Gastrointestinal endoscopic evaluation and management in neonates: a 10-Year clinical experience.

UNLABELLED: Gastrointestinal endoscopy is rarely performed in neonates due to concerns regarding safety and technical feasibility. This study aimed to evaluate its safety, feasibility, and diagnostic yield, and to explore its role as both a diagnostic and therapeutic tool in neonatal gastrointestinal diseases. This retrospective study included 42 neonates, both preterm and term, who underwent gastrointestinal endoscopy at Shanghai Children's Hospital between 2015 and 2025. Indications for endoscopy, endoscopic findings, therapeutic approaches and follow-up outcomes were analyzed. All neonates

A Case Report of Hematemesis in a 4-Week-Old Neonate: Clinical Reasoning and Outcome After Empiric Acid Suppression.

INTRODUCTION: Neonatal hematemesis is an uncommon presentation with a broad differential diagnosis ranging from benign to life-threatening etiologies, and there are no well-established, evidence-based management guidelines. This case highlights the diagnostic challenges and potential role of empiric acid suppression in a clinically stable neonate. CASES PRESENTATION: A 4-week-old term male with a prior episode of coffee-ground emesis at 3 days of life presented with recurrent hematemesis consisting of bright red blood. He was hemodynamically stable with an unremarkable physical exam. Laborato

Upper gastrointestinal bleeding in a term neonate requiring haemostasis by laparotomy.

We report a case of a term neonate who developed massive haematemesis at 15 hours of life without identifiable risk factors. Despite conservative management, the bleeding persisted, resulting in haemorrhagic shock and anaemia. Endoscopy was not feasible due to a large intragastric haematoma, so surgical haemostasis was performed, revealing diffuse gastric erosion and multiple bleeding ulcers and the neonate recovered well postoperatively. This report shows that life-threatening bleeding can occur even in term neonates without identifiable risk factors. In cases of ongoing massive bleeding in w

Gastrointestinal Bleeding as an Atypical Presentation of Neonatal Gastric Perforation in a Preterm Infant: A Case Report.

Neonatal gastric perforation is a rare but life-threatening surgical emergency that typically occurs during the first week of life. Early diagnosis remains challenging because the initial clinical and radiographic findings may be nonspecific, particularly in preterm infants, yet prompt recognition and surgical intervention are essential for improving outcomes. We report the case of a four-day-old preterm, low birth weight female infant admitted to the neonatal intensive care unit with prematurity and respiratory distress. During hospitalization, she developed hematemesis and melena. Initial ab

Efficacy and safety of upper gastrointestinal endoscopy in the neonatal intensive care unit.

BACKGROUND: Upper gastrointestinal (GI) disorders frequently lead to neonatal intensive care unit (NICU) admissions; however, conventional imaging often fails to detect atypical presentations. The development of miniaturized endoscopes has progressively expanded the feasibility of upper GI endoscopy in neonates. Evidence regarding the diagnostic utility and safety of upper GI endoscopy in neonates remains limited, and neonatal-specific guidelines are currently absent in China. This study aimed to evaluate the diagnostic value and safety of upper GI endoscopy in NICU infants. METHODS: We retros

Gastroesophageal Reflux in Infants and Children: Diagnosis and Treatment.

Gastroesophageal reflux is a common physiologic event in infants in which gastric contents pass from the stomach into the esophagus. Gastroesophageal reflux may be asymptomatic or cause regurgitation or "spit up." This occurs daily in approximately 40% of infants. Symptoms often begin before 8 weeks of life, peak at approximately 4 months of age, and usually resolve by 1 year. The prevalence of gastroesophageal reflux is 2% to 8% in children and adolescents. Family physicians should reassure parents that gastroesophageal reflux is self-limited, not pathologic, and does not warrant routine test

Gastric lavage for neonates with coffee-ground hematemesis in early postnatal period: Randomized controlled trial.

BACKGROUND: To evaluate the efficacy and safety of gastric lavage (GL) in neonates with coffee-ground hematemesis due to swallowing maternal blood by postnatal day 1. METHODS: This multicenter, randomized controlled trial included term neonates presenting coffee-ground hematemesis by postnatal day 1. Patients were randomly assigned to GL or non-GL groups. The primary outcome was the rate of feeding intolerance (FI) within 24 h. Secondary outcomes included onset of FI, residual gastric volume, vomiting episodes, weight changes, and bilirubin levels. Adverse events were monitored. RESULTS: Eig

Case Report: Early detection of neonatal volvulus by ultrasound in a 2-day-old neonate: timely intervention prevents complications.

