Falciparum malaria--present day problems. An experience with 425 cases.

Clinical details and present day problems encountered in 425 cases of falciparum malaria (PF) are reported. 10.11% had taken chloroquine prior to reporting to us. Parasitic count done in 23.05% cases lacked correlation with severity of disease. Pattern of fever varied markedly but 5.4% were afebrile throughout and presented only with bodyache and malaise. Apyrexial spell was noted in 5.64%. 28.70% had typical facial looks of anaemia and sallow complexion. Cerebral symptoms were noted in 3.05%. Other symptoms were severe headache 33.4%, pain abdomen 3.29%, gastroenteritis 5.64%, jaundice 2.58%

[Malaria, pregnancy and prophylaxis].

We present two case histories. The first is a case of congenital malaria in a newborn whose Laotian mother immigrated into France 2 years ago. The other is a case of very severe malaria with fetal death in a 28 year old woman who was 8 months pregnant and who had come back to France from Gabon where she had lived for 2 years. The authors wish to draw attention to malaria brought into the country and how serious it can be, and suggest prophylactic measures to be applied for pregnant women and their newborn babies.

Review of 40 children with imported malaria.

Forty cases of imported malaria (1978 to 1988) are reviewed and management principles are discussed. All 15 cases of Plasmodium falciparum malaria were acquired in Africa, 5 of which were probably chloroquine-resistant. Most cases of Plasmodium vivax (80%) were acquired on the Indian subcontinent, including 2 cases of congenital malaria. Six children developed P. falciparum malaria despite chemoprophylaxis. All children had a history of fever, usually with other influenza-like symptoms. Two-thirds had splenomegaly, and one-third were afebrile on admission. Thrombocytopenia (70%) and anemia (70

Epidemiology of HIV infection in Africa.

HIV infection in Africa is primarily acquired through heterosexual activity, accounting for up to 80% of cases. Prostitutes and sexually promiscuous individuals are at particularly high risk of acquiring infection via this route. In the general population, women between the ages of 18 and 30 years are at increased risk of transmission. The role of cofactors, particularly concurrent sexually transmitted diseases (STDs), appears to facilitate heterosexual spread. These groups represent opportunities for targeted prevention programs aimed at education, increased condom use, prompt treatment of ST

Antimalarial drugs and pregnancy.

The use of chloroquine and hydroxychloroquine during pregnancy is controversial. These 4-aminoquinoline drugs cross the placenta and have been reported to cause fetal abnormalities, including loss of vision, ototoxicity, and cochleovestibular dysfunction. However, other reports suggest that 4-aminoquinoline drugs can be taken safely during pregnancy, even in dosages used to control systemic connective tissue diseases. We retrospectively studied eight patients who took antimalarial drugs continuously throughout pregnancy. There was a total of 14 pregnancies in women receiving these drugs; three

[The campaign against sexually transmissible diseases and endemic treponematoses].

From the moment WHO was established in 1948, the control of venereal diseases was felt to deserve highest priority, together with activities to control malaria and tuberculosis. International action was needed in view of the high morbidity and mortality from venereal diseases, their serious human and social consequences, and the prevalence of congenital syphilis and other sexually transmitted diseases (gonorrhoea, chancroid, venereal lymphogranulomatosis, granuloma inguinale). WHO immediately set up a global programme for the control of STDs and, with the participation of other agencies, espec

[Malaria and pregnancy. Comparative study of Central Africa and Western Africa].

The transmission of malaria occurs mainly in and following the rainy season (intermittent transmission) in the Sahelian zone of Africa. On the other hand in rainy Equatorial Africa the transmission of malaria is continuous so that it is stable and can give rise to continuous antigenic stimulation in pregnant women which in turn gives rise to passive early high level immunity in the infant. 150 couples of mother and cord blood and 206 placentae were studied. We found 12.1% of carriers of blood parasites in the blood as compared with 1.6% in the cord bloods (exclusively P. falciparum). Where the

Haemoglobinopathies, malaria, and other interferences with HBA1 assessment.

