Stem cells redirect the neuroinflammatory trajectory of primed microglia in an ovine chorioamnionitis model.

Antenatal infection (e.g. chorioamnionitis), an important risk factor for preterm birth, can induce a fetal systemic inflammatory response (FIRS), initiating neuroinflammation. Microglia, as principal resident immune cells of the brain, are key targets for therapeutic interventions such as Multipotent adult progenitor cells (MAPC). We analyzed the effects of intravenous MAPC therapy on microglia and peripheral immune cells at the blood-brain barrier (BBB) in an ovine chorioamnionitis model. Ovine fetuses were intra-amniotic (i.a.) exposed to lipopolysaccharide (LPS) to induce FIRS, or saline (

Budget Impact Analysis of the Balanced Opioid Initiative: A Cluster Randomized Trial Aimed at Deprescribing Opioids for Chronic Pain in Primary Care Settings.

PURPOSE: This study compared the budget impact of 4 deimplementation strategies designed to promote guideline-concordant opioid prescribing. METHODS: In this cluster randomized trial of 32 primary care clinics, deimplementation strategies targeted the system, clinic, and prescriber levels. All clinics received system-level educational meetings with audit/feedback reports. At month 3, one-half were randomized to add practice facilitation, a clinic-level strategy. At month 9, one-half were again randomized to add prescriber peer consulting, a prescriber-level strategy, resulting in 4 groups: sy

Transamniotic fetal mRNA vaccination: Active immunization against Zika virus in a rodent model.

PURPOSE: We investigated whether transamniotic fetal mRNA vaccination could be a viable strategy for perinatal immunization against Zika virus (ZIKV) infection in a rodent model. METHODS: Twenty pregnant Sprague Dawley dams underwent volume-matched intra-amniotic injections in all their fetuses (n = 280) of either a suspension of a custom-made mRNA encoding for the ZIKV pre-membrane (ZIVK-prM) and envelope (ZIKV-E) structural proteins encapsulated in a semi-synthetic composite lipopolyplex (vaccinated, n = 153), or of the same lipopolyplex without mRNA (control, n = 127) on gestational day 17

Predictive Potential of Plasma Acute Phase- and Adhesion-Related Proteins and Progranulin for Intra-Amniotic Infection/Inflammation and Spontaneous Preterm Birth in Women With Preterm Labor.

OBJECTIVE: To determine whether plasma acute phase-, adhesion-related, and multifunctional proteins, alone or in combination with ultrasound and traditional blood tests, can predict the microbial invasion of the amniotic cavity and/or intra-amniotic inflammation (MIAC/IAI) and spontaneous preterm birth (SPTB) in women with preterm labor (PTL). STUDY DESIGN: A retrospective cohort study was performed on 249 singleton pregnant women with PTL (at 23+0 to 34+0 weeks) who underwent transabdominal amniocentesis. Plasma samples were obtained at amniocentesis. MIAC/IAI was characterized by a positive

Tailored-prom: tailored therapeutic regime in patients with preterm premature rupture of membranes to prolong pregnancy, improve maternal and neonatal outcomes, and reduce antibiotic burden: a randomized controlled trial protocol.

BACKGROUND: Preterm premature rupture of membranes is a major cause of preterm birth and is associated with substantial maternal and neonatal morbidity. Current management is largely uniform and includes routine administration of antibiotics and antenatal corticosteroids, although a considerable proportion of women do not have infection or inflammation at the time of membrane rupture. This approach may lead to unnecessary exposure to medication and suboptimal timing of treatment. Measurement of interleukin-6 in amniotic fluid is a promising marker of intra-amniotic inflammation and may allow a

The microbiology of amniotic fluid sludge: a systematic review and meta-analysis.

PURPOSE: Amniotic fluid sludge (AFS) is a sonographic finding associated with intra-amniotic infection and spontaneous preterm birth. The microbial composition of AFS remains poorly characterized. This systematic review and meta-analysis aimed to consolidate existing data on microbial prevalence in AFS and identify consistent microbial patterns. METHODS: A systematic search of MEDLINE (PubMed), Dimensions, and OpenAlex was conducted from inception to February 11, 2026. Studies reporting the prevalence of microorganisms in AFS (minimum 10 cases) from pregnant women with AFS diagnosed via trans

The cytokine prokineticin-1 in preterm labor with and without intra-amniotic inflammation.

