INTRODUCTION: Hemolytic disease of the fetus and newborn (HDFN) and fetal and neonatal alloimmune thrombocytopenia (FNAIT) are two rare alloimmune diseases of pregnancies where maternal alloantibodies target fetal blood components, potentially leading to risks of serious health conditions and developmental delays for babies, and sometimes loss of the baby. HDFN and FNAIT often have similarly complex journeys, from diagnosis through to burdensome treatments such as intrauterine transfusions and intravenous immunoglobulin.
OUR STORIES: We, the authors, are two mothers with lived experience of t

Increased first-trimester nuchal translucency (NT) may be associated with chromosomal, genetic, and structural abnormalities, although some pregnancies progress with normal invasive testing and reassuring interval imaging. This case involved a 32-year-old primigravida referred at 12+5 weeks' gestation following the identification of markedly increased NT measuring 5.6 mm, with associated body wall oedema. Chorionic villus sampling demonstrated normal fluorescence in situ hybridisation (FISH) and chromosomal microarray results, while subsequent antenatal imaging showed resolution of the body wa

RATIONALE: Iatrogenic blood loss contributes substantially to anemia and transfusion exposure in preterm infants. The effectiveness of interventions aimed at minimizing blood sampling remains unclear. Most prior trials have focused on placental transfusion strategies rather than blood conservation during postnatal care.
OBJECTIVES: To evaluate the benefits and harms of strategies to minimize blood sampling, compared with standard blood sampling, in preterm infants.
SEARCH METHODS: We used CENTRAL, PubMed, two other databases, and two trial registers, together with reference checking and hand

BACKGROUND: Maternal anti-Ro/SSA (Sjögren's syndrome-related antigen A) [± anti-La/SSB (Sjögren's syndrome type B antigen)] antibodies can lead to neonatal lupus, which may present most severely as autoimmune congenital atrioventricular block (CAVB). Although CAVB is uncommon (~ 2% of anti-Ro/SSA-positive pregnancies), once a complete block develops, spontaneous reversal is rare, and many affected infants require permanent pacemaker implantation. Consequently, prevention and early detection are critical components of management, particularly in pregnancies following previous antibody-mediated

BACKGROUND: Due to its outstanding biological properties, the Human Amniotic Membrane (HAM) has been a topic of medical research for many years to treat, repair, and regenerate damages related to the epithelial surface, corneal epithelial, orthopedic, and infected body parts. The HAM shows crucial anti-inflammatory, anti-bacterial, and anti-viral properties and immunomodulating qualities. Therefore, the HAM is one of the most used natural biomaterials in tissue engineering to generate bio-scaffolds.
OBJECTIVE: This study used HAM as a raw material to produce a collagen membrane that fully mee

Posterior reversible encephalopathy syndrome (PRES) is a clinicoradiological entity characterized by headache, seizures, altered consciousness, and visual disturbances, with preeclampsia and eclampsia among its most common triggers. Although visual symptoms occur in roughly one-third of cases, acute cortical blindness as the predominant presenting feature in the absence of significant hypertension is uncommon and diagnostically challenging. We report a 27-year-old woman (gravida 2, abortion 1) at 36 weeks of gestation who presented with a one-day history of headache, a generalized tonic-clonic

BACKGROUND: HLA-G (Human Leukocyte Antigen-G) is a non-classical molecule belonging to the major histocompatibility complex (MHC) class I family, located on chromosome 6 within the MHC region, which is pivotal in promoting immune tolerance. The immune-suppressive ability of Human Leukocyte Antigen (HLA-G) and its binding to fetal maternal interface predispose pregnant women to Plasmodium falciparum (Pf) infection and pregnancy loss, respectively. Alterations in hematological parameters may accelerate the severity of malaria infection. Limited data is available about the effects of malaria infe

Hepatitis E is a liver inflammation caused by the hepatitis E virus (HEV). In pregnant women, the infection significantly increases the risk of acute liver failure, fetal loss, and maternal death. According to the World Health Organization, infection by HEV during the third trimester of pregnancy may increase the risk of maternal mortality in 20-25% of cases. In Tunisia, little is known about HEV infection and its outcome, especially in pregnant women. This study aims to evaluate the prevalence of HEV infection in a large cohort of pregnant women in Tunisia. A total of 891 women who attended t

BACKGROUND: In malaria-endemic regions, escalating sulfadoxine-pyrimethamine (SP) resistance has raised concerns about the effectiveness of intermittent preventive therapy (IPTp) in pregnant women. Consequently, exploring IPTp alternatives has become an urgent priority in this population. Azithromycin (AZ) has garnered attention in this aspect due to its capability to treat sexually transmitted infections and its established safety profile.
METHODS: A systematic literature search was conducted in the PubMed, Embase, CINAHL, Scopus and Cochrane Library databases as of 14 March 2024. We include

Trauma during pregnancy poses a dual threat to the mother and fetus, and disruption of the pelvic ring is among the most challenging injuries a trauma team can encounter in this population. Altered maternal physiology, the presence of a gravid uterus, and concerns regarding fetal radiation exposure all complicate assessment and surgical planning. Evidence to guide management is limited, leaving the sequencing of delivery and skeletal stabilization largely dependent on case-by-case clinical judgment. A 26-year-old woman (gravida 3, para 2) at 35 weeks and 5 days of gestation sustained these inj

