Case report: Clinical utility and limitations of high-frequency ultrasound for monitoring sonidegib response in nodular basal cell carcinoma: a three-case series.

BACKGROUND: Sonidegib, a Hedgehog pathway inhibitor, can induce clinical and imaging remission in locally advanced basal cell carcinoma (BCC). Residual deep tumor may persist after malignant dermoscopic and ultrasound features disappear. This case series assessed the utility and limitations of 20-30 MHz high-frequency ultrasound (HFUS) for confirming complete imaging remission after sonidegib. CASES PRESENTATION: Three patients with nodular BCC received sonidegib 200 mg daily (October 2024-May 2026). Two achieved complete imaging remission, defined as resolution of malignant imaging features

Atypical presentation of subclavian steal syndrome mimicking transient ischemic attack: successful endovascular management with angiographic correlation-a case report.

BACKGROUND: Subclavian steal syndrome is an uncommon vascular disorder caused by stenosis or occlusion of the subclavian artery proximal to the vertebral artery origin. It classically presents with vertebrobasilar symptoms such as vertigo, syncope, or diplopia; however, atypical transient neurological presentations may delay diagnosis. CASE PRESENTATION: A 71-year-old patient presented with recurrent slurred speech and left upper limb weakness without vertigo or diplopia. Examination revealed absent measurable blood pressure and absent radial pulse in the left arm, carotid bruits, and mild di

Plasma p-tau181, Aβ1-42, and GFAP across Alzheimer's disease, mixed-type Alzheimer's disease, mild cognitive impairment, anxiety disorders, and depressive disorders.

BACKGROUND: Blood-based biomarkers including phosphorylated tau181 (p-tau181), amyloid β1-42 (Aβ1-42), and glial fibrillary acidic protein (GFAP) have been widely investigated for Alzheimer's disease (AD) detection. However, their utility for discriminating neurodegenerative cognitive impairment from anxiety and depressive disorders remains poorly established. AIMS: To compare plasma p-tau181, Aβ1-42, and GFAP concentrations across clinically-defined patient groups with AD, mixed-type Alzheimer's disease (MAD), mild cognitive impairment (MCI), anxiety disorders (ANX), and depressive disorders

Gut-brain axis in Alzheimer's disease: neuroinflammatory mechanisms and immunopharmacological interventions.

Despite being defined pathologically by amyloid-β plaques, chronic neuroinflammation and neurofibrillary tangles, the drugs that target these processes directly (lecanemab and donanemab) have only modest effects on the clinical course of Alzheimer's disease (AD) and significant safety concerns have been identified, so there is renewed interest in upstream, systemic contributors to disease. This review aims to provide a synthesis of the latest mechanistic and clinical evidence that gut microbiota dysbiosis is associated with Alzheimer's disease with a specifically immunopharmacological focus. Associations of Total IgE, Eosinophils, and IL-5 with SCORAD-Defined Severity in High-IgE Atopic Dermatitis: A Retrospective Exploratory Study.

BACKGROUND: Atopic dermatitis (AD) is characterized by marked immune heterogeneity, and high-IgE AD is commonly associated with Th2-skewed inflammation and greater disease burden. However, the relationships between downstream Th2-related peripheral blood markers and multidimensional disease severity in high-IgE AD remain incompletely understood. METHODS: This single-center retrospective exploratory study included adult patients with AD diagnosed between June 2023 and June 2025. Among 140 eligible patients with complete core data, 92 patients with high-IgE AD, defined as baseline serum total I

Early micro- and nanoscale microglial responses to blood-brain barrier modulation by transcranial focused ultrasound.

UNLABELLED: Modulation of the blood-brain barrier (BBB) using transcranial focused ultrasound with intravenous microbubbles (FUS) has progressed to clinical trials. Guided by magnetic resonance imaging (MRI), FUS significantly increases local BBB permeability for approximately 6-24 h in mice, with an ensuing mild inflammation. Microglia, as the immune cells of the brain, are stimulated by FUS-BBB modulation, although the timing and nature of their responses remain unclear. In this study, we characterized the cellular (i.e., density, distribution, and morphology) and subcellular (i.e., ultrastr

Multi-omics analysis of potential therapeutic targets for Alzheimer's disease and associated differences in immune cell infiltration.

Alzheimer's disease (AD) represents a progressive neurodegenerative disorder that predominantly impairs the central nervous system. This study integrated single-cell RNA sequencing (scRNA-seq) and Mendelian randomization (MR) analysis to explore AD therapeutic targets and characterize alterations in immune cell infiltration and function. We analyzed the subtypes, developmental trajectories and intercellular communication of peripheral blood T cells from AD scRNA-seq data. Cell clustering identified CD8+ T cells as the key immune subpopulation in AD, with two vital intercellular communication p

Serum 25-hydroxyvitamin D levels in pediatric atopic dermatitis: seasonal variability and association with disease severity.

