Prepartum acquired hemophilia A: managing a double challenge for mother and child.

Acquired hemophilia A during pregnancy is exceptionally rare and poses significant risks to both mother and fetus, including maternal hemorrhage and neonatal bleeding due to transplacental transfer of factor (F)VIII inhibitors. We report four cases of antenatally diagnosed acquired hemophilia A to provide insights into management and outcomes. All patients received corticosteroids during pregnancy, with individualized peripartum hemostatic strategies using bypassing agents when indicated. Transplacental FVIII inhibitor transfer occurred in three neonates, leading to transient low FVIII levels;

Emergent Transfusion and Hemolytic Disease of the Fetus and Newborn Risk Mitigation in Females of Childbearing Potential with Life-Threatening Bleeding: A Clinical Practice Guideline.

BACKGROUND: Timely resuscitation of patients in hemorrhagic shock is lifesaving. Blood product composition for massive transfusion protocols varies (whole blood vs conventional component therapy). Current whole blood inventory is predominantly RhD-positive, raising questions about the transfusion approach for females of childbearing potential (FCP). STUDY DESIGN: Working groups were assembled that included experts in trauma surgery, transfusion medicine, hematology, maternal and fetal medicine, emergency medicine, anesthesiology, neonatology, and pediatrics. Patient stakeholders and ethics re

RhD-Positive Transfusion in Females of Childbearing Potential in Hemorrhagic Shock: Risk, Reality, and Policy.

OBJECTIVES: Prehospital transfusion using Rh-D antigen positive low-titer group O whole blood (LTOWB) or red blood cells is increasingly common when RhD-negative blood products are not available. However, concerns remain regarding D-alloimmunization in RhD-negative females of childbearing potential (FCPs), which could lead to future hemolytic disease of the fetus and newborn (HDFN). The magnitude of these risks, and their policy implications, have been the subject of recent investigation. The objectives of this review are to summarize the biological and epidemiological impacts and post-exposur

Hemolytic disease of the newborn heralds hemorrhagic disease of the newborn.

Hemolytic disease of the fetus and newborn, which normally presents with unconjugated hyperbilirubinemia, has been known to cause cholestatic jaundice, albeit in rare circumstances; seen especially in the setting of Rhesus (Rh) hemolytic disease or when the mother has received intrauterine transfusion(s). Infants presenting with cholestatic jaundice in the setting of hemolytic anemia pose a clinical dilemma with regard to investigative approach and management. There are no evidence-based guidelines as to whether such infants should be managed conservatively or investigated comprehensively for

Rate of Future Hemolytic Disease of the Fetus and Newborn Following the Prehospital Transfusion of RhD-Positive Blood Products to Females of Childbearing Potential in the United States of America.

OBJECTIVES: Transfusing females of childbearing potential (FCP) in hemorrhagic shock with RhD-positive blood products risks inducing the formation of anti-D that can cause hemolytic disease of the fetus and newborn (HDFN) in future pregnancies. A previous study found that 957 FCPs were transfused with RhD-positive blood products over approximately 5-years in the United States (U.S.) in the prehospital phase of their resuscitation. This study used an in silico simulation to calculate the rate of future HDFN amongst these 957 transfused FCPs. METHODS: A model that calculated the overall risk of

A prehospital protocol for transfusion of low-titer O-positive whole blood in patients with hemorrhagic shock in Los Angeles County: Modeling the risk of hemolytic disease of the fetus and newborn.

BACKGROUND: The Los Angeles Development and Rapid Operationalization of Prehospital Blood (LA-DROP) pilot will protocolize prehospital administration of low titer O-positive whole blood (LTO + WB) to patients with hemorrhagic shock in Los Angeles County (LAC). We sought to quantify the risk of death from hemolytic disease of the fetus and newborn (HDFN) associated with RhD-negative alloimmunization in females of childbearing potential (FCPs) as a result of LA-DROP. STUDY DESIGN AND METHODS: Retrospective data from LAC EMS databases were used in combination with estimates from published litera

Management of Pregnancy, Delivery, and Postpartum in Italian Carriers and Women With Haemophilia A and B.

