Potassium Iodide in Dermatology- Recent Advances in Mechanism of Action, Preparation, Uses and Adverse Effects.

Potassium iodide (KI) is a simple yet versatile drug. It has been successfully used for the treatment of various diseases in dermatology, but there are very limited studies on the exact mechanism of action, preparation, and dosage of potassium iodide, causing concerns regarding its toxicity. This article reviews the multimodal mechanism of action of KI and standardises the method of preparation of a saturated solution of potassium iodide (SSKI) on an OPD basis. It also lists the existing and newer indications, optimal dosages, adverse effects, and contraindications of potassium iodide, thereby

Cutaneous Cryptococcosis Manifested as a Large Cystic Mass: A Rare Manifestation of Cryptococcus Infection.

Cryptococcus infection is an invasive fungal infection common in immunocompromised hosts, especially in organ transplant recipients and in patients with HIV. Its presentation varies from localized skin lesions to systemic disseminated infection involving the lungs and the central nervous system (CNS). We present the case of a 50-year-old woman with diabetes mellitus type 2 (DM-2), end-stage renal disease (ESRD) status post deceased donor kidney transplantation seven and a half years ago who presented with a low-grade fever, cough, nausea, vomiting, and a large cystic mass on the right foot. A case of primary cutaneous Cryptococcus neoformans infection.

BACKGROUND: Cryptococcosis is an infectious disease caused by encapsulated heterobasidiomycete yeasts. As an opportunistic pathogen, cryptococcal inhalation infection is the most common. While Primary cutaneous cryptococcosis is extremely uncommon. CASE PRESENTATION: A 61-year-old woman with a history of rheumatoid arthritis on long-term prednisone developed a red plaque on her left thigh. Despite initial antibiotic treatment, the erythema worsened, leading to rupture and fever. Microbiological analysis of the lesion's secretion revealed Candida albicans, Pseudomonas aeruginosa, and methicill

3D Printable Alginate-Chitosan Hydrogel Loaded With Ketoconazole Exhibits Anticryptococcal Activity.

Natural polymers have recently been investigated for various applications, such as 3D printing and healthcare, including treating infections. Among microbial infections, fungal diseases remain overlooked, with limited therapeutic options and high recurrence. Cutaneous cryptococcosis is an opportunistic fungal infection triggered by mechanical inoculation or hematogenous dissemination of the yeast that causes cryptococcal pneumonia and meningitis. Every year, Cryptococcus neoformans endanger the lives of immunosuppressed hosts, resulting in 180,000 deaths per year. Nonetheless, healthy individu

Concurrent Primary Cutaneous Cryptococcosis and Bowen's Disease in a Patient With Multiple Sclerosis on Fingolimod: A Rare Case Report and Review of Literature.

Fingolimod is an oral disease-modifying treatment used for the relapsing-remitting forms of multiple sclerosis, which may render patients susceptible to opportunistic infections and lead to an increased risk of skin cancer. We report a 56-year-old woman with multiple sclerosis in remission on fingolimod treatment for five years presenting with the following skin lesions. Our patient presented with a non-healing ulcerated erythematous lesion on the left lower abdomen for six months and a crusted erythematous plaque on the forehead that had not healed for one year. Skin biopsy from the abdominal

From Histoplasmosis to Cryptococcosis: A diagnostic journey of pediatric scalp abscess.

A 3-year-old boy being treated for disseminated tuberculosis with antitubercular drugs and steroids developed scalp swelling. Needle aspirate revealed yeasts inside macrophages, initially diagnosed as Histoplasmosis but later confirmed as Cryptococcosis. Treatment with liposomal amphotericin B and fluconazole resulted in lesion resolution. Here we report a rare instance of disseminated cryptococcosis with skin involvement, possibly the first documented pediatric case of cryptococcosis from Odisha.

Primary cutaneous cryptococcosis - History, concepts, clinical and therapeutic update.

