SAVI: molecular mechanisms, clinical spectrum and precision medicine approaches beyond type-I IFN.

Type I interferonopathies (TI-IFN) represent a heterogeneous group of autoinflammatory disorders characterized by upregulated type I interferon (IFN) signaling. Among them, STING-associated vasculopathy with onset in infancy (SAVI) is a rare, severe autoinflammatory disease caused by gain-of-function mutations in the STING1 gene. Mechanistically, these mutations lead to a constitutive activation of the STING protein, resulting in excessive type I interferon production, which drives chronic inflammation and damage to various organs, primarily as cutaneous vasculopathy and progressive interstiti

Congenital HSV-2 infection with atypical skin lesions, chorioretinitis, and neurodevelopmental sequelae: a case report.

BACKGROUND: Neonatal herpes simplex virus (HSV) infection, primarily caused by HSV-2, carries a significant risk of severe morbidity, including neurological impairment and developmental delays, without timely diagnosis and intervention. The early identification of atypical cutaneous manifestations at birth and the prompt empirical initiation of high-dose intravenous acyclovir in suspected cases are crucial to prevent systemic dissemination and improve outcomes. Nevertheless, due to the unique neonatal immune response and limitations of current therapies, significant long-term sequelae such as

PHACE syndrome: a systematic literature review and illustrative case report of a patient with severe cerebrovascular and neurodevelopmental sequelae.

BACKGROUND: PHACE syndrome is a rare neurocutaneous disorder defined by the association of large segmental infantile hemangiomas of the head and neck with malformations of the posterior fossa, cerebral and cervical arteries, heart, eyes, and ventral midline structures. Although facial hemangiomas are often the presenting feature, the cerebrovascular, neurodevelopmental, and airway manifestations are responsible for the greatest long-term morbidity. METHODS: A systematic literature review was conducted following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) gu

Neonatal varicella complicated by Staphylococcus aureus lung abscess in a preterm infant: a case report.

BACKGROUND: Neonatal varicella is a rare but potentially life-threatening condition, particularly in preterm infants. Although secondary bacterial infections are common complications, deep organ involvement such as lung abscess formation is exceedingly rare. Reports describing neonatal varicella complicated by Staphylococcus aureus lung abscess are scarce. CASE PRESENTATION: We report a 26-day-old preterm infant (32 weeks' gestation, birth weight 1.94 kg) who developed a progressive vesiculopustular skin lesions and respiratory deterioration following exposure to maternal varicella. Despite i

Unitary fault-tolerant encoding of Pauli states in surface codes.

AIMS: The interpretation of cytokeratin 7 (CK7)-positive cells in the epidermis of the nipple has been controversial. These cells have been described in Paget's disease of the nipple, and they have also been cited as benign 'Toker' cells or as Merkel cells. Having observed CK7+ cells in histologically unremarkable nipple biopsies, we sought to assess the distribution of CK7+ cells in Paget's disease of the nipple and in histologically unremarkable nipple. METHODS AND RESULTS: Representative sections from 37 cases of Paget's disease of the nipple and 32 cases of histologically unremarkable nip

When Skin-Limited Langerhans Cell Histiocytosis Becomes Life-Threatening: Severe Treatment-Related Morbidity in a Prematurely Born Infant-Case Report.

Background and Clinical Significance: Skin-limited Langerhans cell histiocytosis (LCH) is a clinically heterogeneous disease, ranging from self-healing forms to fulminant multi-organ failure, the latter being more often described in infants, especially preterm neonates. The optimal therapy for cutaneous LCH remains controversial; the possibilities vary from a watchful waiting approach to systemic chemotherapy. Case Presentation: This case report describes an exceptionally rare and clinically challenging course of skin-limited LCH in a prematurely born infant treated at the Department of Oncolo

Pseudo-differential operators and generalized random fields over tori.

