Conservative approach for intra-amniotic Candida albicans colonisation. Case report and review of current evidence.

OBJECTIVES: Intra-amniotic colonisation or infection caused by Candida albicans is rare. Given the shortage of reported cases, evidence on antifungal strategies and the choice of type and timing of delivery is limited. CASE PRESENTATION: We report a case of intra-amniotic colonisation by C. albicans in a pregnant woman with a previous history of cervical cerclage and candidal vaginosis at 25 weeks of gestational age (GA). The diagnosis was made following preterm premature rupture of membranes at GA 28 weeks and 6 days. Treatment was started with amphotericin B and was replaced by fluconazole

Preterm and premature rupture of fetal membranes.

Premature rupture of fetal membranes (PRFM) is rupture of fetal membranes before onset of labor (pre-labor) and after 37 gestational weeks. Preterm premature rupture of fetal membranes (PPRFM) is rupture of fetal membranes before 37 gestational weeks. PRFM and PPRFM are associated with a significant perinatal infectious morbidity. Failure to diagnose rupture of fetal membranes (RFM) is associated with failure to implement standard management with subsequent increased perinatal infectious morbidity. Therefore, this review designed to summarize the risk factors, methods of diagnosis and manageme

Transamniotic fetal delivery of recombinant human immunoglobulin monoclonal antibodies: A potential novel strategy for prevention of neonatal Respiratory Syncytial Virus (RSV) disease.

PURPOSE: Modified monoclonal class-G immunoglobulins (IgG) have been shown to protect infants from Respiratory Syncytial Virus (RSV), a prevalent disease with particularly high morbidity in newborns. We sought to determine whether a clinically relevant monoclonal IgG against RSV could be delivered to the fetus via transammiotic fetal immunotherapy (TRAFIT) and remain bioavailable after birth. METHODS: Fetuses (n = 75) from five pregnant dams received volume-matched intra-amniotic injections on gestational day 17-18 (E17-18, term = E21-22) of either an FDA approved and commercially available r

Birth Outcomes for Obstetrician- or Midwife-Led Intrapartum Care.

BACKGROUND: Studies suggest that midwifery care can decrease the rate of unplanned cesarean birth. In this study, we compared unplanned cesarean rates, labor interventions, and birth outcomes for mixed-risk patients receiving intrapartum care from obstetricians or midwives. METHODS: We conducted a retrospective cohort study using perinatal data from a single academic tertiary center from 2013 to 2018. The sample included nulliparous and multiparous patients with a term, singleton, vertex fetus. We included induced and spontaneous labor as well as trial of labor after cesarean. We excluded pat

Candida Susceptibility to Antifungals in Amniotic Fluid: A Preliminary Study.

(1) Background: Fungal infections of amniotic fluid, especially those caused by Candida spp., are rare but clinically important, as they can be correlated with preterm birth and poor neonatal outcomes. The aim of this study was to assess the antifungal susceptibility of Candida spp. isolated from amniotic fluid using an MIC (minimum inhibitory concentration)-based assay. (2) Methods: Forty consecutive, exploratory Candida isolates were identified from amniotic fluid samples at the "Cuza Vodă" Clinical Hospital of Obstetrics and Gynecology, Iași, and were analyzed successively using Sabouraud a

Prenatal administration of Immunoglobulin-M (IgM) via transamniotic fetal immunotherapy (TRAFIT).

PURPOSE: Immunoglobulin-M (IgM) is the first antibody class produced during an immune response, playing a crucial role in early defense mechanisms. Its large pentameric or hexameric structure, however, prevents it from crossing the placenta from mother to fetus during pregnancy. We sought to determine whether IgM is amenable to transamniotic fetal delivery utilizing a healthy rodent model. METHODS: Fetuses (n = 42) from three time-dated pregnant dams received intra-amniotic injections on gestational day 17 (E17, term = E21-22) of either a solution of human IgM antibodies suspended in saline (

Amniotic and cervical fluids progranulin levels in pregnancies complicated by spontaneous preterm delivery with respect to intra-amniotic complications-a retrospective cohort study.

The main aim of the study was to determine progranulin levels in amniotic and cervical fluid samples from pregnancies complicated by preterm prelabor rupture of membranes (PPROM) or preterm labor with intact membranes (PTL), with concomitant microbial invasion of the amniotic cavity and/or intra-amniotic inflammation. A total of 104 and 108 women with PPROM and PTL, respectively, were included. Paired amniotic and cervical fluid samples were obtained using transabdominal amniocentesis and Dacron polyester swabs, respectively. Progranulin levels were assessed with an enzyme-linked immunosorbent

Sustained early postnatal humoral and cellular immunity against human cytomegalovirus after transamniotic fetal mRNA vaccination in a rodent model.

