A novel frameshift RPL15 gene variant associated with hydrops fetalis and perinatal death.

BACKGROUND: The heterozygous RPL15 gene (OMIM*604174) variants are associated with autosomal dominant Diamond-Blackfan anemia 12 (DBA12, OMIM#615550). Fewer than ten RPL15 variants have been reported to date, and they are commonly associated with hydrops fetalis, intrauterine growth restriction (IUGR) and neonatal anemia. Fetal demise attributable to the RPL15 variant identified in our patient was the first documented case of perinatal death associated with RPL15-related DBA. METHODS: Trio-based whole-exome sequencing (trio-WES) was performed on genomic DNA (gDNA) of fetal muscular tissues an

Oxidative Stress in Neonatal Disease: Established Organ Injury and Emerging Evidence for Skeletal Involvement.

Reactive oxygen and nitrogen species participate in physiological signaling and adaptation during fetal and neonatal development, whereas excessive, prolonged, or spatially dysregulated production may contribute to oxidative and nitrosative tissue injury. Redox dysregulation has been extensively investigated in neonatal conditions, including bronchopulmonary dysplasia (BPD), hypoxic-ischemic encephalopathy, retinopathy of prematurity (ROP), necrotizing enterocolitis (NEC), congenital heart disease (CHD), and hyperbilirubinemia. Its relationship with bone metabolism is well established in adult

Laparoscopic Ladd procedure for adult-onset intestinal malrotation with midgut volvulus: a case report.

BACKGROUND: Intestinal malrotation is a rare congenital anomaly resulting from the incomplete rotation of the midgut. While commonly diagnosed in newborns, adult presentation is extremely rare (0.2-0.5%), making its true incidence unknown. This case report describes a rare adult-onset midgut volvulus successfully managed via a minimally invasive approach. CASE DESCRIPTION: A 29-year-old female presented to the emergency department (ED) with acute abdominal pain, nausea, and a history in the last two years of intermittent, self-limiting subocclusive episodes. Abdominal computed tomography (CT)

The implications of maternal sleep disruption on cardiovascular adaptations and cardiometabolic outcomes.

Pregnancy involves profound physiological changes that, although necessary for gestation, might promote maladaptive systemic responses, an important consideration given rising maternal morbidity and mortality in the USA, and the role of cardiovascular disease as a leading cause of pregnancy-related death. This Personal View examines the link between disrupted peripartum sleep and adverse cardiometabolic outcomes. Sleep quality and duration often decline during pregnancy because of many factors, including hormonal changes, physical discomfort, and behavioural influences. Sleep pathologies, such

Global, Regional and National Burden of Hemolytic Disease and Other Neonatal Jaundice, 1980/90-2021, and Projections to 2050: A Global Burden of Disease Study 2021.

BACKGROUND AND AIMS: Hemolytic disease of the fetus and newborn and other forms of neonatal jaundice remain important causes of morbidity and mortality among children under five worldwide. Despite improvements in prevention and treatment such as Rh screening, phototherapy, and exchange transfusion, the burden remains uneven across regions. This study assessed global, regional, and national trends in incidence, prevalence, mortality, Disability-Adjusted Life Years (DALYs), Years of Life Lost (YLLs), and Years Lived with Disability (YLDs) from 1980/1990 to 2021 using Global Burden of Disease (GB

Community-based monitoring of neonatal jaundice using the point-of-care Bilistick® System 2.0.

BACKGROUND: Neonatal jaundice often peaks after discharge, creating the need for reliable post-discharge monitoring. This study evaluated the feasibility and safety of a hospital-to-community pathway using the Bilistick® System 2.0 within routine Community Health Family Services (CHFS). METHODS: This retrospective observational study was conducted within a Local Health Authority in northeastern Italy. Newborns eligible for out-of-hospital bilirubin testing according to a Diagnostic-Therapeutic Assistance Pathway (DTAP) were referred either to CHFS or to hospital follow-up based on bilirubin r

Intravenous immunoglobulin as an adjunct to phototherapy in neonatal haemolytic disease: A systematic review and meta-analysis.

