Clinical and environmental isolates of Cryptococcus neoformans in Bangkok (Thailand).

Cryptococcus neoformans was isolated from 13 patients (7 females and 6 males) suffering from systemic cryptococcosis. Eight patients were suffering from central nervous system cryptococcosis and 5 were suffering from disseminated cryptococcosis. Systemic lupus erythematosus was found to be the common underlying disease in 3 patients. The results of treatment depended on the underlying diseases (7 improved, 6 died). Also, 13 isolates of C. neoformans were obtained from feces of 30 pet birds. All 26 isolates of C. neoformans were cultured in glycine cycloheximide medium and were found to be of s

Profound muscle weakness as the presenting feature of disseminated cryptococcal infection.

A patient with chronic lymphocytic leukemia who was treated with immunosuppressive therapy for a prolonged period presented with profound muscle weakness secondary to disseminated cryptococcosis. The infection developed despite 3 months of continuous ketoconazole therapy and was not responsive to amphotericin B or flucytosine. At autopsy, Cryptococcus neoformans was present in all sampled skeletal muscles, myocardium, and muscularis propria of the gastrointestinal tract but was not identified in either the central nervous system or the lungs. A review of the English-language literature failed

Fatal disseminated cryptococcosis following intraocular involvement.

A 33-year-old man was treated with systemic steroids for a retinal inflammatory lesion before the diagnosis of cryptococcal retinitis and meningitis was suspected. He died from central nervous system disease despite treatment with parenteral antifungals. Histopathological studies demonstrated ocular and disseminated systemic infection with Cryptococcus neoformans. Direct cryptococcal involvement of the eye is rare and is usually associated with disseminated disease. Systemic steroids must be used with caution, and patients who take these drugs require frequent monitoring.

Abdominal cryptococcoma in AIDS: a case report.

Cryptococcosis is a recognized opportunistic pathogen in the acquired immune deficiency syndrome. Although central nervous system infection and disseminated cryptococcosis is common in acquired immune deficiency syndrome, localized infection is rare. We present a case of massive retroperitoneal and mesenteric adenopathy in a male homosexual patient with acquired immune deficiency syndrome with clinical and radiologic features suggestive of lymphoma. However, this was proven pathologically to represent cryptococcal infiltration of the lymph nodes. Our experience indicates that Cryptococcus neof

Oral fluconazole as suppressive therapy of disseminated cryptococcosis in patients with acquired immunodeficiency syndrome.

PURPOSE: Because of the increasing numbers of patients with acquired immunodeficiency syndrome (AIDS) who will require treatment for cryptococcosis and because of the problems associated with long-term administration of intravenous amphotericin B, an alternative therapeutic approach in the form of an efficacious and easily administered oral antifungal drug would be of great benefit. Fluconazole, a new triazole antifungal agent, represents such an alternative. We therefore conducted an open, non-randomized trial of oral fluconazole as maintenance suppressive therapy of disseminated cryptococcos

A case of monosomy-7 eosinophilic leukemia and neurofibromatosis, terminated with disseminated cryptococcosis.

A 17-year-old boy was admitted with fever, generalized lymphadenopathy, hepatosplenomegaly, multiple cāfe-au-lait spots, and a family history of neurofibromatosis. The peripheral blood showed anemia, leukocytosis with 9% blasts, 54% eosinophils of abnormal morphology, and thrombocytopenia. No cause for hypereosinophilia was found. Bone marrow aspiration revealed hypercellularity with 20% myeloblasts and 35% eosinophilic leukocytes. The cytogenetic analysis of bone marrow cells with G-banding techniques showed a karyotype of 45, XY, −7. Based upon these findings, the authors believed that the p

Disseminated cryptococcal infection in a patient with hypogammaglobulinemia and normal T cell functions.

The immunologic status of a patient with disseminated cryptococcosis and no underlying predisposing factor was evaluated. The proportions and numbers of T cells, T cell subsets, B cells and proliferative responses to mitogens and soluble antigens, including cryptococcal antigen, were comparable to those in healthy control subjects; however, the patient had moderately severe hypogammaglobulinemia. The patient continues to do well clinically with intravenous gammaglobulin and antifungal therapy. This study suggests that antibodies might also play a role in the defense against cryptococcal antige

Acquired immune deficiency syndrome presenting as bone marrow and mediastinal cryptococcosis.

