The D-- phenotype is an exceptionally rare Rh blood group phenotype characterized by expression of D in the complete absence of C, c, E, and e. Individuals with this phenotype are at risk of developing anti-Rh17 (anti-Hr0), a clinically significant alloantibody that can severely complicate transfusion support and pregnancy management because compatible blood is exceptionally difficult to obtain. This review critically synthesizes the available published evidence regarding the D-- phenotype and anti-Rh17 alloimmunization, integrating data from case reports, case series, family investigations, a

BACKGROUND AND AIMS: Hemolytic disease of the fetus and newborn and other forms of neonatal jaundice remain important causes of morbidity and mortality among children under five worldwide. Despite improvements in prevention and treatment such as Rh screening, phototherapy, and exchange transfusion, the burden remains uneven across regions. This study assessed global, regional, and national trends in incidence, prevalence, mortality, Disability-Adjusted Life Years (DALYs), Years of Life Lost (YLLs), and Years Lived with Disability (YLDs) from 1980/1990 to 2021 using Global Burden of Disease (GB

BACKGROUND: RhD mismatched transfusions in RhD negative women have the potential to impact future pregnancies through alloimmunization and development of hemolytic disease of the fetus and newborn (HDFN). As use of RhD positive emergency-release blood products in pediatric trauma has increased, it has become clear that a significant alloimmunization risk also exists for RhD negative girls, which has not been addressed by existing literature. This article describes a best-practice guideline to manage RhD negative pediatric females who receive RhD positive transfusions.

OBJECTIVE: Hemolytic disease of the fetus and newborn (HDFN) is a rare and serious immune-mediated condition that can result in poor fetal and neonatal outcomes. During pregnancy, monitoring with middle cerebral artery Doppler ultrasound and intrauterine transfusions (IUTs) are the current standard of care, although intravenous immunoglobulin (IVIg) has been adopted in some centers to attempt to delay the onset of fetal anemia. Significant gaps remain in understanding the safety and effectiveness of current HDFN management, both during pregnancy and in neonates and children. We present the des

BACKGROUND: Although great advances have been made in the prevention of hemolytic disease of the fetus and newborn, especially for anti-D, irregular antibodies still appear. In order to ascertain the situation in Spain, we conducted a survey among hospitals treating pregnant women.
MATERIALS AND METHODS: We sent a survey to Spanish public hospitals comprising demographical data (ethnicity, number of pregnancies, type of fertilization), characteristics of antibodies (specificity, number, antigenic compatibility), cause of alloimmunization and the clinical consequences (fetal death, icterus, an

This report describes a rare case of hemolytic disease of the newborn caused by anti-Cellano (anti-k) antibodies, part of the clinically significant Kell blood group system, the third most important blood group system after ABO and Rh. We present the case of a 43-year-old woman with a history of multiple miscarriages and one previous live birth conceived through in vitro fertilization with a donor oocyte. At 38 weeks of gestation, she delivered a B-positive neonate who subsequently developed anemia and jaundice. Laboratory evaluation demonstrated a positive direct Coombs test and the presence

Hemorrhage is a leading preventable cause of trauma-related death. Early hemorrhage control and resuscitation with blood products are both critical for survival. The use of whole blood and low-titer type O blood has been shown to reduce overall transfusion volume, mitigating the effects of dilutional coagulopathy. Historically, Rh(D)-negative blood products have been prioritized for people of childbearing potential given the feared risk of alloimmunization and hemolytic disease of the fetus and newborn. However, the survival benefit of whole blood resuscitation seems to outweigh the estimated

INTRODUCTION: Targeted routine antenatal anti-D prophylaxis (RAADP) was introduced in Finland in 2014. The aim of this study was to assess the prevalence of anti-D immunizations among pregnant women and the severity of hemolytic disease of the fetus and newborn (HDFN) in affected pregnancies 10 years after RAADP was added to the national prevention program in the context of a rapidly declining birthrate in Finland.
MATERIAL AND METHODS: A nationwide, retrospective cohort study included all RhD-negative pregnancies in Finland between 2014 and 2023. Information on antenatal screening was obtain

Red blood cell (RBC) alloimmunization remains a relevant cause of hemolytic disease of the fetus and newborn (HDFN). Although RhD immunization has significantly decreased since the implementation of systematic prophylaxis, it is still the main cause of alloimmunization in pregnancy. Clinically significant non-RhD alloantibodies, particularly those of the Rh (c, C, E), Kell, Kidd, Duffy, and MNS systems, are associated with variable risk of fetal anemia and account for an increasing proportion of alloimmunized pregnancies requiring specialized prenatal care. Most diagnostic algorithms and manag

This article addresses the diagnostic challenges encountered and resolution of a neonate with blocked D phenomenon, where maternal anti-D had masked the D sites on the neonate's red blood cells (RBCs), initially resulting in false-negative D typing. This article emphasizes the importance of integrating serologic and molecular methods for accurate blood type determination, which is crucial for managing hemolytic disease of the fetus and newborn. A mother, whose RBCs typed as D- and with a history of D isoimmunization, gave birth to a neonate who presented with anemia and jaundice. Initial D typ

INTRODUCTION: The aim of this study was to investigate the clinical consequences of anti-D and non-anti-D antibodies undetected at screening in the first trimester for hemolytic disease of the fetus and newborn (HDFN).
METHODS: This prospective study (conducted from 2010 to 2024) included pregnant women with negative first-trimester antibody screening for red blood cell (RBC) antibodies but positive later in pregnancy for clinically significant antibodies anti-D and non-anti-D. We analyzed the incidence of antibodies, DAT of the newborn, and the treatment and outcomes of HDFN.

