Oropouche virus infection in pregnant women in Rio de Janeiro, Brazil: a prospective cohort study.

BACKGROUND: Although adverse maternal and infant outcomes have been reported following Oropouche virus infection, the frequencies of these outcomes have not been compared with those in uninfected pregnancies. We followed up pregnant women from areas with Oropouche virus transmission in Rio de Janeiro state, Brazil, to evaluate potential adverse maternal and infant outcomes following Oropouche virus exposure. METHODS: In this prospective cohort study across the state of Rio de Janeiro, Brazil, we included pregnant women receiving prenatal care in municipalities with or at high risk of Oropouch

Alexander Disease Diagnosed in an Obstetrics Patient: A Case Report and Review of the Literature.

Alexander disease (AxD) is a rare autosomal dominant leukodystrophy due to pathogenic variants in glial fibrillary acidic protein (GFAP). Adult-onset AxD type II presents with heterogeneous neurologic and neuropsychiatric symptoms. We report a unique case diagnosed in the postpartum period that mimicked posterior reversible encephalopathy syndrome (PRES). A 33-year-old gravida 2 para 1 at 36 weeks gestation presented with 10 days of progressive restlessness, delusions, insomnia, dysarthria, weight loss with inability to feed herself, and urinary/fecal incontinence. She underwent induction of l

Human genetics implicates a BACH2-NRF2 axis in fetal haemoglobin activation.

Human genetic studies have identified key regulators of fetal haemoglobin (HbF) expression, including BCL11A, resulting in therapeutic advances1-8. Yet the mechanisms by which HbF expression is activated remain incompletely understood9. Here we conduct a large multi-ancestry genome-wide association study of HbF levels in 28,279 individuals that identifies 91 conditionally independent associations across 12 genomic regions. In one previously uncharacterized associated region, the high-HbF-linked causal variant rs1010474-C reduces BACH2 expression and elevates HbF levels. Direct perturbation or

Acute Bilateral Visual Loss as an Atypical Manifestation of Wernicke Encephalopathy Secondary to Hyperemesis Gravidarum: A Case Report and Narrative Literature Review.

Wernicke encephalopathy (WE) is a severe neurological condition caused by thiamine deficiency that remains underreported in pregnancy. We report a clinical diagnosis of WE in a 41-year-old woman, gravida 2, para 1, at 16 weeks of gestation. She presented with acute, painless visual impairment, severe dietary restriction, and marked weight loss secondary to severe hyperemesis gravidarum (HG). Examination revealed reduced visual acuity, horizontal nystagmus, psychomotor retardation, and an ataxic gait. The initial diagnosis was delayed due to her atypical presentation and the absence of MRI or b

Access to Arbitrary Stimuli During Fixed-Lean Schedule Thinning.

UNLABELLED: Fixed-lean schedule thinning involves programming the terminal reinforcement schedule from the onset of schedule thinning rather than gradually reducing the delay to reinforcement over time in a dense-to-lean approach. This study extended Sumter et al. (2020) and investigated the effect of continuous access to attention (an arbitrary stimulus; not functionally related to challenging behavior) during fixed-lean schedule thinning for individuals who engaged in challenging behavior maintained by tangible reinforcement or a combination of tangible reinforcement and escape. In some sess

Regulatory T cells and decidual vascular remodelling - A new piece in the preeclampsia puzzle.

Improperly constrained inflammation at the placental-uterine interface is strongly implicated as a causal factor in preeclampsia. Inflammatory mediators and leukocytes in the decidua are key features of the underlying pathophysiology, with potential to cause uterine vascular anomalies that give rise to compromised placental function, particularly in the severe, early onet form of the disease. While uterine natural killer (uNK) cells are clearly involved, a critical question has been whether and how T cells might also contribute. Regulatory T (Treg) cells, immune-regulatory T cells that counter

Effect of Intraoperative Sublingual Nitroglycerin on the Rate of Active Uterine Segment Incision in Preterm Cesarean Birth.

INTRODUCTION AND AIM: Active segment uterine incisions, including classical, T-shaped, and high transverse incisions, are often performed during preterm cesarean deliveries (CD), especially in patients delivering at less than 30 weeks of gestation, when the lower segment can have inadequate space for a safe maternal or fetal delivery. These uterine incisions increase the risk of maternal morbidity due to longer operative time, increased blood loss, and the rate of uterine dehiscence in future pregnancies compared to lower segment uterine incisions. Patients with uterine incisions in the active

Prophylactic carbetocin versus oxytocin following vaginal delivery among women with high risk for postpartum hemorrhage: A before-and-after study.

INTRODUCTION: Data with a high level of evidence is lacking on the use of prophylactic uterotonic drug following vaginal delivery to prevent postpartum hemorrhage (PPH) among women who were considered high-risk for PPH. Our main objective was to compare the effectiveness of prophylactic carbetocin versus oxytocin in preventing PPH after vaginal delivery among women at high risk. MATERIAL AND METHODS: We conducted a retrospective before-and-after single-center comparative study, including all high-risk women after vaginal births. Two consecutive 14-month periods were compared, where the prophy

Differential expression of complement Properdin and Factor H in the placentae and umbilical cords of mothers with Preeclampsia, Gestational Diabetes Mellitus and Recurrent Pregnancy Loss.

BACKGROUND: Properdin and factor H (FH), the two regulatory proteins of the alternative complement pathway, oppose each other to maintain the complement system's activation. While properdin upregulates, FH downregulates the complement alternative pathway. The current study evaluated the expression of properdin and FH transcripts and proteins in the placental tissues and umbilical cords (UC) of preeclampsia (PE), gestational diabetes mellitus (GDM), and recurrent pregnancy loss (RPL) compared to normal healthy pregnancy (N). METHODS: The tissue histology of PE, GDM and RPL were observed using

Selective episiotomy guided by a perineal evaluation form for forceps-assisted vaginal delivery: a retrospective cohort study on maternal and neonatal outcomes.

