Navigating Tuberculosis in Pregnancy and Lactation: A Review of Maternal and Neonatal Considerations.

Tuberculosis (TB) is an infectious disease caused by Mycobacterium tuberculosis. Despite the availability of effective treatments and advances in diagnostic methods, TB remains the leading cause of death from infectious disease globally, with its incidence tending to increase. Pregnant women constitute a population group with particular characteristics, as the diagnosis and treatment of certain conditions can be challenging. Early diagnosis and monitoring of TB by a multidisciplinary team are crucial to guide treatment and reduce complications. Congenital TB, although uncommon, is a serious co

Diagnostic Complexities in Congenital Tuberculosis: Insights From Preterm Twins Conceived via In Vitro Fertilization and a Literature Review.

Congenital tuberculosis (CTB) poses significant diagnostic challenges, particularly in preterm infants. This case report describes the discordant clinical presentation of CTB in dizygotic twins conceived via in vitro fertilization (IVF) and born at 31+6 weeks' gestation. Both twins presented with nonspecific symptoms (fever and respiratory distress) on postnatal day (PND) 31 and 25, respectively, but followed distinct clinical courses: Twin 1 developed disseminated disease with facial palsy and central nervous system (CNS) involvement, whereas Twin 2 presented with a cervical mass and bone des

Congenital multidrug-resistant tuberculosis (MDR-TB) in an extremely low birth weight neonate: diagnostic challenges, management and a narrative review.

BACKGROUND: Congenital tuberculosis(TB) is rare and often fatal, particularly with multidrug-resistant strains. Neonatal diagnosis is challenging due to nonspecific features and low-yield microbiology. Management in extremely preterm infants is further complicated by limited evidence on second-line drug dosing, safety, and monitoring. METHODS: We describe a single patient case alongside a narrative review of the relevant literature. Literature search was conducted in major databases using predefined keywords. Reports describing congenital/presumed congenital drug-resistant tuberculosis in neo

Congenital Tuberculosis in a Stillborn Calf.

Bovine tuberculosis is a zoonotic infectious disease of cattle caused by the bacterium Mycobacterium bovis. In adult cattle, transmission is mainly via the respiratory route, whereas, in young calves, oral infection is also common. Congenital tuberculosis is rare. The purpose of this study was to describe in utero infection of a bovine stillbirth. The fetus was necropsied and tissue samples were processed for histopathology, bacteriology and PCR; a sample of the isolated strain was genotyped using spoligotyping. The whole herd was tuberculin skin tested and the dam's serum was also assessed fo

Immunopathological signatures in congenital tuberculosis-a case-matched study.

INTRODUCTION: Congenital tuberculosis (CTB) is a rare and life-threatening condition with a mortality rate of approximately 53% even with clinical intervention, which is markedly higher than that of adult tuberculosis (ATB). To date, the phenotypic and functional characteristics of immune cells in CTB neonates remain largely uncharacterized. METHODS: In this case-control study, we enrolled nine CTB neonates and nine paired healthy control (pHC) neonates, collecting basic clinical characteristics of the infants and their mothers, and comparing routine blood test results, C-reactive protein (CR

Comparative analysis of clinical manifestations between congenital and non-congenital tuberculosis in infants.

OBJECTIVE: To compare the clinical manifestations and associated characteristics of congenital tuberculosis (CTB) and non-congenital tuberculosis (NCTB) in infants, thereby improving early recognition and enabling precision treatment. METHODS: A retrospective analysis was conducted of 72 tuberculosis cases in infants aged ≤ 1 year admitted to Kunming Children's Hospital between 2014 and 2024. Given the inherent difficulty in distinguishing congenital from early postnatal acquisition in retrospective studies, patients were classified into a CTB group (n = 23) and an NCTB group (n = 49) using m

Congenital tuberculosis transmitted via the placenta: identification by metagenomic next-generation sequencing.

BACKGROUND: Congenital tuberculosis (CTB) is a rare disease with high mortality in neonates. Early diagnosis is crucial but often delayed due to atypical clinical and imaging manifestations. CASE PRESENTATION: We report a 36-day-old female infant presenting with recurrent fever. Laboratory data showed leukocytosis and neutrophilia with mildly elevated C-reactive protein. Chest computed tomography revealed extensive ground-glass opacities, multiple subpleural nodules, and necrotic hilar and mediastinal lymphadenopathy. The asymptomatic mother was subsequently found to have diffuse miliary nodu

The First Documented Case of Congenital Drug-Resistant Tuberculosis in Indonesia: A Case Report.