Cryptococcosis is a disease caused by fungi of the genus Cryptococcus, with the species Cryptococcus neoformans and Cryptococcus gattii being recognized as pathogenic. Cutaneous cryptococcosis can be classified as "secondary", developing from a previous systemic disease, or, on the contrary, "primary", resulting from transcutaneous inoculation of the agent. It can also be classified as "disseminated cutaneous cryptococcosis", when there is an associated systemic disease, or "localized", when it is restricted to the skin. This article uses the term "primary cutaneous cryptococcosis" because it

The management and outcome of cryptococcosis in patients with different immune statuses and treatment protocols: A multicenter real-world study in Jiangsu Province - China.

OBJECTIVE: The incidence of cryptococcosis is increasing in non-immunocompromised patients. However, the evidence on proper management is inadequate in this population. We conducted this multi-center real-world study in pulmonary cryptococcosis patients with different immune statuses, so as to provide practical evidence for optimized clinical management of cryptococcosis, especially for mild-to-moderate immunodeficient diseases patients. METHODS: This is a prospective observational study. The clinical data of patients with proven cryptococcosis were collected and analyzed from 7 tertiary teac

Primary Cutaneous Cryptococcosis in an Immunocompetent Patient: Diagnostic Workflow and Choice of Treatment.

Cryptococcosis is an opportunistic infection in immunocompromised patients, involving mainly the lungs and central nervous system; however, the skin, eyes and genitourinary tract could also be involved as secondary sites of infection. Primary cutaneous cryptococcosis (PCC) is a distinct clinical entity that can occur in both immunocompetent and -compromised patients, usually trough skin injury. In immunocompetent patients, it is a very rare infection, presenting with non-specific clinical pictures and being challenging to diagnose. Herein, we present the case of an immunocompetent man with PCC

Cutaneous Naganishia albida (Cryptococcus albidus) infection: a case report and literature review.

Naganishia albida (Cryptococcus albidus) is considered saprophytic fungi, and is rarely reported as a human pathogen. Cutaneous infections caused by non-neoformans cryptococcus are rare. We describe a case of an immunocompetent older male with cutaneous cryptococcosis caused by Naganishia albida following skin trauma, and conduct a literature review in PubMed, Lilacs, and Embase. Only six previous similar reports were found. The seven cases (including ours) were widely distributed geographically (Brazil, the US, the UK, Hungary, South Korea, and Iran), all males, and their ages varied, ranging

Cutaneous cryptococcosis in an adult male of sub-saharan origin.

Cryptococcosis is caused by the inhalation of the desiccated encapsulated yeast or basidiospores, which are found in the environment. After the infectious particles reach the lungs, get disseminated hematogenously in various organs. Meningoencephalitis is one of the most common manifestations, especially in sub-Saharan Africa. The dermal lesions can be either primary due to the traumatic implantation of infectious propagule or secondary in patients with disseminated infection due to cryptococcus. The typical presentations of the cutaneous lesions are like umbilicated lesions of molluscum conta

BACKGROUND: Infections caused by extended-spectrum beta-lactamases producing Enterobacteriaceae (ESBL-E) are of major concern in clinical practice because of limited therapeutic options effective to treat them. Published studies showed that ESBL-E, widely spread in Europe, United States or Asia; are also frequent in Africa. However, the impact of ESBL-E infections is yet to be adequately determined in Sub-Saharan African countries, particularly in Senegal. The aim of our study was to estimate the incidence rate of ESBL-E infections and to assess their clinical and economic impact in Senegal.

Isolated crater-like tumour in a renal transplant recipient.

Compared with asymptomatic pulmonary cryptococcosis, secondary cutaneous cryptococcosis may be the first clinical manifestation of disseminated infection but it can be difficult to diagnose clinically. The clinical manifestation of cutaneous cryptococcosis is not pathognomonic, varying from nodules to ulcers and plaques, and frequently mimics other pathologies. This case highlights that an isolated tumour can be an initial manifestation of systemic cryptococcosis in a kidney transplant patient.
