Use of new nephrostomy catheter for treatment of renal neonatal candidiasis.

Systemic candidiasis with renal involvement is a rare but well-recognized complication during neonatal intensive care treatment. In addition to intravenous administration of amphotericin B, decompression of the renal pelvis and irrigation of the involved kidney with the same drug through a nephrostomy tube will provide a high concentration of antifungal agent with a flushing effect. This procedure is not always possible due to the small size of the neonatal kidneys. We have conceived a new percutaneous trocar nephrostomy which allows its application directly in an incubator without using X-ray

Nursery outbreak of neonatal fungal arthritis treated with fluconazole.

Eight preterm infants with mean gestational age of 31.6 +/- 1.16 weeks and a mean birth weight of 1310 +/- 201.7 g presented at a mean postnatal age of 26 +/- 11.4 days with knee joint swellings and pedal oedema. There was no other clinical, haematological or microbiological evidence of bacterial sepsis. Fungal cultures yielded growth of Candida spp. from blood in five, from urine in four, from cerebrospinal fluid in one, and from all the three babies in whom the joints were aspirated. Radiographic changes of metaphysitis of the involved joints were noted in all. All infants had received prior

Molecular tracking of Candida albicans in a neonatal intensive care unit: long-term colonizations versus catheter-related infections.

Nosocomial neonatal candidiasis is a major problem in infants requiring intensive therapy. The subjects of this retrospective study were nine preterm infants admitted to the neonatal intensive care unit of the Hospital Central de Asturias between March 1993 and August 1994. The infants were infected with or colonized by Candida albicans. Five patients developed C. albicans bloodstream infections. A total of 36 isolates (including isolates from catheters and parenteral nutrition) were examined for molecular relatedness by PCR fingerprinting and restriction fragment length polymorphism (RFLP) an

Neonatal candidiasis associated with meningitis and endophthalmitis.

From January 1991 to March 1993, a total of 11 infants with positive blood, urine, cerebrospinal fluid (CSF) or stool cultures of Candida albicans were found in the nursery or neonatal intensive care unit of the Chung Shan Medical and Dental College Hospital. Clinical features including respiratory deterioration, poor activity, abdominal distension and/or apnea were noted. Indwelling peripheral venous catheters had been used in all patients to provide parenteral nutrition and broad spectrum antibiotic therapy and six infants had received intravenous fat emulsions. Prolonged endotracheal intuba

Neonatal fungemia and amphotericin B.

Disseminated candidemia is a common nosocomial infection in the neonatal intensive care unit, though only a few studies have reported the outcome of amphotericin B therapy in neonatal candidiasis. Our treatment regimen consisted of an initial daily amphotericin B dose of 0.5 mg/kg. (For infants weighing > 1 kg, the second dose was increased to 1 mg/kg.) At 3 to 5 days, if the blood culture was negative, amphotericin B therapy was changed to every other day and continued for a total of 10 doses. Records of 36 patients given this regimen were reviewed for signs of toxicity or treatment failure.

Invasive neonatal candidiasis: comparison of albicans and parapsilosis infection.

Severe infections caused by non-albicans Candida species are being increasingly reported among infants in neonatal intensive care units. To assess relative severity, mortality rates for C. albicans (CA) and C. parapsilosis (CP) infections in one neonatal intensive care unit from 1980 to 1990 were compared. Invasive candidiasis was defined as Candida recovery from a normally sterile body fluid or site with clinical signs of infection. Invasive candidiasis was diagnosed and systemic antifungal therapy initiated in 45 infants, 29 with CA and 16 with CP. No differences were found between CA and CP

Systemic neonatal candidiasis.

Forty-five cases of systemic neonatal candidiasis were diagnosed over a 9-year period in a neonatal intensive care unit; 42 infants weighted less than 1.5 kg. All had been very ill with preceding bacterial sepsis and other complications of low birthweight. Where treatment was instituted the mortality was low (4 out of 39 dying) and complications of treatment were transitory. We therefore recommend diligent examination for the presence of this infection, and treatment with a combination of amphotericin B and 5-flucytosine.

Neonatal candidiasis: the current challenge.

The current challenge of neonatal candidiasis arises from the increased survival of very low birthweight infants. It is important to understand those factors affecting normal colonization of the neonate, concomitant with those factors that predispose to fungal invasiveness. Disseminated candidiasis may present with subtle signs and symptoms, but has the potential for a wide variety of organ system involvement. Early initiation of aggressive therapy, with careful monitoring, can lead to a successful outcome.

[Congenital cutaneous candidiasis: report of two cases].

Congenital cutaneous candidiasis is a very rare disease. We reported two newborn infants in whom generalized skin eruption was noted at birth, characteristics of erythematous papules and pustules. The eruption involved head, face, neck, trunk and extremities. Candida albicans was demonstrated on direct KOH smear, by surface fungal cultures and skin biopsy. The disease implies a congenital intrauterine infection and is different from neonatal candidiasis which manifests as thrush, diaper dermatitis. The route of infection is ascending in congenital cutaneous candidiasis. The skin eruption is us

Congenital candidiasis limited to the nail plates.

Numerous case reports have been published of patients with cutaneous congenital candidiasis and neonatal candidiasis; however, this is the first reported case of congenital candidiasis confined to the nail plates. A subplacental candidal abscess, funisitis, and demonstration of hyphal invasion of fetal nail plates supported the diagnosis.

Autoinoculated palmar pustules in neonatal candidiasis.

A 2-week-old infant had grouped pustules on the right hand, wrist, and antecubital flexure that appeared after the first week of life. The infant was seen repeatedly to suck the involved areas of the hand and wrist. Clinically typical candidiasis was noted in the oral cavity and the diaper area. Wright's-stained smears of pustular contents, potassium hydroxide preparations of pustular material, and scale from the diaper dermatitis confirmed the presence of pseudohyphae and budding yeasts in all the involved sites. This case demonstrates fairly typical manifestations of neonatal-onset candidias

An assessment of sonography in the diagnosis and management of neonatal renal candidiasis.

The renal sonographic findings in six cases of neonatal candidiasis and 13 cases previously reported in the literature are reviewed. Focal and diffuse parenchymal changes and obstruction due to mycetoma formation were frequent features. In the current series the sonographic changes were found to bear an inconstant relationship to the probable time of onset of the infection. In addition, apparent sonographic deterioration occurred in neonates well after adequate antifungal treatment was initiated. In view of the potential toxicity of systemic antifungal therapy, sonographic renal parenchymal ch

Neonatal candidiasis: renal and pulmonary manifestations.

A description of three newborns with renal candidiasis and a review of 14 others suggest the following pathogenesis: antibiotic therapy in a premature baby with an intravascular catheter is followed by hematogenous renal spread, candiduria, and acute obstructive nephropathy by mycotic bezoars. The radiologic appearance is often characteristic. Prompt therapy is the only possibility for survival. Primary thoracic candidiasis occurs more frequently in the full term baby and is probably aspirated during birth. One patient is described and 20 cases from the literature are reviewed. The radiologic
