Rutin inhibits inflammatory responses by modulating purinergic enzymes and purine metabolism in Cryptococcus neoformans var. grubii-infected rats.

The aim of this study was to evaluate whether intraperitoneally administration of 50 mg rutin/kg body weight exerts a protective effect against Cryptococcus neoformans var. grubii-induced inflammatory damage by modulating serum purinergic enzymes and molecules. In a study, rats treated with a saline solution and infected with C. neoformans var. grubii demonstrated significant reductions (p < 0.05) in E-nucleoside triphosphate diphosphohydrolase (E-NTPDase) activity using adenosine triphosphate (ATP) as a substrate, as well as E-adenosine deaminase (E-ADA) activity, compared to the saline-treat

Disseminated Skeletal Cryptococcosis: A Case Report.

INTRODUCTION: Cryptococcus is a rare cause of osteomyelitis, especially in immunocompromised individuals. This case report discusses a rare case of disseminated cryptococcosis with multiple bone lesions in a patient with isolated CD4 lymphocytopenia. CASE REPORT: A 31-year-old apparently normal Indian male presented with pain and swelling of his right proximal femur for 8 months without any history of trauma. He also reported a similar swelling in his chest wall with allergic respiratory symptoms for 8 years. Laboratory analysis revealed mild elevation in inflammatory markers. Magnetic resona

Immunophenotyping of apparently immunocompetent hosts with cryptococcosis reveals IL-17 deficiency as a unifying susceptibility factor.

INTRODUCTION: We describe the immunophenotyping and genetic analysis of HIV-uninfected apparently immunocompetent adults presenting with disseminated cryptococcosis. Cryptococci are environmentally ubiquitous fungi that may cause disseminated infection including meningitis. Cryptococcosis occurs predominantly in immunocompromised hosts and most commonly in the context of human immunodeficiency virus (HIV) infection. In apparently immunocompetent patients, cryptococcal disease is rare, often diagnosed later and associated with higher mortality. The immunologic work-up and management of this pat

Bone marrow cryptococcosis in an HIV-infected patient: A case report and literature review.

BACKGROUND AND PURPOSE: Cryptococcosis is a common opportunistic fungal infection in immunocompromised individuals. It is typically isolated from cerebrospinal fluid, sputum, or skin lesions. However, disseminated cryptococcosis involving the bone marrow is an extremely rare occurrence. CASE REPORT: This case describes a 34-year-old male patient living with HIV who developed fever, abdominal pain, and pancytopenia. Diagnostic evaluation, including bone marrow aspiration and biopsy, revealed encapsulated yeasts consistent with Cryptococcus sp. Although blood and cerebrospinal fluid cultures fa

Mediastinal cryptococcoma as part of disseminated cryptococcosis in HIV-negative pregnant woman with Myasthenia Gravis: Autopsy case report.

Cryptococcosis is a fungal infection commonly found in immunocompromised individuals, but it can also affect immunocompetent individuals. A pregnant woman with myasthenia gravis, HIV-negative, died after a diagnosis of tuberculosis. An autopsy revealed a mediastinal mass and organ involvement, indicating a disseminated cryptococcal infection. This case highlights the importance of maintaining a high index of suspicion for cryptococcal infection in immunosuppressed individuals, even without HIV/AIDS. Early detection is achieved by measuring serum cryptococcal antigen.

Addison's disease due to adrenal insufficiency in an immunocompetent patient with disseminated cryptococcosis. Case report.

Systemic mycoses, such as cryptococcosis, mainly affect the lungs and central nervous system; however, involvement of other organs, such as the adrenal glands, is rare. This has been described in some cases of primary adrenal insufficiency (PAI) of fungal origin, which are associated with high mortality. We present the case of a 65-year-old immunocompetent man who presented clinical manifestations of adrenal insufficiency. Biochemical tests confirmed PAI, and abdominal tomography revealed hyperplasia of both adrenal glands, predominantly on the left side. Left adrenalectomy was performed, and

Disseminated cryptococcosis presenting as lung mass.

Cryptococcosis is an opportunistic fungal infection in immunocompromised individuals, with a disease spectrum ranging from asymptomatic pulmonary infection to fatal meningoencephalitis. This case report describes the case of a man with diabetes in his late 40s who presented with chest pain and generalised weakness. Contrast-enhanced CT of the chest revealed a mass lesion in the left upper lobe. Ultrasound-guided biopsy of the mass revealed a diagnosis of cryptococcosis. Cerebrospinal fluid and blood samples tested positive for cryptococcal antigens. A final diagnosis of disseminated cryptococc

An 80-year-old man with a rare disease manifestation of disseminated cryptococcosis.