Disseminated cryptococcosis developed as the first manifestation of the acquired immune deficiency syndrome in a previously healthy Haitian man. Following presentation with a febrile illness that included massive mediastinal and peripheral lymphadenopathy, the patient died of overwhelming pulmonary, visceral, and meningeal cryptococcosis.

Pleural cryptococcosis in the acquired immune deficiency syndrome.

Pleural cryptococcosis is extremely rare. We report the first case of cryptococcal pleural effusion in association with the acquired immune deficiency syndrome. Pleural effusion without the evidence of pulmonary parenchymal involvement was the initial and only clinical finding leading to the diagnosis of disseminated cryptococcosis. The pleural effusion resolved spontaneously prior to amphotericin B therapy.

Cryptococcal peritonitis. Report of a case and review of the literature.

Cryptococcus neoformans is a rare cause of peritonitis; only four cases have been reported in the literature. A 63-year-old man in whom isolation of C. neoformans from ascitic fluid provided the initial clue to disseminated cryptococcosis is described. Review of this case and those previously reported reveals striking association between cryptococcal peritonitis and hepatic disease.

The brown colour effect (BCE) of Cryptococcus neoformans in the diagnosis, control and epidemiology of C. neoformans infections in AIDS patients.

Cryptococcus neoformans was found as the causative agent of cryptococcosis in 7 (3.6%) out of 195 HIV-positive persons, most of them being hospitalized. These 7 persons included 6 homosexuals and 1 heroin addict. The examinations were performed at the Mycology Unit of the Robert Koch Institute in Berlin (West) between 1984 and 1986. The brown colour effect (BCE) of the C. neoformans colonies on Guizotia abyssinica creatinine agar (with 0.1% glucose) within 2-5 days at 26 degrees C facilitated the diagnosis of disseminated cryptococcosis. In all 7 cryptococcosis cases, the antigen of C. neoform

Colonization and pathogenesis of Cryptococcus neoformans in gnotobiotic mice.

Congenitally immunodeficient nude (nu/nu) mice and their immunocompetent littermates (nu/+) were used to determine whether the absence of thymus-matured T cells would alter the capacity of Cryptococcus neoformans to colonize their mucosal surfaces or enhance their susceptibility to systemic cryptococcosis, or both, following oral challenge. We present data demonstrating that an encapsulated strain of C. neoformans serotype A colonized the alimentary tracts of germfree, conventional, and antibiotic-treated conventional nu/nu mice. Scanning electron microscopy showed that C. neoformans adhered t

Cryptococcosis in a pup.

Disseminated cryptococcosis was diagnosed as the cause of death of a 2-week-old pup. Cryptococcus neoformans was isolated from the dam's vaginal discharge, but not from the dam's milk, blood, feces, or environment. During succeeding months, C neoformans was isolated repeatedly from the dam's vagina, even though vaginitis was not evident. One year after the death of the 2-week-old pup, cryptococci could not be isolated from the dam's vagina, and evidence of cryptococcosis was not detected in pups of the dam's next litter. This case was unusual because cryptococcosis has been reported previously

Oral manifestations of disseminated Cryptococcus neoformans in a patient with acquired immunodeficiency syndrome.

A palatal lesion was the initial manifestation of disseminated cryptococcosis in a patient with acquired immunodeficiency syndrome (AIDS). The diagnosis was initially verified with cytologic smears in situ and later confirmed with pathologic findings from an excisional biopsy specimen. As cryptococcosis is one of the major opportunistic infections in persons with AIDS, an increased incidence of oral manifestations of cryptococcosis can be expected.

Fatal disseminated cryptococcosis and concurrent ehrlichiosis in a dog.

Laboratory findings in an adult bull terrier presented with a history of anorexia and weight loss included the following: severe anaemia, leukocytosis, neutrophilia, lymphopaenia, thrombocytopaenia, Ehrlichia canis morulae in monocytes, hypergammaglo-bulinaemia, a bleeding tendency, icterus and proteinuria. In addition, a high Haemobartonella canis parasitaemia, non-encapsulated yeasts on urinalysis and a localised Demodex canis infestation were present. Treatment for ehrlichiosis was initiated but the dog died. Lesions found were a severe cryptococcal granulomatous pneumonia and cryptococcal
