Intraventricular hemorrhage (IVH) in newborns is a major cause of mortality and neurodevelopmental morbidity in preterm infants, particularly those with low birth weights. A strong inverse relationship exists between gestational age at delivery and the risk of intraventricular hemorrhage, reflecting the immaturity of the germinal matrix. The germinal matrix is a highly active region of the preterm brain with dense but unstable vasculature that is extremely susceptible to injury. This review examines both the pathogenesis of IVH and evidence-based strategies for its prevention. Antenatal interv

OBJECTIVE: To generate the evidence base, using national cohort data, to design a clinical trial evaluating whether cesarean section (CS) or vaginal birth (VB) offers better outcomes for very preterm infants.
BACKGROUND: Severe intraventricular hemorrhage (IVH) remains a major cause of death and brain injury amongst very preterm infants. Observational studies have suggested CS may reduce the risk, but it is not known which infants might benefit and whether a randomized trial, to generate robust evidence to guide clinical decisions, would be feasible.
METHODS: We examined births in England an

Heterotopic pregnancy, the simultaneous presence of an intrauterine and an ectopic gestation, is rare after spontaneous conception but may be life-threatening, as an intrauterine pregnancy can provide false reassurance during early assessment. Interstitial ectopic implantation is particularly hazardous because rupture can lead to severe hemorrhage. A 35-year-old gravida 3, para 2, presented at 9+4 weeks of gestation with mild lower abdominal discomfort and no vaginal bleeding. Ultrasound confirmed a viable intrauterine pregnancy but also free pelvic fluid, prompting short-interval reassessment

BACKGROUND: Uterine adenomyosis involves the growth of endometrial gland and stroma within the uterine musculature. Patients with uterine adenomyosis experience heavy painful periods, deep dyspareunia and subfertility, reducing their overall quality of life. Hysterectomy is usually the recommended treatment for such patients. Whilst uterine artery embolization (UAE) offers a suitable, fertility-preserving alternative, for patients who have not completed their families, its impacts on pregnancy outcomes have not yet been explored or studied in women with pure adenomyosis.
OBJECTIVE: The object

Neonatal uterine bleeding (NUB) has been hypothesized as a relatively common early-life event potentially linked to the later development of endometriosis through retrograde menstruation. However, its true prevalence and biological nature remain uncertain. This study aimed to determine the prevalence, biological features, and maternal-neonatal correlates of reddish diaper staining in newborns, and to evaluate its purported association with endometriosis. A prospective cross-sectional study was conducted between September 2023 and March 2024 at a tertiary obstetric center with postpartum outpat

AIM: Increasingly, point-of-care testing has been adopted to rapidly assess fibrinogen levels for managing severe postpartum hemorrhage; however, evidence regarding its impact on clinical outcomes is limited. We evaluated whether implementing a transfusion strategy guided by point-of-care testing was associated with changes in blood loss and transfusion outcomes in patients with severe postpartum hemorrhage.
METHODS: We conducted a retrospective single-center interrupted time series analysis at a tertiary perinatal referral center in Japan. Consecutive patients with severe postpartum hemorrha

BACKGROUND: Uterine rupture during the second trimester is an exceptionally rare but life-threatening obstetric complication, presenting significant diagnostic and therapeutic challenges.
CASE SUMMARY: A 33-year-old woman, gravida 4, para 1, at 22 weeks and 3 days of gestation, was admitted due to a shortened cervical length. Four hours after admission, the patient developed acute, severe epigastric pain radiating to the back and shoulders. Her condition deteriorated with persistent hemoglobin decline. Abdominal and contrast-enhanced CT scans revealed perihepatic, perisplenic, and perienteric

Placenta accreta spectrum (PAS) is a major cause of obstetric hemorrhage and maternal morbidity. Despite advances in antenatal imaging, some cases remain undiagnosed until delivery. We report a case of intraoperative diagnosis of placenta accreta in a 32-year-old woman with a history of two prior cesarean sections. Following delivery of a healthy neonate, the placenta failed to separate and was found to be abnormally adherent to the uterine wall. In the setting of ongoing bleeding and absence of a cleavage plane, a subtotal hysterectomy was performed. The patient had a favorable postoperative