The Rh blood group system is the most polymorphic and clinically significant blood group system after ABO. The RhD protein, in particular, encompasses numerous conformation-dependent epitopes that can elicit alloimmune responses, leading to hemolytic disease of the fetus and newborn or hemolytic transfusion reactions. Despite this, traditional serologic methods for RhD typing lack sensitivity and specificity to detect RhD variants like partial D and DEL phenotypes. This chapter describes molecular methods to characterize RhD mRNA expression and identify transcript variants in immortalized leuk

INTRODUCTION: Rh isoimmunization remains a major cause of haemolytic disease of the fetus and newborn, particularly when fetal anaemia develops early in gestation. Antenatal intravenous immunoglobulin (IVIG) has been proposed to improve fetal outcomes, but evidence remains limited, especially from low- and middle-income settings.
METHODS: This study was conducted at a tertiary care centre in India and included pregnant women with severe anti-D isoimmunization managed between 2023 and 2025. Inclusion criteria were pregnant women, with a previous pregnancy complicated by hydrops with perinatal

Hemolytic disease of the fetus and newborn, which normally presents with unconjugated hyperbilirubinemia, has been known to cause cholestatic jaundice, albeit in rare circumstances; seen especially in the setting of Rhesus (Rh) hemolytic disease or when the mother has received intrauterine transfusion(s). Infants presenting with cholestatic jaundice in the setting of hemolytic anemia pose a clinical dilemma with regard to investigative approach and management. There are no evidence-based guidelines as to whether such infants should be managed conservatively or investigated comprehensively for

INTRODUCTION: the ABO and Rhesus systems remain the most clinically significant blood group antigens on red blood cell membranes. Rhesus isoimmunization occurs when maternal Rh antibodies in a RhD-negative woman react with red blood cells of an RhD-positive fetus, leading to adverse fetal and neonatal outcomes.
METHODS: a cross-sectional review of 194 medical records of RhD-negative pregnant women managed at Kenyatta National Hospital (KNH) between 2013 and 2019 was conducted. Data on sociodemographic, obstetric, and clinical characteristics were extracted and analyzed using SPSS version 23.

Background: Maternal anti-D antibody titers are routinely used for risk stratification in RhD alloimmunization before the development of fetal anemia. However, their association with disease severity and perinatal outcomes among pregnancies that have already progressed to severe fetal anemia requiring intrauterine transfusion (IUT) remains unclear. This study aimed to investigate the association between maternal anti-D antibody titers, disease severity, and perinatal outcomes in pregnancies complicated by severe RhD alloimmunization requiring IUT. Methods: This retrospective cohort study inclu

Isoimmunization and fetal anemia pose considerable challenges in obstetrics, especially when there is a documented history of RhD incompatibility or other antibodies that may threaten fetal well-being. These conditions frequently result in serious complications, including hydrops fetalis, fetal anemia, and, in certain situations, fetal loss. Effective management of such pregnancies needs careful monitoring and prompt interventions, such as intrauterine transfusion (IUT), along with thorough post-delivery follow-up to safeguard both maternal and neonatal health. Due to ongoing uncertainties con

BACKGROUND: Hemolytic disease of the fetus and newborn (HDFN) is a life-threatening condition resulting from maternal-fetal erythrocyte antigen incompatibility. Although anti-Rhesus D (RhD) prophylaxis has reduced RhD-associated cases, HDFN persists due to non-RhD antibodies and gaps in prevention. Population-based data on maternal and neonatal outcomes and recurrence patterns are limited.
OBJECTIVE: This study aimed to characterize maternal and neonatal outcomes, health care use patterns, and recurrence rates of HDFN across pregnancies.
METHODS: We conducted a retrospective cohort study of

BACKGROUND: Prenatal RhD screening enables risk stratification for RhD-associated hemolytic disease of the fetus and newborn (HDFN) and supports rational,evidence-based use of prophylactic anti-RhD immunoglobulin. However, noninvasive prenatal RhD screening remains challenging in the Chinese population, primarily due to population-specific RHD allelic diversity and the limited adaptability of current assay designs.
METHODS: We developed and optimized a real-time PCR-based assay for noninvasive prenatal RHD genotyping, specifically designed to accommodate the spectrum of clinically relevant RH

INTRODUCTION: Hemolytic disease of the fetus and newborn (HDFN) and fetal and neonatal alloimmune thrombocytopenia (FNAIT) are two rare alloimmune diseases of pregnancies where maternal alloantibodies target fetal blood components, potentially leading to risks of serious health conditions and developmental delays for babies, and sometimes loss of the baby. HDFN and FNAIT often have similarly complex journeys, from diagnosis through to burdensome treatments such as intrauterine transfusions and intravenous immunoglobulin.
