A Rupture in Silence: A Life-threatening Neonatal Adrenal Bleed.

Neonatal adrenal hemorrhage (NAH) is a rare condition that commonly presents with retroperitoneal bleeding in neonates. We report a term male neonate born through vaginal delivery who presented with respiratory distress and shock on the first day of his life. Examination revealed pallor, abdominal distension and a tender lumbar mass. Laboratory tests showed severe anemia, and imaging studies confirmed a large perirenal hematoma with absent adrenal shadow. The diagnosis was right adrenal rupture causing hemorrhagic shock. Prompt resuscitation and conservative management led to a favorable outco

A congenital portosystemic shunt in the neonatal period: case report and literature review.

A congenital portosystemic shunt (CPSS), a rare developmental malformation of the portal venous system, often presents with atypical clinical manifestations in the neonatal period that are prone to be missed or misdiagnosed. Herein, we report two neonatal CPSS cases. Case 1 initially manifested as respiratory distress, diffuse ecchymosis, and petechial hemorrhages and later caused progressive liver dysfunction, disseminated intravascular coagulation (DIC), and patent ductus arteriosus. Case 2 initially exhibited respiratory distress and multiple cutaneous hemangiomas, subsequently leading to p

A randomized control trial comparing the clinical outcome of thyroxine supplementation in thyroid peroxide antibody- negative cases in patients with subclinical hypothyroidism in pregnancy.

BACKGROUND: A normal free thyroxine level in the presence of an increased thyroid-stimulating hormone is what constitutes subclinical hypothyroidism (TSH). Changes in thyroid physiology during pregnancy result in the normal TSH range being lower than non-pregnant. Subclinical thyroid disease during pregnancy is associated with adverse outcomes, including, risk of adverse pregnancy and neonatal outcomes. Prevalence in India is 6-8%. The prevalence of antibodies varies with ethnicity. Pregnancy with TSH concentrations >2.5 mIU/L needs evaluation for thyroid peroxidase antibody (TPO) antibody sta

Gastrointestinal endoscopic evaluation and management in neonates: a 10-Year clinical experience.

UNLABELLED: Gastrointestinal endoscopy is rarely performed in neonates due to concerns regarding safety and technical feasibility. This study aimed to evaluate its safety, feasibility, and diagnostic yield, and to explore its role as both a diagnostic and therapeutic tool in neonatal gastrointestinal diseases. This retrospective study included 42 neonates, both preterm and term, who underwent gastrointestinal endoscopy at Shanghai Children's Hospital between 2015 and 2025. Indications for endoscopy, endoscopic findings, therapeutic approaches and follow-up outcomes were analyzed. All neonates

Environmental Modulation of Physiological Parameters in the Paediatric Intensive Care Unit After Extracorporeal Cardiac Surgery.

BACKGROUND: The paediatric intensive care unit exposes critically ill children to non-physiological environmental stimuli, such as noise, artificial light and thermal instability, which may interfere with physiological regulation and circadian organization. AIM: To examine how environmental conditions in the paediatric intensive care unit (PICU), including noise, lighting and ambient temperature, affect cardiovascular, respiratory and thermoregulatory physiology in paediatric patients admitted after extracorporeal cardiac surgery. STUDY DESIGN: This prospective cohort study used continuous e

A Neonatal Transfer from Birth Center to Hospital with a Postpartum Hemorrhage During Transport.

There is increasing interest from women and birthing people for midwifery-led care with community birth options in birth centers and for home birth. Birth centers are a high-value and high-quality option for place of birth and also address the increasing need to provide access to perinatal care for women in maternity care deserts. This case report reviews the transfer of a postpartum patient and her newborn from an accredited, freestanding birth center to the designated hospital. The reasons for transfer were neonatal respiratory distress with hypoxemia and an emergent postpartum hemorrhage th

Steroid-Refractory Warm Autoimmune Haemolytic Anaemia Complicating Ulcerative Colitis in Pregnancy: A Case Report and Management Considerations.

Autoimmune haemolytic anaemia (AIHA) arising in pregnancy is an exceptionally rare condition, and its concurrence with active ulcerative colitis (UC) presents considerable diagnostic and therapeutic complexity. We describe a 30-year-old primigravida who presented at 12 weeks' gestation with severe symptomatic anaemia (haemoglobin 4.4 g/dL), serologically confirmed warm AIHA (strongly positive direct antiglobulin test, IgG-mediated), and concurrent gastrointestinal haemorrhage attributable to an acute flare of UC. High-dose corticosteroid therapy failed to achieve a durable response, and the pa

Repeated low-dose lipopolysaccharide exposure during the periconception period on ewe immune responses and lamb performance.

Subclinical inflammatory challenges caused by gram-negative bacterial endotoxin lipopolysaccharide (LPS) may disrupt reproductive physiology and influence offspring development; however, the effects of repeated low-dose exposure during the periconception period remain unclear. Therefore, this study evaluated the effects of repeated low-dose LPS exposure on maternal immune and endocrine responses, reproductive performance, and offspring growth and development. Rambouillet ewes (n = 30; 2 to 8 yr; 61 ± 6 kg BW) were stratified by body weight and age and assigned to receive subcutaneous injection

Umbilical cord ulceration associated with placental abruption: a case report.