BACKGROUND: Forceps-assisted vaginal delivery is prone to perineal laceration and other complications. Episiotomy decisions are often experience-dependent, leading to excessive intervention. The Perineal Evaluation Form (PEF) provides an objective basis for episiotomy, but its clinical efficacy in forceps-assisted vaginal delivery has not been fully validated. METHODS: A single-center retrospective cohort study was conducted on 313 parturients who underwent forceps-assisted vaginal delivery at the Third Affiliated Hospital of Zhengzhou University from January 2017 to December 2024, divided int

The Clinical Benefit of Adding Weight of Blood-Soaked Materials to Quantification of Postpartum Blood Loss.

OBJECTIVE: The objective of this study is to evaluate whether full quantitative blood loss (FBL), which incorporates the weight of blood soaked materials, improves the prediction of hemorrhage-related clinical decision-making compared with volumetric blood loss (VBL) using calibrated drapes alone. STUDY DESIGN: We performed a secondary analysis of an IRB-approved retrospective cohort including all deliveries over 2 years at a large, urban academic center. During the study period, a quantitative blood loss protocol was introduced without other changes to hemorrhage management. VBL consisted of

Intramyometrial injection versus intravenous infusion of oxytocin for maintaining uterine contractility during elective caesarean delivery in a randomised controlled trial.

Oxytocin is commonly administered during caesarean delivery (CD) to prevent postpartum haemorrhage as an initial bolus followed by a maintenance dose. Nearly half of perinatal centres in Japan utilise intramyometrial (IMY) oxytocin. IMY oxytocin is less effective than intravenous (IV) oxytocin as the initial dose for inducing uterine contractions. However, its role as a maintenance dose remains unclear. We evaluated the efficacy of IMY oxytocin versus continuous IV infusion for maintaining uterine contractility during elective CD. Twenty-two women undergoing elective CD were randomised to rece

Blood Product Use for Hemorrhage during Cesarean Delivery.

OBJECTIVE: Postpartum hemorrhage (PPH) remains a leading cause of maternal morbidity and mortality. Little is known about transfusion practices in patients requiring blood tansfusions following cesarean delivery. The objective of this study is to describe the use of blood products, clotting factors, colloids, and antifibrinolytics in a contemporary cohort of patients with hemorrhage during cesarean and compare blood product use with a historic cohort of patients with a similar diagnosis. STUDY DESIGN: Secondary analysis of a multicenter trial of tranexamic acid versus placebo to prevent PPH d

The Role of Immunological Bystander Reaction in Recurrent Pregnancy Loss.

Recurrent pregnancy loss (RPL) affects approximately 1%-2% of women of reproductive age and may be associated with immunological bystander reactions. These reactions occur when the maternal immune system's response to a specific target extends to nearby tissues, causing collateral damage to both maternal and fetal cells. This narrative review explores the mechanisms of immune dysregulation at the maternal-fetal interface and evaluates current therapeutic strategies. A literature search for a narrative review was conducted using PubMed, Scopus, and Web of Science for the period 2010-2025. Keywo

Open Parenchyma-Preserving Enucleation of Giant Hepatic Hemangiomas: A Systematic Cavitron Ultrasonic Surgical Aspirator (CUSA)-Guided Pringle-Free Technique.

Giant hepatic hemangiomas, particularly those involving central hepatic segments (IV, V, and VIII), present unique surgical challenges due to their proximity to major hepatic veins and first-order portal structures. While laparoscopic approaches have been described, detailed guidance for open enucleation across a range of lesion locations and complexities remains limited. This technical report describes a systematic open parenchyma-preserving enucleation technique and presents a retrospective consecutive case series of six patients who underwent the described procedure over an eight-year perio

Study Design of the Global Prospective Hemolytic Disease of the Fetus and Newborn Registry (GERANIUM).

OBJECTIVE: Hemolytic disease of the fetus and newborn (HDFN) is a rare and serious immune-mediated condition that can result in poor fetal and neonatal outcomes. During pregnancy, monitoring with middle cerebral artery Doppler ultrasound and intrauterine transfusions (IUTs) are the current standard of care, although intravenous immunoglobulin (IVIg) has been adopted in some centers to attempt to delay the onset of fetal anemia. Significant gaps remain in understanding the safety and effectiveness of current HDFN management, both during pregnancy and in neonates and children. We present the des

Tranexamic acid use for the prevention of postpartum hemorrhage in high-income countries: a review of recent literature.

BACKGROUND: Postpartum hemorrhage (PPH) is the leading global cause of maternal morbidity and mortality. Tranexamic acid (TXA), an antifibrinolytic agent that has been shown to reduce hemorrhage-related mortality in trauma, is used off-label for the prevention and treatment of PPH, typically in conjunction with uterotonic agents. If TXA is effective in the prevention and management of PPH, this inexpensive medication could improve PPH care and reduce PPH-related morbidity and mortality. The majority of evidence on TXA use for PPH prevention and treatment comes from studies conducted in low- an

Physiological cardiotocography interpretation and neonatal morbidity: A historical pre-post cohort study in a tertiary perinatal center.

INTRODUCTION: Cardiotocography (CTG) interpretation is prone to inter-observer variability and may contribute to both missed fetal compromise and potentially avoidable intrapartum intervention. Physiological CTG interpretation (PCI) reframes fetal surveillance around fetal physiology and the intensity of hypoxic stress, but real-world outcome data after unit-wide implementation remain limited. MATERIAL AND METHODS: Historical pre-post cohort study with case-mix adjustment at University Hospital Ulm, a tertiary perinatal center in Germany (~3200 births/year).