INTRODUCTION: Managing pregnancy in carriers/women with haemophilia involves four key objectives: preconception and prenatal care, minimizing maternal bleeding, choosing the delivery mode to reduce foetal/neonatal bleeding, and postpartum care. The optimal factor VIII/IX level at delivery and the best delivery method to mitigate maternal and foetal complications are still debated. AIM: To share real-world clinical management in Italian haemophilia treatment centres. METHODS: In March-April 2023, members of the Italian Association of Hemophilia Centers "AICE" were invited to an online survey

[Expert consensus on the diagnosis, treatment, and prevention of neonatal dengue, chikungunya, and Zika virus infections (2025)].

Mosquito-borne viruses, including dengue virus (DENV), chikungunya virus (CHIKV), and Zika virus (ZIKV), pose major threats to public health in tropical and subtropical regions worldwide. Neonates are particularly vulnerable, and the associated disease burden has drawn increasing attention. Routes of neonatal infection include vertical mother-to-child transmission (transplacental and peripartum) and postnatal mosquito bites. Clinical manifestations are often nonspecific; a proportion of cases may progress to central nervous system infection, hemorrhagic disease, or long-term neurodevelopmental

Gestational trophoblastic diseases and neonatal choriocarcinoma.

Trophoblastic diseases include benign pre-tumor entities (hydatidiform moles) and malignancies called gestational trophoblastic tumors. Most of the latter arise after a hydatidiform mole and are referred to as post-molar trophoblastic tumors. Their diagnosis relies on elevated human chorionic gonadotrophin (hCG) levels following a mole, without the need for histological confirmation. Other forms, including choriocarcinoma, placental site trophoblastic tumor (PSTT), and epithelioid trophoblastic tumor (ETT), require histology; notably, PSTT and ETT are relatively resistant to chemotherapy and u

Receipt of RhD-positive whole blood for life-threatening bleeding in female children: A survey in alloimmunized mothers regarding minimum acceptable survival benefit relative to risk of maternal alloimmunization to anti-D.

BACKGROUND: Low-titer group O whole blood (LTOWB) for treatment of hemorrhagic shock sometimes necessitates transfusion of RhD-positive units due to short supply of RhD-negative LTOWB. Practitioners must choose between using RhD-positive LTOWB when RhD-negative is unavailable against the risk to a female of childbearing potential of becoming RhD-alloimmunized, risking hemolytic disease of the fetus and newborn (HDFN) in future children, or using component therapy with RhD-negative red cells. This survey asked females with a history of red blood cell (RBC) alloimmunization about their risk tole

Causal Relationships Between Leukocyte Subsets and Adverse Fetal Outcomes: A Mendelian Randomization Study.

Background: The tolerance and dynamic regulation of the maternal immune system during pregnancy are pivotal for ensuring fetal health. Immune cell subsets play a complex and crucial role in this process, closely linked to the neonatal health status. Despite recognizing the significance of dysregulation in the quantity and activity of immune cells in neonatal disease occurrence, their specific roles remain elusive, resulting in a dearth of clinically viable interventions for immune-mediated neonatal diseases. Materials and Methods: Employing two-sample Mendelian randomization (MR) methodology,

Transplacental delivery of factor IX Fc-fusion protein ameliorates bleeding phenotype of newborn hemophilia B mice.

Hemophilia B is an inherited hemorrhagic disorder characterized by a deficiency of blood coagulation factor IX (FIX) that results in abnormal blood coagulation. The blood coagulation is already evident in hemophiliacs at the fetal stage, and thus intracranial hemorrhage and other bleeding complications can occur at birth, leading to sequelae. Therefore, it is important to develop effective treatments for hemophiliacs in utero. In this study, in order to transplacentally deliver FIX from pregnant mice to their fetuses, an improved adenovirus (Ad) vector expressing human FIX fused with the IgG F

Not as "D"eadly as once thought - the risk of D-alloimmunization and hemolytic disease of the fetus and newborn following RhD-positive transfusion in trauma.