BACKGROUND: Umbilical cord ulceration (UCU) is a rare but potentially fatal obstetric condition characterized by ulcerative disruption of the umbilical cord, often leading to fetal hemorrhage and intrauterine fetal demise. Placental abruption, defined as the premature separation of the placenta from the uterine wall, is another major cause of perinatal mortality. When UCU is complicated by occult placental abruption, the risk of adverse perinatal outcomes increases substantially. These associations highlight the importance of vigilant prenatal surveillance and comprehensive imaging to facilita

Prevalence, risk factors, and adverse maternal and fetal outcomes of anemia during pregnancy: a retrospective study in China, 2012-2020.

BACKGROUND: Anemia is a public world health problem among pregnant females especially in developing countries. China is a representatively developing country with vast territory and significant economic and cultural differences in different regions. Currently most studies on anemia in pregnancy ignored economically backward and resource-poor cities or districts and anemia during pregnancy in China was not accurately estimated. The goal of this study was to assess the prevalence, risk factors, and adverse outcomes of anemia among pregnant females across different regions of China using a large,

Outcomes of early enteral feeding in very low birth weight infants: A meta-analysis of randomized controlled trials.

In this study, we aimed to evaluate the outcomes of early enteral feeding among very low birth weight (VLBW) infants. We searched PubMed, the Cochrane Library, Scopus, and Web of Science up to October 2025. The intervention group was identified from randomized controlled trials (RCTs) wherein feeding was started early (≤72 h), and those with delayed initiation served as controls. Meta-analysis was performed using RevMan 5.4 software. Seventeen RCTs were included in the meta-analysis. Early feeding did not significantly increase the risk of mortality (risk ratio [RR] = 0.72; 95% confidence inte

A Case Report of Hematemesis in a 4-Week-Old Neonate: Clinical Reasoning and Outcome After Empiric Acid Suppression.

INTRODUCTION: Neonatal hematemesis is an uncommon presentation with a broad differential diagnosis ranging from benign to life-threatening etiologies, and there are no well-established, evidence-based management guidelines. This case highlights the diagnostic challenges and potential role of empiric acid suppression in a clinically stable neonate. CASES PRESENTATION: A 4-week-old term male with a prior episode of coffee-ground emesis at 3 days of life presented with recurrent hematemesis consisting of bright red blood. He was hemodynamically stable with an unremarkable physical exam. Laborato

How to avoid placental transection during low transverse cesarean delivery for anterior placenta previa.

Placenta previa, particularly when anterior, is a major risk factor for postpartum hemorrhage and may necessitate transplacental fetal extraction during cesarean delivery, which increases maternal and fetal blood loss. Standard strategies to minimize bleeding, such as fundal uterine incisions (when placenta accreta spectrum is suspected) or intraoperative ultrasonographic guidance, may be technically demanding, time-consuming, or unavailable in emergencies or resource-limited settings. Controlled separation and mobilization of a placental edge to avoid placental laceration offers a simple, rep

Case Report: Subcutaneous sensor-augmented insulin pump therapy in a 510-g preterm infant with hyperglycemia.

Neonatal hyperglycemia affects up to 80% of extremely low birth weight (ELBW) infants and is independently associated with mortality, intraventricular hemorrhage, and adverse neurodevelopmental outcomes. Intravenous insulin infusion, which is the standard of care, requires dedicated vascular access. To the best of our knowledge, this is the first report of combined continuous glucose monitoring (CGM) and continuous subcutaneous insulin infusion (CSII) in an ELBW infant (birth weight <1,000 g). Transient hyperglycemia developed on day of life (DOL) 2, necessitating insulin therapy. A 55-min int

Incidence and Predictors of Necrotizing Enterocolitis Among Low-Birth-Weight Neonates Admitted to Southern Ethiopian Public Hospitals.

Necrotizing enterocolitis (NEC) has emerged over the past 25 years as the most common gastrointestinal emergency in neonatal intensive care units (NICUs) worldwide. Despite significant improvements in survival among premature and low-birth-weight (LBW) neonates due to advances in neonatal intensive care, the incidence of NEC remains persistently high, largely because of the physiological vulnerability associated with LBW and prematurity. The purpose of this study was to assess the incidence and identify predictors of NEC among LBW newborns in South Ethiopia. A hospital-based prospective cohort

Maternal Immune Thrombocytopenia With Prior Splenectomy as a Cause of Severe Neonatal Thrombocytopenia: A Case Report.

Severe neonatal thrombocytopenia is a formidable complication of maternal immune thrombocytopenic purpura (ITP). We report the case of a male newborn, preterm at 32 weeks of gestation, presenting on day 4 of life with jaundice and severe cutaneous hemorrhagic syndrome. The maternal history was significant for severe ITP requiring splenectomy. The initial assessment showed a significant drop in platelets, with a count of 6,000/mm³, even though the bone marrow aspirate appeared normal. Treatment included corticosteroids, platelet transfusions, polyvalent immunoglobulins, and phototherapy. This a
