From standard to individualized diazoxide therapy in congenital hyperinsulinism: a narrative review.

PURPOSE: Congenital hyperinsulinism (CHI) is the commonest cause of persistent hypoglycaemia in neonates and infants (blood glucose <3.0 mmol/L in first 2-3 days of life; <3.5 mmol/L after 3 days of life). Diazoxide demonstrates variable efficacy depending on the underlying genetic variant and clinical phenotype. Diazoxide has been associated with side effects that are likely dose dependent. This narrative review synthesizes current evidence on diazoxide's pharmacokinetics and side effect profile to support the development of individualised dosing strategies guided by genotype and patient-spec

Maturity onset diabetes of the young in pregnancy: management, treatment and neonatal outcomes-a systematic review.

Maturity Onset Diabetes of the Young (MODY) is a type of genetically inherited diabetes. As it generally arises in early adulthood, it can affect pregnant women with significant neonatal outcomes. Our study aimed to conduct a systematic review on pregnancy management and outcomes of all subtypes of MODY, browsing the Cochrane, EMBASE and PubMed databases to find clinical studies on the topic. They were last searched on June 29, 2025. The inclusion criteria for our review were: published articles, available in their full-text version, about MODY, reporting data about its management during pregn

Early Versus Delayed Oxytocin Augmentation in Nulliparous Women with Spontaneous Dysfunctional Labour: A Systematic Review and Meta-Analysis.

Background: The timing of oxytocin for slow labour progress rests on custom rather than evidence, and existing syntheses pool clinically distinct populations. Objectives: We restricted this systematic review to nulliparous women in established active-phase labour with a trial-specified diagnosis of slow progress, comparing early versus delayed or withheld intravenous oxytocin. Methods: Cochrane review CD007123 was updated by searching MEDLINE, Embase and CENTRAL; counts were recomputed from primary reports. Results: Three trials (1102 women) were eligible. Early oxytocin did not alter caesarea

A minimalist and robust diagnostic model for neonatal biliary atresia: Harnessing MMP-7 and machine learning in a time-critical setting.

BACKGROUND: Early diagnosis is the most critical determinant of native liver survival in infants with biliary atresia (BA). Accurate differentiation of BA from other neonatal cholestatic disorders remains particularly challenging within the first 28 days of life, a period during which clinical and biochemical features are often nonspecific. This study aimed to develop and validate a minimalist, highly interpretable diagnostic model that integrates serum matrix metalloproteinase-7 (MMP-7) with routinely available clinical parameters to facilitate early BA diagnosis in neonates. METHODS: This r

Timing of birth to improve outcomes in chronic or gestational hypertension: the WILL RCT.

BACKGROUND: For women with chronic or gestational hypertension who remain well, early term birth (at 37-38 weeks' gestation) may reduce maternal complications, caesareans and stillbirths, but it may increase neonatal morbidity compared with expectant care. Expectant care may increase costs. There are no high-quality data to guide care, which currently involves maternal-fetal surveillance and intervention for maternal or fetal compromise, which may be rapid or unexpected. OBJECTIVE: To investigate optimal timing of birth for women with chronic or gestational hypertension who reach term and rem

High- versus low-dose phototherapy for neonatal jaundice.

RATIONALE: Phototherapy is the mainstay of treatment for neonatal hyperbilirubinaemia. Increasing the dosage or intensity of phototherapy by various means is commonly practised for infants with high levels of serum bilirubin. It is unclear whether these measures of increasing phototherapy dosage are beneficial and safe for neonates. OBJECTIVES: To assess the effects of high-dose versus low-dose phototherapy on bilirubin level and associated clinical outcomes that constitute the major conditions in kernicterus spectrum disorder (KSD), such as acute bilirubin encephalopathy and kernicterus, as

Newborn screening for severe combined immunodeficiency in the NHS newborn blood spot screening programme: a systematic review.

BACKGROUND: Severe combined immunodeficiency is an inherited condition arising from mutations in at least 19 known genes. Severe combined immunodeficiency can be identified through screening, family history or clinical presentation. Severe combined immunodeficiency is usually asymptomatic at birth and presents, in infancy, as recurrent and frequently severe infections. Without treatment, severe combined immunodeficiency is usually fatal in the first year of life. OBJECTIVES: To summarise the available evidence relevant to newborn screening for severe combined immunodeficiency in the UK NHS ne

Validation of the ICD-10 Diagnoses of Early-Onset Neonatal Infection in the Danish National Patient Register from 2010 to 2018.