OBJECTIVES: Prokineticin 1 (PROK1), known as endocrine gland-derived endothelial growth factor (EG-VEGF), is a key regulator of pregnancy, playing various roles from implantation to birth. We have shown that maternal serum concentrations of PROK1 are highest during the first trimester and are elevated in pregnancy complications such as spontaneous preterm birth (sPTB). Additionally, PROK1 is described as being largely expressed by the chorion in fetal membranes, suggesting its presence in amniotic fluid. Despite these findings, no information is available about PROK1 concentrations in the amni

Intra-amniotic infection: diagnosis, nomenclature, clinical significance, management, and microbiologic tools used for the diagnosis.

SUMMARYIntra-amniotic infection is the main cause of spontaneous preterm birth and adverse maternal-fetal outcomes; therefore, rapid, robust, and accurate diagnosis remains a clinical priority. Conventional microbiological techniques, especially culture-based methods, are limited by long turnaround times and the inability to detect fastidious or unculturable organisms. This review summarizes the diagnosis, nomenclature, clinical significance, management, and laboratory approaches for diagnosing intra-amniotic infection. Targeted nucleic acid amplification methods, including species-specific po

Osteopathic Manipulative Treatment for the Breastfeeding Dyad: A Scoping Review.

BACKGROUND: Human milk is widely recognized as the optimal source of infant nutrition. While 83.9% of mothers initiate breastfeeding, only 46.3% exclusively breastfeed to 3 months. Common barriers include latch and suck difficulties, painful feeding, and breast infections or engorgement. Osteopathic manipulation, including osteopathic manipulative treatment (OMT) and therapy (OMTh), has been proposed as supportive intervention for breastfeeding challenges. RESEARCH AIM: This scoping review examines the current literature on OMT/OMTh in the breastfeeding dyad and identifies gaps to inform futu

Fetal sex and inflammatory responses in preterm prelabor rupture of membranes.

BACKGROUND: Fetal sex influences placental development and immune regulation. Although sex-related differences in placental vascular pathology have been described, their influence on intra-amniotic inflammation and placental inflammatory responses in pregnancies complicated by preterm prelabor rupture of membranes (PPROM) remains poorly understood. OBJECTIVE: To determine whether fetal sex is associated with differences in the intensity of the intra-amniotic inflammatory response, the distribution of intra-amniotic categories defined according to the presence or absence of microbial invasion

Amniocentesis before cerclage in asymptomatic women with mid-trimester short cervix: time to consider?

The efficacy of cervical cerclage (CC) in cases of asymptomatic mid-trimester short cervix, or the ultrasound finding of a cervical length<25 mm within 24 weeks of gestation, has been documented in women with a previous history of preterm birth (PTB). Conversely, no robust evidence supports the role of cervical cerclage in prolonging pregnancy and improving maternal-neonatal outcomes in pregnant patients with mid-trimester short cervix detected by sonography and no prior history of PTB. However, the limited effect of CC in this population may be partially explained by the lack of preprocedure

Vaginal versus cervical types of visible fetal membranes in cervical insufficiency: Clinical characteristics and pathophysiology.

This study aimed to compare the clinical, inflammatory, and microbiological characteristics of vaginal and cervical types of visible fetal membranes in cervical insufficiency. We retrospectively analyzed 103 singleton pregnant women, classified into vaginal type (V-type; n = 39) and cervical type (Cx-type; n = 64) based on the extent of membrane descent. Cases were most commonly diagnosed at 20-22 weeks of gestation. V-type cases were associated with significantly worse pregnancy outcomes than Cx-type cases, including a higher rate of spontaneous preterm delivery before 28 weeks (97.4% vs. 59.

Home is where the host is: evolutionary history of geographic spread, host switching and adaptive genomic signatures in two generalist group B Streptococcus clonal group.

Group B Streptococcus (GBS) is a pathogen of global relevance in neonatal and maternal disease as well as bovine mastitis. Two closely related clonal group, denoted 103 and 314 (CG103/314), have been detected in humans and cattle on multiple continents in recent decades but are poorly characterized compared to other host-generalist clades. We examined their potential origins, host-switching events and the presence of a suite of genetic markers for antimicrobial resistance, virulence and host association using a newly assembled dataset of 248 CG103/314 genomes from humans, cattle and food origi

Molecular study of specific virulence genes of Pseudomonas aeruginosa isolated from sub clinical bovine mastitis cases.

Pseudomonas aeruginosa-caused bovine mastitis has received increased attention recently because it results in large financial losses for dairy farms and the dairy sector overall. P. aeruginosa mastitis is a complex illness that requires host, pathogen, and environmental interactions. The objective of this study was to examine the distribution and presence of virulence factors in 13 Pseudomonas aeruginosa isolates that were isolated from 70 milk samples from subclinical cases of bovine mastitis, with an incidence of 18.57%. The Vitek 2 system (bioMerieux) verified each isolate. Pseudomonas aeru