The use of blood products to resuscitate injured and massively bleeding patients in the prehospital and early in-hospital phase of the resuscitation is increasing. Using group O red blood cells (RBC) and low titer group O whole blood (LTOWB) avoids an immediate hemolytic reaction from recipient's naturally occurring anti-A and - B, but choosing the RhD type for these products is more nuanced and requires the balancing of product availability and survival benefit against the risk of D-alloimmunization, especially in females of childbearing potential (FCP) due to the possible future occurrence o

Transplacental delivery of therapeutic proteins by engineered immunoglobulin G: a step toward perinatal replacement therapy.

BACKGROUND: Transplacental delivery of maternal immunoglobulin G (IgG) provides humoral protection during the first months of life until the newborn's immune system reaches maturity. The maternofetal interface has been exploited therapeutically to replace missing enzymes in the fetus, as shown in experimental mucopolysaccharidoses, or to shape adaptive immune repertoires during fetal development and induce tolerance to self-antigens or immunogenic therapeutic molecules. OBJECTIVES: To investigate whether proteins that are administered to pregnant mice or endogenously present in their circulat

Postmortem diagnosis of severe factor X deficiency in a fetus with intracranial hemorrhage resulting in intrauterine death.

In patients with severe congenital factor X deficiency, spontaneous intracranial hemorrhage (ICH) is particularly frequent in early childhood. We describe a case of fetal death at 26 weeks due to massive ICH. Gene panel analysis of postmortem samples revealed homozygosity for a pathologic F10 gene variant (c.1210T>C, p.Cys404Arg), which impedes correct folding of the catalytic serine protease domain and, therefore, causes a significant reduction in FX levels. The parents, not consanguineous but of the same ethnic community, were found to be heterozygous for this variant and did not have any pe

Treatment of immune thrombocytopenia during pregnancy with thrombopoietin receptor agonists.

The introduction of thrombopoietin receptor agonists (TPO-RAs) led to a paradigm shift in the management of immune thrombocytopenia (ITP). However, TPO-RAs are not approved for use during pregnancy due to the absence of evidence and concerns for possible effects on the fetus due to their expected transplacental transfer. This comprehensive review examines the safety and efficacy of TPO-RA in 45 pregnancies of women with ITP (romiplostim n = 22; eltrombopag n = 21; both in the same pregnancy n = 2). Mothers experienced failure of the median of three treatment lines during pregnancy prior to TPO

Multidisciplinary management of a pregnancy complicated by Glanzmann thrombasthenia: A case report.

BACKGROUND: Glanzmann thrombasthenia (GT) is a rare, autosomal recessive disorder of platelet glycoprotein IIb-IIIa receptors. Pregnant patients with GT are at increased risk of maternal and fetal bleeding. There is a paucity of literature on the peripartum management of patients. CASE DESCRIPTION: We present the antepartum through the postpartum course of a patient with GT who was managed by a multidisciplinary approach that included communication across maternal-fetal medicine, hematology, transfusion medicine, and anesthesiology services. In addition to routine prepartum obstetric imaging

Von Willebrand Disease, Hemophilia, and Other Inherited Bleeding Disorders in Pregnancy.

Inherited bleeding disorders, which comprise von Willebrand disease (VWD), hemophilia, other congenital clotting factor deficiencies, inherited platelet disorders, defects of fibrinolysis, and connective tissue disorders, have both maternal and fetal implications. Although mild platelet defects may actually be more prevalent, the most common diagnosed bleeding disorder among women is VWD. Other bleeding disorders, including hemophilia carriership, are much less common, but hemophilia carriers are unique in that they are at risk of giving birth to a severely affected male neonate. General guida
