A rare cause of hematemesis in newborn: fibrocystic breast disease of mother.

Hematemesis in a healthy newborn is most often caused by swallowed maternal blood. Maternal blood due to fibrocystic breast disease in human milk has not previously been reported in the literature. We report here a newborn case with hematemesis in which the mother had fibrocystic breast disease, and we want to emphasize this rare entity. Physicians should be aware of this rare condition, and fibrocystic breast disease of the mother should be included in the differential diagnosis of newborns with hematemesis.

Fibropolycystic disease of the liver and kidney in Oman.

BACKGROUND AND STUDY AIMS: Fibropolycystic disease of liver and kidney (FPCDLK) is an uncommon group of conditions inherited in an autosomal fashion. The group encompasses autosomal dominant polycystic disease of the kidney (ADPDK), autosomal recessive polycystic disease of the kidney (ARPDK), congenital hepatic fibrosis (CHF) and Caroli's disease (CD). There are limited data of this disease in the world. We report our experience in the Royal Hospital (RH) in Oman and data regarding long-term follow-up. The aim of the study was to document the frequency of encounter, clinical presentation and

Gastroesophageal reflux disease in infants. Myths and misconceptions, where is the evidence?

Infantile gastroesophageal reflux (GER) is a common self-limited, physiological phenomenon. Infantile gastroesophageal reflux becomes pathological (gastroesophageal reflux disease [GERD]) when symptoms become more severe or are associated with complications such as failure to thrive or hematemesis. Though it is a very common condition, there are several misconceptions and myths on GER/GERD diagnosis and management. Inappropriate investigations are frequently requested and unnecessary medications are increasingly prescribed, particularly in infants with symptoms attributed to possible GER/GERD.

Pyloric atresia associated with Dieulafoy lesion and gastric dysmotility in a neonate.

In this case study, we report a male infant with pyloric atresia, extreme gastric distension, and a caliber-persistent gastric artery (Dieulafoy lesion) with massive gastric bleeding. After a transverse pyloroplasty and endoscopic hemoclip application to the caliber-persistent gastric artery, very slow gastric emptying developed, which required repeated surgical interventions. Gastroduodenostomy failed to promote gastric emptying. The intraoperative and postmortem histologic examinations of the gastric wall revealed a loss of interstitial cells of Cajal, which possibly explains the extreme mot

Apparent life threatening event and gastric antral ulcer in a full-term infant: any possible relationship?

We describe the case of an apparently healthy newborn infant who in 7th day of life showed an episode of haematemesis and in 13th day of life presented an episode of apparent life threatening event (ALTE). A fibroscopy of the upper digestive tract showed a great ulcer of the gastric antrum and esophagitis limited to the mucosa. Gastrinemia in the blood showed high values (121 pg/ml). The relationship between ALTE and gastric ulcer may be casual, however in literature a gastroenteric cause is present in about 50% of ALTE in which an etiologic cause is found. We speculate that in the present cas

Estimating blood loss: comparative study of the accuracy of parents and health care professionals.

OBJECTIVE: Hematemesis and hematochezia are not uncommon presenting complaints in children. The amount of blood loss reported by the parent is likely to influence the pediatrician's decision regarding investigations and management. Currently, there are only very limited data regarding the ability of laypersons to estimate blood losses visually. This study investigated the accuracy of parents, in comparison to pediatric health care professionals, in estimating blood loss volumes. PATIENTS AND METHODS: We performed a prospective, single-blinded study including 227 participants, comprising 131 p

Gastric duplication cyst: a unique presentation.

Duplications of the alimentary tract are rare and occur in 1 of 4500 births (Duplication of the stomach: report of a case and review of the English literature. Arch Surg 1961; 82:634-640). Gastric duplications constitute 8% of these or roughly 17 of every 1,000,000 births (Shew SB, Holcomb GW. Alimentary tract duplications. In: Ashcraft KW, Holcomb GW, Murphy JP, editors. Pediatric surgery. Philadelphia, PA: Elsevier, 2005. p. 543-552). Symptoms often occur by 2 years and can include nausea, vomiting, hematemesis, and vague abdominal pain. Occasionally, a palpable abdominal mass may be identif
