Heat and its effects on maternal and neonatal health care: Evidence from Burkina Faso.

OBJECTIVES: Climate change has a wide range of adverse consequences for human health. The effects of extreme weather on maternal and newborn health, as well as the availability and quality of health services, remain under-researched in the sub-Saharan region. The aim of this study is to examine the effects of heat on pregnant women's use of health services, the health facility working environment and the impact of heat on the quality of care in Burkina Faso, a limited resource country. STUDY DESIGN: We adopted a qualitative approach to address the research objectives. METHODS: This study was

Growth failure and early recurrence in congenital diaphragmatic hernia: An international cohort study.

PURPOSE: The primary objective was to determine whether growth failure (GF) is associated with early recurrence in congenital diaphragmatic hernia (CDH). The secondary objective was to characterize the prevalence and severity of GF in CDH survivors. BACKGROUND: GF is common among CDH survivors, yet it is unclear how growth challenges affect overall morbidity, including hernia recurrence. CDH-specific nutritional guidelines include high calorie and protein targets during periods of acute illness but may benefit from clarifying the relationship between growth and adverse outcomes in this popula

Adherence to a restrictive platelet transfusion guideline in a neonatal surgical population.

BACKGROUND: In 2019, the PlaNeT-2 trial reported an increased risk of death and/or major bleeding among neonates transfused with platelets at liberal compared to restrictive thresholds. However, especially in the perioperative setting, clinicians often administer platelet transfusions to neonates at higher platelet count thresholds in hopes of reducing the risk of bleeding. In this study, we investigated if platelet transfusion practices changed in neonates with primary surgical diagnoses after the implementation of a restrictive platelet transfusion guideline in January of 2019. METHODS: In

Research on age-stratified prevention and control strategies for pediatric RSV infection based on disease burden disparities.

INTRODUCTION: Due to the lack of effective antiviral treatments for respiratory syncytial virus, therefore, understanding the age-specific disparities in the burden of RSV infection can help identify targeted interventions. METHODS: This study stratified pediatric patients with RSV-related hospitalizations by age and compared demographic information, clinical features, imaging data, treatment, and outcomes. RESULTS: The prevalence of RSV infection was higher in preterm infants under 2 years of age compared to those over 2 years. Among children over 1 year, the proportion with atopic disorders

Human Milk Practices in Swedish Neonatal Units: Results From a Nationwide Survey.

AIM: The aim of this study was to describe the routines for handling mother's own milk and donor human milk in Swedish neonatal care units related to national guidelines. METHODS: A web-based survey of 24 of 38 neonatal units and 19 of 28 human milk banks in Sweden. RESULTS: Despite national guidelines, breast milk handling varied widely. All units offered lactation support and free breast pumps during hospital stays; nine continued post-discharge. Use of mother's milk differed; some prioritised fresh, others mixed or used frozen milk by age. Donor milk was mainly given to infants < 34-35 we

Rectus sheath hematoma in pregnancy: a case report with literature review.

Rectus sheath hematoma (RSH) is a rare cause of abdominal pain during pregnancy. In our case, a healthy pregnant woman presented with right upper abdominal pain at 33 weeks of gestation. Ultrasound showed a heterogeneous hypoechoic mass in the abdominal wall, which was suspected of being a rectus sheath hematoma. After half a day of conservative therapy, the progressive decrease in hemoglobin level suggested active bleeding into her rectus sheath. Consequently, surgery was initiated under general anesthesia. During the operation, we found that the right musculus rectus abdominis was ruptured a

Validating CALIPER pediatric reference intervals in a U.S. population using retrospective outpatient data and RefineR.

BACKGROUND: Accurate pediatric reference intervals (RIs) are critical for proper interpretation of laboratory results, yet their development is challenged by age-related physiological variation, limited sample sizes, and preanalytical variability. This study evaluates the applicability of the Canadian Laboratory Initiative on Pediatric Reference Intervals (CALIPER) RIs to a US-based, diverse pediatric outpatient population using both the RefineR inverse modeling approach and the Clinical and Laboratory Standards Institute (CLSI) EP28-A3c recommended approach. METHODS: Outpatient laboratory da

Development and validation of standard-criteria and age-corrected nomograms for post-stroke cognitive impairment risk stratification.

OBJECTIVE: The investigation aimed to develop and validate two complementary prognostic nomograms for post-stroke cognitive impairment (PSCI) among acute ischemic stroke patients. METHODS: In this prospective cohort study, 336 patients were enrolled for model development and internal validation, with 48 patients for external validation. Cognitive performance was evaluated using the Montreal Cognitive Assessment (MoCA) at six months post-stroke. The standard-criteria model defined PSCI as MoCA <26, while the age-corrected model applied age-specific cutoffs (<26 for <60 years, <25 for 60-69, <2

[Evaluating the cause of death in children that died unexpectedly; parents' experiences with the PESUDY-procedure].

OBJECTIVE: The death of a child is (one of) the most devastating life events for parents. A natural order of life is that children outlive their parents. In the Netherlands, parents can choose for investigation into the cause of death using the standardized Postmortem Evaluation of Sudden Unexplained Death in Youth (PESUDY) procedure. Parents' experiences with this procedure have not been well investigated. DESIGN AND METHODS: A qualitative semi structured interview study focusing on the experiences and expectations of parents during a postmortem PESUDY-procedure after the sudden death of the

Resting energy expenditure measured by indirect calorimetry in children with cancer: A systematic literature review.

BACKGROUND & AIMS: Malnutrition can have serious effects on patient outcomes in children with cancer, including a decreased chance of survival, increased risk of treatment-related toxicities and an impaired quality of life. However, it remains insufficiently understood which factors are key determinants in the development of malnutrition in children with cancer. Given that resting energy expenditure (REE) is an important component of energy balance, we aimed to investigate whether REE is altered in children with cancer by reviewing and summarizing current literature comparing measured REE (mRE

Predicting pulmonary hemorrhage in very low birth weight infants.

BACKGROUND: Pulmonary hemorrhage (PH) in very low birth weight (VLBW) infants is a catastrophic event with significant mortality. Predicting PH clinically is challenging, with no tools for early detection. Our objective was to identify candidate physiologic biomarkers using heart rate (HR) and pulse oximetry (SpO2) predictive of PH events in VLBW infants. METHODS: We conducted a retrospective case-control study of VLBW infants from two level IV NICUs (2016-2023). We matched infants with PH to controls using gestational age, birth weight, and postnatal age. Taking HR and SpO2 data from 72 hour
