[MALACOPLAKIA OF THE URINARY BLADDER: A CASE REPORT WITH DIAGNOSTIC DIFFICULTY].

We herein report a rare case of malacoplakia of the urinary bladder that demonstrated significant diagnostic difficulty. The patient was a 65-year-old woman with a history of breast cancer, dermatomyositis, steroid-induced diabetes mellitus, and cutaneous cryptococcosis. She presented with repeated episodes of dysuria and hematuria since the age of 62. Computed tomography (CT) revealed progressive bladder wall thickening, and she was therefore diagnosed with cystitis by her primary care physician and treated with antibiotics. Her symptoms worsened at 63 years of age, and further bladder wall t

Probable disseminated cryptococcosis relapse in a patient with Hodgkin's lymphoma treated with induction therapy resumption and autologous stem cell transplant.

Cryptococcus species are fungi that primarily cause infections in immunocompromised patients. In cases of hematologic malignancies, it can result in central nervous system and disseminated infections which are associated with high morbidity and case-fatality rate. We present the case of a patient with Hodgkin's lymphoma who experienced a probable relapse in disseminated cryptococcosis during the consolidation phase of treatment. After prolonged induction therapy with amphotericin B and flucytosine, followed by isavuconazole maintenance, the patient achieved a favourable outcome. Six months aft

Adrenal Insufficiency due to Disseminated Cryptococcus in an Immunocompetent Individual.

Primary adrenal insufficiency due to infiltrative fungal infections, such as Cryptococcus neoformans, is rare, particularly in immunocompetent patients. We present a case of an immunocompetent 61-year-old man who presented with bilateral adrenal enlargement and adrenal insufficiency, with a 3-month history of generalised fatigue, weight loss and dizziness, as well as initial hyponatraemia and hyperkalaemia. Adrenal computed tomography (CT) revealed bilaterally enlarged adrenals, and fluorodeoxyglucose positron emission tomography (FDG-PET) revealed increased peripheral metabolic surrounded by

Cryptococcosis in Colombia: Analysis of Data from Laboratory-Based Surveillance 2017-2024.

Since 1997, a laboratory-based survey on cryptococcosis has been conducted in Colombia. We present the results for the period 2017-2024. A total of 891 surveys were received. The overall incidence was 0.22 cases per 100,000 people. Among those living with HIV, the incidence was 38, and among HIV-negative people, it was 0.08. Cryptococcosis demonstrated a higher prevalence among men than women (3.2:1). Among patients living with Human Immunodeficiency Virus (HIV), the condition primarily affected younger adults (26-40 years). In contrast, among HIV-negative people, it was mostly observed in old

[Disseminated cryptococcosis resulting in fungal septic shock: a case report].

Fungal sepsis and septic shock are syndromes involving acute organ dysfunction that result from a dysregulated host response to invasive fungal infections. These syndromes are characterized by diagnostic challenges, rapid progression, complex management, high medical costs, and high mortality, particularly among immunocompromised patients. This article mainly presents a case of fungal septic shock caused by disseminated Cryptococcosis, which resulted in death within a short period. It emphasizes the importance of the principle of "early warning and rapid intervention" in clinical practice for

18 F-FDG PET/CT in Disseminated Cryptococcosis Mimicking Malignancy With Rib Osteolysis and Chest Wall Invasion.

Disseminated cryptococcosis is a rare systemic fungal infection that can mimic thoracic malignancies. We report a 56-year-old man with known cryptococcosis on antifungal therapy who underwent 18 F-FDG PET/CT for response evaluation. Imaging revealed a heterogeneously FDG-avid pleural-based right upper lobe mass with lytic destruction of the first and second ribs and extension into chest wall musculature, with pulmonary nodules and nodal disease. Interval imaging demonstrated metabolic regression of adrenal and pulmonary lesions. This case highlights the diagnostic challenge of cryptococcosis p

Successful Long-Term Use of Isavuconazole in a Tacrolimus-Treated Japanese Kidney Transplant Recipient With Disseminated Cryptococcosis and Probable Invasive Aspergillosis.

Disseminated cryptococcosis, particularly cryptococcal meningitis, remains a severe fungal infection with high mortality in immunocompromised patients and usually requires prolonged azole therapy. In kidney transplant recipients receiving tacrolimus (TAC), careful management of drug-drug interactions via CYP3A is essential to maintain stable blood concentrations. We report the case of a 44-year-old man with a history of focal segmental glomerulosclerosis who had undergone a living-donor kidney transplant three years earlier. He was receiving immunosuppressive therapy with TAC, mycophenolate mo

Emerging cryptococcosis cases in Himalayan & sub-Himalayan regions of India.