PURPOSE: To examine the accuracy of the diagnoses of early-onset neonatal infection in the Danish National Patient Register. PATIENTS AND METHODS: All Danish neonates born after 35 weeks of gestation with an ICD10 diagnosis of sepsis or meningitis within the first week of life between 2010 and 2018 were identified. To examine the validity of the diagnoses, medical records were reviewed for 500 neonates diagnosed with sepsis and all 63 neonates diagnosed with meningitis. Proven infection was defined by bacterial pathogens identified from blood or cerebrospinal fluid. Probable sepsis was define

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Imputation of missing clock times - application to procalcitonin concentration time course after birth.

The time course of biomarkers (e.g., acute phase proteins) are typically described using days relative to events of interest, such as surgery or birth, without specifying the sample time. This limits their use as they may change rapidly during a single day. We investigated strategies to impute missing clock times, using procalcitonin for population modelling as the motivating example. 1275 procalcitonin concentrations from 282 neonates were available with dates but not sample times (Scenario 0). Missing clock times were imputed using a random uniform distribution under three scenarios: (1) min

Validation of Peripheral Perfusion Index as a Noninvasive Screening Tool in Predicting Adverse Neonatal Outcome.

INTRODUCTION: Timely identification of suboptimal organ perfusion is crucial to detect adverse outcomes early and to prevent its long-term motor and cognitive effects. Peripheral perfusion index (PI) is a noninvasive screening tool which can identify babies at risk for hemodynamic compromise. AIM: We aimed to study the validity of peripheral PI as an early predictor of adverse neonatal outcome in comparison with other commonly used noninvasive screening modalities such as heart rate (HR), respiratory rate, temperature, capillary refill time (CRT), and blood pressure. MATERIALS AND METHODS: T

Indocyanine green fluorescence in second-look surgery for necrotizing enterocolitis: enhancing the surgeon's perception.

BACKGROUND: A perfusion mismatch in the premature gut is the key component in the development of necrotizing enterocolitis (NEC). Resulting necroses need to be surgically excised, while intestinal salvage is crucial to the survival and rehabilitation of affected preterm neonates. Until now, resection margins have been based on standard visual inspection and surgical experience due to the lack of objective criteria for bowel viability. We hypothesize that by evaluating the vitality and perfusion during NEC surgery, necrosis margins can reliably be defined by indocyanine green (ICG) fluorescent

Epidemiology and management of malignancies in patients with inborn errors of immunity-An ESID registry study of 19,959 patients.

BACKGROUND: Inborn errors of immunity (IEI), or primary immune disorders (PIDs), predispose individuals to infections, autoimmunity, inflammation, allergy, and malignancy. Malignancies are a major cause of morbidity and mortality in patients with IEI/PIDs, with poorer outcomes compared with the general population. OBJECTIVE: We sought to determine the frequency and types of malignancies in patients with IEI/PIDs and to assess clinical management approaches across Europe. METHODS: Descriptive analyses were performed on malignancy data within each IEI category. In addition, a European Society

Early treatment versus expectant management of hemodynamically significant patent ductus arteriosus for preterm infants.

RATIONALE: Patent ductus arteriosus (PDA) is associated with significant morbidity and mortality in preterm infants. Cyclo-oxygenase (COX) inhibitor drugs are used to prevent or treat a PDA. There are concerns regarding adverse effects of COX inhibitors in preterm infants. Controversy exists about whether early targeted treatment of a hemodynamically significant (hs)-PDA improves clinical outcomes. This review updates our previous Cochrane review (2020). OBJECTIVES: To assess the effectiveness and safety of early treatment strategies versus expectant management for hemodynamically significant

Glycaemic control in labour with diabetes: GILD, a scoping study.

BACKGROUND: Diabetes in pregnancy is common, affecting 5-10% of pregnant women. Poor glycaemic control in labour is associated with neonatal hypoglycaemia and other adverse outcomes for mother and baby, but tight glucose control is burdensome, intrusive and may not always be necessary. The ideal intrapartum glucose target level is unknown, traditionally 'tight' control (target 4-7 mmol/l) has been recommended; however, this increases the risk of maternal hypoglycaemia. OBJECTIVE: To determine the feasibility of a randomised clinical trial to compare clinical and cost-effectiveness of permissi