BACKGROUND Cesarean scar pregnancy (CSP), a rare form of ectopic pregnancy, is associated with substantial maternal morbidity and uncertain implications for subsequent pregnancies. This study evaluated the association between prior CSP and adverse obstetric outcomes in subsequent pregnancies; it also explored independent predictors of postpartum hemorrhage (PPH). MATERIAL AND METHODS This retrospective single-center study included 15 439 women with prior cesarean section who delivered at a tertiary hospital (January 2015 to December 2023): 83 patients with CSP (Type I: n=24, ultrasound-guided

BACKGROUND: Fetal and neonatal alloimmune thrombocytopenia (FNAIT) arises from maternal-fetal platelet incompatibility, where maternal alloantibodies-most commonly anti-HPA-1a in Caucasians-cross the placenta and trigger fetal platelet destruction. The most severe complication is intracranial hemorrhage (ICH), which can occur in utero. Despite its severity, significant heterogeneity exists among national and international management guidelines.
METHODS: To align clinical practice with recent evidence, the Leiden University Medical Center and Sanquin Diagnostic Services (Netherlands) revised t

ABSTRACT: This study investigates the relationship between lactate levels - produced during myometrial contraction and transient hypoxia - and maternal and fetal outcomes. Uterine contractions during labour are regulated by hormonal and metabolic processes, including oxytocin release, catecholamine modulation, and the hypoxia-induced force increase (HIFI) mechanism. In a cohort of 303 women receiving neuraxial analgesia, lactate was measured at three time points: at the diagnosis of the active phase of the first stage of labour (T0), at the beginning of active pushing (T1), and at delivery (T2

BACKGROUND: Guidelines for managing a small symphysial fundal height (SFH) measurement during pregnancy are well established, but guidelines identifying and managing a large SFH measurement at term are not.
AIMS: To determine the relationship between a large SFH at term and intra-partum risk, and to determine if there is an SFH cut-off that could predict the following adverse outcomes: emergency caesarean section (CS), instrumental delivery, postpartum haemorrhage (PPH) and admission of neonate to the special care nursery (SCN).
MATERIALS AND METHODS: A retrospective audit was performed on 7

BACKGROUND: The convergence of Advanced Maternal Age (AMA ≥ 35), In Vitro Fertilization (IVF) conception, and Placenta Accreta Spectrum (PAS) represents a clinical nexus of potentially elevated obstetric risk, hypothetically associated with a high probability of massive peripartum hemorrhage (MPH). Based on limited observational evidence, this case-based narrative review hypothesizes that this risk profile warrants protocol-driven management considerations, which are presented as hypothesis-generating rather than definitive.

BACKGROUND: Postpartum hemorrhage (PPH) and Hypertensive Disorders of Pregnancy (HDP) remain leading causes of maternal death. This multi-country survey asked key informants to assess uptake of evidence-based practices; determine their country's implementation of updated WHO global guidelines; and describe the private sector's role in PPH and HDP management. To our knowledge, this is among the few multi-country assessments to examine national implementation of updated WHO recommendations for PPH and HDP alongside private-sector engagement across three major regions-sub-Saharan Africa, South an

Amniotic fluid embolism (AFE) is a rare but catastrophic obstetric emergency characterized by sudden cardiorespiratory collapse, disseminated intravascular coagulation (DIC), and a high case-fatality rate. Because no confirmatory test exists, the diagnosis remains clinical and one of exclusion. We report a case of presumed AFE in a 39-year-old primigravida with uterine myomas, obesity, and chronic hypertension who underwent an elective primary cesarean delivery under spinal anesthesia. During manipulation of the placenta, the patient developed abrupt cardiovascular collapse requiring cardiopul

INTRODUCTION: The majority of low placentation identified on first-trimester transabdominal ultrasound resolves; however, whether this resolution is associated with adverse outcome remains poorly understood. This investigation aimed to determine whether patients with resolved first-trimester low placentation had different delivery outcomes compared to patients who never had low placentation.
METHODS: This is a retrospective cohort study of singleton pregnancies with low placentation, defined as low-lying placenta or placenta covering the internal os, on first-trimester transabdominal ultrasou

Peripartum hysterectomy (PH), the surgical removal of the uterus during the peripartum period, is a rare but potentially life‑saving intervention for severe obstetric complications such as major haemorrhage, uterine rupture, and placenta accreta spectrum (PAS). As a maternal near‑miss event, PH carries substantial maternal and neonatal morbidity, mortality, and long‑term psychological impact. Data from the European INOSS network report an incidence of pH of 5.2 per 10,000 births, with notable variation between countries. Uterine atony and PAS are the leading indications, while major risk facto
