Klebsiella pneumoniae (Kpn) is a leading cause of neonatal invasive bacterial disease especially in low- and middle-income countries (LMICs), with 20-30% of culture-confirmed neonatal sepsis caused by Kpn. This, coupled with the rapid emergence of multidrug-resistant strains, calls for additional therapeutic options. Maternal vaccination represents a promising strategy to confer protection to neonates through transplacental transfer of antibodies, with modelling suggesting a vaccine of moderate efficacy (~70%) could avert over 80,000 neonatal deaths annually. Advancing Kpn vaccine development

Klebsiella pneumoniae is the leading cause of neonatal sepsis in low- and middle-income countries and the foremost cause of infectious deaths in young children. K. pneumoniae isolates are becoming increasingly resistant to antibiotics, creating a great need for additional therapies or preventative strategies. Maternal vaccination serves as an attractive approach to protect from K. pneumoniae neonatal sepsis. Herein, we report a murine model of maternal immunization followed by pup challenge to evaluate putative K. pneumoniae vaccines. Dams were immunized with capsular polysaccharide bioconjuga

IMPORTANCE: Sepsis occurring in the first 72 hours of life is associated with high mortality and complications such as intraventricular hemorrhage (IVH), particularly in preterm infants. Although blood culture is the diagnostic standard, its low detection rate may underestimate the clinical burden of infection.
OBJECTIVE: To determine whether molecular detection of bacterial DNA in umbilical cord tissue improves identification of preterm infants at risk of sepsis-related death or grade IV IVH.
DESIGN, SETTING, AND PARTICIPANTS: Samples were collected from infants with a birth weight less tha

OBJECTIVE: Antibiotic therapy is typically initiated and continued for 36 or 48 hours in neonatal patients while awaiting blood culture results. Our study aims to determine time to positivity (TTP) of positive blood cultures in a level IV neonatal intensive care unit (NICU) to determine the appropriate duration of empiric antibiotics.
PATIENTS AND METHODS: This is a retrospective study of all patients admitted to the NICU with a positive blood culture from January 2021 to March 2025. Only the first positive blood culture per patient was included in the analysis unless a new organism was ident

BACKGROUND: The skin-to-blood route is traditionally considered the main pathway in Candida late-onset sepsis (LOS) development in preterm infants. However, emerging evidence suggests that the gut also serves as a source of infection. We aimed to characterize fecal mycobiota and microbiota profiles preceding onset of Candida LOS to assess the role of the preterm gut microbiome in disease development.
METHODS: This multicenter case-control study included very preterm infants (<30 weeks of gestation) with Candida LOS. Each case was matched to nonaffected controls by gestational and postnatal ag

OBJECTIVE: The randomized trial of azithromycin to reduce maternal and neonatal sepsis (the A-PLUS Trial) found substantial reduction in maternal sepsis among women receiving azithromycin and substantial non-study antibiotic use. This secondary analysis explored the effect modification of non-study antibiotics on azithromycin versus placebo on maternal and newborn infection among A-PLUS participants.
METHODS: Women ≥28 weeks gestation in labor and planning a vaginal delivery at a study hospital in seven low- and middle-income countries (Bangladesh, India [two sites], Pakistan, Guatemala, Keny

UNLABELLED: Dysbiosis in early life has been associated with the development of atopic diseases. In the INCA study, antibiotic treatment in the first week of life was associated with wheezing at 1 year and food allergies at 4-6 years. This follow-up study investigates whether these associations persist at age 9-12, and whether new associations with other atopic diseases have developed. The INCA cohort consisted of 436 children included in 2012-2015 to investigate the long-term effects of antibiotic treatment in the first week of life. Term-born infants from four Dutch hospitals were included,

BACKGROUND: Sepsis is a leading cause of neonatal mortality in sub-Saharan Africa (SSA), where microbiological diagnostic capacity and antibiotic access are limited. High antimicrobial resistance (AMR) rates limit the effectiveness of current treatment guidelines, with concern that available antibiotics are rarely adequate treatment for neonatal sepsis in the region.
METHODS: A cross-sectional online survey was electronically distributed in English, French and Portuguese to neonatal clinicians across SSA between April and June 2025. Questions focused on the management of neonatal sepsis inclu

BACKGROUND: Klebsiella pneumoniae (KPn) is a leading cause of invasive bacterial disease in African children, albeit with a scarcity of genotypic characterization.
METHODS: We sequenced invasive KPn isolates from infants ≤90 days, collected through observational hospital surveillance (n = 226) between March 4, 2019 and February 27, 2021, and between May 13, 2022 and October 31, 2023, and postmortem sampling (n = 111) between February 15, 2018 and April 18, 2023. Postmortem Kpn isolates were attributed in the causal pathway to death by the determination of the cause of death panel, which consi

In a retrospective cohort study, we evaluated the test characteristics of C-reactive protein, alone and in combination with other inflammatory markers, for invasive bacterial infection in well-appearing, febrile infants aged 8-60 days. The combination of procalcitonin and C-reactive protein demonstrated a sensitivity of 91.4% (95% confidence interval: 84.7-95.8) and specificity of 76.5% (95% confidence interval: 75.3-77.6).