Phototherapy remains the first-line treatment of haemolytic disease of the newborn (HDN), a potentially life-threatening condition caused by maternal alloimmunisation. Intravenous immunoglobulin (IVIG) has been proposed as an adjunct to reduce the need for exchange transfusion (ET), a procedure associated with significant risks. This meta-analysis aims to evaluate the safety and efficacy of IVIG combined with phototherapy versus phototherapy alone in neonates with HDN. A comprehensive search of MEDLINE, EMBASE, CINAHL and the Cochrane Library was conducted up to 24 April 2025. Eligible studies

EHA Guidelines on the management of children and adults with hemoglobin SC disease.

HbSC disease is the second most common type of sickle cell disease (SCD) in populations of African origin. Although it is often grouped together with other types of SCD, particularly sickle cell anemia (HbSS), it has unique pathophysiology and a different spectrum of laboratory and clinical features. Compared to HbSS, hemoglobin levels are higher, HbF is lower at less than 5%, and most vaso-occlusive and vasculopathic complications are less frequent, although retinopathy is more common. Despite these differences, there have been no specific guidelines on the management of HbSC disease. This cl

Dynamic landmark models predicting native liver survival after kasai portoenterostomy: a bicentric study.

OBJECTIVES: Native liver survival (NLS) after Kasai portoenterostomy (KPE) for biliary atresia (BA) is highly heterogeneous, and clinically practical approaches that update prognostic estimates across prespecified postoperative follow-up time points remain limited. METHODS: In this retrospective bicentric cohort study (training, n = 168; validation, n = 80), Cox proportional hazards landmark models were developed at five prespecified time points (preoperative and 2 weeks, 1, 3, and 6 months post-KPE) using a LASSO/Boruta dual-algorithm intersection approach with variance inflation factor (VIF

Child drowning in northern Bangladesh: barriers, local initiatives, and opportunities for prevention.

BACKGROUND: Although Bangladesh has achieved substantial progress in reducing mortality from infectious diseases, child drowning, particularly in the northern regions, remains a neglected public health concern and constitutes the leading cause of death among children aged 1-17 years in rural areas. Approximately 18,268 children (aged 0-17 years) die from drowning annually, equivalent to an average of 50 deaths per day. OBJECTIVE: This study aims to identify the socio-demographic characteristics of caregivers, the types of child supervision and reasons for its absence, water safety skills, tra

Early initiation of breastfeeding and perinatal conditions among mothers in southern Ecuador: a descriptive cross-sectional study.

BACKGROUND: Early initiation of breastfeeding within the first hour of life is a key indicator of newborn care quality and is recommended because of its relevance for neonatal health, survival, and the successful establishment of breastfeeding. However, its implementation varies across healthcare settings, and local evidence remains important for understanding the perinatal circumstances in which breastfeeding initiation occurs. OBJECTIVE: To describe breastfeeding practices during the first hour of life and selected perinatal conditions reported by adult mothers (≥18 years) recruited from fo

Fetal hemothorax due to intrapleural rupture of a small anterior thoracic lymphatic malformation: a case report.

BACKGROUND: Fetal pleural effusion is usually chylous, whereas hemothorax is considerably rarer. Trauma, iatrogenic complications, and rupture of intrathoracic tumors have been reported as causes of fetal hemothorax. Lymphatic malformations often enlarge during the fetal period; however, these benign lesions may regress spontaneously after birth or be managed with sclerotherapy. Accurate prenatal diagnosis is therefore crucial for determining whether fetal intervention or postnatal management is the most appropriate strategy. Here, we present a rare case of a fetus with hemothorax presumably c

Lifespan variation during mortality transition in Bangladesh: evidence from a local surveillance and global estimates.

INTRODUCTION: Variation in life expectancy has become a critical dimension in understanding inequality in human populations, complementing average longevity by capturing how unequally years of life are distributed across individuals. Reducing such variation, especially by preventing premature mortality, has emerged as a key priority in global health and demographic research. For low- and middle-income countries, monitoring lifespan variation alongside life expectancy enables the detection of persistent mortality inequalities and uneven survival gains that average indicators alone may fail to r

Prevalence and factors associated with domestic accidents in children in the district of Orodara, Burkina Faso.

BACKGROUND: In West Africa, domestic accidents are generally underestimated, and only few scientific population-based studies have assessed their prevalence. When available, existing data often focus only a single type of accident. This study aimed to determine the prevalence of domestic accidents and their determinants among children aged 0-14 years in the Orodara health district in Burkina Faso. METHODS: We surveyed a fixed number of households across all 37 villages of the Orodara health district, Kénédougou Province, Burkina Faso. The dependent variable was the occurrence of a domestic ac