Cryptococcosis is a potentially life-threatening opportunistic fungal infection presenting with a wide spectrum of clinical manifestations. This retrospective study (2018-2024) from North India analyzed 33 cryptococcosis cases, 66.7% HIV-positive. Fever, headache, and altered sensorium predominated. Overall mortality reached 51.5% at one year. Uncontrolled diabetes and limited amphotericin B use among Himalayan patients correlated with higher mortality. Equitable antifungal access remains crucial.

Invasive Candidiasis in Liver Transplant Recipients Undergoing Antifungal Prophylaxis.

INTRODUCTION: Invasive candidiasis (IC) is the most frequent invasive fungal infection (IFI) affecting liver transplant (LT) recipients. Antifungal prophylaxis (AP) effectively reduces the incidence IFI after LT. However, a considerable proportion of LT recipients undergoing AP are still affected by IC. OBJECTIVES: To assess the incidence and factors associated with IC in high-risk LT recipients undergoing AP. PATIENTS AND METHODS: Retrospective single-center study including high-risk LT recipients undergoing AP between 2015 and 2020. The primary outcome was the occurrence of IC within the f

Disseminated Cryptococcosis in a Non-HIV Patient: Diagnostic Value of Integrating mNGS, Culture, and Antigen Testing.

Cryptococcus neoformans is an opportunistic fungal pathogen most commonly observed in individuals with human immunodeficiency virus (HIV) infection. Disseminated infections involving multiple organs, such as lungs, bloodstream, urinary tract, pleural cavity and central nervous system, are uncommon in patients without HIV. This work describes the case of a 72-year-old man who developed high fever and increased inflammatory markers after coronary artery bypass surgery. C. neoformans (8 sequence reads) was initially detected by metagenomic next-generation sequencing (mNGS) of bronchoalveolar lava

Clinical Significance of Serum (1→3)-β-D-Glucan Positivity in Cryptococcosis: A Retrospective Cohort Study.

Cryptococcosis is associated with negative serum (1→3)-β-D-glucan (BDG) results, but the clinical significance of serum BDG (sBDG) positivity is unclear. We characterized the clinical features of sBDG-positive cryptococcosis in patients with cryptococcal disease. We measured the sBDG levels and classified patients as BDG-positive (>11 pg/mL) or BDG-negative. Clinical characteristics, diagnostic modalities, antifungal treatment, post-treatment BDG level changes, and 90-day cryptococcosis-related mortality were compared. BDG-positive patients showed disseminated cryptococcosis, higher inflammato

A soft tissue lesion on the scalp as a peculiar source of disseminated cryptococcosi.

Cryptococcosis is a potentially fatal invasive fungal infection with considerable health challenges. A seemingly immunocompetent middle-aged man with frequent exposure to pigeons and bird excreta presented with headaches, fever, gait instability and diplopia. Two years prior to his presentation, he sustained a blunt head trauma which resulted in sporadic headaches. On examination, a hard scalp nodule measuring approximately 3 x 3 cm was palpable on the topmost part of his head. Cryptococcus neoformans was cultured from his cerebrospinal fluid, blood and the biopsied scalp nodule. A diagnosis o

Early Disseminated Cryptococcosis Following Orthotopic Heart Transplantation Requiring VA-ECMO: A Case Report.

Disseminated cryptococcosis is a rare but serious opportunistic infection following solid organ transplantation and rarely occurs within the first month of transplantation. We report a case of disseminated Cryptococcus neoformans infection occurring 14 days after orthotopic heart transplantation which required mechanical circulatory support with venoarterial extracorporeal membrane oxygenation (VA-ECMO). Management was complicated by alterations in antifungal pharmacokinetics and the inability to perform serial lumbar punctures while anticoagulated. Antifungal therapy with nonliposomal amphote

Progression of Primary Cutaneous Cryptococcosis to Disseminated Cryptococcosis in an Immunosuppressed Older Patient: A Case Report.

Primary cutaneous cryptococcosis (PCC) is acquired through direct inoculation of Cryptococcus neoformans into wounds or damaged skin, typically resulting in a localized infection confined to a single cutaneous site. Progression of PCC to disseminated cryptococcosis is extremely rare. The purpose of this case report is to describe an unusual instance of PCC progressing into disseminated disease in an immunosuppressed older patient and to highlight the importance of considering cryptococcosis as a differential diagnosis of cutaneous lesions unresponsive to antimicrobial therapy. We report the c

Diagnosis of a patient with pulmonary and skeletal cryptococcosis: A case report and literature review.

Cryptococcosis is an invasive mycosis and can disseminate an infection in immunocompromised hosts. The main sites of infection include the central nervous system and lungs; skeletal infections are rarely reported. We describe the case of a Chinese man in his early 40s with a 1-year history of dermatomyositis and prolonged use of corticosteroids and immunosuppressants. During re-examination of a previously discovered pulmonary nodule, lung computed tomography revealed that the nodule had grown in size, and a bone scan revealed local bone abnormalities and mass formation in the left scapula.
