Mammalian-evolved strains of the fungal pathogen Cryptococcus neoformans show adaptation via heritable transcript level differences specific to mouse MHC haplotype.

BACKGROUND: Organisms adapt to novel environments using changes to genome, gene expression, and protein functions. This study focused on changes that had occurred when a fungal pathogen previously encountered hosts that differed only at the major histocompatibility complex (MHC) region, loci that control immune recognition during the adaptive immune response. To investigate how this fungal pathogen adapted to the host environment, next generation sequencing data were examined from strains of Cryptococcus neoformans (C. neoformans) that had been previously passaged eight times through congenic

A novel mycovirus identified in clinical isolates of Cryptococcus neoformans.

Mycoviruses are widespread throughout the fungal kingdom and are known to infect diverse fungal taxa, including fungal species that are important plant and human pathogens. Although many mycoviruses have been found to have minimal effects on their host, several viruses have been found to modulate fungal physiology, and, as a result, impact fungal virulence. Screens for mycoviruses in clinically relevant fungi have identified numerous mycoviruses within several important human pathogens, but mycoviruses remain uncharacterized in the clinically relevant human pathogen Cryptococcus neoformans. Incidence and mortality of invasive fungal infections among patients admitted in intensive care units in India: findings from a multicentric registry-based study (2023-25).

BACKGROUND: Invasive fungal infections (IFIs) are challenging to treat in critical care, particularly among patients treated in Intensive care units (ICUs). The existing evidence on IFIs from India remains fragmented. This study characterises IFI incidence, risk factors, resistance pattern, treatment and outcome among patients admitted to ICUs. METHODS: This multicentre observational study leveraged registry data on IFIs among patients admitted to 63 ICUs in eight Advanced Mycology Diagnostic and Research centres in India between January 2023 and November 2025. Patients with proven and probab

Bilateral Cryptocococcal orbital cellulitis associated with sinonasal cryptococcosis: a case report and review of the literature.

Cryptococcus neoformans is an opportunistic fungal pathogen that typically affects the lungs and central nervous system. Orbital involvement is exceptionally rare and may mimic bacterial orbital cellulitis, resulting in delayed diagnosis and treatment. We report an 83-year-old immunocompromised man with poorly controlled diabetes and recent corticosteroid exposure who presented with rapidly progressive bilateral orbital cellulitis secondary to invasive cryptococcal infection with extensive sinonasal involvement. Despite broad-spectrum antimicrobial therapy, surgical intervention, and systemic

Birth and clinical outcomes of pregnant participants with advanced HIV disease.

INTRODUCTION: Clinical trials for the evaluation of novel diagnostics and treatments for HIV-associated opportunistic infections have historically excluded pregnant persons. Little information is available regarding the clinical outcomes and pregnancy outcomes of pregnant persons with advanced HIV disease while taking opportunistic infection prophylaxis for tuberculosis (TB) and cryptococcosis. In this study, we describe characteristics at enrollment, clinical outcomes, and birth outcomes of pregnant people with advanced HIV in Uganda. METHODS: We prospectively enrolled pregnant adults with a

Differentiating disseminated cryptococcosis from pulmonary cryptococcosis: Roles of clinical characteristics and cryptococcal antigen.

AbstractDistinguishing disseminated cryptococcosis (DC) from localized pulmonary cryptococcosis (PC) is crucial due to divergent management and disparate prognoses. This study aimed to recognize associated factors for DC from PC in non-human immunodeficiency virus (HIV) patients. Accordingly, 475 pathologically confirmed cryptococcosis cases (391 PC and 84 DC) were retrospectively reviewed over a 20-year period at a tertiary hospital in China. Clinical, radiographic, dynamic serum cryptococcal antigen (CrAg) titer profiles, treatment and prognosis were analyzed. Patients with DC were significa

HIV-1 compartmentalization in the central nervous system during cryptococcal meningitis: Analysis of viral, fungal, and host factors.

Opportunistic infections in the central nervous system (CNS), such as cryptococcal meningitis (CM) have profound consequences for people living with HIV-1 (PWH), including blood-brain barrier impairment and neuroinflammation. To investigate how CM may shape the HIV-1 populations in the CNS, we characterized viral variants in the blood and cerebrospinal fluid (CSF) compartments in PWH with active CM. Thirty-nine participants with CM had paired plasma and CSF viral load measurements prior to the start of anti-retroviral therapy (pre-ART), with overall significantly higher HIV-1 viral loads in pl

A protective hybrid protein vaccine informed by mapping major histocompatibility class II epitopes in Cryptococcus neoformans chitin deacetylases that stimulate CD4+ T-cell responses.

UNLABELLED: Infections due to Cryptococcus species are estimated to cause over 100,000 deaths a year. No cryptococcal vaccines are approved for human use. We have shown that vaccine formulations consisting of C. neoformans chitin deacetylase 2 (Cda2) and Cda1 adjuvanted with Cationic Adjuvant Formulation 01 (CAF01) protect mice against experimental cryptococcosis by a mechanism dependent upon CD4+ T cells. Here, using overlapping peptide libraries, we mapped epitopes in Cda2 and Cda1 that stimulated interferon-gamma (IFNγ) release from splenocytes of C57BL/6 and DR4 mice immunized with these p

An inositol receptor Itr4 coordinates carbon catabolite regulation and virulence in Cryptococcus neoformans.