Intestinal malrotation is a congenital anomaly arising from improper rotation or fixation of the embryonic gut, potentially leading to life-threatening complications such as volvulus. It typically presents within the first month of life with symptoms including bilious vomiting and scaphoid abdomen. In this report, we describe a case involving a 2-day-old term neonate who exhibited two episodes of hematochezia and one episode of hematemesis, without accompanying scaphoid abdomen or bilious vomiting. Initial laboratory investigations revealed metabolic acidosis (lactate 4.6 mmol/L, base excess -

Diagnostic and therapeutic utility of gastric tissue culture in a case of phlegmonous gastritis.

Phlegmonous gastritis is a rare, potentially life-threatening bacterial infection affecting the gastric mucosa and submucosa. Clinical presentation is often non-specific, typically including symptoms such as fever, epigastric vomiting, or hematemesis. Due to its rarity and vague clinical features, diagnosis is frequently delayed, increasing the risk of severe complications such as septic shock and death. We describe the case of a 74-year-old previously healthy female who presented with vomiting, hematemesis, and epigastric pain. Endoscopy revealed diffuse erythema and purulent exudate througho

Torsion of an Accessory Liver Lobe in a Newborn.

Accessory liver lobes are rare. We present the rare case of torsion of an accessory liver lobe in a neonate. A 13-day-old newborn presented with failure to thrive and hematemesis without fever. The initial workup with sonography, magnetic resonance imaging, and upper gastrointestinal study was suspicious of a duplication cyst, most likely in the posterior wall of the stomach. Laboratory and radiological findings were not suggesting a choledochal cyst. We performed a laparotomy with resection of the 3.2 × 2.1 × 1.1 cm mass. Intraoperatively, the cystic formation extended from of the liver bed u

Portal Hypertension in Children: Investigating Umbilical Catheterization in the Neonatal Period.

Portal cavernoma is a major cause of extrahepatic portal hypertension (EHPH) in children. It is a serious condition, due to the frequency and severity of digestive hemorrhages secondary to the rupture of esophageal varices (EV). Neonatal umbilical catheterization is a significant risk factor for the development of portal vein thrombosis (PVT) and portal hypertension. We report a case of a five-year-old male who presented with upper gastrointestinal (GI) bleeding on ruptured esophageal varices resulting from a portal cavernoma, complicating neonatal umbilical vein catheterization. This case ill

Congenital Gastric Teratoma Presenting with Gastrointestinal Bleeding: Case Report and Review of Literature.

BACKGROUND: Gastric teratoma is an extremely rare tumor, representing <1% of all pediatric teratomas, and commonly manifests as a palpable abdominal mass. Upper gastrointestinal tract bleeding in newborns and infants is rare and is mostly caused by a benign lesion. CASE REPORT: We present a 3-month-old boy who presented with recurrent attacks of hematemesis, vomiting, and melena which on work up revealed a gastric teratoma. DISCUSSION/CONCLUSION: Owing to the unique characteristics and the extreme rarity of this entity, accurate preoperative diagnosis has remained elusive.

Idiopathic Pulmonary Hemorrhage in Infancy: A Case Report and Literature Review.

Acute idiopathic pulmonary hemorrhage in infants (AIPHI) is a rare and quite low-described entity. Nowadays, pathophysiological mechanisms are poorly understood, although the lethality remains high. We present an autopsy case report of a 2-day-old male who developed respiratory distress and blood leakage from the endotracheal tube (ET) and suddenly died because of acute pulmonary hemorrhage. A postmortem examination and histological analysis were performed and are reported in this paper. Alveolar spaces were filled with red blood cells and hyaline membranes in all the examined samples. The abs

Age-specific causes of upper gastrointestinal bleeding in children.

The etiology of upper gastrointestinal bleeding (UGIB) varies by age, from newborns to adolescents, with some of the causes overlapping between age groups. While particular causes such as vitamin K deficiency and cow's milk protein allergy are limited to specific age groups, occurring only in neonates and infants, others such as erosive esophagitis and gastritis may be identified at all ages. Furthermore, the incidence of UGIB is variable throughout the world and in different hospital settings. In North America and Europe, most UGIBs are non-variceal, associated with erosive esophagitis, gastr

Case report: Cystic fibrosis with kwashiorkor: A rare presentation in the era of universal newborn screening.

BACKGROUND: Universal newborn screening changed the way medical providers think about the presentation of cystic fibrosis (CF). Before implementation of universal screening, it was common for children with CF to present with failure to thrive, nutritional deficiencies, and recurrent infections. Now, nearly all cases of CF are diagnosed by newborn screening shortly after birth before significant symptoms develop. Therefore, providers often do not consider this illness in the setting of a normal newborn screen. Newborn screening significantly decreases the risk of complications in early childhoo
