The effect of prenatal intervention on postnatal decision-making for infants born with congenital diaphragmatic hernia.

ObjectiveTo determine the effect, if any, of prenatal intervention in fetuses with congenital diaphragmatic hernia (CDH) on the likelihood of attempted trial of resuscitation (TOR) or withdrawal of life-sustaining measures (WLSM) in the postnatal neonatal intensive care environment.Study DesignFour hypothetical patients with either left- or right-sided CDH were devised using prognostic survival data from the literature, depending on whether they received fetoscopic endoluminal tracheal occlusion (FETO) or expectant management in the prenatal setting. Healthcare providers from the CDH Study Gro

Spatial transcriptomics of the developing mouse brain immune landscape reveals effects of maternal immune activation and microbiome depletion.

Immune molecules and their cognate receptors are developmentally regulated in brain and dynamically expressed in the context of neurodevelopmental disorders. Despite the importance of immune molecules to brain wiring, a comprehensive map of the neuroimmune landscape is lacking in developing brain. Here we employed multiplexed in situ spatial transcriptomics to measure the expression of major immune ligands and receptors in developing mouse brain during mid and late gestation. Given the importance of maternal environment in shaping fetal neurodevelopment, we determined how embryonic neuroimmune

Age-specific differences in patient safety incidents: focus on falls and medication using the Korean patient safety incident reporting and learning system (2016-2023).

BACKGROUND: Patient safety incidents (PSIs) pose a significant global healthcare challenge, especially for vulnerable populations such as children and young adults (CYAs, aged 0–19 years), middle-aged adults (MAs, aged 20–60 years), and older adults (OAs, aged 20–60 years). Falls and medication errors are common hospital-based incidents with serious health impacts. Targeted prevention strategies are needed due to differing age-related risks. Therefore, we aimed to analyze the characteristics and key risk factors of falls and medication incidents across different age groups using 8 years (2016–

Maternal Vaccine Receipt and Infant Hospital and Emergency Visits for Influenza and Pertussis.

IMPORTANCE: Influenza and tetanus-diphtheria-acellular pertussis (Tdap) vaccinations during pregnancy offer protection to infants from infections. However, evidence about their effectiveness against hospitalization and emergency department (ED) visits associated with influenza and pertussis remains limited. OBJECTIVE: This study aimed to evaluate the association of maternal influenza and Tdap vaccinations with influenza- and pertussis-related hospitalizations and ED visits in infants younger than 6 months. DESIGN, SETTING, AND PARTICIPANTS: This population-based cohort study used the health

Cross-fostering affects microglia and cell death in the hippocampus of female and male degu pups.

Parental care is essential for social, behavioural, and neural development of offspring. Disruption of parental care, for example through parental separation, can negatively affect offspring development in rodents. While previous work has focused on maternal and paternal deprivation, the effects of cross-fostering, another form of parental-offspring instability, on brain development remain poorly understood. Such disruptions are thought to induce stress, which in turn can suppress neurogenesis and increases inflammation and apoptosis in the hippocampus, with effects that often differ between s

Implementation of basic training in neonatal resuscitation in the outskirts of Conakry, Guinea: evaluation of neonatal mortality by 'before and after intervention' design.

High neonatal mortality remains a major health problem in Guinea (32 deaths /1,000 live births). This represents 15 000 deaths annually, without improvement over the past decade. We evaluated the impact of 2 days of neonatal resuscitation training of health professionals working in the disadvantaged outskirts of Conakry. Non-randomised interventional study with pre-and post-interventional analysis of the very early neonatal mortality with data collection over two 6-month periods, one before and one after intervention. Intervention: Theoretical and practical training given to health professiona

Prenatal exposure to essential and toxic elements in relation to infant growth trajectories.

BACKGROUND: Metal exposures have been associated with adverse growth in utero, but impacts on postnatal growth are not well-understood. OBJECTIVES: To examine relationships between prenatal metal exposures and infant growth trajectories in a rural U.S. pregnancy cohort. METHODS: Participants included 783 mother-infant pairs in the New Hampshire Birth Cohort Study, a rural cohort of pregnant people and their children in northern New England with homes served by private unregulated drinking water. Essential and toxic element concentrations were measured in maternal toenail clippings collected at

Experiencing acute genomic care: perspectives from parents in the neonatal and paediatric intensive care units towards rapid genomic sequencing.

Rapid genomic sequencing (rGS) is increasingly used in neonatal and paediatric intensive care units (ICUs) to inform diagnosis and guide management of critically ill infants and children. Although rGS has a high diagnostic yield and potential to influence treatment and care planning decisions, little is known about how families experience rGS in the ICU and the emotional and contextual factors influencing their testing-related decisions. We conducted semi-structured interviews with twenty-three parents of infants who consented to rGS in an ICU at two tertiary hospitals in Toronto, Ontario, Can

Prenatal exposure to oral glucocorticoids and risk of long-term neurodevelopmental disorders.