INTRODUCTION: The diphtheria, tetanus, and acellular pertussis combined vaccine (DTaP) is a trivalent formulation containing acellular pertussis antigens, diphtheria toxoid, and tetanus toxoid, and provides simultaneous protection against pertussis, diphtheria, and tetanus. This study aimed to systematically evaluate the effects of repeated intramuscular administration of an adsorbed acellular diphtheria-tetanus-pertussis (reduced-dose) combined vaccine (Tdacp) on gamete maturation, mating performance, fertility, and embryo-fetal development in Sprague-Dawley (SD) rats, thereby providing exper

BACKGROUND: Due to their immature immune system, neonates are at high risk of infection. This vulnerability when combined with limited resources and health education in developing countries can lead to sepsis, resulting in high global neonatal mortality rates. Many of these deaths are preventable. The BabyGel pilot trial tested the feasibility of conducting the main randomised trial, with the provision of alcohol handgel to postpartum mothers for prevention of neonatal infective morbidity in the rural community. This secondary analysis sought to evaluate the methods of detecting infections in

IMPORTANCE: Unencapsulated Haemophilus influenzae frequently causes noninvasive upper respiratory tract infections in children but can also cause invasive disease, especially in older adults. A number of studies have reported an increased incidence in neonates and suggested that pregnant women may have an increased susceptibility to invasive unencapsulated H. influenzae disease.
OBJECTIVE: To describe the epidemiology, clinical characteristics, and outcomes of invasive H. influenzae disease in women of reproductive age during a 4-year period.
DESIGN, SETTING, AND PARTICIPANTS: Public Health

Pertussis is typically more severe in neonates and young infants, with fulminant forms possibly linked to age-specific systemic immune responses. However, the early events that shape the development of the immune response in neonates are poorly understood, mainly due to limited sample access and the absence of human ex vivo infection models. Here, multi-omic profiling of an ex vivo whole-blood infection model was used to investigate systemic immune responses to Bordetella pertussis. We observed a stronger pro-inflammatory immune response in cord blood (CB) compared to adult blood (AB), marked

Bordetella hinzii is a rare, opportunistic Gram-negative bacterium traditionally associated with poultry but increasingly identified as a human pathogen in immunocompromised individuals. We report the first Indian case of B. hinzii causing antenatal sepsis in a 27-year-old pregnant woman with homozygous sickle cell anaemia and β-thalassemia minor, leading to intrauterine fetal demise at 24 weeks of gestation. The patient presented with breathlessness, edema, and absent fetal movements. Laboratory results showed severe anaemia, leucocytosis, and raised inflammatory markers. Blood cultures yield

OBJECTIVE: To establish whether digital foetal scalp stimulation (dFSS) performs better than foetal blood sampling (FBS) in terms of reducing the rate of caesarean section (CS) in labour, without adversely affecting perinatal outcomes.
DESIGN: A multicentre parallel-group randomised controlled trial.
SETTING: Maternity centres in Ireland.
POPULATION: The study aimed to randomise 2500 nulliparous women who required a second-line test of foetal well-being in labour due to abnormal cardiotocography (CTG).
METHODS: Participants were randomly allocated to dFSS or FBS in a 1:1 ratio. Analysis wa

Hypophosphatasia (HPP) is a congenital bone disease caused by tissue-nonspecific mutations in the alkaline phosphatase gene. It is classified into six types: severe perinatal, benign prenatal, infantile, pediatric, adult, and odonto. HPP with femoral hypoplasia on fetal ultrasonography, seizures, or early loss of primary teeth can be easily diagnosed. In contrast, pediatric, adult, and odonto types of HPP over 4 years of age are less likely to be diagnosed because they do not have typical symptoms. Consequently, it may be misdiagnosed as common osteoporosis, and treatments incompatible with HP

OBJECTIVE: To compare low-cost "Suction Tube Uterine Tamponade" (STUT) treatment for refractory postpartum hemorrhage (PPH) with uterine balloon tamponade (UBT) using a randomized feasibility study.
METHODS: After verbal assent, we allocated participants with refractory PPH by randomly ordered envelopes to STUT or routine UBT at 10 hospitals in South Africa and one tertiary referral center in Colombia between January 10, 2020, and May 3, 2024. In the STUT group, we inserted a 24 FG Levin stomach tube into the uterine cavity and applied suction. The control group received standard UBT, mainly

OBJECTIVE: Low placentation is associated with increased risk of postpartum hemorrhage (PPH). There is a paucity of data on the association between second trimester low placentation that later resolves, and PPH. Our objective was to investigate the association of resolved low placentation and other prenatal ultrasound markers, and PPH with delivery.
STUDY DESIGN: This was a retrospective matched case-control (1:1) study of full term, singleton pregnancies complicated by PPH who delivered vaginally at our academic center between 2018-2022. All patients received a transvaginal ultrasound in the
