Case Report: Severe persistent transaminitis during pregnancy: a complex differential diagnosis involving suspected toxoplasmosis, possible drug-induced liver injury, and intrahepatic cholestasis.

INTRODUCTION: Severe transaminitis during pregnancy represents a diagnostic challenge because multiple etiologies may coexist. We report a pregnant woman with suspected toxoplasmosis, intrahepatic cholestasis of pregnancy, and possible drug-induced liver injury who developed prolonged severe liver dysfunction despite extensive investigation and treatment. CASE PRESENTATION: A 33-year-old primigravida was referred at 15 weeks' gestation because of positive Toxoplasma gondii serology and elevated liver enzymes (ALT/AST 460/150 U/L). Despite treatment with spiramycin, later changed to pyrimetham

Toxoplasma Transmission to the Fetus during the Periconceptional Period.

In Tunisia, toxoplasmosis screening in pregnancy is common but not regulated. This study evaluated congenital toxoplasmosis after primary infections during the periconceptional period and the accuracy of diagnostic tools. We analyzed 209 women at the Pasteur Institute of Tunis (2005-2021) with primary infections classified as periconceptional (n = 50, 23.9%), first trimester (n = 121, 57.9%), or second trimester (n = 38, 18.2%). Congenital toxoplasmosis was confirmed in 29 cases (13.9%), diagnosed prenatally in 22 cases, and diagnosed postnatally in 7 cases. Transmission rates were 14%, 8.2%,

Malaria in children aged <6 months: a narrative review of current evidence, recommendations and practice gaps.

BACKGROUND: Although clinical malaria occurs far less frequently in newborns and young infants aged  <6 months than in older children, its importance and public health relevance should not be ignored. This population is often perceived to have protection against malaria, and making a diagnosis of malaria in this age group can be challenging, as the symptoms generally overlap with those of many other common conditions in infancy, possibly leading to misdiagnosis or treatment delay. This narrative review synthesises current evidence, explores existing treatment recommendations, and highlights pr

Irrational and Inappropriate Use of Antifungals in the NICU: A Narrative Review.

Invasive Candida infections in the neonatal intensive care unit (NICU) are associated with significant morbidity and mortality, particularly among extremely preterm neonates. Early treatment with antifungals is critical to improve survival rates and avoid long-term adverse outcomes. Prevention with antifungal prophylaxis in high-risk neonates has been shown to reduce the prevalence of invasive Candida infections effectively. However, the irrational and/or inappropriate use of antifungals has been documented. This narrative review aims to provide an overview of the rationales for the inappropri

Antifungal susceptibility, FKS1 gene expression, and virulence determinants of Candida albicans in pediatric and neonatal candidiasis.

BACKGROUND: Candida albicans is a major cause of invasive fungal infections in pediatric patients, including subgroups of neonates and young infants, particularly in intensive care settings where premature and low birth weight infants are at high risk. Management is increasingly complicated by antifungal resistance, notably to echinocandins. Understanding basal FKS1 expression, antifungal susceptibility, and virulence traits in neonatal isolates, as a subgroup within pediatric patients, provides preliminary insights for clinical management. METHODS: A cross-sectional study was conducted on ped

Navigating Tuberculosis in Pregnancy and Lactation: A Review of Maternal and Neonatal Considerations.

Tuberculosis (TB) is an infectious disease caused by Mycobacterium tuberculosis. Despite the availability of effective treatments and advances in diagnostic methods, TB remains the leading cause of death from infectious disease globally, with its incidence tending to increase. Pregnant women constitute a population group with particular characteristics, as the diagnosis and treatment of certain conditions can be challenging. Early diagnosis and monitoring of TB by a multidisciplinary team are crucial to guide treatment and reduce complications. Congenital TB, although uncommon, is a serious co

Monitoring Rubella Immunity and Transmission in Central India Amidst Elimination Efforts.

India along with other countries in the World Health Organization (WHO) Southeast Asia region has set a goal to eliminate rubella by December 2026 through active tracking and vaccination program. However, the incidence of sporadic events and outbreaks in recent years has posed a significant challenge to this goal. To access the ongoing infections and the effectiveness of immunization efforts, we evaluated the presence of anti-rubella IgM and IgG antibodies, which serve as indicators of ongoing infection and immune protection, respectively. Serum samples from individuals presenting with suspect

Diagnostic Complexities in Congenital Tuberculosis: Insights From Preterm Twins Conceived via In Vitro Fertilization and a Literature Review.