Nutrient sensing and utilization are critical for microbes to adapt to their environment and evade predators. The encapsuled yeast Cryptococcus neoformans has a unique ability to sense and utilize the sugar inositol as both a signaling molecule and as a carbon source. This trait is advantageous not only in the natural environment but also to promote its pathogenesis, including invasion of the central nervous system. In this study, we have identified a novel inositol transporter-like receptor (transceptor) Itr4 that coordinates inositol and glucose metabolism by regulating inositol transporters

[MALACOPLAKIA OF THE URINARY BLADDER: A CASE REPORT WITH DIAGNOSTIC DIFFICULTY].

We herein report a rare case of malacoplakia of the urinary bladder that demonstrated significant diagnostic difficulty. The patient was a 65-year-old woman with a history of breast cancer, dermatomyositis, steroid-induced diabetes mellitus, and cutaneous cryptococcosis. She presented with repeated episodes of dysuria and hematuria since the age of 62. Computed tomography (CT) revealed progressive bladder wall thickening, and she was therefore diagnosed with cystitis by her primary care physician and treated with antibiotics. Her symptoms worsened at 63 years of age, and further bladder wall t

The Cryptococcus neoformans titanide is a pathogenic cell type that arises from typical yeast cells in response to host conditions.

Fungal morphogenesis is a major driver of disease outcome. For the opportunistic fungal pathogen Cryptococcus neoformans, extreme morphological heterogeneity within the lung environment drives dissemination, immune evasion, and drug resistance. One such cell type is the small, oval, titanide (2-4 µm). This poorly understood cell type is prevalent in lung histology as well as host-relevant in vitro conditions. In conditions that generate cellular heterogeneity, titanides comprise 4%-10% of cells 24 hours post-induction and by 72 hours are the dominant cell type. Despite their prevalence and the

Diagnosis and treatment of four cases of pulmonary cryptococcosis: case report and literature review.

INTRODUCTION: Cryptococcosis has attracted global attention due to its high mortality rate, unique geographical distribution, and tendency to cause severe and rapid disease progression. Metagenomic next-generation sequencing (mNGS) is a novel detection technology that provides diagnostic insights for identifying pathogens, particularly rare ones. The present article describes 4 individuals diagnosed with pulmonary cryptococcosis (PC) using mNGS of bronchoalveolar lavage fluid (BALF). CASE PRESENTATION: Among these patients, 3 had cancer, 1 had sarcoidosis and diabetes, and 2 were exposed to p

Clinical Phenotypes, Immunosuppressive Exposures, and Outcomes in Pulmonary vs Extrapulmonary Cryptococcosis Among Non-HIV, Non-Transplant Adults: A U.S. Claims Analysis.

INTRODUCTION: Cryptococcosis increasingly affects non-HIV, non-transplant (NHNT) adults, but factors associated with extrapulmonary dissemination remain incompletely defined. We evaluated associations between comorbidities, immunosuppressive therapy, and cryptococcal disease phenotype. METHOD: NHNT adults with cryptococcosis in Merative MarketScan were classified as having pulmonary or extrapulmonary disease. Extrapulmonary disease was further classified as central nervous system (CNS) or non-CNS disseminated disease. Comorbidities and immunosuppressant, systemic glucocorticoid, and antineopl

Evaluation of a multiplex qPCR assay for rapid diagnosis of fungal pneumonia and tuberculosis in people with advanced HIV disease.

Fungal pneumonias and tuberculosis (TB) remain major causes of mortality among people living with advanced HIV disease (PLWHA). Their overlapping clinical manifestations delay diagnosis in developed and resource-limited settings, with greater impact where access to rapid diagnostic tools is limited. This study evaluated the performance of a multiplex quantitative PCR (qPCR) assay for simultaneous detection of Histoplasma capsulatum, Cryptococcus neoformans, Pneumocystis jirovecii, and Mycobacterium tuberculosis complex in respiratory samples from PLWHA. A cross-sectional study was conducted at

Cryptococcosis in a seemingly immunocompetent patient presenting with knee and lower limb soft tissue infection complicated by meningitis: a case report.

Cryptococcus neoformans typically causes pulmonary or central nervous system (CNS) infections. However, osteoarticular manifestations, particularly as the initial presentation, are rare, especially in immunocompetent hosts. We report the case of a 65-year-old apparently immunocompetent woman admitted for generalized pain following a fall. Ten days post-admission, she developed fever and rapidly progressing soft tissue swelling in the right lower limb and knee. Despite the absence of overt immunodeficiency, the infection disseminated, leading to meningitis. Diagnosis was confirmed via fungal cu

Rare Fatal Event of Multidrug-resistant Shigella Septicemia and Cryptococcal Meningitis in an Immunocompromised Host.

Bacteriemia due to Shigella spp. is a rare event. However, an individual with impaired immunity remains at risk of such invasion where the infective dose is as low as 10 viable bacilli. This case report describes one such fatal event where a 40-year-old immunocompromised male presented to a tertiary-level healthcare facility with a history of diarrhea for 10 days and seizures. Prompt microbiological workup established multidrug-resistant Shigella flexneri septicemia and cryptococcal meningitis.

Pulmonary cryptococcosis mimicking metastatic progression despite systemic disease stability in advanced colon cancer: A case report and literature review.

RATIONALE: In patients with advanced cancer, newly detected pulmonary nodules are often interpreted as metastatic progression. The distinctive feature of this case was the appearance of multiple new lung nodules during otherwise controlled systemic disease: the primary colonic lesion and hepatic metastases remained stable while tumor marker levels declined. This radiologic-clinical discordance prompted reconsideration of metastatic progression and broadened the differential diagnosis. PATIENT CONCERNS: A patient with advanced colon cancer, who was undergoing systemic chemotherapy and had othe
