Right-sided cryptorchidism and contralateral intrascrotal testicular torsion in a young dog: a rare case report.

A 7-month-old, 20-kg male mixed-breed dog with a history of right retained testis was presented to the hospital. Over the past three days, he developed swelling for two days and then hardening in the scrotum, which only included the left testis. Hematological and biochemical analyses revealed mild anemia and elevated urea levels. Ultrasonography examination revealed the left testis to be mildly enlarged, heterogeneous, with abnormal mediastinum, and with an insufficient Doppler signal. On surgical exploration, the left testis was necrotic, dark in color, and twisted clockwise. A left orchiecto

Right-Sided Infectious Endocarditis in the Patient With Williams Syndrome: Importance of Recognizing Disease-Specific Pathophysiology in Adults.

Infectious endocarditis (IE) in patients with Williams syndrome is usually associated with left-sided heart lesions, whereas right-sided endocarditis is rarely observed. This can be explained by the natural course of congenital heart lesions in Williams syndrome, in which right-sided heart lesions are likely to spontaneously regress with patient growth. We encountered a 29-year-old patient diagnosed with right-sided infective endocarditis. His diagnosis was supported by the modified Duke criteria, one major criterion of positive blood culture twice with Streptococcus oralis, a type of gram-pos

Origin of poorly galactosylated IgA1 other than mucosa: a viewpoint from a report on patient with IgA vasculitis.

Patients presenting monoclonal gammopathy of renal significance (MGRS) and IgA vasculitis collectively have rarely been reported. This study reports one patient with monoclonal IgA and λ, and with IgA positive-mesangial proliferative glomerulonephritis. The patient manifested slight chronic nephritic syndrome, and his serums tested positive for M protein (monoclonal IgA and λ). As a result of bone wear, the plasma cell ratio of these patients was confirmed to be mildly increased in peripheral blood smears. Just like typical IgA nephropathy or IgA vasculitis patients, serum poorly galactosylate

IgA vasculitis associated with chronic myelomonocytic leukemia.

IgA vasculitis is a predominantly pediatric autoimmune disease characterized by IgA deposit in small vessels. Chronic myelomonocytic leukemia (CMML) is a rare hematological malignancy classified within myelodysplastic syndromes. Here, we present a previously unrecognized case of CMML associated with IgA vasculitis. A 62-year-old woman presented with necrotic and infiltrated purpura and mild arthralgia, primarily affecting the knees and wrists, without gastrointestinal or kidney involvement. A comprehensive screening for other etiologies was unremarkable. Blood tests showed an increase of monoc

Anemia of Inflammation.

Anemia of inflammation (AI), also known as anemia of chronic disease, is the most common anemia in hospitalized patients and considered to be the second most common anemia worldwide after iron deficiency anemia (IDA). The hallmark of AI is iron restriction within macrophages of the mononuclear phagocyte system (MPS) resulting in hypoferremia and hyperferritinemia together with suppression of erythropoiesis and shortened erythrocyte lifespan. Symptoms are comparable to other anemia entities and often related to the underlying disease. Patients usually present with normocytic, normochromic, hypo

Recurrent pericarditis with normal C-reactive protein: Clinical and laboratory features, biomarkers and outcome in a non-inflammatory phenotype.

BACKGROUND: Differences in recurrent pericarditis with normal vs. elevated C-reactive protein (CRP) are unknown. MATERIALS AND METHODS: We studied 448 patients with recurrent or incessant pericarditis. CRP levels <10 mg/L were considered normal. Forty-one patients with normal CRP were tested for interleukin 6 (IL-6), serum amyloid A (SAA) and soluble urokinase plasminogen activator receptor (suPAR). RESULTS: Among the cohort (median age 43 years, 48.4% females), 336 patients (75%) had elevated CRP, while 112 (25%) had normal levels. CRP-negative patients were younger (38.6 vs. 43.5 years, p=

Henoch-Schönlein Purpura Triggered by Amebiasis in a Young Female Child: A Case Report and Literature Review.

Immunoglobulin A (IgA) vasculitis, formerly known as Henoch-Schönlein purpura (HSP), is the most common form of systemic vasculitis in children, characterized by the deposition of IgA within small blood vessels. While HSP typically manifests with purpuric rash, arthritis, and gastrointestinal involvement, reports of HSP triggered by parasitic infections, such as amebiasis, are rare. This case examines the association between HSP and Entamoeba histolytica infection, highlighting the importance of recognizing parasitic triggers in systemic vasculitis. A 12-year-old female with no significant pas

Atypical presentation of immunoglobulin A vasculitis mimicking acute appendicitis in a pediatric patient: a case report.

