Labor and delivery outcomes with remifentanil patient-controlled analgesia versus labor epidural analgesia among women with and without obesity: a retrospective population-based cohort study (2015-2024).

BACKGROUND: Evidence regarding the safety and effectiveness of intravenous remifentanil patient-controlled analgesia (PCA) in women with obesity remains limited. We compared labor and delivery outcomes associated with remifentnail PCA and labor epidural analgesia according to maternal body mass index. METHODS: This retrospective population-based cohort study included women with singleton, term, cephalic pregnancies undergoing spontaneous or induced labor who received either remifentanil PCA or labor epidural analgesia in Slovenia between 2015 and 2024. Analyses were performed separately in wo

Pregnancy Outcomes in Women With Vascular Ehlers-Danlos Syndrome: A Systematic Review of Maternal and Fetal Complications.

The Ehlers-Danlos syndromes (EDS) comprise a clinically and genetically heterogeneous group of heritable connective tissue disorders marked by skin hyperextensibility, joint hypermobility, and tissue fragility. Vascular EDS (vEDS) is an autosomal dominant disorder caused by pathogenic variants in COL3A1, the gene encoding type III collagen. The diagnosis of vEDS is confirmed by identifying a pathogenic variant in COL3A1. This systematic review was conducted according to the PRISMA guidelines. Major outcomes included maternal, fetal, and neonatal complications in EDS pregnancies. A comprehensiv

Extensive cervico-vaginal ischemic injury reaching the bladder base following obstructed labor in a macrosomic pregnancy: a case report.

INTRODUCTION: Fetal macrosomia, defined as a birth weight (BW) ≥4000 g, is associated with increased maternal and neonatal morbidity, particularly when BW exceeds 4500 g. While complications such as shoulder dystocia and postpartum hemorrhage are well recognized, extensive ischemic soft-tissue injury of the lower genital tract and adjacent urinary structures due to obstructed labor alone is exceedingly rare. CASE PRESENTATION: A 22-year-old gravida 3, para 2 woman at 40 weeks of gestation presented in active labor with signs of cephalopelvic disproportion, a prominent caput succedaneum, and p

Cervical ripening compared to amniotomy and/or oxytocin administration in twin population with unfavorable cervix: a propensity score analysis.

PURPOSE: To compare the effectiveness of prostaglandins versus amniotomy and/or oxytocin for labor induction in twin pregnancies with an unfavorable cervix, and to assess differences in induction and labor duration. METHODS: We conducted a retrospective cohort study including twin pregnancies with a Bishop score <7 undergoing labor induction at ≥34 weeks of gestation in two tertiary maternity units between 2017 and 2023. Advanced gestational age was defined as >37 weeks for monochorionic and >38 weeks for dichorionic twin pregnancies. Women receiving prostaglandins were compared with those un

Effects of different oral intake management during labor on maternal and neonatal outcomes: A protocol for a systematic review and network meta-analysis.

INTRODUCTION: Fasting or restricting oral intake during labor may adversely affect maternal and neonatal outcomes. Although several interventions regarding oral intake during labor have been proposed, the optimal strategy for clinical practice remains controversial. This study aims to conduct a network meta-analysis to identify the most effective intervention for managing maternal oral intake during labor. METHODS AND ANALYSIS: A comprehensive search will be conducted in the following electronic databases: Web of Science, PubMed, Embase, Ovid, Cochrane Library, ClinicalTrials.gov, the WHO Int

Third-trimester polyunsaturated fatty acid supplementation and increased blood loss at vaginal delivery: A prospective birth cohort study.

The association between polyunsaturated fatty acid (PUFA) supplementation, particularly omega-3 and omega-6 fatty acids, during the third trimester of pregnancy and obstetric outcomes is not well established. This prospective birth cohort study included 12,050 women who delivered at Kuopio University Hospital (KUH) between 2013 and 2024, and completed a questionnaire about supplementation after 28 weeks of gestation. Data were obtained from the KUH Birth Register and the questionnaire. The primary outcomes were maternal blood loss and the incidence of postpartum haemorrhage (PPH), while second

Low-Dose Aspirin is Not Associated with Increased Risk of Postpartum Hemorrhage.

OBJECTIVES: Aspirin (ASA) is recommended for preeclampsia prophylaxis, but it is uncertain whether higher doses, such as the 150 mg/day used in the ASPRE trial, increase the risk of postpartum hemorrhage (PPH). In Canada, the available formulation is 81 mg/day. We hypothesized that ASA 162 mg/day may increase the risk of PPH and other bleeding complications. We aimed to evaluate whether ASA 162 mg/day for preeclampsia prevention is associated with increased risk of PPH or other maternal or neonatal bleeding events. METHODS: This retrospective cohort study included patients taking ASA 162 mg/d

A Robson classification-based audit of cesarean section rates and public health implications in a tertiary care center in India.