PURPOSE: We sought to examine the humoral and cellular immune responses to transamniotic fetal mRNA vaccination against a human cytomegalovirus (hCMV) antigen over time in early postnatal life in a rodent model. METHODS: Seven pregnant Sprague Dawley dams underwent volume-matched intra-amniotic injections in all their fetuses (n = 82) of a custom-made mRNA encoding for hCMV envelope glycoprotein-B (hCMV-gB) antigen encapsulated by a lipid-polymer composite on gestational day 17 (E17; term = E21-22). At three time points between 1 and 3 months after birth, serum levels of antigen-specific hCMV

Intra-amniotic infection and/or inflammation is associated with fetal cardiac concentric hypertrophy and diastolic dysfunction in preterm labor and preterm prelabor rupture of membranes.

BACKGROUND: Preterm delivery is associated with cardiovascular remodeling and dysfunction in children and adults. However, it is unknown whether these effects are caused by the neonatal consequences of preterm birth or if these are already present in utero. OBJECTIVE: We evaluated fetal cardiac morphology and function in fetuses of mothers admitted for preterm labor or preterm prelabor rupture of membranes and the association of these changes with the presence of intra-amniotic infection and/or inflammation. STUDY DESIGN: In this prospective cohort study, fetal echocardiography and amniocent

Anaphylaxis-induced premature uterine contractions: a case report and literature review.

BACKGROUND: Preterm labor is caused by multiple etiologies, including intra-amniotic infection and/or intra-amniotic inflammation, vascular disorders, cervical disease, decidual senescence, and breakdown of maternal-fetal tolerance. Accumulating evidence in vivo and in vitro has shown that an allergic reaction, including anaphylaxis, can induce preterm uterine contractions. This report describes a case of a pregnant woman who developed anaphylaxis and regular uterine contractions after the ingestion of a strawberry-coated biscuit. We also review the mechanism of allergic reaction (hypersensiti

Angiogenic imbalance in pregnancies with preterm prelabor rupture of membranes between 34 and 37 weeks of gestation.

INTRODUCTION: This study aimed to identify whether microbial invasion of the amniotic cavity and/or intra-amniotic inflammation in women with late preterm prelabor rupture of membranes (PPROM) was associated with changes in concentrations of soluble fms-like tyrosine kinase-1 (sFlt-1), placental growth factor (PlGF) and its ratio in maternal serum, and whether placental features consistent with maternal vascular malperfusion further affect their concentrations. MATERIAL AND METHODS: This historical study included 154 women with singleton pregnancies complicated by PPROM between gestational ag

Anti-infectious and anti-inflammatory effect of amniopatch in the treatment of spontaneous previable rupture of membranes.

Spontaneous previable rupture of membranes complicates approximately 0.4-0.7% of pregnancies and is associated with severe maternal and neonatal morbidity and mortality. Intra-amniotic inflammation is present in up to 94.4% of cases, most often caused by a bacterial infection. In comparison, the effectiveness of antibiotic therapy in its eradication reaches less than 17%. Inflammatory activity in the amniotic cavity disrupts the physiological development of the fetus with an increase in maternal, fetal, and neonatal inflammatory morbidity through the development of fetal inflammatory response

Evidence of brain injury in fetuses of mothers with preterm labor with intact membranes and preterm premature rupture of membranes.

BACKGROUND: Brain injury and poor neurodevelopment have been consistently reported in infants and adults born before term. These changes occur, at least in part, prenatally and are associated with intra-amniotic inflammation. The pattern of brain changes has been partially documented by magnetic resonance imaging but not by neurosonography along with amniotic fluid brain injury biomarkers. OBJECTIVE: This study aimed to evaluate the prenatal features of brain remodeling and injury in fetuses from patients with preterm labor with intact membranes or preterm premature rupture of membranes and t

Antenatal creatine supplementation reduces persistent fetal lung inflammation and oxidative stress in an ovine model of chorioamnionitis.

Chorioamnionitis is a common antecedent of preterm birth and induces inflammation and oxidative stress in the fetal lungs. Reducing inflammation and oxidative stress in the fetal lungs may improve respiratory outcomes in preterm infants. Creatine is an organic acid with known anti-inflammatory and antioxidant properties. The objective of the study was to evaluate the efficacy of direct fetal creatine supplementation to reduce inflammation and oxidative stress in fetal lungs arising from an in utero proinflammatory stimulus. Fetal lambs (n = 51) were instrumented at 90 days gestation to receive