UNLABELLED: Vitamin D has important immunomodulatory and skin barrier-protective effects that may influence the pathogenesis of atopic dermatitis (AD). However, the relationship between serum 25-hydroxyvitamin D [25(OH)D] levels and AD severity remains controversial. This study aimed to evaluate the association between serum 25(OH)D levels and disease severity assessed by the SCORAD index in pediatric patients with AD and to examine seasonal variability in vitamin D levels. In this retrospective study, pediatric patients diagnosed with AD according to the Hanifin and Rajka criteria were includ

A machine learning-driven and explainable prediction model for in-hospital mortality in patients with diabetes mellitus and congestive heart failure: integrating XGBoost and SHAP approaches.

BACKGROUND: Patients with congestive heart failure (CHF) and type 2 diabetes mellitus (T2DM) are at elevated risk of in-hospital mortality, particularly in intensive care units (ICUs). Conventional scoring systems may not fully capture the clinical complexity of such comorbidities. This study aimed to construct an interpretable machine learning model to predict in-hospital mortality in this high-risk group. METHODS: A retrospective study included 573 ICU patients with CHF and T2DM admitted to three hospitals in Henan, China (Jan 2025-Jun 2026). Seven predictors-body mass index (BMI), myocardi

When to Start Oral Anticoagulant Therapy Post-Stroke: From Common to Rare Conundrums.

PURPOSE OF REVIEW: This review summarizes recent evidence on when to start oral anticoagulant therapy following stroke, including both common and rare clinical conundrums. RECENT FINDINGS: While previous recommendations based on Diener's rule suggested waiting up to 12 days based on stroke severity and neuroradiological findings, newer data support earlier initiation or resumption of anticoagulation, particularly for mild and moderate ischemic strokes, generally within 4 days. In patients with a history of intracerebral hemorrhage, anticoagulation resumption involves weighing thromboembolic r

Impact of hypothermia level on neurological and renal outcomes in emergency aortic surgery: a single-centre experience.

BackgroundTemperature management during emergency surgery for acute aortic syndrome is critical for neurological, renal, and systemic protection. Recent European Association for Cardio-Thoracic Surgery (EACTS) guidelines recommend moderate hypothermia with antegrade cerebral perfusion for hemiarch procedures. Nevertheless, deep hypothermic circulatory arrest (DHCA) remains common. This study evaluates neurological and other postoperative outcomes across different intraoperative temperature ranges.MethodsA single-centre retrospective study of consecutive patients undergoing surgery for acute ao

MTHFR C677T genetic polymorphisms are not a decisive factor in delayed methotrexate excretion in central nervous system lymphoma patients.

BACKGROUND: High-dose methotrexate (HD-MTX) serves as a cornerstone treatment for central nervous system lymphoma. However, its clinical utility is limited by delayed drug excretion and associated adverse events. The methylenetetrahydrofolate reductase (MTHFR) C677T polymorphism, a critical genetic variant influencing MTX metabolism, has been proposed by some experts as an indicator for HD-MTX dose reduction due to its potential association with delayed excretion. OBJECTIVE: This study aimed to identify clinical risk factors for delayed MTX excretion and evaluate the predictive value of MTHFR

Safety considerations and clinical monitoring during pulsed field ablation.

Pulsed field ablation (PFA) has emerged as a transformative technology in cardiac electrophysiology, offering a potentially superior safety profile compared with thermal ablation due to its non-thermal mechanism of irreversible electroporation. Clinical experience confirms a substantial reduction in traditional complications such as pulmonary vein stenosis, atrio-oesophageal fistula, and permanent phrenic nerve injury, though PFA introduces a distinct spectrum of physiologic effects requiring consideration. Coronary vasospasm is the most characteristic phenomenon, ranging from transient, subcl

Serum β-synuclein predicts dementia conversion and mortality beyond amyloid-β and phospho-tau.

Synaptic dysfunction is among the earliest pathological events in Alzheimer's disease and among the strongest neuropathological correlates of cognitive impairment, yet blood-based synaptic biomarkers are lacking. Whether serum β-synuclein provides prognostic information independent of established amyloid-β (A; CSF Aβ42/Aβ40 ratio) and tau (T1; CSF pTau181) biomarkers has not been determined. We studied 259 non-demented individuals with subjective or mild cognitive impairment from a single-centre memory clinic, classified by CSF biomarkers using externally validated cut-offs into A-T1- controls
