OBJECTIVE: To assess the epidemiology, clinical features, and genotypic characteristics of congenital toxoplasmosis (CT) in Martinique over a 12-year period.
STUDY DESIGN: We conducted a retrospective study of CT cases diagnosed at the University Hospital of Martinique between 2010 and 2022. We then compared the clinical, genotypic, and epidemiological data of these cases with those from the Limousin region in mainland France.
RESULTS: Twenty-five CT cases were identified in Martinique, with no pregnancy terminations or in utero deaths. Clinical findings showed low severity at birth, with oc

Congenital toxoplasmosis results from transplacental transmission of Toxoplasma gondii during maternal primary infection. While the classic triad-hydrocephalus, intracranial calcifications, and chorioretinitis-defines severe forms, hypothalamic-pituitary axis involvement is extremely rare. Endocrine complications, particularly central hypothyroidism, are seldom described in this context. We report a preterm newborn at 35 weeks' gestation, representing the first documented case of isolated central hypothyroidism associated with severe congenital toxoplasmosis. Neuroimaging revealed major triven

During pregnancy, a finely regulated balance between inflammatory and regulatory immune responses supports maternal-fetal tolerance while preserving defense against pathogens. Various congenital infections can disrupt this equilibrium and impair fetal development. Amniotic fluid plays a central role in fetal protection, particularly through its cytokine composition, which mirrors the immune status during pregnancy. In the context of congenital infection (bacterial, viral, or parasitic), the cytokine profile is altered, thereby influencing fetal prognosis. Numerous studies have examined the imp

Congenital toxoplasmosis (CT) results from vertical transmission of Toxoplasma gondii during maternal infection in pregnancy. Early diagnosis in newborns is crucial to initiate timely therapy and prevent long-term sequelae. The IgM Immunosorbent Agglutination Assay (ISAGA) has historically been considered an important diagnostic tool for CT; however, its recent market withdrawal necessitates alternative approaches. We conducted a retrospective observational study at Fondazione IRCCS Policlinico San Matteo, Pavia, Italy, including 44 newborns born to mothers with confirmed toxoplasmosis between

The impact of climate change on environmental pathogens is a question whose importance will amplify in coming years. The protozoan parasite and global zoonosis Toxoplasma gondii is one such: empirical evidence indicates that oocyst survival is reduced at high temperatures. Paradoxically, a decline in incidence of T. gondii infections could amplify the burden of this disease, as the most damaging outcome occurs subsequent to first infection during pregnancy, and reductions in the incidence of the infection will increase the average age of first infection. We blend models of infection dynamics r

Toxoplasma gondii is an apicomplexan parasite causing reproductive failure in small ruminants. In this study, we evaluated the capacity of an inactivated vaccine to prevent reproductive failure caused by congenital toxoplasmosis in sheep. The vaccine is based on an antigen extract obtained from a low passage T. gondii Type III isolate (TgPigSp1) preserving the ability to spontaneously produce cysts in vitro and following a procedure involving parasite inactivation via hyperosmotic shock and membrane proteins solubilisation. The vaccine prototype, consisting of 40 µg of parasite extract adjuvan

OBJECTIVES: To describe a rare case of sulfadiazine-associated nephrolithiasis in an infant treated for congenital toxoplasmosis and to highlight the clinical implications for monitoring and management during prolonged therapy. METHODS: This study describes a single case report of an 11-month-old male infant diagnosed with congenital toxoplasmosis who developed fever, urinary tract infection, bilateral nephrolithiasis (maximum diameter approximately 1.2 cm), hydronephrosis, and renal dysfunction after 9 months of continuous treatment with sulfadiazine, pyrimethamine, and folinic acid. RESULTS:

The search for effective therapies to eradicate Toxoplasma gondii, the causative agent of toxoplasmosis, has spanned decades with limited success. Current drugs, including sulfadiazine, spiramycin, and pyrimethamine, face limitations such as parasite resistance, high cellular toxicity, inconsistent efficacy in the acute phase of disease, and the lack of effect in the chronic phase. This study evaluates an innovative adjunctive approach using light-emitting diode phototherapy (BLP) to control T. gondii acute infection during pregnancy. For that, pregnant female Swiss mice (8-9 weeks) were orall

INTRODUCTION: Toxoplasmosis is a widespread protozoan infection that exhibits increased pathogenicity in its congenital form. The diagnosis and management of this condition require high accuracy and rapid intervention throughout the maternal-fetal and neonatal periods.
AREAS COVERED: The aim of this review is to provide an updated overview of screening and diagnostic confirmation strategies for congenital toxoplasmosis, covering prenatal screening, neonatal assessment and long-term follow-up. It also examines therapeutic options for prenatal treatment based on gestational age.