Acute appendicitis is a common pediatric surgical emergency, but its clinical presentation can overlap with other conditions, complicating diagnosis. We present a case of a 4-year-old female who initially presented with symptoms suggestive of appendicitis, including right iliac fossa pain, vomiting, and fever. Imaging studies were inconclusive, and a laparoscopic appendectomy was performed. No signs of appendicitis were found, but vasculitic changes were noted in a segment of the ileum. Postoperatively, the patient developed a mild purpuric rash, and elevated C-reactive protein (CRP) levels pr

Guidelines for the diagnosis and management of childhood immunoglobulin A vasculitis.

BACKGROUND: Immunoglobulin A vasculitis (IgAV) is the most common cause of systemic vasculitis in childhood. Due to the continued use of the disease name "anaphylactoid purpura" in China, several misunderstandings have arisen in clinical practice and treatment regimens differ widely. In addition, new research and evidence-based data have grown. The Subspecialty Group of Immunology, Society of Pediatrics, Chinese Medical Association and the Chinese Alliance of Pediatric Rheumatic and Immunologic Diseases initiated an update of guidelines for the diagnosis and management of childhood IgAV. The a

Global, regional, and national anemia burden among women of reproductive age (15-49 years) from 1990 to 2021: an analysis of the Global Burden of Disease Study 2021.

BACKGROUND: Anemia remains a significant global health challenge, disproportionately affecting women of reproductive age (WRA, 15 to 49 years) due to physiological and socioeconomic factors. However, there is a lack of high-quality data on anemia burden and causes analysis in this population. The aim of this study is to provide a comprehensive global assessment of anemia burden and its underlying causes among WRA. METHODS: Using data from the Global Burden of Disease Study (GBD) 2021, we evaluated the prevalence, years lived with disability (YLDs), and underlying causes of anemia among WRA at

MicroRNA sequencing analysis in pediatric patients with influenza-associated acute necrotizing encephalopathy: Potential biomarkers for early diagnosis and therapy.

Acute necrotizing encephalopathy (ANE) secondary to influenza infection is characterized by fulminant neurological deterioration and a high mortality rate. The underlying mechanisms remain unclear, and specific treatments are currently lacking. Therefore, understanding the pathogenesis and identifying diagnostic and therapeutic targets for influenza-induced ANE are crucial. Peripheral blood samples were collected from two groups: influenza-infected patients without ANE (mild) and influenza infection with ANE patients (severe). Differentially expressed genes (DEG) were identified through microR

Surgery for the treatment of arterial hypertension in patients with unilateral adrenal incidentalomas and mild autonomous cortisol secretion (CHIRACIC): a multicentre, open-label, superiority randomised controlled trial.

BACKGROUND: Adrenal incidentalomas are found in 3-10% of adults undergoing abdominal imaging. Of these, 30-50% are responsible for mild autonomous cortisol secretion (MACS), which is frequently associated with hypertension. The impact of adrenalectomy on hypertension in patients with unilateral incidentalomas and MACS remains uncertain. The aim of the CHIRACIC study was to prospectively assess the impact of surgical excision of the incidentaloma on blood pressure with a randomised trial combining accurate blood pressure measurement and standardisation of antihypertensive treatment. METHODS: C

Very-early-onset autoimmune hypothyroidism: a report of two cases with STAT3 gain-of-function variant.

Autoimmune hypothyroidism occurs rarely before 3 years of age. Two siblings were diagnosed with autoimmune hypothyroidism at age 5 and 16 months, presenting with classic symptoms of hypothyroidism, abnormal thyroid function tests (TSH: 200 and 660 mU/L; reference range (RR): 0.73-8.4 mU/L; Free T4: 5.9 and <1.3 pmol/L; RR: 11.9-25.6 pmol/L), and high thyroid peroxidase antibody levels. Thyroxine medication alleviated their symptoms. Apart from mild infections, the siblings exhibited no other major disorders. Whole exome sequencing identified a pathogenic STAT3 gain-of-function variant, most co

Acute necrotising encephalopathy (ANE) due to influenza: an unrecognised encephalopathy in childhood.

Acute necrotising encephalopathy (ANE) of childhood is a rare, rapidly progressive and often fatal form of encephalopathy characterised by distinctive clinical and neuroradiological features. It typically presents with a sudden and progressive alteration of consciousness, along with the characteristic finding of multifocal, symmetric lesions involving the thalami, brainstem, cerebellum and cerebral white matter. Although its exact pathogenesis remains unclear, it is thought to result from a cytokine storm triggered by viral infections, leading to blood-brain barrier disruption and brain injury
