Cost-effectiveness of implementing a multidisciplinary team approach in the management of placenta accreta spectrum.

OBJECTIVE: Placenta accreta spectrum (PAS) is associated with high maternal morbidity. Recent studies have shown that a multidisciplinary team (MDT) approach to managing PAS can improve outcomes. This study aimed to evaluate the cost-effectiveness of implementing an MDT for the management of PAS. METHODS: A decision-analytic model was built using TreeAge software to compare the outcomes and cost-effectiveness of the MDT approach versus the standard of care. The theoretical cohort included 5000 individuals in the United States with PAS annually. Maternal outcomes included severe postpartum hem

Intertwin differences in umbilical venous diameter and flow reflect placental sharing and perinatal outcome in monochorionic twins with selective fetal growth restriction.

BACKGROUND: In monochorionic diamniotic (MCDA) twin pregnancies with selective fetal growth restriction (sFGR), antenatal risk stratification relies primarily on umbilical artery (UA) Doppler assessment of the smaller twin. However, UA patterns may change, and placental sharing cannot be measured accurately. Umbilical venous (UV) diameter and flow may reflect each twin's oxygenated blood supply more directly. OBJECTIVE: To evaluate if intertwin differences in UV diameter and flow provide information on placental sharing and perinatal outcomes, in addition to established ultrasound markers. S

Dysregulated differentiation kinetics underlie the essential role of DNA damage repair in placental development.

The placenta orchestrates maternal-fetal exchanges, and its dysfunction compromises pregnancy outcomes. Somatic cell nuclear transfer (SCNT) placentas offer a model to investigate such dysfunction. However, SCNT placentas exhibit severe pathological features that remain poorly understood. Using single-nucleus multi-omics profiling, we uncover defective differentiation programs in SCNT placentas, including the persistent multipotency of arrested trophoblast precursors and an aberrant differentiation trajectory in the junctional zone. In addition, SCNT placentas demonstrate impaired VEGF signali

IFN-γ+ NK cells as a potential predictor of pregnancy loss in unexplained recurrent pregnancy loss.

BACKGROUND: Natural Killer (NK) cells are the largest population of lymphocytes in the endometrium during early pregnancy and play a key regulatory role in trophoblast cell invasion and embryo implantation at the maternal-fetal interface. Previous studies on the roles of NK cells with distinct phenotypes in pregnancy loss were based on NK cells derived from the peripheral blood or decidua, and it is difficult to determine whether the reported changes in decidual NK cells are causes or consequences of pregnancy loss. OBJECTIVE: We aimed at exploring the functional NK cells during the implantat

Internal iliac artery balloon occlusion for placenta accreta spectrum disorder: outcomes and factors influencing efficacy.

BACKGROUND: Placenta accreta spectrum disorder (PASD) is a major obstetric complication associated with life-threatening haemorrhage. Prophylactic internal iliac artery balloon occlusion (IIABO) is used in many centres to reduce intraoperative blood loss but success rates are variable. METHODS: We reviewed 20 consecutive patients with PASD over a twenty-one-month period stratified by procedure type [total abdominal hysterectomy (TAH) versus conservative myometrial repair (MR)] and balloon occlusion use. Estimated blood loss (EBL) was recorded. Welch's t-tests compared groups, and a two-way an

The role of lactate metabolism in uterine spiral artery remodelling.

Lactate is a key metabolite of glycolysis. Since the proposal of the lactate shuttle hypothesis, accumulating evidence indicates that lactate is not only an essential energy substrate but also a key signalling molecule that regulates cellular functions under both physiological and pathological conditions. Uterine spiral artery (SpA) remodelling is a critical event in successful pregnancy. It involves complex interactions among trophoblasts, vascular smooth muscle cells (VSMCs), decidual immune cells, and endothelial cells (ECs). Similar to the tumour microenvironment, physiological hypoxia at

Clinical characteristics and outcomes of uterine compression sutures for postpartum hemorrhage during cesarean delivery.

OBJECTIVE: Uterine compression sutures (UCS) serve as a uterus-preserving option for the management of postpartum hemorrhage (PPH); however, the clinical characteristics associated with the need for additional treatment after UCS remain incompletely characterized. This study aimed to explore clinical factors associated with escalation to additional treatment after UCS. METHODS: We conducted a single-center retrospective cohort analysis of 33 patients who underwent UCS for PPH during cesarean delivery between January 2011 and July 2025. Patients were categorized based on management with UCS al

Multiscale computational modeling to quantify how spiral artery remodeling alters wall shear stress on placental villi.