We present an extraordinary manifestation of disseminated cryptococcosis in a man who developed a triad of acute panuveitis, autoimmune hemolytic anemia, and pulmonary cryptococcal infection. The patient exhibited progressive clinical deterioration with mediastinal lymphadenopathy, fever and severe hemolysis. Despite initial negative cryptococcal antigen testing, histopathological examination of a pulmonary nodule demonstrated yeast forms consistent with cryptococcal infection. The diagnosis was confirmed through molecular diagnostics and isolation of Cryptococcus neoformans from vitreous humo

Disseminated cryptococcosis involving the adrenal gland and brain in an immunocompetent patient: A case report.

RATIONALE: Cryptococcosis is a fungal infection that primarily occurs in immunocompromised individuals. This infection typically affects the lungs and central nervous system, while adrenal involvement is relatively uncommon. Our research reports a rare case of adrenal cryptococcosis in a patient with normal immune function, with suspected involvement of the nervous system. PATIENT CONCERNS: A 51-year-old male patient with normal immune function presented with progressive symptoms of dizziness, palpitations, and fatigue, which had persisted for 2 years. Imaging studies revealed a hypoechoic ma

Cryptococcal Infection in a Patient With Chronic Lymphocytic Leukaemia Receiving Acalabrutinib, a Bruton's Tyrosine Kinase Inhibitor: A Case Report.

A male patient in his mid-sixties with chronic lymphocytic leukaemia (CLL), Binet stage B, receiving the Bruton's tyrosine kinase (BtK) inhibitor acalabrutinib, presented with fever and other non-specific systemic symptoms. Following the investigation, they were found to have Cryptococcal meningitis. Cryptococcal infections are known to occur in immunocompromised patients, but they are rarely encountered or thought about during the initial acute medical assessment. This case highlights the importance of considering the possibility of opportunistic infections and the atypical presentation of su

Successful desensitization to liposomal amphotericin B for the treatment of disseminated cryptococcosis in pregnancy: a case report.

BACKGROUND: Cryptococcosis is the most common opportunistic fungal infection in people with human immunodeficiency virus. On a global scale, it causes 15% of acquired immunodeficiency syndrome-related mortality. CASE SUMMARY: We report the case of a 21-year-old cisgender pregnant woman living with human immunodeficiency virus who presented with headache and photophobia. Her clinical course was complicated by recurrent hospital admissions for disseminated cryptococcosis and a life-threatening reaction to liposomal amphotericin B, the mainstay of treatment. A desensitization protocol was implem

Cutaneous cryptococcosis resembling keratoacanthoma.

Cutaneous cryptococcosis is a manifestation of systemic cryptococcal infection, characterized by polymorphic skin lesions that can make diagnosis challenging. We report an HIV-positive patient with a solitary facial nodule, initially diagnosed as keratoacanthoma. Histopathology, fungal culture, and cerebrospinal fluid analysis confirmed disseminated cryptococcosis with central nervous system involvement caused by Cryptococcus neoformans genotype VNI. He was treated with amphotericin B followed by fluconazole, resulting in complete clinical resolution. This case underscores the need to consider

Low Antigen Titre Disseminated Cryptococcosis in Immunocompromised Hosts: Two Challenging Case Reports.

BACKGROUND Cryptococcus is a yeast-like fungus commonly found in soil and typically causes disease in immunocompromised individuals due to impaired cell-mediated immunity. While the central nervous system (CNS) and lungs are most frequently affected, disseminated disease can involve multiple organs, including the skin, liver, spleen, kidneys, and bone marrow. Symptoms such as headache, fever, and altered mental status often require extensive diagnostic evaluation. CASE REPORT We present 2 unusual cases of cryptococcal growth in the blood without meningoencephalitis, with low cryptococcal antig

Trend of Candida species profiles and their susceptibility to various antifungals in Jakarta, Indonesia.

BACKGROUND AND PURPOSE: Pathogenic fungi, including both true and opportunistic pathogens, pose significant health risks, particularly in immunocompromised individuals. Species, such as Candida albicans, Aspergillus fumigatus, and Cryptococcus neoformans, cause fatal infections and frequently develop resistance to conventional antifungal therapies. Limitations of current antifungal medications, such as drug toxicity, resistance development, and environmental concerns, highlight the urgent need for novel therapeutic strategies. Earthworm extracts, particularly those derived from Eisenia fetida,

Invasive pulmonary cryptococcosis mimicking metastatic lung cancer: A case report and review of literature.

BACKGROUND: Worldwide, there has been a steady increase in the number of cases of pulmonary cryptococcosis diagnosed in immunocompetent patients, where symptoms can range from mild to severe. Clinical and radiological distinction of disease may be made when compared with immunodeficient cases and in those presenting with primary lung carcinoma. In the latter case confusion can lead to initial misdiagnosis and delayed treatment. We report a case of disseminated cryptococcosis in an immunocompetent patient which mimicked a primary lung carcinoma with brain metastases. CASE SUMMARY: A 51-year-ol