Neonates are particularly susceptible to infections because of a reduced ability to fight bacterial pathogens while simultaneously exhibiting enhanced inflammatory damage. Neonatal neutrophilic cells (NNC) are a heterogeneous population of innate immune cells, which differ substantially from adults. They display functional deficits in pro-inflammatory responses and include a suppressive subpopulation known as myeloid-derived suppressor cells (MDSC). It remains unclear whether pro- or anti-inflammatory properties of NNC predominate immediately after birth. This study aimed to clarify the role o

BACKGROUND: Neonatal septic shock is a life-threatening emergency with high mortality. Campylobacter jejuni is a rare but potentially severe pathogen in neonates. CASE PRESENTATION: We report a 24-day-old female neonate admitted in July 2025 with fever, diarrhea, and rapid progression to septic shock with multiple organ dysfunction. Blood culture confirmed Campylobacter Jejuni infection. The infant received comprehensive treatment including respiratory support, fluid resuscitation, vasoactive drugs, and targeted antibiotics (meropenem plus azithromycin). She fully recovered and was discharged

BACKGROUND: India contributes to around one-fifth of all preterm births globally. Reproductive tract infections (RTIs), colonization of fetal membranes, and associated maternal inflammatory immune response are recognized as a major trigger associated with preterm birth. Azithromycin is a broad-spectrum macrolide antibiotic with anti-inflammatory properties and is commonly used for symptomatic reproductive tract infections. Evidence from African trials suggests that oral azithromycin in the antenatal period can substantially reduce preterm births in malaria holoendemic settings (where all pregn

BACKGROUND: The umbilical cord is a natural site for bacterial colonization, and insufficient care could lead to omphalitis, sepsis, or delayed separation. While antiseptics and dry care are broadly used, breast milk utilization remains common in many cultures due to its antimicrobial and anti-inflammatory properties.
OBJECTIVE: To compare the effect of breast milk utilization, open dry care, soap-and-water cleansing, and alcohol application in terms of cord separation time and omphalitis incidence.
METHODS: A prospective randomized trial of 200 healthy newborns (1-7 days old) was assigned t

Bacterial sepsis is a leading cause of neonatal mortality. Pro-inflammatory MR1-restricted T (MR1T) cells may help protect from sepsis by recognizing bacterial pathogens producing the canonical MR1 antigen 5-OP-RU. Most adult MR1T cells are mucosal-associated invariant T (MAIT) cells expressing a semi-invariant TCRα, while neonatal MR1T cells express diverse TCRα chains. Here, we perform combined single-cell RNA-sequencing and TCR repertoire analyses on MR1/5-OP-RU tetramer-positive cells from neonatal cord blood (CB) and adult blood. Compared to adult MR1T cells, CB MR1T cells exhibit greater

BACKGROUND: Sepsis is an important cause of morbidity and mortality in foals. Early diagnosis can improve outcome but is complicated by non-specific clinical signs and delayed confirmation of infection via blood culture. Molecular assays represent a rapid and more sensitive alternative.
OBJECTIVES: To evaluate and compare bacterial load and composition in blood and blood culture media (BCM) of sick and healthy foals using molecular assays and to compare results with traditional bacterial culture.
STUDY DESIGN: Cross-sectional observational clinical study.
METHODS: Thirteen septic foals, 10

Neonatal dengue is an underrecognized infection that can mimic neonatal sepsis, leading to diagnostic challenges, particularly in endemic regions and in the context of perinatal or household exposure. We report the case of a term male neonate, delivered by emergency cesarean section for abruptio placenta, who developed early respiratory distress, thrombocytopenia, and petechial rash shortly after birth. Initial management included respiratory support and empirical antibiotics for suspected sepsis; however, bacterial cultures remained negative. Dengue infection was confirmed by polymerase chain

Klebsiella pneumoniae is a gram-negative, opportunistic pathogen, with high rates of antimicrobial resistance, and is responsible for a wide range of infections of the urinary tract, lungs, and bloodstream, among others. Disease burden is particularly high in neonates, where K. pneumoniae is a leading cause of sepsis. Renewed interest in vaccine development against this critical priority pathogen has focused on this vulnerable population. Vaccination in pregnancy is a promising approach for prevention of neonatal sepsis, however efforts to understand the dynamics, specificity and function of m