We investigated whether prenatal exposure to oral glucocorticoids (GCs) is associated with long-term neurodevelopmental disorders (LNDDs) in offspring. Using South Korea’s National Health Insurance Service database, we identified all live births from January 1, 2011, to December 31, 2014 (n = 1,553,505) and followed offspring through December 31, 2023. Maternal oral GC exposure during pregnancy defined the exposed group. After 1:10 propensity-score matching, 33,940 exposed and 338,921 unexposed offspring were analyzed. LNDD incidence was 11.5% in the GC group versus 9.8% in the unexposed group

Guidelines for anaesthesia and sedation for patients who are breastfeeding: Guidelines from the Association of Anaesthetists.

INTRODUCTION: Breastfeeding is acknowledged widely as one of the most effective ways to ensure the health and well-being of both child and birth parent. Historically, advice given to patients who required an anaesthetic while breastfeeding was variable and inconsistent, sometimes resulting in the interruption of feeding for ≥ 24 h, or expressing and discarding breastmilk because of concerns regarding the possible adverse effects secondary to medicines passing into the breastmilk. This can be a contributory factor in the early cessation of breastfeeding. Peri-operative decisions can normally be

Rapid, effective, and affordable randomisation for emergency neonatal research in a low-resource setting: a feasibility randomised controlled trial.

BACKGROUND: Randomisation is an essential component of any clinical trial to eliminate selection bias and ensure similar distribution of confounders between the treatment groups. For randomisation to be successfully implemented, there must be adequate allocation concealment. Many low-cost randomisation and allocation concealment methods have the potential to introduce bias by ineffectively concealing the allocation sequence from the researcher and are now rarely used in high-resource settings. In such settings, centralised randomisation services have been developed to prepare the randomisation

Initial supplementary oxygen concentration for moderate-late preterm infants receiving respiratory support in the delivery room: statistical analysis plan for the multicenter, cluster-randomized crossover AIROPLANE Trial.

BACKGROUND: Moderate-late preterm infants born at 32-35 completed weeks' gestation constitute a large proportion of all preterm births (< 37 weeks' gestation), yet they are not well represented in the newborn resuscitation literature. Preterm infants often receive respiratory support in the delivery room, and recommendations exist to guide the use of supplemental oxygen when providing this support. However, there are minimal data regarding the best initial supplementary oxygen concentration for moderate-late preterm infants requiring respiratory support at birth, resulting in practice variatio

Benefits and challenges of voucher-based transport for skilled birth attendance in Kitui County, Kenya: the health stakeholders' perspectives.

BACKGROUND: Reducing maternal, newborn and child mortality remains one of the top global public health priorities. Each year, approximately 303,000 mothers, globally, die during delivery. Stillbirths and neonatal deaths within the first 28 days of life are estimated at 2.6 million and 2.7 million, respectively. Rural residents face challenges in accessing health care due to difficulties in transportation. Given the voucher-based transport's potential to address inaccessibility to health services, this study explored the health stakeholders' perspectives on the benefits and challenges of this i

Women's experiences of unexpected situations during childbirth: A qualitative study.

BACKGROUND: An unexpected situation during childbirth is a significant risk for a negative or traumatic birth experience. Existing knowledge of such situations is fragmented, and research has tended to focus on medical problems, interventions, or women's expectations rather than on their overall experience. OBJECTIVE: To describe women's experiences of unexpected situations during childbirth. METHODS: This qualitative descriptive study used purposive sampling to recruit participants by posting an invitation on two social media sites popular among mothers of newborns. Eighteen women who had g

Avoiding routine gastric residual volume measurement in neonatal critical care (the neoGASTRIC trial): study protocol for a multi-centre, unblinded, randomised, controlled trial.

BACKGROUND: Routine measurement of gastric residual volumes involves regularly aspirating the entire stomach contents to assess the volume and colour of the aspirate to inform feeding. This is an established practice in many United Kingdom and Australian neonatal units for preterm infants receiving gastric tube feeds. The rationale is to assess feed tolerance and to predict and potentially prevent necrotising enterocolitis, a serious gut condition. Routine measurement of gastric residual volumes may also be associated with adverse outcomes and harm, including delayed achievement of full entera

A single-cell transcriptomic atlas of the periventricular proliferative zone in the late gestation fetal brain in the pigtail macaque.

BACKGROUND: The fetal brain undergoes rapid changes in late gestation, when waves of neurogenesis and gliogenesis shape cortical circuitry. The periventricular proliferative region and adjacent white matter are enriched in neuroprogenitor cells, newborn neurons, and interneurons, which is challenging to study in the late gestation human fetal brain. The nonhuman primate (NHP) provides a powerful translational model to overcome this limitation, given its close similarity to human neurodevelopmental trajectories. The study objective was to construct a single-cell RNA-Seq (scRNA-Seq) atlas of the

Pathways of change for essential newborn care practices and health care seeking: a process evaluation of Mamás del Río, a community-based, maternal and neonatal health intervention in the Peruvian Amazon.

In rural Indigenous communities in the Peruvian Amazon, access to quality care is difficult, home births are frequent, and neonatal mortality remains high. Peru has a large cadre of Community health workers (CHW), yet their potential is not harnessed. A recent outcome evaluation of a community-based intervention showed improvements in essential newborn care (ENC) for home births and small increases in facility births. To explain these findings, elucidate the pathways of change, and derive policy recommendations, we conducted a mixed-methods process evaluation. Implementation strength, mechanis