Starting from a synthetic sample with composition Al2(SO4)3·16.6H2O, the high-temperature- and moisture-dependent behavior of alunogen has been unraveled by TGA measurements, in situ powder X-ray diffraction as well as by gravimetric moisture sorption/desorption studies. Heating experiments using the different techniques show that alunogen undergoes a first dehydration process already starting at temperatures slightly above 40 °C. The crystalline product of the temperature-induced dehydration corresponds to the synthetic equivalent of meta-alunogen and has the following chemical composition: A

Cutaneous Tuberculosis in a 22-Day-Old Neonate:A Diagnostic Challenge and Case Report.

BACKGROUND: Cutaneous tuberculosis (CTB) is a rare extrapulmonary manifestation of tuberculosis, particularly in neonates. Its diagnosis is often challenging due to non-specific presentations that can mimic common conditions. CLINICAL DESCRIPTION: We report the case of a 22-day-old male neonate who presented to the emergency room with fever and a scalp swelling following a reported fall from bed. Initial assessment suggested a subcutaneous abscess. The patient did not respond to multiple courses of intravenous antibiotics. A blood culture grew Burkholderia species, but targeted therapy also f

Modified STOP-Bang questionnaire for detecting obstructive sleep apnea in individuals with a body mass index below 35 kg/m2.

BACKGROUND: Obstructive sleep apnea (OSA) is a common clinical condition. Due to its high prevalence, the waiting list for polysomnography is often long. A screening tool is needed to identify individuals at high risk for OSA who should undergo polysomnography. The STOP-Bang questionnaire is a widely used screening tool; however, it may require modification for individuals with a body mass index (BMI) below 35 kg/m2. This study aimed to evaluate whether the STOP-Bang questionnaire should be modified for patients with a BMI under 35 kg/m2. METHODS: This retrospective analytical study included

Management dilemma of anti-GBM disease and p-ANCA-associated vasculitis with necrotizing skin lesions in a pediatric patient.

Diffuse alveolar hemorrhage and acute glomerulonephritis characterize pulmonary-renal syndrome, in which anti-glomerular basement membrane antibodies (anti-GBM) and anti-neutrophil cytoplasmic antibodies (ANCA) are often assessed. The treatment is complex, requiring a multidisciplinary approach, based on immunosuppressant therapies, mechanical ventilation, plasma exchange (PLEX), and kidney replacement therapy. This clinical case describes a 7-year-old female hospitalized for edema, hypertension, and acute kidney failure. Laboratory tests showed the coexistence of ANCA and anti-GBM autoantibod

Congenital erythropoietic porphyria: the overlooked inherited disorder.

Congenital erythropoietic porphyria (CEP) also known as Gunther's disease is a subtype of porphyria. It is an autosomal recessive disorder caused by a mutation in the uroporphyrinogen III gene (URO III) coding for the enzyme UROS synthase, an essential enzyme in the heme synthesis pathway. The condition may present as non-immune hydrops in foetuses, dark-red urine-stained diapers in neonates and skin blistering and mutilation in sun-exposed areas in older children. Enzyme assays and genetic studies are costly and not easily available in low-resource settings; therefore, awareness of the typica

Unusual Presentation of Coronary Artery Fistula in Capillary Malformation Arteriovenous Malformation 2 Syndrome: A Case Report.

Capillary malformation-arteriovenous malformation syndrome (CM-AVM) is a germline vascular dysplasia that is typically characterized by cutaneous capillary malformations and central nervous system arteriovenous malformations (AVM). We report an atypical presentation of CM-AVM2 featuring a giant coronary fistula. A 22-day-old male exhibited a cardiac murmur, leading to the discovery of a large fistula from the left circumflex coronary artery. The patient developed left eye exophthalmos due to a left-sided basilar to pontomesencephalic vein fistula. Genetic testing demonstrated a previously repo

High serum estradiol levels on the day of frozen blastocyst transfer are associated with increased early miscarriage rates in artificial cycles using transdermal estrogens.