Congenital tuberculosis (CTB) poses significant diagnostic challenges, particularly in preterm infants. This case report describes the discordant clinical presentation of CTB in dizygotic twins conceived via in vitro fertilization (IVF) and born at 31+6 weeks' gestation. Both twins presented with nonspecific symptoms (fever and respiratory distress) on postnatal day (PND) 31 and 25, respectively, but followed distinct clinical courses: Twin 1 developed disseminated disease with facial palsy and central nervous system (CNS) involvement, whereas Twin 2 presented with a cervical mass and bone des

E-protein variability in Zika virus strains: A possible new O-glycosylation site and its implications.

Background and objectives Zika virus (ZIKV) is a flavivirus transmitted by the bite of infected Aedes mosquito. In 2015-16, Brazil reported cases of ZIKV virus infection followed by Guillain-Barre syndrome and congenital birth defects. India has reported ZIKV virus infections sporadically since 2016, without adverse events. This prompted us to conduct this in-silico investigation and identify reasons for this variation. The objective was to study ZIKV envelope protein (E-protein) to identify possible mutations and their potential role in virus entry into the host cell. Methods Using multiple s

Congenital Toxoplasmosis With Atypical Neuroimaging: Hydrocephalus and Bilateral Chorioretinitis Without Intracranial Calcifications.

We report a rare case of congenital toxoplasmosis in a 2-month-old female infant from rural Nepal, born to a primigravida mother without prenatal care. The infant presented with progressive lethargy, feeding difficulties, intermittent fever, and progressive macrocephaly. Neuroimaging revealed obstructive hydrocephalus and multiple cerebral ring-enhancing lesions without intracranial calcifications. Ophthalmologic examination demonstrated bilateral chorioretinitis. Serologic testing confirmed congenital toxoplasmosis. Despite initiation of standard therapy with pyrimethamine, sulfadiazine, and

Expiration Date of Extemporaneous Pyrimethamine Formulations for Treating Congenital Toxoplasmosis: Physicochemical and Microbiological Evaluation.

This study evaluated the stability of extemporaneous pyrimethamine oral suspensions prepared from commercial tablets using 85% sucrose or 70% sorbitol as vehicles. A validated HPLC method was employed to quantify pyrimethamine and assess its chemical stability. Physicochemical parameters including macroscopic appearance, pH, particle size, and drug content were monitored for 90 days at 5 and 25 °C. Both formulations maintained acceptable pH values and pyrimethamine content within pharmacopeial limits. Particle size increased over time, likely due to aggregation, while appearance remained uncha

Treatment of congenital toxoplasmosis with pyrimethamine combined with sulfadiazine in a Chinese neonate: a case report.

This work reports a case of neonatal congenital toxoplasmosis in which the standard regimen of pyrimethamine combined with sulfadiazine was administered under conditions of limited drug accessibility in China, with multidisciplinary collaboration and long-term follow-up. The infant was a male born at 36⁺⁶ weeks of gestation. Postnatal serological screening and cerebrospinal fluid (CSF) next-generation sequencing (NGS) confirmed the diagnosis of mildly symptomatic congenital toxoplasmosis. In the initial phase of treatment, due to the lack of routine access to standard therapeutic drugs in Chin

Congenital multidrug-resistant tuberculosis (MDR-TB) in an extremely low birth weight neonate: diagnostic challenges, management and a narrative review.

BACKGROUND: Congenital tuberculosis(TB) is rare and often fatal, particularly with multidrug-resistant strains. Neonatal diagnosis is challenging due to nonspecific features and low-yield microbiology. Management in extremely preterm infants is further complicated by limited evidence on second-line drug dosing, safety, and monitoring. METHODS: We describe a single patient case alongside a narrative review of the relevant literature. Literature search was conducted in major databases using predefined keywords. Reports describing congenital/presumed congenital drug-resistant tuberculosis in neo
