DOCK2 protects against bacterial sepsis by constraining T helper 1 response.

BACKGROUND: Sepsis is a systemic host response to infection with life-threatening consequence which ranks among the top ten causes of death worldwide. Nevertheless, our understanding of the molecular and cellular impact of sepsis remains rudimentary. METHODS: A mouse sepsis model was established through LPS induction and Escherichia coli (E. coli) infection. Flow cytometry and enzyme-linked immunosorbent assay (ELISA) were used to detect T helper 1 (Th1) cell subsets and serum pro-inflammatory cytokines in septic mice. Additionally, in vivo neutralization experiments were conducted to block I

High prevalence of antimicrobial resistance to initial empirical antibiotic therapy in neonatal sepsis in Bengaluru, India-a multicentre study.

Data about epidemiologic and microbiologic patterns of neonatal sepsis in specific regions of low- and middle-income countries can help improve management and stimulate prevention efforts. We conducted a multicentre study within a large metropolitan region in South India to describe the burden of neonatal sepsis; and identify the antimicrobial sensitivity patterns of causative organisms. In a collaborative network of six neonatal intensive care units, standardized data were collected on every admitted neonate with a positive blood culture from June 2020 to May 2022. The frequency of sepsis, th

High diversity of Escherichia coli causing invasive disease in neonates in Malawi poses challenges for O-antigen based vaccine approach.

BACKGROUND: Escherichia coli is an important cause of neonatal sepsis and the third most prevalent cause of neonatal infection in sub-Saharan Africa, often with negative outcomes. Development of maternally administered vaccines is under consideration, but to provide adequate protection, an understanding of serotypes causing invasive disease in this population is essential. We describe the genomic characteristics of a collection of neonatal E. coli isolates from a tertiary hospital in Blantyre, Malawi, with specific reference to potential protection by vaccines under development. METHODS: Neon

Interleukin-27-producing cells in gram-negative neonatal sepsis display diverse phenotypes and functions in the liver.

Neonates have increased vulnerability to life-threatening infections due to the distinct immune landscape. Interleukin (IL)-27 is a key component of this immune profile that we have previously shown to be elevated in both newborn humans and mice. IL-27 continues to increase in the serum and tissues consistent with poor outcomes during gram-negative neonatal bacterial sepsis. Presently, we dissected the IL-27 producer profile at a single-cell level using IL-27p28eGFP reporter mice in our previously established model of neonatal sepsis with luciferase-expressing K1-encapsulated Escherichia coli.

Vaginal colonization, vertical transmission rate, antimicrobial susceptibility profile, and associated factors of potential neonatal pathogens among pregnant women at public health facilities of Northeast Ethiopia.

BACKGROUND: Vaginal colonization by pathogenic bacteria increases the risk of bacterial infections such as sepsis, which is associated with high neonatal mortality. More than half a million newborn deaths occur globally each year due to infections that lead to sepsis. However, the problem is worsening in Ethiopia the evidence of vaginal colonization and vertical transmission is scarce. METHODS: A healthcare facility-based cross-sectional study was conducted in Dessie town from April 1 to June 30, 2023, among 348 pregnant women and their newborns. Socio-demographic, clinical, and related data

Enterobacterales gut colonization and late-onset sepsis in neonates: a multicentre prospective study across 18 neonatal intensive care units in six countries.

OBJECTIVES: Gram-negative bacteria cause a significant proportion of neonatal late-onset sepsis (LOS) and are associated with high mortality. Emerging evidence implicates the gut as a reservoir for invasive pathogens; however, the mechanisms of gut-to-blood translocation and the role of virulence factors remain unclear. METHODS: We conducted a secondary analysis of microbiological samples from the NeoMero-1 trial, a multicentre study of neonatal LOS. Whole-genome sequencing was performed on paired blood and faecal Enterobacterales isolates from 22 neonates with gram-negative bacteria bloodstr

The Role of clbF in the Pathogenicity of Avian Pathogenic Escherichia coli.

The genotoxin colibactin, a complex secondary metabolite, targets eukaryotic cell cycle machinery and contributes to neonatal sepsis and meningitis. Avian pathogenic Escherichia coli (APEC) XM, which produces this genotoxin, is an agent of poultry diseases with zoonotic potential. In this study, we confirmed that clbF was necessary for the APEC XM strain to produce colibactin, but it did not affect the growth, adhesion, or invasion of cells. Deletion of clbF substantially diminished both virulence and systemic dissemination, but it also changed the gene expression of the antiserum survival fac

Inhibition of IL-27 signaling regulates chemokine levels and sustains CXCR2 receptor expression on mononuclear cells to improve disease outcomes during gram-negative neonatal sepsis.

BACKGROUND: Interleukin-27 (IL-27) is a cytokine that belongs to the IL-6/IL-12 cytokine family with diverse influences on the immune response. Elevated levels of IL-27 cytokine during the neonatal period predispose neonatal mice to more severe infection. Neonatal pups deficient in IL-27 signaling exhibit improved survival and bacterial clearance with reduced systemic inflammation. However, the precise molecular mechanisms that regulate bacterial clearance and the overall immune response in IL-27 receptor a-deficient (KO) mice during neonatal sepsis remain incompletely defined. METHODS: Analy

Pathogen distribution and antimicrobial resistance among neonatal bloodstream infections in Southeast Asia: results from NeoSEAP, a multicentre retrospective study.

BACKGROUND: Progress in reducing morbidity and mortality due to neonatal sepsis has slowed in recent decades and is threatened by the global rise of antimicrobial resistance. The populous Southeast Asian region has a high burden of both neonatal sepsis and antimicrobial resistance (AMR). Despite this, there remains a lack of robust data on the epidemiology of neonatal sepsis and the prevalence of AMR within the region. METHODS: We evaluated positive blood cultures and susceptibility profiles responsible for neonatal sepsis across 10 clinical sites in five countries in South and Southeast Asia

Characterization of gut microbiota signatures in Indian preterm infants with necrotizing enterocolitis: a shotgun metagenomic approach.

INTRODUCTION: Necrotizing enterocolitis (NEC) is an inflammatory bowel disease that primarily affects preterm infants. Predisposing risk factors for NEC include prematurity, formula feeding, anemia, and sepsis. To date, no studies have investigated the gut microbiota of preterm infants with NEC in India. METHOD: In the current study, shotgun metagenomic sequencing was performed on fecal samples from premature infants with NEC and healthy preterm infants (n = 24). Sequencing was conducted using the NovaSeq X Plus platform, generating 2 × 150 bp paired-end reads. The infants were matched based

Probiotic biogeography and sepsis prevention in the neonatal intestine.

Neonatal infection is one of the leading causes of neonatal morbidity and mortality worldwide, particularly in those born prematurely or with low birth weight. Probiotic bacteria have been demonstrated to protect against the development of neonatal intestinal dysbiosis and are widely used in peri- and post-natal clinical settings. However, formulations and efficacy are highly variable, highlighting a critical gap in the current understanding of the mechanistic underpinnings of successful probiotic interventions in this population. Furthermore, current studies on probiotic efficacy largely rely