STUDY QUESTION: Do serum estradiol (E2) levels on the day of frozen blastocyst transfer (FBT) affect pregnancy outcomes in hormonal replacement therapy (HRT) cycles using transdermal estrogens? SUMMARY ANSWER: E2 levels ≥313 pg/ml on the day of FBT are associated with increased early miscarriage rates (EMRs), but do not significantly impact the live birth rate (LBR). WHAT IS KNOWN ALREADY: E2 plays a crucial role in endometrial receptivity and placentation. The effect of serum E2 levels measured around the time of FBT in HRT cycles remains debated, with some studies indicating a negative imp

Analysing tumours for genetic diagnosis in mosaic neurofibromatosis type 1.

Neurofibromatosis type 1 (NF1) is an autosomal dominantly inherited disorder caused by pathogenic variants in the NF1 gene, resulting in diverse clinical manifestations, especially multiple cutaneous neurofibromas. In approximately 50% of cases, variants occur de novo, and a portion of these cases involves genetic mosaicism, where variants are present in a subset of cells of an individual. Mosaic NF1 often presents with a milder phenotype and reduced transmission risk, complicating clinical diagnosis and genetic consulting. Conventional blood-based genetic testing may fail to detect the pathog

Arteriovenous cerebral high-flow shunts: genetic analysis of patients from a pediatric tertiary care center.

INTRODUCTION: Arteriovenous cerebral high-flow shunts include the vein of Galen aneurysmal malformation (VGAM) and vein of Galen dilatation, which are considered secondary to arteriovenous malformations or arteriovenous fistulas. These entities are often sporadic but are found in association with variants of the RASA1 and EPHB4 genes (capillary malformation-arteriovenous malformation, CMAVM; OMIM #608354) or ACVRL1, ENG, and SMAD4 genes (hereditary hemorrhagic telangiectasia, HHT; OMIM #187300). The clinical phenotypes associated with these conditions are highly variable, with incomplete penet

Morphometric analysis of the postnatally developing marsupial lung: the gray short-tailed opossum (Monodelphis domestica).

The postnatal lung development of the gray short-tailed opossum (Monodelphis domestica) was investigated to assess the morphofunctional status in this immature born marsupial. Lung volume, surface densities and surface areas, parenchymal (air spaces and septa) and non-parenchymal (airways, blood vessels, connective tissue) volume proportions were determined in Micro-CT images using morphometry. The lung of the neonate was at the canalicular stage and consisted of large terminal air spaces and a rudimentary bronchial tree. The absolute lung volume was only 0.002 cm3 with an air space surface de

Transdermal delivery of antenatal steroids to promote fetal lung maturation: Proof of principle data from sheep and non-human primate models.

BACKGROUND: Antenatal steroids (ANS) are routinely administered to women at risk of preterm birth to accelerate fetal lung maturation. Despite extensive clinical use, dosing and route of ANS administration remain unoptimized beyond intramuscular (IM) injection. We aimed to undertake a proof-of-principle assessment of transdermal ANS administration for accelerated fetal lung maturation. METHODS: We formulated and tested a transdermal ANS (betamethasone) patch in ex vivo studies. To confirm in vivo transdermal distribution of the ANS patch, we undertook a 24-h pharmacokinetic study in non-pregn

Identification of Keratin 5-Expressing Fibroblasts for Regenerating Keratinocytes in the Necrotic Skin Graft.

Skin transplantation is commonly used to compensate for normal cutaneous tissues in full-thickness wounds or surgically excised skin. Despite complete termination of initial blood flow, transplanted skin usually maintains its dermal and epidermal structures without scar formation. However, the precise role of engrafted cells in maintaining epidermal and dermal homeostasis remains unclear. In this study, we investigated the chronological changes in neonatal full-thickness skin grafts on mice at the histological and single-cell transcriptomic levels. We found devastating necrotic changes in the
