[Bacteremia caused by Bacteroides. An analysis of 50 episodes].

During the 32 months between October 1984 and May 1987 there were 72,800 admissions in our center; 31,523 blood cultures were undertaken and in 3,831 of them (11.5%) at least one significant organism was recovered; 6.7% of blood cultures yielded anaerobic bacteria, and 3.1% Bacteroides spp. In the 1,478 instances of bacteremia recorded during that period, the relative frequencies of bacteremia by anaerobic bacteria and by Bacteroides were 8.6% and 3.9%, respectively. In overall 50 Bacteroides bacteremias (47 due to Bacteroides fragilis and 3 to Bacteroides spp), 32 (64%) were hospital acquired

[Meningitis caused by Bacteroides fragilis in children].

Bacteroides fragilis is an obligated anaerobic bacillus which forms part of the normal intestinal flora of the colon and is often seen as a common pathogen in intraabdominal infections. It is an infrequent pathogen in cases of meningitis; a review of the literature reports only eight cases of this disease in children, especially in neonates with conditioning factors such as abdominal sepsis, chronic middle ear otitis and atrial-ventricular derivations. A case of a newborn baby girl with lumbosacral myelomeningocele is reported. After the defect was surgically corrected, the wound became infect

Anaerobic bacterial bacteremia: 12-year experience in two military hospitals.

Clinical and microbiologic data of 296 patients with anaerobic bacteremia were reviewed. Anaerobes were isolated with aerobic or facultative bacteremia in 23 instances. The Bacteroides fragilis group accounted for 148 (70%) of 212 isolates of Bacteroides species. B. fragilis accounted for 78% and B. thetaiotaomicron for 14%. Among other species, there were 20 (6%) Fusobacterium organisms, 63 (18%) Clostridium isolates, and 53 (15%) anaerobic cocci. Seventy-five patients died: 40 had B. fragilis group isolates - B. fragilis, 28, and B. thetaiotaomicron, 8 - and 21 had Clostridium organisms isol

Management of empyema in children.

The antibiotic era has changed the incidence, causal factors, and gravity of empyema. Between 1977 and 1988, 27 children with empyema were surgically managed. Ten cases occurred after an operation (8 esophageal and 2 abdominal). There were 15 girls and 12 boys. The age range was newborn to 12 years. Symptoms included fever, cough, tube drainage postoperatively, anorexia, weight loss, chest pain, tachypnea, and lymphadenitis. Multiple aerobic and anaerobic organisms were cultured. Treatment included thoracentesis and antibiotics, tube thoracostomy, tube thoracostomy and bronchoscopy, decorticat

Comparison of bacteriologic culture of blood and necropsy specimens for determining the cause of foal septicemia: 47 cases (1978-1987).

Diagnosis of bacterial septicemia was confirmed by results of bacteriologic culture of antemortem blood samples and/or necropsy specimens obtained from 47 foals up to 8 days old. Gram-negative bacteria were isolated from all 47 foals, and mixed infections with more than one organism were involved in 26 (55%). Escherichia coli, Actinobacillus spp, and Klebsiella pneumoniae were the most frequent isolates, infecting 55, 34, and 23% of foals, respectively. Gram-positive bacteria and anaerobic bacteria were isolated only from foals with mixed infections with gram-negative organisms. Clostridium pe

Invasive streptococcal infections in the era before the acquired immune deficiency syndrome: a 10 years' compilation of patients with streptococcal bacteraemia in North Yorkshire.

Significant streptococcal (non-pneumococcal, non-enterococcal) bacteraemia was detected in 100 patients in two Health Districts of North Yorkshire in the decade 1978-1988. Patients with these infections accounted for 11% of the total 902 patients in the districts in whom bacteraemia was diagnosed during the period. Infection was most often seen with beta-haemolytic streptococci (52 patients) comprising Lancefield group A (Streptococcus pyogenes) (20 patients), group B (13), group C (5), group G (9), haemolytic Streptococcus milleri and non-groupable streptococci (5). The wide variety of seriou

Intraamniotic infection in low-birth-weight infants.

Intraamniotic infection (IAI) complicating births of low-birth-weight infants (less than 2500 g) was compared with IAI in births of infants weighing greater than or equal to 2500 g for differences in neonatal and maternal infectious morbidity and mortality, as well as differences in microbiological isolates in amniotic fluid. Four hundred four cases of clinically diagnosed IAI were prospectively evaluated. Thirty-seven patients (9.2%) delivered neonates less than 2500 g, and 367 patients (90.8%) delivered neonates greater than or equal to 2500 g. The low-birth-weight group had a significant in

The microbiology of neonatal peritonitis.

To review the bacteriology of neonatal intra-abdominal sepsis, we reviewed peritoneal cultures from 86 newborns undergoing operation for necrotizing enterocolitis (NEC) for the type and incidence of microorganism recovered. As a control, we conducted a similar review in 59 children with perforated appendicitis during the same period. Necrotizing enterocolitis was characterized by a lower incidence of polymicrobial contamination (1.7 organisms per patient vs 2.4 organisms per patient, NEC vs appendicitis) and an uncharacteristic pattern of isolates. Although enteric gram-negative bacilli were r

Pharmacokinetics of intravenous metronidazole in neonates.

