of E. coli relies on detection of virulence genes. However, by targeting a limited set of genetic markers, routine Polymerase Chain Reaction (PCR)-based approaches may hinder the detection of atypical pathogenic isolates, especially in bacteria with high genomic plasticity. METHODS AND RESULTS: Genomic approaches were used to characterize a hybrid ETEC/ExPEC E. coli strain isolated from lambs s

IL-27 neutralization with and without antibiotics as an approach to prevent and treat neonatal sepsis.

Neonatal sepsis is a predominant cause of neonatal mortality and long-term morbidity that severely affects preterm and low-birth-weight newborns. Antibiotic resistance and long-term developmental issues associated with neonatal sepsis necessitate finding new and improved treatment options. IL-27 has diverse influences on the immune response, is elevated during the neonatal period compared with adulthood, and continues to rise further during infection. Elevated levels of IL-27 early in life predispose the host to impaired control of the pathogen burden and increased mortality. This study explor

Comprehensive genomic characterization of extraintestinal pathogenic Escherichia coli isolated from neonates: multiple center insights into virulence, resistance, and transmission dynamics.

BACKGROUND: Neonatal extraintestinal pathogenic Escherichia coli (ExPEC), which can cause severe long-term sequelae by systemic infections, is gradually becoming the primary pathogen threatening neonatal health. The lack of large-scale genomic epidemiological investigation hinders further understanding of neonatal ExPEC. We conducted this nationwide multicenter study to support further strategies for improving neonatal ExPEC management. METHODS: The neonatal ExPEC strains and clinical information, including antimicrobial resistance phenotype, were collected from nine centers within 7 province

Ecology of protection: probiotic biogeography and sepsis prevention in the neonatal intestine.

Neonatal infection is a leading cause of morbidity and mortality worldwide, particularly among preterm and low birth weight infants. Probiotic bacteria are widely used in peri- and postnatal care and can reduce neonatal intestinal dysbiosis. However, formulations and efficacy remain highly variable, highlighting a critical gap in our understanding of the mechanisms that drive successful interventions in this population. Furthermore, current studies on probiotic efficacy rely on indirect or relative measures of intestinal bacterial burden. Here, we directly mapped the biogeography of intestinal

Vertical Transmission of Extended-Spectrum β-Lactamase (ESBL)-Producing Escherichia coli Infection in Premature Neonates: An Emerging Concern in Developed Countries.

Early-onset neonatal sepsis (EONS) is predisposed by maternal colonisation and neonatal risk factors. We present two cases of preterm neonates with vertically transmitted extended-spectrum β-lactamase (ESBL)-producing Escherichia coli sepsis. In the United Kingdom (UK), the common organisms causing EONS episodes are susceptible to the routinely used first-line empirical antibiotics, i.e., benzylpenicillin and gentamicin. These cases observed in a local neonatal unit highlight the growing concern of drug-resistant E. coli infections in this highly vulnerable age group. Gram-negative bacterial s

Epidemiology of injury-related bloodstream infections in Queensland, Australia: a population-based data linkage study.

INTRODUCTION: Bloodstream infections (BSIs) are an important complication among injured patients, yet existing studies have focused on selected populations or specific settings, limiting the generalisability of the findings. In this study, we conducted a population-based study to examine the incidence, demographic and clinical variation of injury-related BSIs. METHODOLOGY: The study population consisted of all residents of Queensland, Australia, who developed an injury-related BSI identified between 1 January 2000 and 31 December 2019. The linked data used for this study consisted of three st

Colonization of selected antibiotic-resistant bacteria among pregnant women: a cross-sectional study at a tertiary care hospital in the Eastern Province of Sri Lanka.

INTRODUCTION: Antimicrobial Resistance (AMR) is a global health issue with profound implications for human health, economy, and agriculture worldwide. Maternal colonization with antibiotic-resistant bacteria serves as a surrogate marker for the prevalence of such bacteria in the community and also gives a clue to the likely bacteria that cause early onset neonatal sepsis. This study aims to determine the proportion of colonization with selected antibiotic-resistant bacteria, namely Methicillin Resistant Staphylococcus aureus (MRSA) and Extended spectrum beta-lactamase (ESBL) -producing E. coli

Neonatal Infections Caused by Multidrug-Resistant Bacteria: An Analysis of Prevalence, Risk Factors, and Therapeutic Implications-A Narrative Review.

Neonatal infections remain a leading cause of morbidity and mortality worldwide, particularly among preterm and low-birth-weight infants and in low- and middle-income countries. This burden has intensified with the global increase in multidrug-resistant (MDR) bacteria, especially in neonatal intensive care units, where prolonged hospitalization, invasive interventions, and exposure to broad-spectrum antibiotics promote colonization, transmission, and invasive infection. In this narrative review, we explore the epidemiology and microbiological characteristics of MDR bacterial infections in newb