INTRODUCTION: The rising global cesarean section rate, surpassing 21% globally and reaching up to 50% in some Indian settings, represents a growing public health challenge. In the absence of clear obstetric indications, unnecessary cesarean section contributes to increased maternal morbidity, neonatal complications, and avoidable healthcare expenditures. India's healthcare system has witnessed a steady escalation in cesarean section rates, necessitating urgent audit mechanisms to guide corrective strategies. The World Health Organization (WHO) Robson Ten-Group Classification System offers a st

Improving postpartum hemorrhage diagnosis in low-resource settings: development and verification of Quantavag, a novel uterine blood loss quantification tool.

BACKGROUND/OBJECTIVES: Postpartum hemorrhage (PPH) is the leading cause of maternal morbidity and mortality in low-resource settings globally. In Ghana, PPH accounts for approximately 27% of maternal deaths, driven by inaccurate or inaccessible blood loss quantification methods. To address this gap, we developed and verified Quantavag, a novel device designed to accurately measure uterine blood loss for timely PPH diagnosis and treatment in low-resource settings. METHODS: We conducted 45 customer discovery interviews with healthcare professionals, midwives, and patients in Ghana and the Unite

A Rupture in Silence: A Life-threatening Neonatal Adrenal Bleed.

Neonatal adrenal hemorrhage (NAH) is a rare condition that commonly presents with retroperitoneal bleeding in neonates. We report a term male neonate born through vaginal delivery who presented with respiratory distress and shock on the first day of his life. Examination revealed pallor, abdominal distension and a tender lumbar mass. Laboratory tests showed severe anemia, and imaging studies confirmed a large perirenal hematoma with absent adrenal shadow. The diagnosis was right adrenal rupture causing hemorrhagic shock. Prompt resuscitation and conservative management led to a favorable outco

A randomized control trial comparing the clinical outcome of thyroxine supplementation in thyroid peroxide antibody- negative cases in patients with subclinical hypothyroidism in pregnancy.

BACKGROUND: A normal free thyroxine level in the presence of an increased thyroid-stimulating hormone is what constitutes subclinical hypothyroidism (TSH). Changes in thyroid physiology during pregnancy result in the normal TSH range being lower than non-pregnant. Subclinical thyroid disease during pregnancy is associated with adverse outcomes, including, risk of adverse pregnancy and neonatal outcomes. Prevalence in India is 6-8%. The prevalence of antibodies varies with ethnicity. Pregnancy with TSH concentrations >2.5 mIU/L needs evaluation for thyroid peroxidase antibody (TPO) antibody sta

A Neonatal Transfer from Birth Center to Hospital with a Postpartum Hemorrhage During Transport.

There is increasing interest from women and birthing people for midwifery-led care with community birth options in birth centers and for home birth. Birth centers are a high-value and high-quality option for place of birth and also address the increasing need to provide access to perinatal care for women in maternity care deserts. This case report reviews the transfer of a postpartum patient and her newborn from an accredited, freestanding birth center to the designated hospital. The reasons for transfer were neonatal respiratory distress with hypoxemia and an emergent postpartum hemorrhage th

Umbilical cord ulceration associated with placental abruption: a case report.

BACKGROUND: Umbilical cord ulceration (UCU) is a rare but potentially fatal obstetric condition characterized by ulcerative disruption of the umbilical cord, often leading to fetal hemorrhage and intrauterine fetal demise. Placental abruption, defined as the premature separation of the placenta from the uterine wall, is another major cause of perinatal mortality. When UCU is complicated by occult placental abruption, the risk of adverse perinatal outcomes increases substantially. These associations highlight the importance of vigilant prenatal surveillance and comprehensive imaging to facilita

Prevalence, risk factors, and adverse maternal and fetal outcomes of anemia during pregnancy: a retrospective study in China, 2012-2020.

BACKGROUND: Anemia is a public world health problem among pregnant females especially in developing countries. China is a representatively developing country with vast territory and significant economic and cultural differences in different regions. Currently most studies on anemia in pregnancy ignored economically backward and resource-poor cities or districts and anemia during pregnancy in China was not accurately estimated. The goal of this study was to assess the prevalence, risk factors, and adverse outcomes of anemia among pregnant females across different regions of China using a large,

How to avoid placental transection during low transverse cesarean delivery for anterior placenta previa.

Placenta previa, particularly when anterior, is a major risk factor for postpartum hemorrhage and may necessitate transplacental fetal extraction during cesarean delivery, which increases maternal and fetal blood loss. Standard strategies to minimize bleeding, such as fundal uterine incisions (when placenta accreta spectrum is suspected) or intraoperative ultrasonographic guidance, may be technically demanding, time-consuming, or unavailable in emergencies or resource-limited settings. Controlled separation and mobilization of a placental edge to avoid placental laceration offers a simple, rep

Screening for large-for-gestational-age neonates at term: evidence of a labeling effect and increased intervention without neonatal benefit.

BACKGROUND: Routine third-trimester ultrasound screening for suspected large-for-gestational-age (LGA) and macrosomic fetuses is increasingly used to guide obstetric management and reduce delivery-related morbidity. However, ultrasound estimation of fetal weight at term has limited diagnostic accuracy, and evidence supporting interventions to reduce morbidity in large fetuses remains limited. Together, these uncertainties raise questions regarding the benefit of routine ultrasound screening for fetal largeness at term. OBJECTIVE: To evaluate the diagnostic accuracy of routine third-trimester