BACKGROUND: Toxoplasma gondii is a major congenital parasitic pathogen that can be transmitted vertically and is associated with a wide spectrum of adverse fetal and neonatal outcomes. In Iran, data on concurrent maternal-neonatal serological assessments remain limited, particularly in hospital-based settings. Neonatal serology is often interpreted without parallel maternal data or contextual information on awareness and risk factors. This hospital-based cross-sectional study in Yasuj, southwestern Iran, aimed to determine the seroprevalence of anti-Toxoplasma IgG and IgM among hospitalized in

BACKGROUND: Unlike mouse models of congenital toxoplasmosis, pregnant sheep models provide the opportunity to evaluate treatment strategies that more closely resemble clinical practice in pregnant women, including chemotherapeutic interventions initiated after specific IgM seroconversion. BKI-1748, which targets Toxoplasma gondii CDPK1 and MAPKL-1 protein kinases, has demonstrated an excellent safety profile and efficacy when administered repeatedly to pregnant sheep, starting at 2 and 7 days after challenge.
METHODS: In this study, treatment was initiated at day 14 post-infection (p.i.), fol

Gestational infection with Toxoplasma gondii remains a significant concern for the adequate progression of pregnancy, fetal health, and neonatal well-being. The current status of toxoplasmosis screening in Spain is heterogeneous, with a growing trend toward abandoning universal screening for this infection during pregnancy. However, its incidence has only declined marginally, and based on the current scientific evidence, we consider it beneficial to support gestational screening for toxoplasmosis.

Congenital toxoplasmosis, resulting from vertical transmission of Toxoplasma gondii during pregnancy, poses serious risks to the fetus. This study investigated the efficacy and therapeutic potential of spiramycin, spiramycin-loaded chitosan nanoparticles and Aluvia (a combination of lopinavir and ritonavir) in mitigating fetal infection during late gestation in Swiss albino mice. Forty pregnant mice were included in the study and divided into control and experimental groups (10 and 30 respectively). Each pregnant mouse was injected subcutaneously with 30 tachyzoites of the virulent T. gondii R

Congenital toxoplasmosis is a parasitic infection with the potential for severe central nervous system involvement. We assessed agreement between cranial ultrasound (US) and computed tomography in 73 affected infants. Agreement between modalities was fair (κ = 0.33; 95% confidence interval: 0.10-0.57), with US showing limited sensitivity (52%). These findings support a selective use of computed tomography when clinical or laboratory suspicion remains high despite normal US results, highlighting its complementary role in evaluation.

Toxoplasmosis affects approximately one-third of the global population. Despite the high seroprevalence in Brazil, contemporary data on the incidence and clinical spectrum of congenital toxoplasmosis (CT) in tertiary-care settings remain limited. Few studies have specifically addressed the incidence of symptomatic CT, limiting the identification of risk factors, preventive strategies, and optimal patient management, particularly in the context of recent healthcare disruptions and environmental changes. This study aims to determine the incidence of exposure to gestational toxoplasmosis and CT a

Toxoplasma gondii infection poses a substantial global health burden, yet transmission pathways and population susceptibility in urban informal settlements remain poorly characterised, particularly for women of childbearing age. We analysed archived samples from a cross-sectional serosurvey of 728 children and adolescents aged 4-18 years living in a marginalised urban community in Salvador, Brazil, to characterise exposure patterns and identify demographic, socioeconomic, behavioural, household, and environmental factors associated with seropositivity and to assess spatial heterogeneity in exp

BACKGROUND: Current neonatal diagnostic tests for congenital toxoplasmosis (CT) have limited performance, which is particularly problematic in resource-limited settings and where monitoring for toxoplasmosis during pregnancy is lacking. The TOXODIAG study (NCT03385499) aimed to detect specific IgGs as soon as they appear in pregnant women with acute infection with Toxoplasma gondii (Tg) and their newborns suspected of infection.
METHODS: Seventy pregnant women were included in five perinatal centers in Paris, France. They were divided into three groups based on their Toxoplasma seroconversion

Maternal infection with Toxoplasma gondii can disrupt fetal brain development, yet the mechanisms underlying the long-term neurobehavioral consequences of congenital toxoplasmosis remain incompletely understood. In this study, we investigated the effects of congenital toxoplasmosis on adult offspring behavior, with particular emphasis on how the gestational timing of maternal infection and offspring sex influence the nature and severity of these alterations. We also evaluated neuroinflammation, neurotrophism, and N-methyl-d-aspartate receptor (NMDAR) subunit expression. Pregnant dams were infe

Congenital toxoplasmosis has become rare in France. Primary prevention is based on hygiene and dietary measures. Serological screening is mandatory in France. When maternal infection occurs in pregnancy, prophylactic treatment (spiramycin for first trimester; pyrimethamine and sulfonamide for second or third-trimester infections) reduces the risk of placental transmission. Prenatal diagnosis is performed by amniocentesis. In case of congenital toxoplasmosis, prompt treatment with pyrimethamine and sulphonamide reduces the risks of brain or eye damage; long-term follow-up of the child is requir