Proper placental development is essential for a healthy pregnancy. It depends on the remodeling of the maternal uterine vasculature to meet fetal demands while maintaining physiological intervillous space (IVS) hemodynamics for biochemical exchange. Terminal villi, the primary sites of feto-maternal exchange, exhibit impaired development in pregnancies complicated by intrauterine growth restriction and preeclampsia, which are also associated with incomplete spiral artery (SA) remodeling. Despite this association, the mechanistic link between maternal blood flow and villous development remains

A physiological link between uterine perfusion and endocervical length reveals a novel interaction during early placentation.

OBJECTIVE: Endocervical length is a strong predictor of spontaneous preterm birth, yet the physiological processes shaping its baseline architecture during early placentation remain unclear. We explored whether endocervical length relates to uterine perfusion in first-trimester pregnancies, assessed by vascular resistance-based measurements and quantitative uterine artery (UtA) blood flow volume. METHODS: In a prospective cohort of 446 consecutive pregnancies undergoing ultrasound examination between 6 and 13 weeks' gestation, UtA impedance indices and blood flow volume were measured using pu

Glycolysis-derived lactate contributes to H3K18la modification and CCL27 expression to promote placental angiogenesis.

Numerous studies have shown that impaired placental angiogenesis results in a spectrum of gestational diseases, posing significant risks to both maternal and fetal health. Yet, the metabolic patterns and underlying mechanisms that govern placental angiogenesis remain poorly understood. Here, we examined the metabolic reprogramming during placental development and the role of its metabolic derivative, lactate, in placental angiogenesis and the associated mechanistic pathways. Firstly, a pronounced metabolic shift toward glycolysis, accompanied by significant lactate accumulation, was identified

Minimizing blood sampling in preterm infants.

RATIONALE: Iatrogenic blood loss contributes substantially to anemia and transfusion exposure in preterm infants. The effectiveness of interventions aimed at minimizing blood sampling remains unclear. Most prior trials have focused on placental transfusion strategies rather than blood conservation during postnatal care. OBJECTIVES: To evaluate the benefits and harms of strategies to minimize blood sampling, compared with standard blood sampling, in preterm infants. SEARCH METHODS: We used CENTRAL, PubMed, two other databases, and two trial registers, together with reference checking and hand

Maternal-Fetal Letermovir Exposure In Vivo and Physiologically Based Pharmacokinetic Modeling: A Rationale for Congenital Cytomegalovirus Therapy.

BACKGROUND: Cytomegalovirus (CMV) affects 0.5%-2.0% of all live births and is the main nongenetic cause of congenital sensorineural hearing loss and neurological damage, yet no validated prenatal treatment exists for infected fetuses. Letermovir, a CMV-specific antiviral, is approved for prophylaxis in transplant recipients. This study aimed to quantify maternal-fetal exposure to letermovir following maternal administration, focusing on CMV-target fetal organs, particularly the brain. METHODS: Seven pregnant women undergoing second-trimester termination of pregnancy for fetal anomalies receiv

Complement Component 1q Deficiency Impairs Maternal Hemodynamic Adaptations to Pregnancy, Decidual Vascular Remodeling, and Fetal Growth in Mice.

BACKGROUND: Pregnancy complications such as preeclampsia and fetal growth restriction are caused by compromised placentation associated with impaired maternal cardiovascular and immune adaptation to pregnancy, but the pathophysiological pathways underlying these impairments are not fully defined. C1q (complement component 1q) plays a central role in immune cell recruitment, functional regulation, and immune tolerance. In this study, we investigated the impact of maternal C1q deficiency on maternal uterine and systemic cardiovascular parameters in pregnancy. METHODS: C1q null mutant (C1qa-/-)

Gut microbiota in patients receiving dialysis: Mechanisms, complications and the road to personalized microbiome‑based care (Review).

UNLABELLED: In vivo evaluation of the immunotoxicity of xenobiotics is mainly carried out in rodents. Recent trends following the 3R (reduction, refinement, replacement) principle have driven development of in vitro methods for screening immunotoxic chemicals. The recently reported IMMUNOTOX-T immunotoxicity assay uses the human monocytic leukemia cell line THP-1, and evaluates potential immunotoxicity by profiling production of various cytokines by cells treated with test chemicals. Development of the assay involved small-scale evaluation of transferability, within-laboratory repeatability (W

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l
