The relationship between umbilical arterial pH, newborn heart rate and ventilation at birth - a prospective observational study.

OBJECTIVE: To investigate the relationship between umbilical arterial pH (UA-pH), heart rate (HR) and positive pressure ventilation (PPV) at birth. METHODS: A prospective cohort study conducted at Stavanger University Hospital, Norway, June 2019-November 2021. Newborns of gestational age ≥28 weeks were eligible for inclusion. UA-pH was routinely obtained at birth and newborn HR captured using dry-electrode ECG. Relationships were evaluated using linear correlation and logistic regression. UA-pH was categorised as <7.00, 7.00-7.09, 7.10-7.19, with reference group 7.20-7.50. Outcome measures we

Molecular sex determination in primates including non-invasive umbilical cord samples from neonates of three endangered species.

BACKGROUND: Molecular sex determination is a valuable tool for an effective population management in captive bred endangered species. The molecular approaches are particularly valuable in species with limited sexual dimorphism. Moreover, the utilization of accessible non-invasive samples from newborn animals enables early sex determination beneficial for the animal population management and planning. METHODS AND RESULTS: A rapid molecular sex determination method using PCR with specific primers targeting the male-specific SRY locus and the control c-myc gene present in both sexes was develope

Hemodynamic effects of umbilical cord milking versus early cord clamping in nonvigorous intrauterine growth restricted neonates: a randomized controlled trial.

UNLABELLED: The effect of placental transfusion on the nonvigorous intrauterine growth restricted (IUGR) neonates with pre-existing subclinical myocardial dysfunction and altered cerebral hemodynamics is largely unknown. The present study was planned to evaluate the effect of umbilical cord milking (UCM) on left ventricular output (LVO) of nonvigorous IUGR neonates of > 28-weeks' gestation. In the present randomized control trial, 74 nonvigorous IUGR neonates were randomly allocated to UCM (4-times squeezing of 20-cm intact cord, n = 37) and ECC (cord clamping within 30 s, n = 37). The primary

Velamentous Cord Insertion With Vaginal Delivery: A Case Report and Review of the Literature.

Velamentous cord insertion (VCI) is a rare placental abnormality in which the umbilical cord enters the chorionic membranes instead of the placenta, leaving the umbilical vessels unprotected. This pathological condition carries an increased risk of adverse perinatal outcomes, such as fetal growth restriction, preterm delivery, abnormal fetal heart rate patterns during labor, and perinatal morbidity, which often lead healthcare professionals to choose cesarean section to reduce postnatal risks. Early prenatal diagnosis using color Doppler imaging and comprehensive ultrasound is of utmost import

Effect on neonatal outcomes of maintenance of maternal blood pressure targets with noradrenaline after spinal anaesthesia for caesarean delivery: a multicentre, randomised, controlled trial.

INTRODUCTION: Hypotension after spinal anaesthesia for caesarean delivery can cause maternal and neonatal harm. However, excessive vasopressor therapy may also result in harm, and the optimal blood pressure target is unclear. We compared two different maternal blood pressure targets during caesarean delivery. METHODS: Women with term pregnancies undergoing caesarean delivery under spinal anaesthesia were assigned randomly to maintenance of systolic blood pressure within 90% (SBP-90% group) or 80% (SBP-80% group) of baseline readings, from intrathecal injection until delivery. The primary outc

Challenging the paradigm that neonatal uterine bleeding represents the first menstrual episode.

Neonatal uterine bleeding (NUB) has been hypothesized as a relatively common early-life event potentially linked to the later development of endometriosis through retrograde menstruation. However, its true prevalence and biological nature remain uncertain. This study aimed to determine the prevalence, biological features, and maternal-neonatal correlates of reddish diaper staining in newborns, and to evaluate its purported association with endometriosis. A prospective cross-sectional study was conducted between September 2023 and March 2024 at a tertiary obstetric center with postpartum outpat

Novel use of the Nuwellis® dual extended length catheter (dELC) as umbilical venous access for neonatal kidney replacement therapy: case series.

BACKGROUND: Neonates with congenital kidney failure or acute kidney injury often require continuous kidney replacement therapy (CKRT). Historically, venous access for CKRT in this population has relied on non-approved adult catheters, commonly via the internal jugular (IJ) vein. Limited international reports describe using the umbilical vein for CKRT, but this practice is not widespread in the USA. The Nuwellis® dual lumen extended length catheter (dELC), originally created for adult ultrafiltration, may support alternative access for neonatal CKRT. METHODS: This case series describes the cli

Histopathological placental inflammation and umbilical cord blood inflammatory biomarkers in preterm infants: a prospective cohort study.