Metronidazole is being extensively used in neonates for the prevention and treatment of anaerobic infections. However, detailed pharmacokinetic studies are not available. Thirty neonates receiving intravenous metronidazole 7.5 mg/kg at eight-hour intervals were studied with serial blood levels. The levels increased up to the third day of treatment and then reached a plateau. Neonates less than seven days of age showed higher levels than older infants. Septicemic neonates, however, showed continually rising levels. The half-life of metronidazole was 22.3 to 22.5 hours, approximately twice that

Anaerobic bacteremia in a neonatal intensive care unit: an eighteen-year experience.

A review of anaerobic bacteremia in the Neonatal Intensive Care Unit identified 29 episodes of clinically significant bacteremia occurring over the past 18 years. This experience suggested that certain clinical settings were associated with specific anaerobic infections. Although Gram-positive and Gram-negative anaerobes were isolated with similar frequency, 8 of 12 infants bacteremic within the first 48 hours of life were infected with Gram-positive, penicillin G-susceptible organisms whereas 11 of 17 infants older than 2 days were bacteremic with Gram-negative, penicillin G-resistant anaerob

Subclinical intra-amniotic infection in asymptomatic patients with refractory preterm labor.

The purpose of this prospective investigation was to determine the incidence of subclinical intra-amniotic infection in asymptomatic patients who had intact membranes and refractory preterm labor. Refractory preterm labor was defined as persistent uterine contractions despite maximum recommended doses of parenteral tocolytics, or recurrent preterm labor within three days of successful transition to oral tocolytics. Amniotic fluid was cultured aerobically and anaerobically and prepared for Gram stain and group B streptococci latex fixation test. One of 24 women had a positive latex fixation tes

Isolation of genital mycoplasmas from the blood of neonates and women with pelvic infection using conventional SPS-free blood culture media.

Standard blood culture media used in our laboratory were tested for their ability to support the growth of Mycoplasma hominis and Ureaplasma urealyticum. Small inocula (approximately 10 colony forming units per ml) of both organisms grew in diphasic tryptone soya medium but not in any of several media containing sodium polyanetholesulphonate (SPS) including a modified Schaedler broth (RWH anaerobic medium) and two BACTEC media (6B and 7D). Both organisms were inhibited even by very low concentrations of SPS but grew well in the Royal Women's Hospital (RWH) anaerobic medium when SPS was omitted

Infective endocarditis in children with congenital heart disease: comparison of selected features in patients with surgical correction or palliation and those without.

The diagnostic and prognostic features of 44 episodes of infective endocarditis in 42 children with congenital heart disease were reviewed. Endocarditis occurred in 18 patients who had not had surgical correction or palliation of the defect (non-operated group). There were 26 episodes in 24 patients who had been treated surgically (operated group) (16 open and eight closed cardiac operations). Endocarditis occurred soon after open heart surgery in eight patients and as a late complication in the other 16. It recurred in two patients (operated group). Invasive monitoring and low cardiac output

SIGNAL blood culture system for detection of bacteremia in neonates.

In the SIGNAL (Oxoid Ltd., Basingstoke, United Kingdom) blood culture system, gas produced by bacterial metabolism displaces medium from the culture bottle into an upper reservoir via a hollow needle. Displacement of media may provide a visual indication of the presence of both aerobic and anaerobic organisms in a single medium. The single-bottle SIGNAL system was compared with paired BACTEC 16B and 7D (Johnston Laboratories, Inc., Towson, Md.) radiometric system bottles by using bacterial inocula and conditions which simulated those found in neonatal and pediatric populations. The single SIGN

[5-year evaluation of bacteriological samples (CSF, blood cultures, urine) in neonatology].

We report the result of the bacteriological samples performed for 5 years (1980-1984) in neonates, referred to the neonate unit of the hospital of Clermont-Ferrand, for infection (2,894 infants). The CSF tests show aseptic meningitis in 4% of the cases and in 0.4% meningitis with bacteria on the direct examination and in culture. We emphasize the interest of carrying out the soluble bacterial antigen assay (Strepto B, Coli K1) to find out the bacteria involved in the meningitis. 13.7% of the infants have one or several positive blood culture. The germ is a E. Coli in 30% of the cases, and less

Once daily ceftriaxone for central nervous system infections and other serious pediatric infections.

Ceftriaxone has a very long serum half-life and enhanced in vitro activity against common pediatric pathogens. Therefore we evaluated the efficacy and safety of once daily ceftriaxone therapy in 57 children with serious infections including: meningitis (26 patients); ventriculitis (3); pyelonephritis (7); osteomyelitis (6); abscess (4); septic arthritis (3); sepsis (2); and miscellaneous infections (6). The most common isolates were Haemophilus influenzae (23), Escherichia coli (9) and Staphylococcus aureus (8). Ceftriaxone was given intravenously or intramuscularly in a dose of 50 mg/kg for n

Perforation of the gastrointestinal tract and peritonitis in the neonate.

A review of 60 neonates with perforation of the gastrointestinal tract and peritonitis was undertaken to evaluate efficacy of current treatment. Perforation was most frequently associated with necrotizing enterocolitis, spontaneous gastric perforation, intestinal obstruction and feeding tube perforation of the duodenum. Primary closure of proximal gastrointestinal tract perforations and resections with diversion for distal perforations were the commonly used operative procedures. Mortality was 33 per cent, with most deaths (80 per cent) a result of sepsis or its complications. Increased risk o