Total glycosylated haemoglobin (HbA1) was determined by a rapid minicolumn chromatography technique in 438 diabetic patients and correlated with the mean of fasting and post-prandial blood glucose values for the preceding six weeks. In 360 of them, free of congenital haemoglobinopathies and other detected causes of HbA1 mis-interpretation (reference group), a significant correlation was established between the HbA1 and glucose values: y = 0.54 X + 4.91; r = 791; (p less than 0.01). In 28 of the 29 patients with heterozygous haemoglobinopathies (Hb S = 17; Hb C = 8; Hb D Pundjab = 1; Hb E = 2)

[Congenital Plasmodium falciparum malaria. Apropos of a case in Paris].

A 28 years old parturient woman, coming back from Cameroon, was admitted with fever. Blood smear for malaria revealed trophozoïtes of Plasmodium falciparum. Because of fetal troubles, the infant was born by a caesarean. Physical examination of the new-born revealed an hepatomegaly. Cord blood film showed trophozoïtes of Plasmodium falciparum. With treatment with chloroquine, the child looked healthy in a few days. So, congenital malaria is not seldom in areas of high malaria endemicity, but we must be very careful with travellers parturient in other countries. The difference between congenital

The human materno-foetal relationship in malaria. II. Histological, ultrastructural and immunopathological studies of the placenta.

Histological and ultrastructural studies of four placentae heavily infectd with Plasmodium falciparum revealed large intervillous accumulations of erythrocytes containing parasites together with monocytes which had ingested pigment. These appearances were associated with focal syncytial necrosis, loss of syncytial microvilli and proliferation of cytotrophoblastic cells. In addition, marked irregular thickening of trophoblastic basement membranes and protrusion of tongue-like projections of syncytiotrophoblast into the basement membrane were observed. In six other placentae which contained scan

A case of congenital malaria in Malaysia with IgM malaria antibodies.

Congenital malaria from Malaysia is reported here for the first time. It occurred in a baby boy born to a 16-year-old primigravida who contracted Plasmodium falciparum infection during pregnancy. She suffered malaria during the later stages of pregnancy and at parturition. The placenta was heavily infested with various asexual stages of P. falciparum. Gametocytes were not seen. Extensive search did not show other species. Cord blood showed very light infection with young trophozoites of P. falciparum. Serological studies using IFA technique showed specific IgG and IgM antibodies to P. falcipar

Congenital malaria: a rare cause of splenomegaly and anemia in an American infant.

A 38-day-old infant had fever, jaundice, hepatosplenomegaly, and a hemolytic anemia. A peripheral blood smear demonstrated intraerythrocytic malarial parasites identified as Plasmodium vivax. Maternal and infant sera contained antibodies to this species. A directed history revealed the mother had suffered several febrile illnesses in Mexico during her pregnancy. Malaria had not been diagnosed nor was it considered at the time of her delivery at this hospital. Review of this and six other cases of congenital malaria reported in this country since 1950 indicates clinical manifestations seldom ap

Epidemiological assessment of malaria transmission in an endemic area of East Pakistan and the significance of congenital immunity.

A longitudinal epidemiological study was carried out in a hyperendemic malarious area of East Pakistan. Transmission of malaria was observed with two peaks, one in the premonsoon period, the other in the pre-winter period. New infections occurred in 51.7% of the infants surveyed who were 2 months old or under when first examined: there were frequent superinfections. The maternal immunity passively transferred to the infants did not play a significant role in restricting the development of the asexual erythrocytic stage or that of gametocytes, as indicated by the fact that the average parasite

THE DIAGNOSTIC VALUE OF THE PLACENTAL BLOOD FILM IN AESTIVO-AUTUMNAL MALARIA.

1. The placental blood film examination is worthy of routine application wherever aestivo-autumnal malaria is endemic. This type of malaria when associated with labor and the early days of the puerperium can be more easily and certainly diagnosed by the use of this film and a polychrome stain than by employing the usual films made from the mother's peripheral blood at the time of labor. The placental film in such an infection offers an abundance of adult parasites and far more evidence of the presence of pigment, while the peripheral blood film frequently offers but a scant number of the small