BACKGROUND: Histopathological placental inflammation provides objective evidence of intrauterine inflammatory exposure in preterm birth, but placental histopathology is not immediately available after delivery. We aimed to determine whether placental inflammatory pathology is associated with cord blood and postnatal inflammatory biomarkers in preterm infants. METHODS: In this prospective cohort study, 87 preterm infants with birth weight < 2500 g were enrolled. Cord blood collected at birth was analyzed for procalcitonin (PCT), presepsin (P-SEP), and high-sensitivity C-reactive protein (hs-CR

Sex-specific signatures of fetal programming for gestational diabetes mellitus in umbilical cord: A cohort study from Tanzania.

BACKGROUND: Gestational diabetes mellitus (GDM) is defined as glucose intolerance first recognized during pregnancy. Offspring of GDM mothers have an increased risk of developing type 2 diabetes (T2D), obesity, and other metabolic disorders later in life. METHODS: We analyzed metabolic, genetic, epigenetic, and transcriptional profiles in umbilical cord blood (UCB) from male and female offspring of GDM mothers versus normoglycemic controls in Tanzania. We constructed polygenic and methylation risk scores for T2D and GDM to assess whether diabetes-related susceptibility is detectable at birth.

Intracranial hemorrhage identified after initiating therapeutic hypothermia: two case reports of neonatal hypoxic-ischemic encephalopathy due to hypovolemic shock with anemia: a case report.

BACKGROUND: Therapeutic hypothermia for moderate-to-severe hypoxic-ischemic encephalopathy in neonates effectively improves neurological outcomes when initiated within 6 hours of birth. However, coagulopathy is a potential side effect of therapeutic hypothermia and requires careful monitoring for signs of hemorrhage. Uncontrolled hemorrhage is the primary exclusion criterion for therapeutic hypothermia. Herein, we report two cases of intracranial hemorrhage that, despite being massive enough to cause hypovolemic shock with anemia, could not be detected by cranial ultrasonography before the ini

Linking maternal blood pressure with fetal cerebral haemodynamics and cortical growth in congenital heart disease.

BACKGROUND: Children with congenital heart disease (CHD) experience neurodevelopmental challenges, arising in part from altered brain maturation beginning in utero. There are currently no prenatal interventions to mitigate this risk. Foetal CHD is associated with alterations in maternal blood pressure (BP), for which therapies are available, so we investigated associations between maternal BP, foetal cerebral haemodynamics, and brain growth in foetuses with and without CHD. METHODS: Our single-centre retrospective cohort study first analysed maternal BP during pregnancy stratified by foetal C

Transfusion Histories of Neonates Who Developed Severe Retinopathy of Prematurity.

INTRODUCTION: The recent BORN study suggests the incidence of retinopathy of prematurity (ROP) might be significantly reduced by transfusing extremely low gestational age neonates (ELGANs) with red blood cells (RBCs) not from adult donors, but from umbilical cord blood. We are uncertain what proportion of infants with severe ROP had no RBC transfusions and thus would not benefit from changing transfusion practice. METHODS: We created detailed transfusion histories of all infants in our health system (2021-2024) who developed severe ROP. RESULTS: Severe (stage ≥3) ROP was diagnosed in 34 infa

Limited placental transfer and clearance of risankizumab by 6 months following in utero exposure.

BACKGROUND AND AIMS: The recent Global Consensus supports the use of risankizumab during pregnancy. However, infant clearance following in utero exposure remains unknown, and data on placental transfer are scarce. We report on infant clearance following in utero exposure to risankizumab, and characterize placental transfer and infant outcomes. METHODS: Two pregnant women with inflammatory bowel disease exposed to risankizumab during pregnancy were included. Maternal serum and umbilical cord blood were collected at delivery, and infant serum at 3 and 6 months postpartum. Infant development was

Blood sLOX-1 Is Beneficial for Determining Severity of Neonatal Hypoxic-Ischemic Encephalopathy: A Nationwide Prospective Cohort Study.

Background/Objectives: Neonatal hypoxic-ischemic encephalopathy (HIE) is a major cause of neurodevelopmental diseases. In a previous retrospective study, we revealed that the levels of the soluble form of lectin-like oxidized low-density lipoprotein receptor-1 (sLOX-1) are linked to HIE severity. We conducted a prospective investigation to evaluate the association between sLOX-1 levels and HIE severity using the Sarnat staging system, as well as a multicenter study to assess neurodevelopmental sequelae in the short term. Methods: A total of 193 infants were enrolled in the study between April