Congenital drug-resistant tuberculosis (DR-TB) is a rare condition with no standardized treatment. Congenital TB itself is a life-threatening condition. The clinical manifestations can be nonspecific, particularly in preterm infants, which can further reduce diagnostic awareness. We report a case of a 32-week preterm baby with rifampicin-resistant TB. She was born with respiratory distress and early-onset jaundice, initially suspected of sepsis and admitted to the NICU. The infant initially showed clinical improvement at 3 days of life, but at 23 days of life, the clinical condition worsened d

Use of Levofloxacin in Congenital Tuberculosis: Two Case Reports and a Review of the Literature.

BACKGROUND: Congenital tuberculosis (CTB), a serious and often fatal infection acquired in utero or during delivery, continues to pose significant challenges in treatment. METHODS: We describe 2 cases of CTB treated with levofloxacin from the inpatient department of Anhui Provincial Children's Hospital. In addition, we performed a literature review and identified 18 eligible cases from 17 published case reports of CTB treated with levofloxacin-containing antituberculosis regimens, which were reported in both Chinese and English journals. These cases were analyzed together with our patients.

Cerebral tuberculoma in pregnancy (Jan 1975-May 2025): a systematic review and descriptive analysis of 33 published cases.

BACKGROUND: Cerebral tuberculoma is an infrequent but life-threatening manifestation of tuberculosis whose diagnosis and treatment are uniquely complicated by pregnancy-related physiological changes. Evidence to guide clinicians remains fragmented and largely anecdotal. METHODS: We systematically searched MEDLINE, LILACS and grey literature to May 2025 for reports of cerebral tuberculoma in pregnant or postpartum women. Twenty-seven studies (24 case reports, 2 descriptive studies, 1 case series) from 20 countries met inclusion criteria, yielding individual-level data on 33 patients. Demograph

Congenital tuberculosis presenting as acute cervical lymphadenitis in a 28-week preterm infant.

Congenital tuberculosis is a rare infection in infants with extremely low birthweight. We report the case of an infant born at 28 weeks' gestation who developed isolated, cervical lymphadenitis at age 84 days. Despite the infant's continuous hospitalisation in the neonatal intensive care unit and a negative maternal history, Mycobacterium tuberculosis was identified through culture and molecular testing of surgical drainage fluid. The diagnosis was substantially delayed due to several factors: a low index of clinical suspicion based on the infant's isolation from external sources, the presence

Effects of Stannous Fluoride Toothpaste and Chios Mastiha Toothpaste on the Prevention of Enamel Erosion.

PURPOSE: Erosive tooth wear (ETW) is an increasingly prevalent condition characterised by the loss of dental hard tissue due to repeated effects and interactions of acids and mechanical forces. The aim of this study was to investigate the primary preventive effect of salivary pellicle combined with either stannous fluoride (SnF2) toothpaste or mastic toothpaste against ETW in permanent teeth. MATERIALS AND METHODS: Using a three-arm invitro model, 54 enamel samples (18 in each group) were prepared with a salivary pellicle alone (control), salivary pellicle with SnF2 toothpaste, or salivary pe

Immune-neuroendocrine reactivity and features of tuberculosis in pregnancy.

The combination of tuberculosis and pregnancy always raises questions about therapy, the specialness of management of pregnancy, obstetrics, postpartum period, and lactation; the effect of therapy on fetal development and the peculiarities of the tuberculosis course. Until recently, tuberculosis and pregnancy were considered a rare combination, but with the growing problem of HIV infection and worsening tuberculosis screening among adults, this combination has become quite common. Moreover, cases of congenital tuberculosis in newborns have begun to emerge. In this review, we analyzed features

Customized feeding plate for nutritional and respiratory support in an infant with Pierre Robin sequence and cleft palate complicated by severe respiratory infections: A case report.

Pierre Robin sequence (PRS) is a congenital condition characterized by micrognathia, glossoptosis, and airway obstruction, often accompanied by cleft palate. Feeding difficulties and respiratory compromise pose significant challenges in early management. While feeding plates are commonly used to improve feeding and airway stability, their application in infants with severe respiratory infections remains underreported. The aim of this study was to describe the challenges of impression-taking and feeding plate fabrication in an infant with PRS complicated by multiple congenital anomalies and sev

Congenital tuberculosis in preterm infants in a high-burden setting in southwest China: a single-center cross-sectional study.

BACKGROUND: Congenital tuberculosis (CTB) is an extremely rare and potentially life-threatening infection in premature infants that is frequently misdiagnosed. There is still a lack of thorough characterization of this condition in preterm neonates, particularly in high TB-burden settings. METHODS: Premature infants with CTB were identified from hospital medical records from January 2016 to December 2023 in a high-burden, resource-limited neonatal intensive care unit (NICU) in southwest China. Diagnosis was based on etiology and clinical evidence. Data extracted included demographics, materna
